Statement of Work

(SOW)

For

RAPID PROTOTYPE SYSTEM (RPS)

Prepared By:

INTRODUCTION

The Aircraft Maintenance Group (AMXG) requires the purchase, delivery, installation, and training of a Rapid Prototype System (RPS) for use in support of the Air Force Programmed Depot Maintenance (PDM) at Building 3, Tinker Air Force Base (Tinker AFB). The overall RPS is a “turn-key” type procurement.

REQUIREMENTS

General The equipment shall be new (not a prototype) and one of the manufacturer’s standard current production models capable of operations in accordance with the requirements herein. This means that on the date this solicitation is issued, the equipment has been designed, engineered and sold, or is being offered for sale through advertisements of manufacturer’s published catalogs or brochures. Products such as prototype unit, preproduction model, or experimental unit do not qualify as meeting this requirement. Equipment being offered shall be capable of performing it’s intended function in accordance with the operation and performance requirements specified herein. The machine shall be complete, so that when connected to the utilities identified herein, it can be used for any function for which it is designed and constructed.

Safety and Health: Covers, guards, and other safety devices shall be provided for all parts of the machine that present safety hazards. The safety devices shall not interfere with the operation of the machine. The safety devices shall prevent unintentional contact with the guarded part and shall be removable to facilitate inspection, maintenance, and repair of the parts. All machine parts, components, mechanisms, and assemblies furnished on the machine shall comply with all of the requirements of 29 CFR 1910. The machine shall have positive stops at the limits of travel in each axis to prevent overrun. The machine shall have overload protection which automatically stops the movements of the slides should sit stall from overload.

Caution-Warning Plates: Corrosion resistant “Caution” or “Warning” plates shall be securely attached to the equipment in visible locations, with any safety precautions to be observed by the operator or maintenance personnel permanently marked on the plates.

Prohibited Materials: Asbestos, mercury, lithium or polychlorinated biphenyl (PCB) shall not be used in any part or component of the system. Lithium batteries used in the computer system are exempt from this requirement.

Painting: The machine shall be painted in accordance with the manufacturer’s commercial practice. Yellow or orange safety color-coding shall be used for designating physical hazards.

Commercially Available Items: The following items shall be considered commercially available items and shall be supplied as described herein:

Transportation to TAFB, including all labor, tarping and materials (section 3.1).

RIGGING, MOVING, and ELECTRICAL cost for all of the following:

Transportation of components, as required, from warehouse to Tinker Uncrating of equipment and cleaning of components Proper equipment staging and component installation with factory representatives Build material for RPS (section 3.6).

Testing material, tooling, part programming, all personnel to perform tests (section 5).

On-Site Training (section 7).

Machine Installation The contractor shall provide all personnel, equipment, tools, supervision, transportation, materials, structural installation requirements, field testing, site oversight, waste handling/ transportation /disposal and any other incidentals necessary to install the RPS and associated machine startup/testing and training. The contractor shall warranty all construction installation and materials for a period of one year.

MACHINE TRANSPORTATION, RIGGING, INSTALLATION, AND STARTUP

Transportation The contractor shall provide for the "enclosed" transportation of the RPS from the point of origination to the installation site at Tinker AFB.

Rigging The contractor shall furnish all labor and material handling equipment required for offloading the RPS at Tinker AFB. The contractor shall be responsible for the disposal of all crating, skids and packing materials.

Installation/Assembly The contractor shall furnish all personnel, equipment, tools and materials necessary to off-load, clean, set on-site machine components, grout and/or weld anchor bolts/leveling device and the associated assembly of said components with the supervision of factory representatives.

Leveling The contractor shall furnish all personnel, equipment, tools and materials necessary to level the machining cell on the foundation IAW manufacturer's direction.

Utilities Hook-Up The contractor shall furnish all personnel, equipment and materials necessary to connect the RPS to Tinker AFB provided "rough-in" 240V electrical and compressed air services that are available within twenty-five (25) feet of final connection points. Contractor furnished transformers shall be of the "shielded type" and all electrical work shall be IAW NFPA 70: National Electrical Code (NEC). The contractor shall provide three sets of installation drawings showing a dimensioned top view of the machining cell, along with applicable electric and air requirements 90 days ARO to enable government lead time of roughed-in utilities. Equipment grounding requirements should be indicated on drawings and the installation of such grounding requirement shall be the responsibility of the contractor.

System Charging The contractor shall provide all build materials, fluids, etc. IAW manufacturer's instruction. MSDS shall be provided to the Government representative 90 days in advance to obtain the necessary authorization of chemical use.

The contractor shall provide a large “starter kit” of building materials for initial operation and training.

