Continuation of block 14. DESCRIPTION OF AMENDMENT/MODIFICATION
The purpose of this amendment is to:
1). Extend the due date for the Receipt of Offers as follows:

2). Replace the following Attachments in their entirety as incorporated herein:
Attachment 2 Bailment Agreement Attachment 4 Field Assessment Questionaire Purchase Description (PD) Statement of Work (SOW) CDRL ELIN A005 Instructional Media Package
3). Paragraph II.A.2 of the Proposal Requirements provision on page 70 of the solicitation document is hereby replaced in its entirety as follows:
"Print shall be no smaller than a font size of 12, with exception to charts, graphs, drawings, diagrams, supporting illustrations, or spreadsheets, which shall be no smaller than a font size of

Paper size shall be 8 1/2” X 11,” excluding any drawings, diagrams, and/or supporting illustrations, spreadsheets, etc. Charts, graphs, drawings, diagrams, supporting illustrations, or spreadsheets shall not be greater than 11” x 17.” Each volume within the proposal notebook shall be separately tabbed and identified. Margins shall be no smaller than 1 inch and each page within a volume shall be numbered consecutively. The page limitations specified include any drawings, diagrams, and illustrations. Offerors are cautioned that any pages that exceed the limitations shall not be read and shall be removed and sent back to the offeror. Elaborate formats, bindings, color presentations are not desired or required. In order to verify the font size and margins utilized, the offeror shall provide an electronic copy of the technical proposal in Word format."
4). Paragraph II.C.2.ii of the Proposal Requirements provision on page 72 of the solicitation document regarding Technical Subfactor One: Field Evaluation, is hereby replaced in its entirety as follows:
"In accordance with the completed/signed Bailment Agreement, the offeror shall provide four (4 EA) of its existing commercial SCBA units for evaluation and assessment once a date/time has been established by the AFLCMC/WNKAC contracting office. A date/time for receipt of the SCBA units to be delivered to the two initial evaluation locations (as outlined in the Bailment Agreement) will be established after proposals are received. The four SCBA units shall be the same model and shall represent the base unit that the offeror intends to provide on the contract effort should it receive the contract award. Additionally, the SCBA units shall meet NFPA standard 1981 and 1982 (Current Effective Edition as of the issuance date of the RFP). The offeror shall ensure that the SCBA units comply with all relevant portions of section 3 (3.4.1, 3.4.2.1, 3.4.2.3, 3.4.2.4, 3.4.2.6, 3.4.3, 3.4.4, 3.4.5, 3.4.6, 3.5 (to include all subparagraphs under 3.5), 3.6 (to include all subparagraphs under 3.6), 3.7.1, 3.7.2, 3.7.3, 3.7.5, 3.7.6, 3.9, 3.10, 3.11, Amendment/Modification FA8534-20-R-0006-0002
3.12, 3.13, 3.14, 3.15, 3.16, 3.17, 3.18, 3.19, 3.20, and 3.21) in the Purchase Description (excluding any CWC or SAR specific requirements). Systems shall be complete units in that all the accessories that are used with the unit are included, along with the units’ standard commercial operations and maintenance manuals. Offerors shall provide one (1) each mask for each size with every SCBA unit that will be submitted for evaluation; i.e. for an offeror that has three (3) mask sizes (small, medium, large), one (1) each small, one (1) each medium, and one
(1) each large masks shall be submitted for SCBA #1; one (1) each small, one (1) each medium, and one (1) each large masks shall be submitted for SCBA #2 and so forth. (The SCBA units will be returned to the offeror as outlined in the Bailment Agreement.)"
5). All other terms and conditions remain unchanged.
