DESCRIPTION OF SERVICES

The contractor shall provide all management, facilities, tools, supplies, equipment, and labor necessary to remove, refurbish, test, deliver and reinstall two (2) Servo Component Test Systems (SCTS), National Stock Number (NSN) 4920-01-272-0071. The upgraded SCTS systems shall be of the same configuration and configured as identified in Table 1 and Table 2 unless otherwise specified herein. The contractor shall also provide one (1) Hydraulic Power Module (HPM), identified in Table 3, to support the two upgraded SCTS. The contractor shall provide associated technical data, software, tooling, material, and training as specified herein.

Each refurbished SCTS shall test all assets per their respective governing technical data and associated Test Program Sets (TPS) as described in Table 4. Additional design requirements or constraints are further outlined herein.

BASIC SERVICES / BASIC DESIGN REQUIREMENTS

Any components replaced or added for the refurbished SCTS shall be new. To the extent possible, the refurbished SCTS shall incorporate Commercial-Off-The-Shelf (COTS) components and software.

All components requiring scheduled maintenance or calibration shall be easily accessible for adjustment, repair, or replacement.

All mechanisms requiring lubrication including gears, slide ways, and bearings (except sealed-for-life type) shall be provided with means for lubrication. All oil holes, grease fittings, and filler caps shall be easily accessible.

ELECTRONIC TEST CONSOLE (ETC) UPGRADES

Although the AAI/ACL SCTS listed in Table 1 and Table 2 have different Part Numbers, these machine are built and configured identically. The refurbished SCTS ETCs shall be of the same configuration and configured as AAI/ACL SCTS Serial Number 5051-001 identified in Table 1.

The new ETCs shall include the same functionality of all TPS listed in Table 4 as well as the TPS listed in Table 5.

Each TSP in table 4 shall include an auto-save function to save all test results automatically. Saved tesults shall be stored in folders accessible to the user.

The new ETC will include a digital module that will duplicate the existing digital (PC) module assembly (P/N 1391-0126).

All software will be hosted on the 64-bit version of the Windows 7 Operating System or higher, including software programs Manual Mode II Software All existing Units Under Test (UUT) TPS and manual mode programs shall be converted to run on the new digital module assembly. Converting existing TPS programs:

Capture and determine the existing TPS files, set up validation procedures, back up all existing data, verify configuration control, interview operators, and create tools for import of TPS software to the new system.

Refurbishment will include verification and validation of the following:

All upgrades to the Hydraulic Test Console (HTC).

The conversion of the existing four (4) UUT TPS P/Ns (8500-0162, 8500-0163, 8500-0165, 8500-0166) to run on the upgraded ETC Windows operating system.

The digital module hardware will include the functional interface with the new signal conditioning module and existing hydraulic console hardware.

The new digital module will be installed within the new ETC cabinet assembly.

The following items shall be included as part of the digital module upgrade:

Laser printer capable of printing graphs at least single sided in black and white.

Keyboard 19" touchscreen monitor CPU – Intel 2 Core, 2.66 GHz, 4 Gb RAM Hard Drive – 2X 500 Gb ADC – 16-bit resolution analog to digital converter, 100 KHz sampling rate, and programmable gains Signal Generator – Arbitrary waveform generator and programmable offset and gain Digital I/O – 72 channels, buffered TTL Operating System – Windows 7 or higher

The Contractor shall provide with the refurbished SCTS software containing all utilities, drivers, libraries, and applications necessary to run the TPS listed in Tables 4 and 5. The Contractor shall supply software code for all TPS listed in Tables 4 and 5.

The refurbished SCTS shall maintain the Government’s current capability to back up and load any TPS software TPS and Operational Test Program Set (OTPS).

The software shall be the latest released (stable) version of a windows based commercial application package commonly used for data acquisition, instrument control, and industrial automation.