QUALITY ASSURANCE PROVISIONS

Responsibility for Inspection The contractor shall be responsible for the performance of all inspection requirements (examinations and tests) as specified herein. The contractor may utilize his own or any other facilities suitable for the performance of the inspection requirements specified herein, unless disapproved by the government. The government reserves the right to perform any of the inspections set forth in the specification when such action is deemed necessary to assure supplies and services conform to prescribed requirements. The contractor shall provide all tooling, fixturing, measurement indicating devices, material and programs required of the compliance, accuracy alignment and machining acceptance tests.

Responsibility for Compliance The machine shall meet all requirements of section 2. The inspections set forth in this specification shall become a part of the contractors overall inspection system or quality program.

The absence of any inspection requirements in the specification shall not relieve the contractor of the responsibility of ensuring that all products or supplies submitted to the government for acceptance comply with all requirements of the contract.

Examination at origin The equipment offered shall be examined for design, construction, components, electrical equipment and workmanship to determine conformance with the requirements of this specification. The fit of parts shall be observed with particular reference to interchangeability of such as are likely to require replacement.

Quality Conformance Inspection Quality conformance inspection shall be conducted on machine prior to being offered for acceptance under the contract. Quality conformance inspection shall be conducted at origin and monitored by government personnel. The inspection shall be in accordance with section 5.

Acceptance Test Final inspection, testing and acceptance shall occur at destination (Tinker AFB, OK). The acceptance test shall be in accordance with section 5.

Test Conditions All tests shall be performed in an indoor facility with ambient conditions of 41°F to 104°F and 20% to 95% relative humidity.

Examination The machine shall be visually examined to determine compliance with all requirements within section 2.

TESTS

Operational Test The equipment shall be operated at no load (warm up), during which time all functions listed in section 2 shall be tested. The equipment shall be operated in accordance with the builders standard operating test procedure for warm-up and run-off. The run-off test shall include test of all mechanical and electrical functions of the machine and controls to determine proper functioning of operation provided. After warm up the equipment shall be cycled continuously for a period of not less than four (4) hours. During this period the machines shall perform all ranges listed in section 2. The software routines shall also be demonstrated. Government personnel shall be present during this testing.

Performance Test A performance test shall be conducted prior to final acceptance. Testing shall be conducted at destination by contractor personnel and be monitored by government personnel. The test shall consist of contractor's standard performance tests. The performance test results shall meet or exceed the requirements of section 2. The contractor's standard performance tests shall be reviewed and approved by government personnel prior to awarding of contract.

Examination at destination Examination at destination shall consist of inspection for damage in shipment, verification of quantities and any visual inspection deemed necessary by the receiving activity to verify supplies received conform to the requirements of the specification.

TECHNICAL DATA

Manuals/Drawings The contractor shall furnish digital copies of all technical manuals/drawings containing all information necessary for the programming, operation and maintenance (parts lists, electrical schematics, ladder logic and I/O diagrams) of the RPS. Digital copies shall be received concurrent with machine delivery and shall be in the form of PDF. Any other digital edition shall require pre-approval.

The contractor shall furnish three (3) hard copies of all technical manuals/drawings containing all information necessary for the programming, operation and maintenance (parts lists, electrical schematics, ladder logic and I/O diagrams) of the RPS. One (1) complete set of manuals/drawings shall be sent to the receiving activity 60 days prior to the ship date and the remaining two (2) sets concurrent with machine delivery.

Installation Drawings The contractor shall provide to the receiving Government facility, engineering installation drawings showing the necessary compressed air, water, and electrical utilities of the RPS. The installation drawings shall be provided 60 days after-receipt-of-order (ARO).

Acceptance test report The contractor shall provide to the receiving Government facility, acceptance tests reports completed at origin. The acceptance test reports shall be provided 30 days prior to shipment of the RPS.

Documentation All documentation, i.e. operation manuals, maintenance manuals, programming manuals, student training materials, drawings, vendor literature, laminated note cards, metal name and service plates, service bulletins, upgrade bulletins, and all associated charts/graphs, shall be written in the English language and grammatically correct.

TRAINING

Operator/programmer Within 10 days of acceptance of the RPS at Tinker AFB, the contractor shall conduct a minimum of 24 hours of on-site familiarization over 4 consecutive workdays (excluding Government holidays and weekends), during the hours of 0730 to 1600 for up to four (4) operators. The familiarization shall include instruction and demonstration of equipment operation as well as use of unique features, accessories and all programming functions/routines. The instructor shall utilize organized course material and visual aids to provide comprehensive programming, operator and maintenance training. Instructions and all printed material shall be in the English language. The contractor shall notify the receiving activity in writing not less than 30 days prior to delivery of the machine(s) as to the training schedule and the class room facilities required by their personnel. Training shall be at a time mutually agreeable to the contractor and the Government.

