* 9 step shutdown o We believe that this has been updated to 12 steps now, so we aren’t sure that the 9 steps that was written was correct

- Response: Yes, the template is correct as submitted since we are following AFI 91-203, chapter 21.

There are 9 steps in the LOTO process and 3 steps in the release to from LOTO process.

* Print front/back of LOTO procedures, don’t print lockseq on every page o Can we please clarify what you mean by "print front and back" but "don't include the shutdown process on the back of every procedure"?

- Response: I do not believe we used that verbiage. To clarify - In the case that you have a multi-paged LOTO instruction, each page should be double sided, but you only need to include the 9 Step process on the last page of that series.

* Procedures inspection sheet o We need to clarify that all we are doing is printing and laminating one copy of the blank inspection sheet and hanging it up with the rest of the procedure.

- Response: Yes.

* Multiple pages to be stapled in binder o Can the reference binder be single sided, no backside printed? (do you need to be printed out like the ones that will be hanging on the equipment)

- Response: The unlaminated copy to be stored at a management point should be printed double sided just like the copy which is laminated for the equipment. This should be easily accomplished by simply printing using the double sided option when printing the selected pages (tabs) of the workbook.

* Their approved adhesive o We need more information about this adhesive before we can quote. We need to know who is supplying the adhesive (it could cost more than our normal). We also need to understand what it is. If it's an epoxy or similar. Can we please find our more information first?

-Response: We typically use an RTV Silicone adhesive. Other adhesives may be approved. Contact our contract representative with any particular requests.

o Also to clarify, the stainless steel ties, we can use our existing crimp ties? Or do they have a specific stainless steel tie in mind?

-Response: Please use an unlockable, stainless steel ring / shower curtain pin / similar stainless steel assembly so they may be detached, utilized, or updated as necessary. You can search for "stainless steel shower curtain pin" for an example.

* Service report o We need more information. Everything bulleted is mandatory according to the statement. Some of this would not be applicable to a 100 LOTO project. We don't charge by labor hours. We could break it down, but it wouldn't be hours and rate, because it's a fixed cost. It also reference only using new parts and materials. We can do that, but that's more if we were installing actual equipment or motors, our "parts" are just tags, laminate, etc. Also what about the "equipment ID"? Do we need to provide a list of all equipment we worked with? Also we wouldn't be providing a written verification statement about the performance check because we aren't working on the equipment. We would need this Service Report section completely updated before we could accept these terms.

-Response: The PWS will be amended to address some of the concerns.

• Labor Hours – will not be needed as it is a firm fixed price.

• Equipment ID - answer will be items In Accordance With (IAW) Appendix A

• New Parts and Materials – changed.

• ADDED: Time in and Time Out