Each OTPS and TPS shall include and display a Computer Program Identification Number (CPIN) assigned by the Government in compliance with United States Air Force (USAF) Automated Computer Program Identification Numbering System (ACPINS) outlined in T.O. 00-5-16. CDRL A004/DI-MISC-81454C

HYDRAULIC TEST CONSOLE (HTC) UPGRADES

The refurbished SCTS HTCs shall be of the same configuration and configured as AAI/ACL SCTS including the following:

The refurbished SCTS HTCs shall include all new directional control valves. The pneumatic positioners and all air lines and fittings shall be new.

The refurbished SCTS HTCs shall include all new pressure control valves.

The refurbished SCTS HTCs shall include new manual and solenoid operated shut-off valves.

The refurbished SCTS HTCs shall include all new pressure and temperature measurement components.

The refurbished SCTS HTCs shall include a new sump pump.

HYDRAULIC POWER MODULE (HPM)

The Hydraulic Power Module shall be configured as follows:

The HPM including the electrical cabinet shall have a footprint of 60” wide x 78” long x 82” high.

HPM shall have two independent pumps each rated for 20 gpm at 4000 psi (Total of 40 gpm at 4000 psi).

HPM shall include a remote controls module that will allow the HPM system 1 and system 2 to be operated independently.

Existing Hill AFB Facility Capabilities:

Power: 480VAC, 3ph, 60hz, 250Amp Cooling Water: 50GPM, 50 deg F, 40 psi minimum Hydraulic Fluid: MIL-PRF-83282 150 gallons

IDENTIFICATION

All identification plates shall be replaced. All markings on identification plates shall be permanent and legible on a contrasting non-glare background.

New updated corrosion-resistant nameplate shall be securely attached to the refurbished SCTS. The nameplate shall contain the information listed below:

a. Noun

b. Manufacturer’s name

c. Manufacturer’s serial number

d. Manufacturer’s model number

e. National Stock Number (NSN)

f. Power input (volts total amperage phases frequency)

g. Contract number or order number

h. Date of manufacture (day month and year) The test stand shall contain a Unique Identification (UID) marking IAW MIL-STD-130N. The Contractor shall maintain the IUID tags (markings) IAW MIL-STD-130N, dated 16 November 2012, contract clauses, DFARS (252.211-7003, 252.211-7007), and the Contractor's IUID Marking Plan. The Contractor shall report using the IUID registry, located on the internet at http://www.acq.osd.mil/dpap/pdi/uid/data_submission_information.html. The IUID Marking Plan and registry shall be approved before delivery of refurbished test stands. The Contractor is responsible for identifying new candidate parts for inclusion in the IUID program. CDRL A010/ DI-MGMT-81803 and A011/ DI-MGMT-81804.

MANAGEMENT PLAN

The contractor shall provide a management plan to the Program Engineer (PE) and Contracting Officer Representative (COR) within 21 calendar days of contract award IAW with Contract Data Requirements List (CDRL) A009/DI-MGMT-80004A. The purpose of this plan is to ensure compliance with customer requirements and applicable industry standards, and safety. The management plan shall include the following: identification of all Government PWS requirements, CDRLs, applicable Data Item Descriptions (DID(s)), and Government standards.

The management plan shall include a schedule. The schedule shall include key milestone meetings for kick-off, In-Process Review (IPR) of technical data package, factory test, delivery, and acceptance. CDRL A009/DI MGMT 80004A The management plan shall identify project team members, communication pathways between the Government and contractor, and how contractor status will be reported. CDRL A009/DI-MGMT-80004A The management plan shall include a risk analysis and mitigation plans to minimize impact of identified risks to project quality, schedule, and budget.

The contractor shall provide any revision of schedule, if required, no later than 10 business days from PE and/or COR written notification. CDRL A009/DI-MGMT-80004A Safety Program: Establish a complete Contractor Safety Program to ensure the requirements of this contract are provided as specified in appendix C. The Contractor shall ensure safety is maintained throughout the life of the contract, and shall establish and provide a contractor’s Safety Plan. The plan shall meet requirements of Appendix C. The contractor shall make appropriate corrections and modifications (at no additional costs to the Government) and obtain acceptance of the plan by the contracting officer, through coordination with OO-ALC Safety office, before the start of the performance period. (CDRL A013/DI-SAFT-81626 and A014/DI-SAFT-81563).