Electronics/mechanical The contractor shall provide one (1) separate 1 day maintenance familiarization session; one session for mechanical maintenance and electronics maintenance of the RPS. Maintenance familiarization shall be on-site at the receiving activity. Familiarization shall be provided for a total of four personnel, two (2) each personnel per discipline. On-site maintenance familiarization shall be conducted during the hours of 0800 - 1600. Maintenance familiarization shall be scheduled so as not to interfere with Federal Holidays or weekends.

Follow-up operator/programmer The contractor shall conduct a minimum of twenty-four (24) hours of on-site familiarization over 3-1/2 consecutive workdays, during the hours of 0730 to 1600 for up to four (4) operators for the purpose of follow-up familiarization. The follow-up familiarization shall be completed within a 6 to 8 weeks after completion of the initial operator/programming familiarization conducted after final acceptance.

OPERATIONS

Secured Area(s) Project is not located in a secured area; however, badges (including temporary badges) shall be displayed at all times.

Working Hours Construction shall take place during standard working hours (0730-1600) except for holidays and weekends. No work shall occur on holidays and weekends without written approval from the contracting officer.

Environmental, Safety & Quality Control Environmental Protection & Safety: The Contractor shall be knowledgeable of and comply with all applicable federal, state and local laws, regulations, instructions and requirements regarding environmental protection and occupational safety and health. The Contractor shall comply with any laws, instructions or regulation changes during the term of this contract.

Contractors shall be compliant with Tinker Air Force Base (TAFB) Supplement to Air Force Instruction (AFI) 32-7086, “Hazardous Materials Management”, Tinker AFB procedures per Tinker Air Force Base Instruction (TAFBI) 32-7004, “Hazardous Waste Management”, and Complex procedures per Oklahoma City Air Logistics Complex (OC-ALC) Supplement to AFI 32-7086, “Hazardous Material Management” and OC-ALCI 32-101 “Air Quality Record Keeping Procedure”. Publications are available digitally on e-Publishing website at www.e-publishing.af.mil for downloading or ordering.

Material Safety Data Sheet/Safety Data Sheet (MSDS/SDS): The Contractor shall not transport, ship or use any chemical, material or substance used in the performance of this SOW on Tinker AFB before written approval is provided by 72 ABW/CEIE, Natural Infrastructure Management (Environmental Compliance). The Contractor shall be responsible for submitting all Material Safety Data Sheets (MSDSs) or Safety Data Sheets (SDSs) to 72 ABW/CEIE. Contractor shall comply with the effective dates for SDS requirements IAW the Occupational Safety and Health Administrations (OSHA) Hazardous Communication Standard, Title 29, Code of Federal Regulation (CFR) Part 1910.1200. The Contractor shall maintain copies of manufacturer MSDSs/SDSs for all chemicals and MSDSs/SDSs shall be readily accessible at all times by Contractor personnel as well as government employees.

Hazardous Material (HAZMAT): The Contractor shall comply with applicable federal and military procurement policies IAW OC-ALC Supplement to AFI 32-7086, “Hazardous Material Management”. The Contractor shall obtain access to the standardized Air Force HAZMAT tracking system and manage HAZMAT within the system. The Contractor shall establish and reconcile the inventory, print the standardized Air Force HAZMAT tracking system bar code labels, and apply the labels to containers of HAZMAT so that the Occupational Safety and Health Administration (OSHA) compliant labels are not covered. Any failure to comply with proper HAZMAT management procedures shall be reported to the CO and COR who will notify 776 MXSS/MXDEB, OC-ALC Environmental and Occupational Health Compliance.

Air Quality Recordkeeping: The Contractor shall be responsible for specific recordkeeping and reporting requirements, when applicable, mandated by Tinker Air Force Base Facility-Wide Operating Permit IAW OC-ALCI 32-101. The COR will contact the UEC for environmental requirements. The Contractor shall submit all applicable records on a monthly basis to the COR no later than the third (3rd) day of the month following the reporting period. If the third (3rd) day of the month is a weekend or holiday, the logs are required by the last business day 'PRIOR' to the (third) 3rd. The COR will forward the records to the UEC no later than the fourth (4th) day of the following month or the first subsequent business day. Contractor shall maintain a signed copy of the form(s) required per OC-ALCI 32-101 on site for one (1) year.

Hazardous Waste: The Contractor shall dispose of contaminated solid waste, spent or processed chemical solutions, used rags and hazardous waste in government furnished waste containers IAW TAFBI 32-7004.

The Contractor shall be responsible for the Initial Accumulation Point (IAP) within the Contractor assigned space. The IAP shall be under the direct control of the Contractor who generates the waste. Before accumulation begins, the Contractor shall ensure appropriate hazard labels are affixed to government furnished waste containers as specified in 49 Code of Federal Regulation (CFR) 172.400. Management procedures for IAPs include written weekly inspections and reporting. When the container has been filled, the Contractor shall seal the container, mark the Certified Filled Date on the container label using an indelible pen, place the container at a designated pick up location, and call the Hazardous Waste Management Facility (HWMF) at 734-3285 for pick-up service. The Contractor shall ensure the HWMF picks up the container within three (3) calendar days after the Certified Filled Date. If container is not picked up within three (3) calendar days, Contractor shall notify the COR who will notify 776 MXSS/MXDEB, OC-ALC Environmental and Occupational Health Compliance.

