STATEMENT OF WORK

(SOW)

B-52 CONNECT AND

CROWN STANDS

16 December 2015

1. INTRODUCTION

The 76th Aircraft Maintenance Squadron is contracting DAE Industries to manufacture and deliver two (2) complete sets of CONECT, Crown and Flight Deck Access Stands consisting of: Two (2) sets of Crown Skin Stands, two (2) left and two (2) right exactly like the most current version built and/or modified by DAE. Two sets of CONECT Stands, two (2) left and two (2) right exactly like the most current version built and/or modified by DAE. Two (2) Aluminum Flight Deck Access Stands. The existing stands are currently located in Building 2121 at Tinker AFB in the B-52 production line. These works stands are fabricated by DAE Industries and they hold the load and stress analysis testing and data results which will be used to procure the additional Connect Mod, Crown Skin, and Flight Deck Access Stands.

1.1. SCOPE OF WORK

The Contractor shall provide for the manufacture and delivery two (2) full sets of CONECT, Crown Skin and Flight Deck Stands exactly like the most current version built and/or modified by DAE.

The Contractor shall provide two (2) sets of Crown Skin Stands, two (2) left and two (2) right as per attached drawing (see Appendix B). Two sets of CONECT Stands, two (2) left and two (2) right as per attached drawing (see Appendix B). The labor and delivery of two (2) Aluminum Flight Deck Access Stands (see Appendix B). Only the hand rails will be assembled by user. All of the other parts and components will be pre-assembled. The addition of these stands will allow for ease of access and safety of movement in and around the B-52 aircraft while it is undergoing production maintenance. The B-52 Stand Sets shall maintain all minimum load ratings per the original design after the manufacture and installation of the Crown, Connect, and Flight Deck Access Stands.

Project management, manufacture, inspection, testing, documentation including drawings with parts list, user manuals, operation manuals, maintenance manuals, hoist certification, delivery, tools, assembly, labor, and other incidentals necessary to provide a complete installation. Contractor shall provide components, equipment, and parts that are commercial-off-the-shelf (COTS) when possible.

If COTS parts aren’t available, Contractor shall identify all components, equipment, and parts that are not COTS.

1.2. EQUIPMENT REQUIREMENTS

1.2.1. Quantity. The Contractor shall provide for the manufacture and delivery of two (2) sets of

Crown Stands (two left and two right), two (2) sets of Connect Stands (two left and two right), and two (2) Flight Deck Access Stands per the submitted drawing (see Appendix B).

1.2.2. Utilities. The utilities are to be installed as needed.

1.2.3. Supportability. These Stands will allow for ease of access and safety of movement in and around the B-52 aircraft while it is undergoing production maintenance. The Stands shall maintain all minimum load ratings per the original design after installation is complete.

1.2.4. Weather proof. The Stands must be weather proof and be capable of being stored outside in inclement weather.

1.3. CONTRACTOR REQUIREMENTS

1.3.1. Periods of Performance. Project management, manufacture, inspection, testing, documentation including drawings with parts list, user manuals, operation manuals, maintenance manuals, hoist certification, delivery, tools, assembly, labor, and other incidentals necessary to provide a complete installation shall be completed in no more than 120 calendar days after contract award.

1.3.2. Delivery. Contractor is fully responsible for offloading all materials during delivery/deliveries.

Contractor is fully responsible for all tools and equipment necessary to complete the delivery and installation effort.

1.3.3. Travel. The Contractor shall travel, as required, to complete this effort.

1.3.4. Function and Fit Test. Contractor shall perform function and fit test to ensure that equipment will provide requirements as listed in section 1.2. Contractor is fully responsible for verifying field measurements.

1.3.5. Assembly. Safety equipment shall be used during assembly. Safety equipment shall contain up-to-date certifications and shall be in good working condition. Remediation of any component that cannot be assembled properly as a result of errors in manufacturing or deformation due to handling or transportation shall be the responsibility of the Contractor.

The Contractor shall immediately report any discoveries to the CO. Repair methods shall have prior approval by the CO before any work is performed. The straightening of steel base frames, tubing, angles, connecting devices, casters or other components shall be done by methods approved by the original component manufacturer and shall follow industry standards. The Contractor shall replace all items damaged beyond acceptable repair.