OSHA and EPA requirements are the sole responsibility of the contractor, however if the government observes the contractor in violation of these regulations, they may stop the contractor , if there is imminent danger to Air Force personnel or AF property, until such times the contractor mitigates the hazard or deficiency.

TECHNICAL DATA DELIVERABLES

The contractor shall provide a technical data package as stated in Table 6 and further explained below:

CALIBRATION

The Contractor shall ensure the HPM and SCTS calibration is verified in accordance with (IAW) AFMETCAL requirements. Calibration and measurement traceability shall be identified and documented IAW MIL-STD-1839D. MIL-HDBK-1839B will be used for interpretive guidance to clarify requirements invoked by MIL-STD-1839D. The Contractor shall provide a means to perform calibration to meet the following requirements: CDRL A001/DI-QCIC-80278C and A003/T-86-01Q The Contractor shall provide, with the PCU test console, all software and special hardware required to perform calibration and calibration verification.

The Contractor shall develop and deliver a system calibration procedure verifying system performance to the performance specification contained within this PD (commercial/contractor format).

The contractor shall ensure AFMETCAL and PMEL are able to provide a system certification per AFMETCAL requirements IAW with AFTO Form 108 Technical Measurement and Diagnostic Equipment (TMDE) that allow use to test overhauled assets for field use after delivery to Hill AFB.

The contractor shall provide a manufacturer calibration certificate for the consoles that will be accepted by AFMETCAL (commercial/contractor format). CDRL A002/DI-QCIC-80798C Final Calibration shall be conduct at Hill AFB in the presence of 2 PMEL technicians for training purposes.

TECHNICAL MANUALS

The contractor shall provide HPM and SCTS technical manual for operation and maintenance, with illustrated parts breakdown and IAW CDRL# A012/TM-86-01Q. CDRL A012 is governed by TM-86-01Q, tailored for the refurbished SCTS.

The contractor shall prepare the technical manuals in a manner that is consistent with Air Force Technical Orders (T.O.). There shall only be one T.O. assigned for both refurbished SCTS.

The contractor shall provide backup and recovery procedures for system software. The contractor shall also provide procedures on data acquisition hardware setup, including Field Programmable Gate Array (FPGA) programming necessary for system operation. CDRL A005/ DI-IPSC-81488 and A012/TM-86-01Q The contractor shall submit a final draft thirty (30) days before delivery of upgraded stands and final edition of technical manuals within ten (10) days after receipt of Government review of initial submission. CDRL A012/TM-86-01Q

ACCEPTANCE TEST PROCEDURES AND REPORT

The contractor shall submit TPS and OTPS, Acceptance Test Procedures (ATPs) and Acceptance Test Report (ATR) IAW CDRLs A006/DI-ATTS-80282B and A007/DI-ATTS-80283B/T.

The contractor shall develop OTPS, and ATP IAW CDRL A006/DI-ATTS-80282B to establish the criteria for test stands’ acceptance. The ATPs shall be submitted fifteen (15) calendar days before factory test of upgraded stands. The ATP shall include a Functional Configuration Audit (FCA) and Physical Configuration Audit (PCA) outline along with TO verification.

The FCA outline shall ensure the upgraded test stand performs as intended without a UUT.

The contractor shall insert non-destructive faults into the test system for self-test verification. The FCA outline shall include a Fault Candidate List (FCL) for the Test Set and any Test Set Adapters. The FCL shall consist of 15 faults for the console and any test adapters. Each FCL shall describe how they shall be inserted and what the anticipated outcome is. Destructive faults shall not be included on the FCL. The FCL shall be provided in contractor format.

The PCA outline shall ensure the upgraded test stand is configured as described in the PWS and supplied Technical Data Package required in Table 6.

The ATP shall refer to the TO(s) associated with Government furnished assets for UUT testing and provide for documentation of test results.

The contractor shall provide a preliminary ATR IAW CDRL A007/DI-ATTS-80283B/T documenting results of the contractor’s acceptance testing at contractor’s facility per the ATP submitted IAW CDRL #A006/DI-ATTS-80282B fifteen (15) calendar days before delivery of upgraded stands. PE and/or COR will approve ATR prior to shipment of upgraded stands. The contractor shall submit the final ATR within 15 calendar days after completion of acceptance tests at Hill AFB. If changes are required, the contractor shall submit a subsequent ATR revision within fifteen (15) calendar days of receipt of Government comments.

SOFTWARE DATA

The contractor shall submit computer software IAW CDRL A005/DI-IPSC-81488 properly identified for Automated Computer Program Identification Numbering System (ACPINS) IAW CDRL A004/DI-MISC-81454C thirty (30) days before delivery of upgraded stands.

The contractor shall provide source and executable codes for the refurbished SCTS, including any translators, operating systems, runtime, and development environment software in support of TPS listed in Table 4 and Table 5. CDRL A005/DI-IPSC-81488 The contractor shall provide licensing for all software used to develop the system codes and run system programs for the test stands. CDRL A005/ DI-IPSC-81488 The contractor shall comply with T.O. 00-5-16 when implementing or revising any software code in the refurbished SCTS. If changes are required, final delivery shall be submitted thirty (30) days after receipt of Government comments. Any changes shall be accompanied with properly labeled media identified by a new or revised CPIN, visible in the software program itself, assigned by the government.

The Government will issue CPINs for all TPS.

Information Assurance and Malicious Logic Protection.

The COR shall be notified upon discovery of any virus or malware associated with the equipment being procured.

The contractor shall ensure incoming and outgoing non-volatile storage devices, via utilization of human and technological preventative measures such as good cyber hygiene and virus scanning, are free of malicious logic.

DATA RIGHTS

The contractor shall provide government purpose rights to all technical data developed in any part for this PWS and requested herein. The distribution of this data shall be as stated in the CDRLs, Table 6.

MSDS

The contractor shall submit Material Safety Data Sheets IAW CDRL A008/FED-STD-313D fifteen (15) days before upgraded stands delivery if any hazardous materials are present upon delivery.

DELIVERY AND ACCEPTANCE

The stands shall be upgraded in two phases. In phase one, SCTS Serial Number 5051-001 shall be upgraded and delivered within fifty-three (53) weeks of contract award IAW the PWS. The Hydraulic Power Module will be delivered at time of installation of the first refurbished unit included in phase one. In phase two, SCTS Serial Number 4978-002 shall be upgraded and delivered within thirty (30) weeks of phase one acceptance IAW the PWS. Phase two stands shall not be upgraded until after phase one stands have been accepted by Government.

Prior to installation of phase one stands at Hill AFB, the Contractor shall host Government personnel at their facility in order to demonstrate testing capability using the OTPS and ATP developed IAW CDRLs A006/DI-ATTS-80282B and A007/DI-ATTS-80283B/T. The Contractor shall document the results of testing in a preliminary ATR IAW CDRL A007/DI-ATTS-80283B/T. Any deficiencies identified at the factory test shall be corrected prior to shipping the test stand to Hill AFB.

The stands shall be upgraded at the contractor’s facility. The contractor shall be responsible for packaging and shipment of stands to and from their facility. The contractor shall be responsible for all shipping costs and held liable for Freight On Board (FOB) destination.

The upgraded stands shall be shipped to Flight Controls Shop, 6045 Dogwood Ave, Building 1266, Hill Air Force Base, UT 84056. The contractor shall be responsible for installation and setup of stands at Hill AFB.

The contractor shall provide eight (8) total hours of On the Job Training (OJT) for a minimum of 4 technicians but not to exceed 10 technicians on the new systems at HAFB of Government technician, maintenance and engineering personnel for operation, maintenance and repair functions associated with test stands. The OJT shall include software / hardware functionality, fault diagnosis, and specific maintenance requirements and is required only for test stands upgraded in phase one.

The contractor shall conduct acceptance testing with Government personnel at Hill AFB for phase 1 stands. The contractor shall ensure the test stands meet the criteria outlined in the ATP (see paragraph 2.3) within seventy-five (75) days from date of delivery. If the contractor fails to meet all criteria within this timeline the Government will require the stands be shipped back to the contractor’s facility and the process repeated at contractor’s expense. The Government shall conduct acceptance testing for phase 2 stands. The following applies to acceptance testing:

The Government, at its discretion and coordinated with the contractor, may visit the contractor’s facility before shipment to Hill AFB to assist preparation for factory acceptance testing.

The contractor shall ensure a COR can successfully follow the ATP as submitted per CDRL A006/DI-ATTS-80282B and pass government furnished field acceptable servo valves, as confirmed by supplied test results, per TO requirements.

The contractor shall submit the final ATR IAW CDRL A007/DI-ATTS-80283B/T to document the results of acceptance testing per the ATP submitted IAW CDRL A006/DI-ATTS-80282B.

The ATR shall document the test results of each asset as identified in Table 8, tested per their applicable TOs for each stand.

The contractor shall submit any necessary technical data package revisions before completion of acceptance testing. CDRLS A001/DI-QCIC-80278C, A002/ DI-QCIC-80798C, , A004/DI-MISC-81454C, A005/DI-IPSC-81488, A006/DI-ATTS-80283B, A007/DI-ATTS-80283B/T, A008/FED-STD-313D, andA012/TM-86-01Q.

The contractor shall ship HPM to HAFB, verify installation, start-up and train on the new system at HAFB within thirty-four (34) weeks of contract award.

GENERAL INFORMATION

COTS PRODUCTS

The contractor shall use Commercial off the Shelf (COTS) products to the fullest extent possible consistent with applicable Air Force Technical Data (AFTD).

GOVERNMENT FURNISHED PROPERTY (GFP)

In addition to the test stands referenced in Table 1, the Government will provide a minimum of one (1) each of the equipment outlined in Table 8 consistent with USAF policies at time of contract award.

The following further defines government requirements in regards to provided property listed in Table 8.

The contractor shall be responsible for all shipping costs of GFP back to the government and shall be held liable for FOB destination.

The contractor shall be responsible to return all GFP to the government immediately following successful testing in the same condition as it was provided, as indicated by the condition tag attached to each asset. Failure to do so represents a contract failure and will be noted as such in contract performance evaluation. If necessary, time extensions of GFP can be negotiated by the contractor with the government. The contractor shall be required to reimburse the Government the current stock list price of any asset that is not returned in the condition it was received.

PUBLICATIONS

Compliance with all publications, regulations and operating instructions listed in this PWS is required, including MIL-PRF-83282D and T.O. 35M27-3-13-1.

TECHNICAL ORDERS (TOs): The Contractor shall provide a current DD Form 2345 and must comply with AFI 61-201 (Management of Scientific and Technical Information (STINFO) and verify the DD Form 2345 through the Joint Certification Program http://www.dla.mil/HQ/InformationOperations/Offers/Products/LogisticsApplications/JCP.aspx Once verified the Contractor shall receive access to all TO excerpts with distribution statements “B” and “D” during contract solicitation following a request made with DD Form 2345 through the Contracting Officer..

REPORTING OF CONTRACT LABOR HOURS

The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the refurbished SCTS via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.

Reporting inputs shall be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. The contractors may direct questions to the Contractor Manpower Reporting Application CMRA help desk at http://www.ecmra.mil.

Reporting Period: The contractors are required to input data by 31 October of each year.

Uses and Safeguarding of Information: Information from the secure web site is considered proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.

SECURITY REQUIREMENTS

The contractors coming onsite once or twice within the year will only require a one day visitor pass or weekly pass. The contractor shall provide the COR 2 (two) weeks advance notice of scheduled visit. The sponsor is authorized to arrange call to Pass and Identification (ID) office morning of visit. Call on same day of visit will be accepted. Call made a day ahead will not be accepted. Pass and ID office is located near Southgate Entrance, Phone: 801-777-8631 Office Hours: 0700-1630. The contractor shall be a U.S. Citizen. Proof of ID required: Driver’s license. If driving on base, the contractor shall supply Gate Security with proof of drivers’ insurance and registration. The sponsor will be responsible to escort the contractor in and out of controlled area.

