Transmittal Letter for Government Sources to Accompany Present/Past Performance Questionnaire

SUBJECT: Present/Past Performance Questionnaire for Contract(s) .

1. We are currently responding to the Department of the Air Force (AF), Warner Robins Air Logistics Complex, Request for Proposal FA8574-19-R-0002, for the procurement of the Link-16 Alaska (LAK) CLS Support. This RFP is being conducted as a Tradeoff Source Selection and specifically requires that we, as an offeror, do the following:

The offeror shall send out – and track the -- Present/Past Performance Questionnaire to each of the offeror’s, critical subcontractors’, teaming contractors, and/or joint venture members’ Points of Contact (POCs) identified in the FACTS Sheet. The responsibility to send out and track the Present/Past Performance Questionnaires rests solely with the offeror - i.e., it shall not be delegated to any other entity. The Transmittal Letter (see RFP Attachment 6) shall be used by the offeror in sending out the Present/Past Performance Questionnaires. Exert your best effort to ensure that at least two POCs per relevant contract submit a completed Present/Past Performance Questionnaire directly to the Government not later than the date established in the RFP for receipt of proposals. POCs may submit their completed Present/Past Performance Questionnaire either electronically or by mail. If sending electronically, follow the procedures outlined below:

Exchanges of source selection information between the Government and offerors after submission of the initial proposal packages will be controlled by the Contracting Officer. E-mail may be used to transmit such information preferably with the e-mail being sent encrypted, and must include the statement “Source Selection Information – See FAR 2.101 and 3.104” in the subject line of the e-mail. Otherwise, source selection information may be transmitted via direct mailing. In order to facilitate the sending and receiving of encrypted e-mails, offerors must use MS Outlook e-mail configured to support encryption or a different e-mail product that is S/MIME compatible and configured to support encryption. If you intend to submit your source selection information via encrypted e-mail, you will need to contact the PCO/Buyer indicated on the face page of the solicitation prior to that first submittal to coordinate the exchange of certificates used for encryption. To ensure the process is working correctly, send a test encrypted message first (without including any source selection information). If an offeror’s computer system cannot support encryption technology, the offeror shall send the Contracting Officer correspondence at the time that they submit their proposal stating they accept the risk of sending and receiving unencrypted electronic mail if they choose to utilize unencrypted e-mail as a channel of communication.

2. We have identified subject contract(s) as relevant to this acquisition and you as our POC. As such, please take a few moments of your time to fill out the attached questionnaire and send it directly back to WR-ALC. The information contained in the completed Present/Past Performance Questionnaires is considered sensitive and cannot be released to us, the offeror. If you have any questions about the acquisition or the attached questionnaire, your questions must be directed back to the Government’s points of contact identified above.