1.3.5.1. Assembly Site Limitations. The Contractor shall take the following precautions on-site:

a. All assembly work shall be completed in such a manner as to not disrupt government activities. The Contractor shall perform any on-site work that shall disrupt government activities during non-production hours and shall be coordinated and approved by the COR.

b. The Contractor’s work area shall be maintained clean and orderly. Debris shall be removed and disposed of daily by the Contractor.

c. Material cutting shall not introduce shavings or chips in, onto or around the aircraft, aircraft access panels, or aircraft engines.

d. All phases of work shall be accomplished safely to comply with OSHA 29 CFR 1910.29, 1926.450, 1926.451, 1926.452, and AFOSH 91-501.

e. The Contractor shall be responsible for handling equipment and supplies during progress of the contract.

f. Utility requirements shall be the responsibility of the Contractor. Sources of electrical power and compressed air shall be provided on an “as available” basis. The Contractor shall be responsible for ensuring adequate utility supplies and hookups are available on-site prior to the time of need.

g. No welding is permitted within fifty (50) feet of an aircraft.

h. Storage of flammable materials such as paints and solvents shall be in authorized areas and approved through the CO.

i. The Contractor shall comply with base requirements on handling hazardous materials.

Disposal of hazardous materials shall be coordinated with the CO and performed by the Contractor.

j. The Contractor employees shall not wear watches, rings, or jewelry within aircraft maintenance areas. Proper personal protective equipment (safety glasses, hearing protection, hard hats, safety shoes, etc.) shall be worn in and around all appropriate production areas.

1.3.6. Contractor Point of Contact. The Contractor shall provide a point of contact (POC) and an alternate who shall act for the Contractor when the manager is absent and shall designate these individuals in writing to the Contracting Officer within two (2) days of contract award.

The Contractor shall provide telephone numbers of the Contract Manager and alternate, and where they may be contacted at any time. The Contract Manager shall have full authority to act for the Contractor on all matters relating to operation of this contract on Tinker Air Force Base and MROTC. The Contract Manager or alternate shall be available to meet on the installation with Government personnel designated by the Contracting Officer to discuss problem areas within Two (2) weeks of notification. The Contractor shall provide a Contract Manager who can read, write, speak, and understand English fluently. Contractor’s employees shall present neat, clean, safe, and professional appearances at all times, and are easily recognized.

1.3.7. Secured Area(s). The project is located in a secured area. Badges shall be displayed at all times.

1.3.8. Security Requirements. All personnel employed by the Contractor in the performance of this contract, or any representative of the Contractor entering the governmental installation, shall abide by all security instructions and directives of Tinker Air Force Base and MROTC.

Employees are responsible for safeguarding any government property provided for Contractor use.

1.3.8.1. The Contractor shall comply with existing government procedures for obtaining Contractor credentials to work on-site, and government procedures for Contractor access to government legacy data systems. While inside DOD facilities the Contractor shall be wear government-approved identification badges on their outer garment in the chest area. DOD facilities are defined as any space, open or covered, that is directly owned by or leased to the Department of Defense for the United States of America.

1.3.9. Personnel Requirements. The Contractor and any subcontractors, as applicable, shall not employ persons for work on this contract if such employee is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population; nor shall the Contractor or subcontractor employ persons under this contract who have an outstanding criminal warrant.

1.3.9.1. Contractor shall ensure their employees and those of their subcontracts have the proper credentials allowing them to work in the United States. Employees and subcontractors later found to be undocumented or illegal aliens will be remanded to the proper authorities.

1.3.9.2.The Contractor shall not be entitled to any compensation for delays or expenses associated with complying with the provisions of this SOW. Furthermore, nothing in this SOW shall excuse the Contractor from proceeding with the contract as required.

1.3.10. Foreign Nationals. A foreign national is defined as any person who does not possess a legal certificate of citizenship in the United States of America. The Prime Contractor shall be responsible for acquiring access to the Base for Foreign Nationals. The Contractor shall provide a list of all Foreign Nationals that it proposes to work on Base and should document its efforts to acquire access to the Base for these Foreign Nationals. The Contractor and his/her employees shall comply with all requirements within AFI 16-201, Air Force Foreign Disclosure and Technology Transfer Program.

1.3.11. Working Hours. All work shall be performed during normal operating hours, 0645-2400 local Monday thru Friday, except on federal holidays when the facility shall be closed.

