PERFORMANCE WORK STATEMENT
FOR
ANNUAL LEASE AND MAINTENANCE
OF
DE-IONIZED WATER PURIFICATION SYSTEMS
(Commodities and Propulsion Maintenance Groups)
15 May 2014

PWS – Leased De-ionized Water Purification Systems
1. DESCRIPTION OF SERVICES
1.1. Scope. The contractor shall provide all labor, tools, equipment, test equipment personal protective equipment and clothing, hearing protection, eye protection including prescription safety glasses, materials and parts, transportation and other incidentals necessary for the Lease and Maintenance of thirty-eight (38) De-ionized Water Purification Systems to the 76 Maintenance Wing Group at Tinker AFB, OK and to the various locations listed in Appendices A1 and A2.
1.2. Hours of Operation. The contractor shall perform maintenance work required under this contract during the following hours Monday through Friday, 7:00 am -4:00 pm, excluding Federal holidays and furlough days.
1.3. Requested Services. The contractor shall lease and maintain the De-ionized (DI) Water Purification Systems (WPS) listed in Appendices A1 and A2. The services shall include at a minimum, the following requirements:
1) Deliver the types of DI WPS described below and as specified in Appendices A1 and A2 at their locations stated.
2) DI WPS descriptions (listed in Appendices A1 and A2):
a) 9" Portable Exchange (PE) Tri Bed DI WPS Description - 3 ea. Poly glass cylinders in series
- Cylinder Size is 9"diameter x 42" length
b) 9" PE 2-Bed DI WPS Description - 2 ea. Poly glass cylinders in series
- Cylinder Size is 9"diameter x 42" length
c) 14" PE 2-Bed DI WPS Description - 2 ea. Poly glass cylinders in series
- Cylinder Size is 14" diameter x 47” length
d) 14” PE 5 Bed DI Water Purification System Description – 5 ea. Poly glass cylinders in series
e) General requirements for DI WPS
- National Sanitation Foundation (NSF) Design Parameters Safety factor: 4:1 Minimum burst at 600 Pounds per Square Inch (PSI) Tested to 100,000 cycles without leakage
- Poly glass cylinders must not be exposed to temperatures below - 20 degrees Fahrenheit (F).
- Operating Specifications:

