PERFORMANCE WORK STATEMENT

Crane Rental – 250 Ton

18 June 2019

1.0 DESCRIPTION OF SERVICES: The contractor shall provide all travel, per diem, personnel, equipment, tools, materials, transportation, and services for a 250 ton capacity hydraulic truck crane with NCCCO operator to relocate a vertical 11,000 gal nitrogen tank, weighing 60,000 pounds, 34 feet tall. The nitrogen tank needs to be moved 110 feet east of its current location and anchored in its new locate. The tank will need to be anchored in 6 ea. location with 1-1/4” X 20” HAS-R stainless steel threaded rods with a tensile strength 32,000 lbs. each or greater. Each anchor will need two nuts to prevent self-loosening. The Contractor will need to drill anchors holes 16 in. or greater in existing pad and apply injectable mortar or adhesive capsule approved for concert use, to epoxy threaded anchors in the concrete pad.

1.1 Over and Above Work. Over and above work is work identified by the Contractor or by the Government not included in the Contractor’s proposal that necessitates the need for additional parts or other corrective action. If over and above work is required, the Contractor shall prepare and submit an estimate identifying the over and above work along with recommended corrective actions to the CMXG Government Representative. The CMXG Government Representative will provide the estimate to the Government Contracting Officer. The Government Contracting Officer will review the estimate, negotiate a formal quote and authorize the work to be completed. Once the Government Contracting Officer authorizes the work, the Contractor shall schedule a date and time to perform the work with the CMXG Government Representative.

At a minimum, Contractor estimated shall contain the following information:

· Date of evaluation

· Equipment identification number(s)

· Specific location of the work

· Information describing the type of work required

· All costs of performing the repairs to include estimated labor hours, travel, equipment, and material

1.1 Contractor will contact Government POC within 5 business days after contract award to schedule this service.

1.2 Crane will be needed a minimum of two (2) days for eight (8) hours each day.

1.4 Delivery: The Contractor shall provide FOB Destination delivery no later than three (3) weeks after contract award to the following POC and address:

1.5 ACCEPTANCE: Upon completion of relocation and anchoring of the nitrogen tank, the Government POC, Brian Biegel, will accept.

2.0 GOVERNMENT FURNISHED PROPERTY: None.

3.0 SERVICES SUMMARY: None.

4.0 GENERAL INFORMATION:

4.1 Access to Government Facilities:

4.1.1 Base Access: The contractor will be escorted by Government personnel at all times during all service calls. The Air Force has mandated visitors, to include all non-DOD card holders, to consent to a check of their criminal history prior to being allowed access to installations. The check is similar to a driver’s license check and be conducted at the Visitor Control Center (VCC) off of Watson Boulevard.

4.1.1.1 The Contractor shall ensure Contractor personnel who require access to Robins AFB or other Government facilities comply with the security procedures of the facility.

4.2 PLACE OF PERFORMANCE: Building 169 North Side, Robins AFB, GA.

4.3 SCHEDULE: The services will be performed between the hours of 7:00 am and 4:00 pm (EST), Monday through Friday, excluding Federal holidays.

4.4 Contractor’s Safety Requirement. (See Appendix C, Part II, Para 7) The Contractor shall establish and maintain an acceptable safety program in accordance with their accepted Safety and Health Plan. The Air Force is part of the OSHA Voluntary Protection Program (VPP). All shall familiarize themselves with the requirements of VPP. Information on the VPP can be accessed at http://www.osha.gov/dcsp/vpp/index.html. Additionally, the Contractor is responsible for the safety and health of all subcontractor employees. No more than two Class C mishaps per contract year and Zero Class A or B mishaps will be tolerated. All changes to the plan must also be submitted in writing and approved by the Government. (CDRL A001, DI-SAFT-82080, Contractor's Safety Plan)