Contractor shall coordinate all work through Contracting Officer.

1.3.12. Crisis or Severe Weather Conditions. This requirement has been determined not to be essential and does not require continued support during a crisis as defined in DoDI 3020.37, E2.1.1, and E2.1.3. For Base Operating Status go to Tinker’s Facebook page at:

1.3.13. Quality Control. The Contractor is responsible for ensuring and enforcing industry safety practices and quality control of the job site, including those performed by subcontractors. The Contractor shall comply with all standard industry practices and procedures and manufacturer’s specifications.

1.3.14. Parts and Materials. The Contractor shall use only new equipment, hardware, parts, and materials in performance of this statement of work, unless authorized by the CO who shall seek technical guidance from the assigned QPE engineer. All equipment, hardware, parts and materials shall be COTS items to the fullest extent possible.

1.3.14.1. Spare Parts and Proprietary Tools. The Contractor shall provide a recommended spare parts list with prices for each stand to the Government. On each line item note if the item is COTS, and if not, provide the approximate current lead time. The Contractor shall provide all proprietary or uncommon tools required for maintenance or assembly of the stands. The tools shall be transferred to the Government at the end of the contract.

1.3.15. Warranty. The Contractor shall warranty all components and workmanship for a period of

365 calendar days after the date of final acceptance. Contractor shall provide all repairs during evaluation period and 365 calendar days after the date of final acceptance.

1.3.16. Training. After completion and acceptance, the Contractor shall provide a training session for the government personnel to include the operation, maintenance, preventive maintenance, diagnostic features, and use of provided technical data for the systems and all the system components.

1.3.17. Storage and Staging Areas. The Contractor shall limit all storage of applicable materials, equipment, and incidentals to the MROTC facility. If the Contractor requires additional storage area(s) other than the project site, a request shall be submitted, in writing, to the Contracting Officer for approval. These areas are on a first come first served basis and are very limited. A request shall not be considered a guarantee that space is available. The

Contractor shall determine the square footage needed and request the area prior to bid submittal. It is recommended that the Contractor make arrangement for storage of equipment between its delivery and its installation, as availability of space and security during non-working hours cannot be guaranteed.

1.3.18. Tool Control and Accountability. While performing contract services in or around

Maintenance Production Shops, including ramp areas, the Contractor shall comply with AFMCI 21-107, Tool Control and Accountability Program and Tinker Instruction 21-112, Tool Management. Minimum requirements.

1.3.18.1. All tools brought into the 76 MXW Wing areas including ramp areas shall be secured in a container or bag.

1.3.18.2. Each tool shall be identified and listed separately on a “tool listing” kept inside each container or bag.

1.3.18.3. Contractor personnel shall make the “tool listing” available to government personnel when requested.

1.3.18.4. Contractor personnel shall ensure all tools are secured when not in use.

1.3.18.5. Contractor personnel shall ensure no tools are left unattended at any time.

1.3.18.6. Contractor personnel shall inventory the container or bag of tools at the beginning of the day.

1.3.18.7. Contractor personnel shall inventory the container or bag of tools at the end of each day.

1.3.18.8. Contractor personnel shall immediately notify the Contract Monitor of any tools that are missing. Tools not located within one hour of notification will be reported by the Contract Monitor to appropriate government personnel.

1.3.19. Foreign Object Damage (FOD) and Dropped Object Prevention (DOP). Contractor personnel who, in performance of their assigned duties on this contract, work in or travel though areas near operational and production aircraft, engines, munitions, missiles, drones, space systems, support equipment, Aerospace Ground Equipment (AGE), trainers and the Contractor personnel operating vehicles and equipment on flight lines, runways, taxiways, parking ramps and in aircraft hangers and maintenance areas are required by AFMCI 21-122 to receive FOD/DOP training and to comply with AFMCI 21-122. Contractor personnel shall obtain FOD/DOP Awareness and Prevention Training which is available on the Tinker Intranet, https:/ /wwwmil.tinker.af.mil/76mxw/FOD.asp.

1.3.20. Environmental Protection and Safety. The Contractor shall be knowledgeable of and comply with all applicable federal, state and local laws, regulations, instructions and requirements regarding environmental protection and occupational safety and health. The Contractor shall comply with any laws, instructions or regulation changes during the term of this contract. The Contractor shall follow manufacturer’s guidelines and professional recommendations for control of humidity, temperature, cleanliness and materials handling (including hazardous materials and hazardous waste).

1.3.20.1. Hazardous Material: Contractor not occupying space on base: The Contractor shall not store chemicals/HAZMAT on Tinker AFB. All chemicals/HAZMAT must be approved prior to being transported on base for use by any Contractor. The Contractor shall prepare and submit to the QAE/QAP, ten (10) calendar days after contract award date, one

(1) each, Chemical/Hazardous Material Request Authorization Form (Air Force Form 3952) and MSDS for each chemical, materials and substance proposed to be used in performance of this SOW. The QAE/QAP shall forward the completed AF Form 3952s and MSDS to 72 ABW/CEAN for approval. The Contractor shall not transport or use any chemical, material or substance on Tinker AFB before written approval is provided by 72 ABW/CEAN.

Contractor shall maintain copies of MSDSs for all chemicals. The MSDSs must correspond with the manufacturers of the chemicals on hand. Manufacturer, material, and MSDS must match. NOTE: MSDS refers to manufacturer MSDS. Once material is approved for use, Contractor shall contact base EM (72 ABW/CEAN) and provide a list of all material. 72 ABW/CEAN shall determine any additional material requirements based on the hazard code of the material.

1.3.20.2. Hazardous Waste: Contractor not occupying space on base: Contractor shall dispose of any spent/processed chemical solutions and used rags in waste drums located in the shop/area the work is being performed in.

1.3.20.3. Discharges: Any discharges into Tinker’s industrial waste treatment plant (IWTP) or sanitary sewer shall be done in accordance with Tinker Industrial Waste Water Discharge Permit as instructed by 72 ABW/CEAN. The Contractor shall maintain records of written authorization issued by 72 ABW/CEAN for discharges into IWTP and sanitary sewer.

1.3.20.4. Spills: Upon occurrence of any spills that could enter the storm system or cause any harmful environment effects, the Contractor shall immediately call 911 to report the incident.

When possible, the Contractor shall use a base telephone to receive help from Tinker AFB

911. If using a cell phone, request Tinker AFB 911. The Contractor shall report all environmental violations to the QAE/QAP. The Contractor shall submit a typed or written report about the incident within four (4) clock hours. The Contractor shall provide five (5) copies of the written report. The Contractor shall provide one copy to the CO, two copies to the QAE/QAP, a copy for Unit Environment Coordinator (UEC) and a copy for record. The format of the report shall be left up to the Contractor, but the report shall contain the following information:

1.) Name and contact number of person reporting the spill 2.) Date and time of incident 3.) Location and source of spill 4.) Substance or pollutant spilled 5.) Amount spilled and rate of discharge 6.) Any damages or injuries involved 7.) Extent of area impacted 8.) Potential hazards

9.) Actions taken 10.) Organizations contacted 11.) Remarks Section - that addresses the cause of the spill (e.g. human error, equipment, etc.) and whether 911 was called and which agencies responded.

1.3.20.5. Environmental and Safety Training: The Contractor personnel shall have Resource Conservation Recovery Act (RCRA) environmental and safety training required by the state and federal government. The training shall be IAW the following regulations:

Resource Conservation Recovery Act (RCRA) 40 Code of Federal Regulation (CFR) 265.16 “Personal Training”, Occupational Safety and Health Administration (OSHA) Hazardous Waste Operations and Emergency Response Standard Title 29, CFR Part 1910.120 “Hazardous Waste Operations and Emergency Reponses” 40 CFR Part 63.741-40 CFR 63.753, Subpart GG- National Emission Standards for Aerospace Manufacturing and Rework Facilities, Oklahoma City Air Logistics Center Tinker Air Force Base Instruction (OC-ALC TAFB) 32-7004, Hazardous Waste Management Instruction, Section 13, and Air Force Instruction (AFI) 32-7042, “Solid and Hazardous Waste Compliance”, Section 2, 76 MXW OI 32-7 “Hazardous Material Management Program” (In its entirety), 76 MXW OI 32-101 “Air Quality Record Keeping Procedure” (In its entirety) & 76 MXW OI 32-7004 “Hazardous Waste Management Program” (In its entirety) . The Contractor shall provide the environmental and safety training certificate for all employees to the CO and QAE/QAP, within ten (10) calendar days after contract award date.