3) Technical Requirements for all systems are to produce water that has less than 5 ppm (parts per million) TDS (total dissolved solids) in final water produced.
4) Set-up, install and demonstrate to the Contracting Officer Representative (COR) identified Government Point of Contact (POC) and the COR that all De-ionized Water Purification Systems delivered are properly functioning in accordance with manufacturer’s equipment specifications and this Performance Work Statement (PWS).
5) Replace the exhausted ion exchange cylinders with fully recharged cylinders when limit in no. 3 above is exceeded. The schedule in Appendices A1 and A2 (the last column) shows the estimated annual usage of each DI water system at each location.
Water quality will be tested by the government periodically. Upon any over limit findings, the COR will notify the contractor to replace the cylinder/tank set with a fresh set.
1.3.1. Delivery of De-ionized Water Purification Systems. The contractor shall deliver the De-ionized Water Purification Systems, stated in Appendices A1 & A2, within ten (10) work days after contract award date.
1.3.2. Contractor Scheduled Visit. The contractor shall notify the COR at least ten (10) work days prior to any scheduled visit as estimated in Appendices A1and A2 last column.
1.3.3. Removal of De-ionized Water Purification Systems. Upon formal notification that the De-ionized Water Purification Systems listed in Appendices A1 & A2 are no longer required or contract end date, the contractor shall return to Tinker AFB to pick up and return the De-ionized Water Purification Systems to the contractor site within ten (10) work days from date of formal notification.
1.3.4. Emergency Maintenance (EM). Upon notice of equipment malfunctions, the COR will place a telephone call to the designated contractor support point of contact to request repair service. The COR will provide the equipment identification number, a brief description of the malfunction, the location of the De-ionized Water Purification System, and the name and telephone number of the COR identified Government Point of Contact (POC).
1.3.4.1. Emergency Maintenance (EM) – Contractor Response Back. The contractor shall respond back to the COR’s phone call within four (4) work hour and the contractor’s technician shall arrive to Tinker AFB OK at the downed equipment location within eight (8) work hours after telephone notification of malfunctions.
1.3.4.2. Equipment Evaluation. The contractor shall evaluate, troubleshoot, and diagnose the De-ionized Water Purification System(s) to isolate the cause of its described malfunction.
1.3.4.3. Repaired Systems. The contractor shall repair and return the De-ionized Water Purification System(s) to operational status within twenty four (24) work hours in accordance with manufacturer’s specifications.
1.3.4.4. Replacement Systems. The contractor shall be responsible for transporting a comparable replacement to Tinker AFB OK at no additional cost to the government at the time of removal of the malfunctioning De-ionized Water Purification System(s).
1.3.4.5. Operations Demonstration. After completion of the EM, the contractor shall demonstrate to the COR identified Government Technical POC that the repairs have been completed.
1.4. Quality Assurance. According to the contract’s inspection clause, the Government will evaluate the contractor’s performance under this contract. The COR will participate in the administration of this contract. Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the Contracting Officer (CO). All services to be performed by the contractor during the period of this contract will be subject to review by the CO or COR.
1.4.1. Correction Action Report (CAR)/Customer Complaint Response Time. The contractor shall provide written response addressing the root cause, corrective action and preventive action(s). The contractor will be given two (2) work days to answer a submitted CAR or Customer Complaint.
1.5. Environmental Protection & Safety. The contractor shall be knowledgeable of and comply with all applicable federal, state and local laws, regulations, instructions and requirements regarding environmental protection and occupational safety and health.
The contractor shall comply with any laws, instructions or regulation changes during the term of this contract.
1.5.1. Compliance. Contractors shall be compliant with Air Force Instruction (AFI) 32- 7086, “Hazardous Materials Management” and Tinker AFB procedures per Tinker Air Force Base Instruction (TI) 32-7004, “Hazardous Waste Management”.
1.5.2. Material Safety Data Sheet/Safety Data Sheet (MSDS/SDS). The contractor shall not transport or use any chemical, material or substance used in the performance of this PWS on Tinker AFB before written approval is provided by 72 ABW/CEAN, Natural Infrastructure Management (Environmental Compliance). The contractor shall submit to the contracting officer representative (COR), ten (10) calendar days after contract award date all material safety data sheets (MSDSs) or safety data sheets (SDSs). Contractor shall comply with the effective dates for SDS requirements in accordance with the Occupational Safety and Health Administrations (OSHA) Hazardous Communication Standard, Title 29, Code of Federal Regulation (CFR) Part 1910.1200. The COR will forward all MSDS/SDS submittals to 72 ABW/CEAN for approval. 72 ABW/CEAN will determine any additional material requirements based on the hazard code of the material. Contractor shall maintain copies of manufacturer MSDSs/SDSs for all chemicals and MSDSs/SDSs shall be readily accessible at all times by contractor personnel as well as government employees.
1.5.3. Hazardous Material (HAZMAT). The contractor shall not store chemicals or hazardous material (HAZMAT) on Tinker AFB. Contractors shall be compliant with HAZMAT management procedures per Air Force Instruction (AFI) 32-7086, “Hazardous Materials Management”. The contractor shall maintain a log of all HAZMAT that is used on base. This usage log shall be provided to the contracting officer representative (COR) monthly, the last business day of each month, or when the required work is completed.
1.5.4. Hazardous Waste. The contractor shall dispose of contaminated solid waste, spent or processed chemical solutions, used rags and hazardous waste in government furnished waste containers located in the shop or area the work is being performed.
The contractor shall notify the contracting officer representative (COR) if the required waste containers are not available on site.
1.5.5. Discharges. Any discharges into Tinker’s industrial waste treatment plant (IWTP) or sanitary sewer will be approved and done in accordance with (IAW) Tinker Industrial Waste Water Discharge Permit as instructed by 72 ABW/CEAN, Natural Infrastructure Management (Environmental Compliance). The contracting officer representative (COR) will submit the required notice of intent documentation to the unit environmental coordinator (UEC) for 72 ABW/CEAN approvals. The contractor shall maintain records of written approval issued by 72 ABW/CEAN for discharges into IWTP and sanitary sewer.
1.5.5.1. Off-Site Discharges. For discharges outside of Tinker AFB, the contractor shall ensure waste water discharges, that may contain pollutants resulting from any contracted service, go to a state or federal permitted treatment facility such as a municipal Publicly Owned Treatment Works (POTW). The contractor shall provide a letter ten (10) calendar days after contract award date to the COR from the POTW that states the POTW is permitted for all pollutants that the contractor may discharge during the contracted service.
1.5.6. Spills. Upon occurrence of any spill that could enter the storm system or cause any harmful environment effects, the contractor shall immediately call 911 to report the incident. When possible, the contractor shall use a base telephone to receive help from Tinker AFB 911. If using a cell phone, the contractor shall request Tinker AFB 911.
The contractor shall immediately report all environmental violations to the contracting officer representative (COR) and unit environmental coordinator (UEC). The contractor shall provide five (5) copies of a typed or written report about the incident within four (4) clock hours as follows:
1.5.6.1. Spillage Records. One (1) copy to the contracting officer (CO), two (2) copies to the COR, a copy for Unit Environment Coordinator (UEC) and a copy for contractor’s record. The format of the report shall be left up to the contractor, but the report shall contain the following information:
1.) Name and contact number of person reporting the spill 2.) Date and time of incident
3.) Location and source of spill 4.) Substance or pollutant spilled 5.) Amount spilled and rate of discharge 6.) Any damages or injuries involved 7.) Extent of area impacted 8.) Potential hazards 9.) Actions taken
10.) Organizations contacted 11.) Remarks Section - that addresses the cause of the spill (e.g. human error, equipment, etc.) and whether 911 was called and which agencies responded.
1.5.7. Environmental Training. The contractor personnel performing duties in the service of this PWS shall have training that acquaints the person with the details of Resource Conservation Recovery Act (RCRA) regulations in 40 CFR 260 through 279;
