(IAW FAR 12.603)
Solicitation Number: FA8126-21-Q-0029
Building 9001 Post E45 Acoustic Testing
This is a combined synopsis/solicitation for commercial items prepared in accordance with the formats in Subpart 12.6 and Subpart 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued.
The Air Force Sustainment Center (AFSC/PZIMB) at Tinker Air Force Base Oklahoma, intends to award a 100% Small Business Set-Aside, Firm Fixed-Price contract for the acquisition of commercial services listed below.
This solicitation is being issued as Request for Quote (RFQ), FA8126-21-Q-0029

Contractor is required to sign/date and fill in the pricing and delivery on page 2, complete the Unit Pricing on page 3 and the required provisions on pages 27 - 47, along with proof of technical evaluation criteria needed to fulfill this requirement. Contractor is required to provide commercial product literature for all services quoted. Please return 1 copy to issuing office. Contractor agrees to provide all products/services set forth or otherwise identified above and on any additional sheets subject to the terms and conditions specified herein.
The required testing and all reports must be completed between 1-16 April 2021.
Notice to Offeror(s)/Supplier(s): The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs.

EVALUATION OF RESPONSES:
Award will be conducted and evaluated under the provisions of FAR Part 12 Commercial Items and
FAR Part 13, Simplified Acquisition Procedures.
The responsible offerors must be registered in the System for Award Management (SAM) database (available at: https://www.sam.gov/portal/public/SAM/). Lack of registration in SAM will qualify contractor as ineligible for award.
Contract award will be made to the offeror who is technically acceptable with the lowest evaluated price.
Offerors must agree to complete all testing and reports required herein between 01 - 16 April 2021.
Failure of an offeror to agree to this completion date may render the quote technically unacceptable.

Deliveries or Performance
CONTRACT HOLIDAYS
(a.) The prices/costs in Section B of the contract holiday observances; accordingly, the Government will not be billed for such holidays, except when services are required by the Government and are actually performed on a holiday. Holidays in addition to those reflected in this contract, which are designated by the Government, will be billable provided the assigned Contractor employee was available for performance and was precluded from such performance.
(b.) The following days are contract holidays: New Years Day, Martin Luther King, Jr. Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day.
TRUCK ENTRY GATE NOTICE
IMPORTANT DELIVERY INFORMATION
All trucks making deliveries to Tinker Air Force Base must use the "Truck Entry Gate" (Gate 33), which is located at the corner of SE 59th Street and Air Depot Blvd (southwest corner of the base). The gate is open from 0500-1600 Monday-Friday.
Gate 33 shall be used for all deliveries by truck to this base. (TRUCKS WILL NOT BE ALLOWED TO GAIN
ACCESS THROUGH ANY OTHER GATE.)
Since your firm's delivery on this order may be affected, please advise your delivery carrier to use this gate. This should minimize the impact of your delivery.
DELIVERIES TO BUILDING 3001 (B/3001)
Be advised that delivery vehicles destined for B/3001 are prohibited from using "Staff Drive".
Delivery vehicles may make deliveries only in designated unloading areas.
Since your firm's delivery may be affected, ensure that your delivery carrier receives proper instructions on delivery/unloading requirements for B/3001. Unloading in designated unloading areas will minimize any inconvenience and attendant ramifications, which may accrue, to your delivery/delivery carrier for failure to adhere to this requirement.
INSURANCE
The following insurance amounts are required by FAR Clause 52.228-5 - Insurance Work on A Government Installation that is incorporated in full text in this solicitation.
(a) Workers' compensation and employer's liability. Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 shall be required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers. (See 28.305(c) for treatment of contracts subject to the Defense Base Act.)
(b) General liability.
(1) The contracting officer shall require bodily injury liability insurance coverage written on the comprehensive form of policy of at least $500,000 per occurrence.
(2) Property damage liability insurance shall be required only in special circumstances as determined by the agency.
(c) Automobile liability. The contracting officer shall require automobile liability insurance written on the comprehensive form of policy. The policy shall provide for bodily injury and property damage liability covering the operation of all automobiles used in connection with performing the contract. Policies covering automobiles operated in the United States shall provide coverage of at least $200,000 per person and $500,000 per occurrence for bodily injury and $20,000 per occurrence for property damage. The amount of liability coverage on other policies shall be commensurate with any legal requirements of the locality and sufficient to meet normal and customary claims.