The Contractor shall perform written weekly inspections using Tinker AFB IMT Form 487, Initial Accumulation Point Management Aid. Weekly denotes once per working week. The Contractor shall submit Tinker AFB IMT Form 487 on a monthly basis to the COR no later than the third (3rd) day of the month following the reporting period. If the third (3rd) day of the month is a weekend or holiday, the logs are required by the last business day 'PRIOR' to the third (3rd). The COR will forward the form(s) to the UEC no later than the fourth (4th) day of the following month or the first (1st) subsequent business day. Contractor shall maintain a signed copy of the form(s) on site for one (1) year.

Discharges: Any discharges into Tinker’s industrial waste treatment plant (IWTP) or sanitary sewer shall be approved and done IAW Tinker Industrial Waste Water Discharge Permit as instructed by 72 ABW/CEIE, Natural Infrastructure Management (Environmental Compliance). The COR will submit the required notice of intent documentation to the UEC for 72 ABW/CEIE approval. The Contractor shall maintain records of written approval issued by 72 ABW/CEIE for discharges into IWTP and sanitary sewer.

Spills: Upon occurrence of any spill that could enter the storm system or cause any harmful environment effects, the Contractor shall immediately call 911 to report the incident. When possible, the Contractor shall use a base telephone to receive help from Tinker AFB 911. If using a cell phone, the Contractor shall request Tinker AFB 911. The Contractor shall immediately report all environmental violations to the COR and UEC. The Contractor shall provide five (5) copies of a typed or written report about the incident within four (4) clock hours as follows:

One (1) copy to the CO, two (2) copies to the COR, a copy for UEC and a copy for record. The format of the report shall be left up to the Contractor, but the report shall contain the following information:

1.) Name and contact number of person reporting the spill 2.) Date and time of incident 3.) Location and source of spill 4.) Substance or pollutant spilled 5.) Amount spilled and rate of discharge 6.) Any damages or injuries involved 7.) Extent of area impacted 8.) Potential hazards 9.) Actions taken 10.) Organizations contacted 11.) Remarks Section - that addresses the cause of the spill (e.g. human error, equipment, etc.) and whether 911 was called and which agencies responded.

Housekeeping: The contractor shall keep the work areas clean and neat in accordance with Industry Standards, Occupational, Safety and Health Administration (OSHA), fire and safety standards and this SOW.

Tool Control and Accountability While performing contract services in or around Maintenance Production Shops including Ramp areas, the contractor shall comply with AFMCI 21-107, Tool Control and Accountability Program and Tinker Instruction 21-112, Tool Management.

Contractor personnel shall ensure all tools are secured when not in use.

Contractor personnel shall ensure no tools are left unattended at any time.

Contractor personnel shall inventory the container or bag of tools at the beginning of the day.

Contractor personnel shall inventory the container or bag of tools at the end of each day.

Safety Program Requirements The contractor will implement a safety program that ensures protection of Government personnel and property. The program will consist of, as a minimum:

Mishap notification/reporting as defined in paragraph I.B. below.

A Safety Program that addresses, as a minimum, the subjects listed in Safety Program Elements, (Section 8.3.14) and to be used during performance of the work described in the contract.

Mishap Notification The contractor shall notify the designated Contracting Officer Representative (COR) within one (1) hour of all mishaps or incidents. A written report of the mishap/incident shall be sent within three calendar days to the COR, who will forward it to the Procuring Safety Office. For information not available at the time of initial written report, the contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the Safety Office.

Mishap notifications shall contain, as a minimum, the following information:

a. Contract, Contract Number, Name and Title of Person(s) Reporting

b. Date, Time and exact location of accident/incident

c. Brief Narrative of accident/incident (Events leading to accident/incident)

d. Cause of accident/incident, if known

e. Estimated cost of accident/incident (material and labor to repair/replace)

f. Nomenclature of equipment and personnel involved in accident/incident

g. Corrective actions (taken or proposed)

h. Other pertinent information

If requested by the designated government representative, the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by the Procuring Safety Office.

The safety provisions of this contract, shall apply to any subcontracts/subcontractors.

Safety requirements listed herein that do not concern contractor’s operations or services shall be considered self-deleting as mutually agreed by the contractor and the procuring office.

Safety Program Elements

The contractor’s Safety Program shall clearly define procedures, personnel qualifications, facilities and required equipment necessary to fulfill the following elements:
