PERFORMANCE-BASED WORK STATEMENT (PWS)
FOR
MAINTENANCE SERVICES
Service and Maintenance on ARL 3580 ICP Spectrometer
(809 MXSS/MXDEC)
20 DEC 2012

1.0 DESCRIPTION OF SERVICES:
1.1 Provide preventative maintenances and repair/service on one (1) ARL 3580 ICP Spectrometer that is over 20 years old, and is used to run all metal analysis for all hazard waste at Hill Air Force Base. Services will also include an unlimited number of on demand site service visits, and Factory certified replacement parts.
2. BASIC SERVICES:
2.1 The contractor shall repair one (1) ARL, Model 3580, Sequential Simultaneous ICP Spectrometer, and all equipment listed in Appendix B, to fully operational and serviceable condition, and NELAP certification standards, per OSHA required methods. Final inspection and acceptance will be performed by the user.
2.1.1 The contactor shall use only new, original equipment manufactured parts in making repairs.
2.1.2 All replaced parts will be returned to the Government Representative.
2.2 The contractor shall provide Two (2) Preventive Maintenance visits per year for the ARL, Model 3580, Sequential Simultaneous ICP Spectrometer and all equipment listed in Appendix B. Maintained items will be maintained to fully operational and serviceable condition, and conform to NELAP certification standards, per OSHA required methods. Final inspection and acceptance will be performed by the user. These visits shall be scheduled by the contractor, but coordinated with the user.
2.3 On-site service within 72 hours from the time contractor is contacted and it is determined that on-site services is necessary. Unlimited number of service calls.
2.3.1 The contractor shall provide the Government with a phone number for service calls.
2.3.2 This phone number shall have a low frequency of use and shall be monitored 10 hours a day BETWEEN 6:30 AM and 4:30 PM. Recording devices are acceptable if the user is notified within a 24 hour period.
2.4 Contractor shall provide reports on all maintenance, and repair within five (5) days of completion of work. (CDRL A001).

4. GOVERNMENT FURNISHED PROPERTY AND SERVICES
4.1 The government will provide necessary working space including heat, light, ventilation and electrical powered outlets for the use of contractors’ maintenance personnel. The Government shall provide access to the necessary facilities where the, equipment resides. The place of performance shall be on Hill AFB, UT.
4.2 All contractor personnel shall comply with the Air Force facility requirements for vehicle registrations and personnel identification badges that allow access to facilities to accomplish the tasks defined within this PWS.
4.3 Government telephones shall be used for official business only.
4.4 The contractor shall be responsible for safeguarding all Government property provided for contractor use.
5. GENERAL INFORMATION:
5.1 SPECIAL QUALIFICATIONS:
5.1.1 In performing the tasks required by the PWS, the contractor shall be required to efficiently and effectively interface with other contractors and Government personnel.
5.1.2 Contractor personnel shall present a neat appearance and be easily recognized. During summer months, contractor personnel shall be authorized to wear tee shirts. All clothing and safety equipment shall comply with Code of Federal Regulation (CFR) 1910 Occupational Safety and Health Act (OSHA) requirements.
5.1.3 Contractor shall be factory trained personnel with appropriate certifications.
5.1.4 Contractor shall assign employees who possess the certification in preventative maintenance inspection, corrective maintenance, and machine certification for all equipment listed in Appendix B.
| 5.2 | SAFETY REQUIREMENTS: |
| In performing work under this contract, the contractor shall: |
5.2.1 Conform to the safety requirements contained in this contract for all activities related to the accomplishment of the work.
5.2.2 Comply with additional, immediate precautions that the CO may require for safety and mishap prevention purposes.
5.2.3 Provide protection to government property to prevent damage during the period of time the property is under the control, or in possession of, the contractor.
5.2.4 Include a clause in all subcontracts to require subcontractors to comply with the safety provisions of this contract as applicable.
5.2.5 In the event of an accident or mishap, the contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the CO.
5.2.6 Record and report promptly (within one hour) to the CO and/or authorized GR, all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.
5.2.7 If the government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed.
5.3 HOURS OF OPERATION:
5.3.1 If work is to be completed at Hill AFB, the contractor is responsible for scheduling work hours with the GTR .
5.3.2 Contractor shall have access to perform services during normal working hours up to Force Protection Condition (FPCON) C. Normal working hours at Hill AFB are from 6:30 AM – 4:30 PM.
5.3.3 Unless otherwise required by the Government, the Contractor shall not have access to Government facilities to work on the following federal holidays: New Year’s Day, Martin Luther King’s Birthday, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans’ Day, Thanksgiving Day, and Christmas Day.
5.3.3.1 If the holiday falls on Sunday, it is observed on Monday. If the holiday falls on Saturday, it is observed on Friday. There may also be up to 6 AFMC Goal Days per year awarded to Government civilian employees as well as possible weather related days and government directed holidays. On these days, the Contractor shall not generally be required to work, and the Government shall not be billed for work not performed.
5.4 ENVIRONMENTAL CONTROLS:
5.4.1 Compliance with Laws and Regulations. The contractor shall be knowledgeable of and comply with all applicable Interstate, Federal, State, and Local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost as a result of the change, the contractor shall inform the Contracting Officer pursuant to notice requirements and negotiate a modification to the contract.
5.4.2. Notification of Environmental Spills. If the contractor spills or releases any substance contained in 40 CFR 302 into the environment, the contractor or its agent shall immediately call 911. If the call is made from an on base phone it will go directly to Hill AFB dispatch. If using a cell phone it will go off base, ask the 911 dispatcher to be transferred to Hill AFB. The liability for the spill or release of such substances rests solely with the contractor and its agent.
5.4.3. Material Storage and Use. If the contractor spills or releases any substance contained in 40 CFR 302 into the environment, the contractor or its agent shall immediately call 911. This includes hazardous materials.
5.6 PUBLICATIONS.
5.6.1 Compliance with all publications, regulations and operating instructions provided by the government is required when:
5.6.2 They pertain to the procedures for materials expediting herein and where the contractor is authorized by the performance-based statement of work to accomplish the work specified in the publication, regulation or operating instructions.
5.6.3. The publications prescribe USAF policies, use of materials, procedures and processes applicable to the work requirements.
5.6.4. The contractor is required to acquire and work on the latest version of the publication.