1.3.21. Bioenvironmental Protection. The Contractor shall ensure all activities do not subject workers to bioenvironmental hazards including excessive noise, production of dust particles, toxic fumes, inadequate lighting, inadequate ventilation and excessive heat.

1.3.22. Safety. The Contractor shall be knowledgeable of and comply with all applicable federal, state and local laws, regulations and requirements regarding environmental and bioenvironmental protection. In the event environmental and bioenvironmental laws or regulations change during the term of this contract, the Contractor shall comply with such laws when enacted. The Contractor shall comply with all safety practices posted in the assigned work area as well as the Industrial Safety Requirements provided in Appendix A of this SOW.

2. APPLICABLE DOCUMENTS

2.1. Industry Standards. Design and installation must be in accordance with state and local codes, ordinances, and regulations of Oklahoma City, Oklahoma and the latest edition of the following codes and standards, where applicable:

1. Federal Publications: 29 CFR 1910: Occupational Safety and Health Standards.

2. American Society for Testing and Materials: ASTM D 3951-90: Standard Practice for

Commercial Packaging.

2.2. Reference Documents. The following are general references that must be followed in addition to all requirements specified in this document. Contractor shall notify the Contracting Officer (CO) if any additional clarification is required. Unless otherwise indicated, the issue in effect on date of proposal applies. Use the following documents as a guideline:

1. MIL-STD-130: Identification Marking of US Military Property

2. MIL-STD-808: Finish, Protective and Coded for Finishing Schemes for Ground and Ground Support and Equipment.

3. MIL-STD882C: System Safety Program Requirements.

4. MIL-STD-889: Dissimilar metals

5. MIL-STD-1472: Human Engineering Design Criteria for Military Systems, Equipment and

Facilities.

6. MIL-STD-45662: Calibration Systems Requirements.

7. MIL- H- 5440: Hydraulic Systems, Aircraft, Types I and II, design and Installation

Requirements.

8. FED-STD-595: Colors Used in Government Procurement.

2.3. Code Requirements: Equipment and materials furnished under this statement of work to be designed and constructed in accordance with the latest applicable requirements of standard specifications and codes referenced in this document, including the following:

1. ANSI: American National Standards Institute.

2. ASME: American Society of Mechanical Engineers.

3. ASTM: American Society of Testing and Materials.

4. AWS: American Welding Society.

5. SMACNA: Sheet Metal and Air Conditioning Contractors’ National Association

APPENDIX “A”

INDUSTRIAL SAFETY REQUIREMENTS

SECTION I - GENERAL REQUIREMENTS

A. Safety Program Requirements. The Contractor shall implement a safety program that ensures protection of Government personnel and property. The program shall consist of, as a minimum:

1. Mishap notification/reporting as defined in Section I.B. below.

2. A Safety Program that addresses, as a minimum, the subjects listed in Safety Program Elements, Section II and to be used during performance of the work described in THE CONTRACT.

B. Mishap Notification.

1. The Contractor shall notify the procuring Safety Office or a designated Government Representative (GR) within one (1) hour of all mishaps or incidents. A written report of the mishap/incident shall be sent within three calendar days to the GR, who shall forward it to the Procuring Safety Office. For information not available at the time of initial written report, the Contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the Safety Office.

2. Mishap notifications shall contain, as a minimum, the following information:

a. Contract, Contract Number, Name and Title of Person(s) Reporting

b. Date, Time and exact location of accident/incident

c. Brief Narrative of accident/incident (Events leading to accident/incident)

d. Cause of accident/incident, if known

e. Estimated cost of accident/incident (material and labor to repair/replace)

f. Nomenclature of equipment and personnel involved in accident/incident

g. Corrective actions (taken or proposed)

h. Other pertinent information

3. If requested by the designated Government representative, the Contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by the Procuring Safety Office.

C. The safety provisions of this contract, shall apply to any subcontracts/subcontractors.

Safety requirements listed herein that do not concern Contractor’s operations or services shall be considered self-deleting as mutually agreed by the Contractor and the procuring office.

SECTION II - SAFETY PROGRAM ELEMENTS

The Contractor’s Safety Program shall clearly define procedures, personnel qualifications, facilities and required equipment necessary to fulfill the following elements Element/Requirement.