applicable State Laws and regulations; DoD, Air Force and local requirements. This training can be obtained through creditable training institutions or companies. All training shall be approved by 72 ABW/CEAN, Natural Infrastructure Management (Environmental Compliance). RCRA training is available at http://www.natlenvtrainers.com/main.htm and http://www.access-safety.net/EPARCRA (These are examples of companies that offer training to meet RCRA regulation requirements) at the contractor’s expense. Prior to obtaining training, the contractor shall provide the name of the training institution or company and course description to the contracting officer representative (COR) for approval within ten (10) calendar days after contract award date. Once written approval is received from the COR, The contractor shall provide the environmental training certificates for employees performing duties in the service of this PWS to the contracting officer representative (COR), within thirty (30) calendar days after contract award date. In addition to the required environmental training, contractors shall be compliant with Tinker AFB procedures per Tinker Air Force Base Instruction (TI) 32-7004, “Hazardous Waste Management”.
1.6. Health and Safety on Government Installations. In performing work under this contract on a Government installation, the contractor shall comply with federal, state and local regulations and AFI 91-203 Air Force Consolidated Occupational Safety Instruction as established by this contract and Appendix C. The contractor shall notify the CO of any injuries that occur to contractor personnel while on the installation. The contractor shall notify the CO of damage to government property or equipment during the execution of the contract.
1.7. Housekeeping. The contractor shall keep the work areas clean and neat in accordance with Industry Standards, Occupational, Safety and Health Administration (OSHA), fire and safety standards and this PWS.
1.8. Foreign Object Damage (FOD) and Dropped Object Prevention (DOP). The contractor personnel who, in performance of their assigned duties on this contract, work in or travel though areas near operational and production aircraft, engines, munitions, missiles, drones, space systems, support equipment, Aerospace Ground Equipment http://www.natlenvtrainers.com/main.htm http://www.access-safety.net/EPARCRA
(AGE), trainers and the contactor personnel operating vehicles and equipment on flight lines, runways, taxiways, parking ramps and in aircraft hangers and maintenance areas are required to receive FOD and DOP training and to comply with, Air Force Materiel Command (AFMC) Sup1, Chapter 14, paragraph 14.41 and TAFBI 21-449, Chapter 4.
1.8.1. Certificate for Foreign Object Damage (FOD) and Dropped Object Prevention (DOP) Certificate. The contractor and personnel shall acknowledge the Foreign Object Damage (FOD) and Dropped Object Prevention (DOP) training by viewing the DVD at http://www.youtube.com/watch?v=tYcbodjFPnM and providing a contractor devised certificate to the COR for each employee within ten (10) calendar days of contract award. The certificate(s) shall have the title of the training, name of the person who received the training and the date. The contractor shall also ensure all new employee(s) view the FOD/DOP Training DVD and provide a contractor devised certificate of training to the COR prior to starting work in the OC-ALC shops. Annually, the contractor and personnel shall be required to review the FOD/DOP Training DVD and provide an updated contractor devised certificate of training to the COR.
1.9. Tool Control and Accountability. While performing contract services in or around Maintenance Production Shops including Ramp areas, the contractor shall comply with AFI 21-101_AFMC Sup 1, 21-101, Chapter 10 (all paragraphs) and OC-ALCI 21-112, paragraph 14.1 & 14.5.3. The contractor or contractor’s personnel shall inventory the container or bag of tools at the beginning of each work day and at the end of each work day. The contractor shall turn in a Contractor Tool Listing, MXRIWRS Form 32 checklist to the COR prior to starting work in the OC-ALC Industrial Areas. The contractor personnel shall immediately notify the COR of any tools which are missing or lost from the work area or container (i.e. tool or job box, or tool bag). The COR will report to appropriate Government personnel of any missing Contractor’s tool(s) not located within one (1) hour after notification of lost or missing tools. The contractor can obtain the Form 32 from the COR.
1.10. Housekeeping/FOD/Tool Control Briefing. The Contractor Manager shall ensure each employee on the job site is briefed on Housekeeping, FOD and Tool Control prior to starting work in OC-ALC Industrial Areas from the COR.
1.11. Contractor Personnel. The Contractor shall provide the name of the Contract Manager (CM) and any alternate(s) who shall act for the contractor when the manager is absent and they shall be designated in writing to the COR within five (5) work days after contract award. The contractor shall provide telephone numbers and email addresses of the CM and alternate(s) where these persons may be contacted at any time. The CM or alternate shall have full authority to act for the contractor on all matters relating to operation of this contract at Tinker AFB. The CM or alternate shall be available to meet on the installation with Government personnel designated by the CO to discuss problem areas. The contractor shall provide a CM who can read, write, speak and understand English fluently.