TRAVEL REQUIREMENTS

The contractor shall be responsible for all contractors’ travel cost associated with this contract to support the objectives of this PWS. The government will be responsible for the travel of all government personnel travel requirement associated in support of the objective of this PWS.

SECTION I - GENERAL REQUIREMENTS

A. Safety Program Requirements.

The contractor will implement a safety program plan that ensures protection of Government personnel and property. As part of the Contractor Safety and Health Program, the contractor shall assign in their plan, by name and phone number, a person who will be the primary point of contact for safety and health issues for the on-site operation. The program will consist of, as a minimum:

1. Mishap reporting, as defined in paragraph B1 below.

2. A Safety Plan that addresses, as a minimum, the subjects listed in Section II –Specific Requirements, and will be used during the performance of the work described in the contract. The Safety Plan will be accepted by OO-ALC/SE (Safety Office) prior to commencement of any work described in this contract.

3. Routine and recurring surveillance to ensure the safety requirements of this contract are enforced.

4. Competent personnel to provide surveillance of the Safety Plan.

5. Identification of segregated work site locations for operations that cannot be co-mingled with general industrial operations and the process for ACO approval of operations and changes at these specific sites.

6. All contractor personnel shall be trained and qualified to perform their duties safely.

7. The contractor shall include a clause in all subcontracts requiring the subcontractors to comply with the safety provisions of this contract, as applicable.

B. Mishap Notification

1. For operations outside of Hill AFB contractor shall comply with local military installation mishap reporting procedures as well as those listed below.

1.1 The contractor shall notify OO-ALC/SE (801-586-6038) or the Hill AFB Command Post (801-777-3007) after normal duty hours, and the designated Government Representative (GR), i.e., the ACO, PCO, or DCMA COR (Contracting Officer's Representative) within one (1) hour of all mishaps or incidents at or exceeding $2,000 (material + labor) in damage to DOD property entrusted by this contract, even if the government is wholly or partially reimbursed. This notification requirement shall also include physiological mishaps/incidents. A written or email copy of the mishap/incident notification shall be sent within three calendar days to the GR, who will forward it to OO-ALC/SE. For information not available at the time of initial notification, the contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the ACO.

Mishap notifications shall contain, as a minimum, the following information:

(a) Contract, Contract Number, Name and Title of Person(s) Reporting

(b) Date, Time and exact location of accident/incident

(c) Brief Narrative of accident/incident (Events leading to accident/incident)

(d) Cause of accident/incident, if known

(e) Estimated cost of accident/incident (material and labor to repair/replace)

(f) Nomenclature of equipment and personnel involved in accident/incident

(g) Corrective actions (taken or proposed)

(h) Other pertinent information

2. The contractor shall cooperate with any and all government mishap investigations. Additionally if requested by government personnel or designated government representative (GR), i.e., the ACO, PCO, or DCMA COR, the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by safety investigators.

3. The contractor shall provide copies of contractor data related to mishaps, such as contractor analyses, test reports, summaries of investigations, etc. as necessary to support the government investigation.

4. The contractor shall support and comply with the safety investigation and reporting requirements of AFI 91-204, Chapters 1 – 5.

C. General Safety Requirements:

The contractor is solely responsible for compliance with all federal, state and local laws, the Occupational, Safety and Health Act (OSHA) (Public Law 91-596) and the resulting standards, OSHA Standards 29 CFR 1910 and 1926, as applicable, and the protection of their employees. Additionally, the contractor is responsible for the safety and health of all subcontractor employees. All documents required as a result of OSHA 29 CFR 1910 and 1926 regulations, (i.e. certifications, training, respiratory protection program, workplace hazard assessments), shall be made available to the COR upon request.
The contractor shall ensure assigned personnel are adequately trained and qualified for the task being performed. Brief all personnel on the hazards involved with operations and applicable precautions to be taken. Should unidentified hazards arise, cease operations until actions are taken to eliminate or mitigate hazards to safe levels.
Hazard Assessment. In accordance with 1910.132(d)(2), the contractor shall conduct a certified Hazard assessment on all work areas. A follow-on survey shall be conducted by the contractor when the workload or environment conditions change. If respirators are necessary to protect the health of the employee or whenever respirators are required by the contractor, the contractor employer shall establish and implement a written respiratory protection program according to 29CFR 1910.134.

