Performance Work Statement Varian Clinac Linear Accelerator Trilogy 22 May 2019

1. SCOPE: This performance work statement establishes maintenance service for the 88 MDOS/SGOMR on the Varian Clinac Linear Accelerator system CL-IXS Trilogy with multi- leaf collimator (MLC), on-board imaging (OBI), and cone-beam computed tomography (CBCT) capabilities. The services required are telephone and email consultations, quarterly preventative maintenance (PM), software and hardware updates, and emergency diagnostic and repair visits.

1.1 Federal Holidays:

For the purpose of this contract Federal Holidays shall include the following days; New Year's Day, Martin Luther King Day, Washington's Birthday (aka President's Day), Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day.

2. REQUIREMENTS:

2.1 Preventative Maintenance

2.1.1 Description of PM:

This will include preventative maintenance inspections and calibrations. The contractor shall be responsible for all costs incurred, including all labor, travel, materials, and shipping (as required) in providing the services for maintenance and repair of normal wear and tear on the equipment. This PM shall include FDA-mandated updates, as well as functionality updates that are required to maintain original equipment manufacturer (OEM) functionality standards.

2.1.2 Parts and Materials: All defective parts/materials which the contractor has replaced with new parts shall become the property of the contractor. “New parts” are defined as replacement items that are new to the WPAFB Varian Clinac Linear Accelerator system, which shall be: OEM brand-new parts, OEM never-before-used parts, or OEM-refurbished parts.

2.1.3 Minimum Uptime Guarantee: The contractor shall provide a minimum uptime guarantee of 97% over 1 year (excluding weekends and federal holidays).

2.1.4 Quarterly PM Schedule: The Government shall contact the contractor each quarter to schedule the quarterly PM for this equipment.

2.1.5 Equipment Removal: The contractor shall not remove any accessory from WPAFB without prior written approval from the WPAFB Medical Equipment Repair Center (MERC).

2.2 Telephone and Email Support

2.2.1 Description:

The contractor shall provide telephone and email support to try to resolve issues before a technician is sent to WPAFB.

2.2.2. Schedule of Telephone & Email Consultations: Telephone or email support shall available between 0800 and 1700 EST, Monday-Friday, excluding federal holidays. The vendor shall acknowledge receipt of a request or inquiry within 24 hours, during normal duty hours (0800-1700 EST), Monday-Friday, excluding federal holidays.

2.3 Diagnostic and Repair Visits (Beyond Normal Preventative Maintenance)

2.3.1 Description:

If there is an issue that cannot be resolved via telephone or email consultation, the Government shall request that the contractor send a representative to WPAFB in a timely manner to run diagnostic tests on this equipment to determine what the issue is and to repair the issue. The contractor shall be responsible for all costs incurred for these emergency visits and repairs, including: all labor, all travel, and all OEM parts and materials.

2.3.2 Responsiveness to emergency visit requests: The contractor shall make every effort to acknowledge receipt of an emergency visit request within 24 hours (0800-1700, EST; Monday- Friday, excluding federal holidays).

2.3.3 Scheduling of Emergency Diagnostic and Repair Visits: The Government shall contact the contractor as needed to schedule any emergency visits that are deemed necessary for this equipment. The Government and the contractor shall work together to determine an appropriate repair schedule to ensure 97% machine uptime over the course of 1 year.

4. Enterprise-wide Contractor Manpower Reporting Application (eCMRA)

The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force via a secure data collection site.

The contractor is required to completely fill in all required data fields using the following web address: https://www.ecmra.mil

Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30.

While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year.

Reporting Period: Contractors are required to input data by 31 October of each year. Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.