WIDE AREA WORKFLOW PAYMENT INSTRUCTIONS (DEC 2018)
(a) As used in this clause- Definitions.
"Department of Defense Activity Address Code (DoDAAC)" is a six position code that uniquely identifies a unit, activity, or organization.
"Document type" means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).
"Local processing office (LPO)" is the office responsible for payment certification when payment certification is done external to the entitlement system.
"Payment request" and "receiving report" are defined in the clause at 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.
(b) The WAWF system provides the method to electronically process vendor payment requests and receiving reports, as Electronic invoicing.

(c) To access WAWF, the Contractor shall- WAWF access.

(d) The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice WAWF training.

(e) Document submissions may be via web entry, Electronic Data Interchange, or File Transfer WAWF methods of document submission.

(d) Comptroller General Examination of Record. If the solicitation includes the provision 52.222-18, Certification Regarding Knowledge of Child Labor for Listed End Products, or the equivalent at 52.212-3(i), the Contractor agrees to cooperate fully with authorized officials of the contracting agency, the Department of the Treasury, or the Department of Justice by providing reasonable access to records, documents, persons, or premises upon reasonable request by the authorized officials.
(c) Violations. The Government may impose remedies set forth in paragraph (d) for the following violations:
(1) The Contractor has submitted a false certification regarding knowledge of the use of forced or indentured child labor for listed end products.
(2) The Contractor has failed to cooperate, if required, in accordance with paragraph (b) of this clause, with an investigation of the use of forced or indentured child labor by an Inspector General, Attorney General, or the Secretary of the Treasury.
(3) The Contractor uses forced or indentured child labor in its mining, production, or manufacturing processes.
(4) The Contractor has furnished under the contract end products or components that have been mined, produced, or manufactured wholly or in part by forced or indentured child labor. (The Government will not pursue remedies at paragraph (d)(2) or paragraph (d)(3) of this clause unless sufficient evidence indicates that the Contractor knew of the violation.)
(d) Remedies.
(1) The Contracting Officer may terminate the contract.
(2) The suspending official may suspend the Contractor in accordance with procedures in FAR Subpart 9.4.
(3) The debarring official may debar the Contractor for a period not to exceed 3 years in accordance with the procedures in FAR Subpart 9.4.

(a) In performing work under this contract on a Government installation, the contractor shall:
(1) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and
(2) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes.
(b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract.

(a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.
(b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver s license, current vehicle registration, valid vehicle insurance certificate, and <<1>> to obtain a vehicle pass.
(c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.
(d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with <<2>> citing the appropriate paragraphs as applicable.
(e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office.
(f) Failure to comply with these requirements may result in withholding of final payment.

OWNERSHIP OR CONTROL OF OFFEROR (AUG 2020)
Definitions. As used in this provision–(a) means–Commercial and Government Entity (CAGE) code
An identifier assigned to entities located in the United States or its outlying areas by the Defense Logistics Agency (DLA) Commercial (1) and Government Entity (CAGE) Branch to identify a commercial or government entity by unique location; or
An identifier assigned by a member of the North Atlantic Treaty Organization (NATO) or by the NATO Support and Procurement (2) Agency (NSPA) to entities located outside the United States and its outlying areas that the DLA Commercial and Government Entity (CAGE) Branch records and maintains in the CAGE master file. This type of code is known as a NATO CAGE (NCAGE) code.
means the entity that owns or controls an immediate owner of the offeror, or that owns or controls one or more entities Highest-level owner that control an immediate owner of the offeror. No entity owns or exercises control of the highest level owner.
means an entity, other than the offeror, that has direct control of the offeror. Indicators of control include, but are not Immediate owner limited to, one or more of the following: ownership or interlocking management, identity of interests among family members, shared facilities and equipment, and the common use of employees.
The Offeror represents that it has or does not have an immediate owner. If the Offeror has more than one immediate owner (such as (b) a joint venture), then the Offeror shall respond to paragraph (c) and if applicable, paragraph (d) of this provision for each participant in the joint venture.