4.5 Mishap Notification. (See Appendix C, Part II, Para 5) The contractor shall notify the Robins AFB Safety Office by telephone at (478)-926-1708 during duty hours and (478)-926-3567 after hours, within four (4) hours after initial notification of all mishaps or incidents for damage to DoD property (material plus labor) entrusted by this contract, and employee injuries as a result of contract operations. A follow-up (written or e-mail) of the mishap/incident notification shall be sent within three (3) calendar days to the Government Representative, the Procurement Contracting Officer (PCO) or the Administrative Contracting Officer (ACO). The Government Representative will forward it to the Safety Office. The information that is not available at the time of the initial notification shall be provided within 20 calendar days after the mishap. Mishap notification shall contain, as a minimum, the following information:

a. Contract, Contract Number, Name and Title of Person(s) Reporting

b. Date, Time and exact location of accident/incident

c. Brief Narrative of accident/incident (Events leading to accident/incident)

d. Cause of accident/incident, if known

e. Estimated cost of accident/incident (material and labor to repair/replace)

f. Impound Authority Name and Phone Number (as applicable)

g. Nomenclature of equipment and personnel involved in accident/incident

h. Corrective actions (taken or proposed)

i. Other pertinent information (CDRL A002, DI-SAFT-81563 NOT 1, Accident/Incident Report)

4.6 Agency Affirmative Procurement Programs (formerly GPP). In accordance with FAR 23.404(b)(1), 100% of purchases of Environmental Protection Agency (EPA)-designated products included in the Comprehensive Procurement Guidelines (CPG) list [https://www.epa.gov/smm/comprehensive-procurement-guideline-cpg-program] and/or United States Department of Agriculture (USDA)-designated products included in the bio-based product listings [http://www.biopreferred.gov] shall meet the EPA or USDA standards for recovered materials or bio based content, respectively, unless an item cannot be acquired competitively within a realistic timeframe, meet appropriate performance standards, and/or be acquired at a reasonable price.

4.6.1 In accordance with FAR 23.404(a)(iii), the Contractor shall provide estimates and verification of recovered material for EPA-designated CPG products and certification for both EPA-designated CPG products and USDA-designated bio based products. In accordance with the Robins AFB Green Procurement Program (GPP) Plan, the Contractor shall provide estimates and certifications to the Government for completion of the applicable attachment(s) to be included in the Purchase Request (PR) package.

4.7 Environmental Management Systems (EMS) Awareness Training. All contractor personnel working on Robins AFB who perform activities on the installation are required to complete Air Force-provided initial EMS Awareness Training. It is the responsibility of the Prime Contractor to ensure that all sub-contractors, vendors, and employees complete this training prior to beginning work on Robins AFB. Notification of training completion shall be sent to both the Robins AFB Contracting Officer (CO) to be maintained in the contract file and Environmental Management, 78 CEG/CEIE, (78ceg.cev.FrontOfc@us.af.mil) for tracking purposes. Failure to provide documentation of EMS Training may result in termination of the contract.

4.7.1 EMS training is provided by Environmental Management at (478) 327-8104 or (478) 926-9645. Contractors have two options to satisfy the mandatory EMS Awareness Training requirement. Contractors that do not have a Common Access Card must use Option 2.

4.7.2 Option 1 (Preferred Option): The Air Force version of EMS Awareness Training is available through the Advanced Distributed Learning Service (ADLS) website: https://golearn.adls.af.mil/kc/rso/login/ADLS_login.asp. EMS Awareness Training is located under the miscellaneous tab. This option is only available to contractors who have a CAC and takes approximately 10 minutes to complete.

4.7.3 Option 2: Contractor requests a copy of the Robins EMS Awareness Training Power Point Presentation from Environmental Management. The request should be sent to the Environment Management Workflow Box at the following email address, 78ceg.cev.FrontOfc@us.af.mil. Once the presentation is received, the contractor is responsible for ensuring that all his/her employees view the training.

4.8 Essential Services During Crisis. The Functional Commander/Director (FC/FD) has determined, by placing in writing their decision, that this requirement is not Mission Essential (M-E) in accordance with DFARS 237.76, Continuation of Essential Contractor Services.