1.11.1. The contractor and all their employees shall be required to identify themselves as contractor personnel by introducing themselves or being introduced as contractor personnel and displaying distinguishing badges or other visible identification for meetings with Government personnel. In addition, the contractor and all their employees shall identify themselves as contractor employees in telephone conversations and in formal and informal written correspondence.
1.11.2. Listing of Contractor Employees. The contractor shall provide an initial listing of contractor employees’ names and position titles to the COR within five (5) work days after contract award. The contractor shall also provide an updated listing of personnel to the COR when employees affecting the work of this contract, status changes or position changes within five (5) work days.
1.11.3. The contractor shall not employ persons on this contract if such employee is identified to the contractor as a potential threat to the health, safety, security and/or general well-being or operational mission of the installation and its population. The contractor shall not employ any person who is an employee of the United States Government if the employment of the person would create a conflict of interest, nor shall the contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to the DoD 5500.7-R Joint Ethics Regulations (JER). In addition, the contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in AFI 64-106, Air Force Industrial Labor Relations Activities. The contractor shall be prohibited from employing off-duty CORs who are providing surveillance on any contracts/subcontracts awarded to the contractor. All contract employees depending on particular condition, occasion or place or job assignment shall require professional, suitable attire. All contractor employees shall conduct themselves in a professional, courteous manner.
The selection, assignment, reassignment, transfer, supervision, management and control of contractor employees in performance of this PWS shall be the responsibility and prerogative of the contractor; however, the contractor shall comply with the requirements set forth in this PWS and Tinker AFB regulations concerning conduct of employees as referenced herein. The contractor shall employ and maintain a technically trained and experienced work force.
1.11.4. Foreign National(s). The contractor shall notify the COR before sending a Foreign National representative to perform services. Special rules apply for foreign national(s) visiting a US Government facility.
1.12. Non-Common Access Card (CAC) Holders. Non-CAC Card holders shall complete a “Request for Identification Credential” (AFMC Form 496) and submit it to Pass and Registration. The Government will provide a completed "Identification Credential" (Defense Biometric Identification Data System (DBIDS) ID, which shall be issued, displayed and surrendered in accordance with Installation Integrated Defense Plan, Tinker Plan 31-101, Installation Security Program (in its entirety).
1.12.1. Contractor IDs. All badges shall be surrendered when the contract expires, when no longer or when the contract personnel no longer work for the contract/Contractors. IDs expire on the last day of the current contract. If contract is extended, they must get new IDs. This includes the CAC ID, the new DBIDS ID and AF Form 1199C Controlled/Restricted Area Badge.
1.12.2. CAC Cards Return. All CAC cards shall be returned to the Pass and ID, Building 6611, upon the completion of the contract. The CAC card shall be issued displayed and surrender in accordance with Installation Integrated Defense Plan, Tinker Plan 31-101, (in its entirety). The contractor shall provide the COR with copies of the turn-in receipts by either US Postal Service or email within ten (10) days of return.
1.12.3. CAC Cards Assessment. Contractors are assessed a fine of $50.00 if CAC IDs, AF Form 1199C’s AFMC Form 387s or DBIDS IDs are lost, stolen or not returned to the Pass and ID, Building 6611 when expired or no longer required. 72 SFS will attempt to fine the contract if money cannot be collected from the contractor.
1.13. Privately Owned Motor Vehicles (POV). Contractor personnel shall comply with the directives pertaining to operation of POVs on Tinker AFB OK per AFMAN 31-116 (in its entirety), Motor Vehicle Traffic supervision. The contractor shall comply with TAFBI 24-301 (in its entirety) for the operation of POVs within OC-ALC Complex.
1.14. Emergency Procedures. Contractor employees shall follow the direction of Government employees in regards to emergency procedures, i.e. fire, tornado, active shooter, bomb threats. It is the responsibility of the contractor to provide guidance and establish procedures for responding to emergency for their employees. It is the contractors' responsibility to account for their employees following an emergency evacuation. Contractor Personnel shall participate with Government personnel in all fire and tornado drills and no additional cost to the government.
1.15. Forms, Technical Orders (T.O.) Air Force Instructions (AFI), Air Force Material Command Instruction (AFMCI), and Publications. The contractor shall obtain the forms and publications expressly required to perform the work in this contract via internet at www.e-publishing.af.mil. In the event a form or publication is not available online, the COR will provide the form, instruction, or publication to the contractor upon written request (e-mail is acceptable) by the contractor. In the event laws, regulations, TO’s, Air Force Instructions (AFI’s), Air Force Material Command Instructions (AFMCI’s) change during the term of this contract, the contractor is required to comply as such changes come into effect. Contractors delivering or handling official USAF records shall comply with the applicable records management regulations, AFI 33-322, all of Chapter 3.
1.16. Continuation of Essential DOD Contractor Services During Crisis. This requirement has been determined not to be essential and does not require continued support during a crisis as defined in DFARS 252.237-7023.