Contractor personnel must receive Explosive Safety Awareness Training if their duties require entry into the Explosive Clear Zone. This training will be provided by the OO-ALC Safety Office. Contact Eric Covington at 586-0168.

A Risk Assessment will be required prior to any work being accomplished, if munitions will remain in the facility.

Compliance with OSHA and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor. Note: The government shall assume no liability or responsibility for the contractor's compliance or non-compliance with such requirements. The contractor shall furnish to each of his/her employees a place of employment, which is free from recognized hazards. The contractor shall brief his/her employees on the safety requirements of this contract and on hazards associated with prescribed tasks. The contractor is responsible for compliance with OSHA Public Law and the resultant standards identified within. In addition, the contractor is required to flow down the safety requirements/specification to all subcontractors. This applies to Federal Acquisition Regulation (FAR) 12 commercial acquisitions as well. This contract shall in no way require persons to work in surroundings or under working conditions which are unsafe or dangerous to their health. The contractor must coordinate and perform work so as not to impact the safety of government employees or cause damage to government property. This requires providing personnel with protective equipment and associated safety equipment as may be necessary. The contractor must also protect personnel from hazards generated by the work. If the contractor employs BILINGUAL speaking employees, they must post bilingual signs and have written procedures for specific tasks in applicable languages.

SECTION II – SPECIFIC REQUIREMENTS

The contractor’s prepared Safety Plan shall:

· Demonstrate a management commitment to employee safety and health

· Identify applicable rules and regulations

· Identify the roles and responsibilities of Management, Supervisors, Employees and Safety Coordinator

· Identify work to be performed and location of expected operations

· Provide a description of safety program, safety monitoring responsibilities, organizational structure, and contact information for on-site personnel

· Include a work hazard analysis of the worksite and operations to be performed to include baseline hazard identification and required control measures

· Identify employee safety and health training requirements and the documentation process

· Include emergency response plans and procedures that relate to protection of government personnel and property

· Include hazard reporting procedures and identify individual(s) responsible for the correcting identified hazards

· Identify procedures for accident reporting and investigation

· Identify the process for tracking controlled hazards in contractors work area

The contractor shall ensure that each element identified below is adequately addressed in detail in the safety and health plan:

PEDESTRIAN CROSSWALKS: All contractor personnel are required to use the closest crosswalk, or traffic controlled intersection when crossing the road. Pedestrians must look both ways to ensure the coast is clear before stepping out into the crosswalk. Pedestrians DO NOT have the right of way unless they are already in the crosswalk. Contractor vehicle operators have the same responsibilities as pedestrians, to share the road and mutually observe and yield to pedestrians.

MOTOR VEHICLES: Contractor shall comply with the standards in: DoD Directive 5525.4, Enforcement of State Traffic Laws on DoD Installations", Nov 2, 1981, Para 3-4; DODI 6055.4, DoD Traffic Safety Program, 20 Jul 99; AFI 91-207, USAF Traffic Safety Program, 22 May 07; and AFI 91-207 AFMC SUP1, The US Air Force Traffic Safety Program, 1 Oct 07. Each applies to all persons at any time on an Air Force Installation and includes all leased, owned, or privatized property including housing areas. In addition: AFI 13-213, Airfield Management, applies to all contractors, sub-contractors, vendors, commercial delivery companies, and all other private business vehicles who operate anywhere on Hill Air Force Base, including the airfield (to include the industrial areas and any buildings or hangars located upon the airfield) in support of their mission.

HOUSEKEEPING: Housekeeping shall be conducted according to the requirements in OSHA Standard 29 CFR 1910.141. CLEAN AS YOU GO will be enforced.

The authorized Government Representative (OO-ALC Safety Office) shall accept/reject the contractor’s procedures within the Health and Safety Plan as meeting the intent of the below referenced standards, with the exception of Mishap Notification / Reporting Procedures, which are outlined in Section I, paragraph B of this Appendix C.
