STATEMENT OF WORK
FOR
PROJECT #
REPLACE WYNN DINING FACILITY (DFAC) EAST, MIDDLE and WEST DINING ROOM FLOORING, BLDG 768
1. GENERAL: The contractor shall provide all labor, equipment, transportation, and supervision for the following:
2. PERIOD OF PERFORMANCE: (3 days per Clin)
a. Hours of Work: Work to be accomplished from 0800 – 1700 hrs. Mon-Sat.
THREE OPTION CLINS, SCOPE OF WORK for each CLIN:
· To remove and replace furnishing’s as needed, to accomplish removal and installation of flooring. Furnishing’s can be placed on the back patio area and/or back dock area.
· Remove and dispose of existing carpet tile
· Prepare sub floor with leveling compound
· Provide and Install 20 MIL commercial grade Luxury Vinyl Planks (LVP) or Luxury Vinyl Tile (LVT) and 4.5” Rubber Cove Base (Black). Color of the LVP or LVT will be the same or equivalent to the Mohawk Premium Wood 330 Almond color

3. MEASUREMENTS: The contractor shall confirm all measurements to ensure all materials will fit.
4. COORDINATION: The Contractor shall provide 30-day notification prior to the start of project construction.
5. CONTINUED OCCUPANCY: The DFAC shall remain in operation during the entire duration of the project. The contractor will be given access to the project area.
6. SECURITY REQUIREMENTS
A. Contractor will provide a list of installers to Wynn Dining Facility at least 2 weeks prior to installation so that base passes can be obtained.
B. The Contractor and contractor employees shall comply with all base traffic regulations.
C. Weapons, Firearms and Ammunition are prohibited from possessing on themselves or within their contracted privately owned vehicles while on base.
D. The Contractor shall be responsible for safeguarding all government property, and controlled forms provided for contractor use.
E. At the end of each work period Contractor shall secure all Government equipment and materials.
7. SAFETY
A. The Contractor shall have a safety plan for their employees. The Contractor shall instruct each employee in the recognition and avoidance of unsafe conditions and regulations to his or her work environment to control or eliminate any hazards or other exposure to illness or injury. The Contractor shall provide their employees appropriate Personal Protective Equipment (PPE) and mandate proper wear.
B. Accident/Incident Reporting and Investigation. Contractor shall record and report to DFAC management all available facts relating to each instance of accidental damage to Government property or injury to either Contractor or Government personnel. The Contractor shall secure the accident scene and wreckage until released by the accident investigative authority through the center POC. If the Government elects to investigate the incident, the Contractor shall cooperate fully and assist the Government personnel until the investigation is completed.
8. DISPOSAL OF REMOVED MATERIAL: The Contractor shall provide a dumpster for all removed materials. It shall be placed on the East side of the building as directed by the project manager. Its proper removal and disposal shall be the responsibility of the Contractor.
9. SITE MAINTENANCE, CLEAN UP, AND RESTORATION
A. Maintain the work site in a neat, orderly, and safe manner.
B. Remove scrap, waste, and excess materials promptly. Provide signs, barricades, and lights as required to protect base personnel.
C. Do not allow trash and debris to accumulate and become unsightly. Sweep up and collect in contractor-maintained disposal containers daily. Dispose of collected debris weekly as a minimum.
D. Restore the project site to its final condition as required by the contract as soon as possible.
10. GOVERNMENT FURNISHED PROPERTY: Will not be provided in this project.
11. OTHER REQUIREMENTS
A. Smoking is prohibited in all facilities. Smoking outside of facilities/buildings must be in approved areas.
B. Conform to current NFPA and RAFB Fire Protection Codes.
C. Conform to current NEC and Robins base facility standards.
D. Parking of contractor’s vehicles will be in the East side of the South parking lot.
12. GOVERNMENT REPRESENTATIVES:
A. The contractor shall not make any changes to or deviate from the requirements listed in the SOW unless approved by the Contracting Officer.
B. Contractor and DFAC Management will conduct a joint quality assurance inspection of installation prior to completion of project.