1.17. Training – General. Contractor employees shall complete all required training, to include but not limited to the list below, within thirty (30) work days of the start of performance date and/or contractor employee hire date if the hire date is after the period performance start date. The COR will coordinate the training with the contractor and training monitor at the government expense. The contractor shall notify the COR in writing within five (5) work days of new employee who will be working under this contract.

1.17.1. Receipt of Training - General. The contractor shall receive training provided by the government either in a classroom, video or by the Government COR. The government will be responsible for all approved costs associated with the training identified above in Table 1-1. The COR will document training on and/or maintain copies of completed AF Form 1155s as part of the contract surveillance file.
1.18. Contractor Manpower Reporting. The Contractor shall report ALL Contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the United States Air Force via a secure data collection site. The Contractor is required to completely fill in all required data fields at http://www.ecmra.mil.
1.18.1. Reporting Data Input. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs, 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year. 2. SERVICE SUMMARY (SS).
2.1. Performance Assessment. The contractor shall be aware that in the absence of a n y contract requirement from the SS does not detract from its enforceability nor limit the rights or remedies of the Government under any other provision of the contract. The contractor shall ensure the following service summary requirements are accomplished:

3) Technical Requirements for all systems are to produce water that has less than 5 ppm (parts per million) TDS (total dissolved solids) in final water produced.
4) Set-up, install and demonstrate to the Contracting Officer Representative (COR) identified Government Point of Contact (POC) and the COR that all De-ionized Water Purification Systems delivered are properly functioning in accordance with manufacturer’s equipment specifications and this Performance Work Statement
(PWS).
5) Replace the exhausted ion exchange cylinders with fully recharged cylinders when limit in no. 3 above is exceeded. The schedule in Appendices A1 and A2 (the last column) shows the estimated annual usage of each DI water system at each location.
Water quality will be tested by the government periodically. Upon any over limit findings, the COR will notify the contractor to replace the cylinder/tank set with a fresh set.

The contractor shall provide written response addressing the root cause, corrective action and preventive action(s). The contractor will be given two (2) work days to answer a submitted CAR or Customer Complaint.

Material Safety Data Sheet/Safety Data Sheet (MSDS/SDS). The contractor shall not transport or use any chemical, material or substance used in the performance of this PWS on Tinker AFB before written approval is provided by 72 ABW/CEAN, Natural Infrastructure Management (Environmental Compliance). The contractor shall submit to the contracting officer representative (COR), ten (10) calendar days after contract award date all material safety data sheets (MSDSs) or safety data sheets (SDSs). Contractor shall comply with the effective dates for SDS requirements in accordance with the Occupational Safety and Health Administrations (OSHA) Hazardous Communication Standard, Title 29, Code of Federal Regulation (CFR) Part 1910.1200.
The COR will forward all MSDS/SDS submittals to 72 ABW/CEAN for approval. 72 ABW/CEAN will determine any additional material requirements based on the hazard code of the material. Contractor shall maintain copies of manufacturer MSDSs/SDSs for all chemicals and MSDSs/SDSs shall be readily accessible at all times by contractor personnel as well as government employees

Off-Site Discharges. For discharges outside of Tinker AFB, the contractor shall ensure waste water discharges, that may contain
1.5.5.1. 100% pollutants resulting from any contracted service, go to a state or federal permitted treatment facility such as a municipal Publicly Owned Treatment Works (POTW). The contractor shall provide a letter ten (10) calendar days after contract award date to the COR from the POTW that states the POTW is permitted for all pollutants that the contractor may discharge during the contracted service.
Environmental Training. The contractor personnel performing duties in the service of this PWS shall have training that acquaints the person with the details of Resource Conservation Recovery Act (RCRA) regulations in 40 CFR 260 through 279; applicable State Laws and regulations; DoD, Air Force and local requirements. This training can be obtained through creditable training institutions or companies. All training shall be approved by 72 ABW/CEAN, Natural Infrastructure Management (Environmental Compliance). RCRA training is available at http://www.natlenvtrainers.com/main.htm and http://www.access-safety.net/EPARCRA (These are examples of companies that offer training to meet RCRA regulation requirements) at the contractor’s expense. Prior to obtaining training, the contractor shall provide the name of the training institution or company and course description to the contracting officer representative (COR) for approval within ten (10) calendar days after contract award date. Once written approval is received from the COR, The contractor shall provide the environmental training certificates for employees performing duties in the service of this PWS to the contracting officer representative (COR), within thirty (30) calendar days after contract award date. In addition to the required environmental training, contractors shall be compliant with Tinker AFB procedures per Tinker Air Force Base Instruction (TI) 32-7004, “Hazardous Waste Management”.

Certificate for Foreign Object Damage (FOD) and Dropped Object Prevention (DOP) Certificate. The contractor and personnel shall acknowledge the Foreign Object Damage (FOD) and Dropped Object Prevention (DOP) training by viewing the DVD at http://www.youtube.com/watch?v=tYcbodjFPnM and providing a contractor devised certificate to the COR for each employee within ten
(10) calendar days of contract award. The certificate(s) shall have the title of the training, name of the person who received the training and the date. The contractor shall also ensure all new employee(s) view the FOD/DOP Training DVD and provide a Contractor devised certificate of training to the COR prior to starting work in the OC-ALC shops.
Annually, the contractor and personnel shall be required to review the FOD/DOP Training DVD and provide an updated contractor devised certificate of training to the COR.

Contractor Personnel. The Contractor shall provide the name of the Contract Manager (CM) and any alternate(s) who shall act for the contractor when the manager is absent and they shall be designated in writing to the COR within five (5) work days after contract award. The contractor shall provide telephone numbers and email addresses of the CM and alternate(s) where these persons may be contacted at any time.
The CM or alternate shall have full authority to act for the contractor on all matters relating to operation of this contract at Tinker AFB. The CM or alternate shall be available to meet on the installation with Government personnel designated by the CO to discuss problem areas.
The contractor shall provide a CM who can read, write, speak and understand English fluently.

Listing of Contractor Employees. The contractor shall provide an initial listing of contractor employees’ names and position titles to the COR within five (5) work days after contract award. The contractor shall also provide an updated listing of personnel to the COR when employees affecting the work of this contract, status changes or position changes within five (5) work days.

Non-Common Access Card (CAC) Holders. Non-CAC Card holders shall complete a “Request for Identification Credential” (AFMC Form
496) and submit it to Pass and Registration. The Government will provide a completed "Identification Credential" (Defense Biometric Identification Data System (DBIDS) ID, which shall be issued, displayed and surrendered in accordance with Installation Integrated Defense Plan, Tinker Plan 31-101, Installation Security Program (in its entirety).

Contractor Manpower Reporting. The Contractor shall report ALL Contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the United States Air Force via a secure data collection site. 3. GOVERNMENT FURNISHED PROPERTY AND SERVICES.
3.1. Government Furnished Property (GFP). GFP is not applicable to this contract in accordance with FAR Parts 45 and 52.245 plus supplements.
3.2. Shared Property of the Government. Not Applicable
3.3. Government Furnished Services (GFS). Government Furnished Services are applicable during the performance of this contract. The government will furnish the following service at Tinker AFB, OK.
3.3.1. Security Forces. The government will provide general on-base security forces’ services. The security forces’ telephone numbers are 911 for emergencies, 734-2000 for crime in progress, and 734-3737 for non-emergency calls.
3.3.2. Fire and Emergency Services. The government will provide general on-base fire protection services. The contractor shall comply with fire prevention rules and regulations enforced by the Tinker AFB Fire Prevention Branch. Call 911 for emergencies. For routine fire prevention matters call 734-3981. For routine calls to dispatch call 734-7964.
3.3.3. Utilities. The Government will furnish utility services. These utilities include natural gas, electricity, water, and sewer for the sole purpose of accomplishing this contract. If the contractor requires additional utility services, it shall be the contractor’s responsibility to provide. The contractor shall use due care and diligence in efforts to conserve utilities to reduce utility costs.
3.3.3.1. De-ionized Water. The government will provide the water to be de-ionized for performance of the maintenance support services to the contractor.
3.3.4. Refuse Collection. The government will provide refuse collection services limited to the extent provided in the Base Refuse Collection Contract(s) for government provided facilities. The government will only allow non-hazardous refuse to be emptied into the nearest authorized refuse dumpster for trash collection. Do not use the government dumpsters to dispose of construction debris.
3.3.6. Workspace. The Government will provide adequate workspace around the equipment area.
3.3.7. Training-General. The Government will provide training either in a classroom, video or by the Government COR for required contractor’s training as stated in Para.
