PERFORMANCE WORK STATEMENT (PWS)
Department of the Air Force (AF) August 2017 Task Force True North (TFTN) Resiliency and Prevention Support Contract
1.0. BACKGROUND
The Air Force Task Force True North’s (TFTN) mission is to develop and test effective models to address the seams and shortfalls with resources/capabilities in order to achieve the goals of optimizing Airman performance, increase resilience of the force and their families, and decrease negative and violent outcomes. This specific contract requirement includes the establishment and support of integrated inter-disciplinary personal services teams consisting of Government Program Management personnel and Contractor support. Contractor support will be organized to ensure embedded mental health, & medical operational support services at the wing and group level under the management of the Government Contracting Officer Representative (COR).
1.1. SCOPE
1.1.1. This effort to establish capability supporting Resiliency and Prevention programs levies the following duties and responsibilities on the contractor:
1.1.1.2. Embedded Mental Health Teams.
1.1.1.2.1 Mental Health Technician. The contractor shall assist with the management and treatment of outpatient behavioral health activities in the operational unit primarily and the mental health clinic occasionally. The Mental Health Technician is primarily responsible for providing a wide range of behavioral health activities for individuals and groups, including education, prevention, and treatment. Under the supervision of a credentialed provider, the contractor conducts intakes and other forms of psychological assessments and psychometric tests, assists with care and treatment of psychiatric, drug and alcohol patients, counsel’s clients/patients with personal, behavioral or psychological problems, and assists with command consultation. The contractor accomplishes work on a substantially independent basis, performing a full range of standard and nonstandard work assignments and resolving a variety of non-recurring problems encountered on own initiative. Extensive guidelines are available; however, the contractor must use judgment in locating and selecting the most appropriate guideline for application to the work situation at hand. Unusual matters which may not be specifically covered by policy statements or published regulations shall be referred to licensed behavioral health staff. The contractor shall observe patients to detect behavior patterns and reports observations to licensed behavioral health staff. The contractor leads prescribed individual or group therapy sessions as part of specific therapeutic procedures under the guidance of a licensed mental health professional.
Depending on the location and type of assignment, the contractor may be required to perform one or more of the following tasks:
· Populate and maintain a computerized database to compile monthly statistical data to track the delivery of the team’s services, the team’s training requirements, the team’s productivity, and the team’s outcome measures.
· Prepare daily correspondence (i.e., training agendas, meeting agendas, purchase orders, clinical appointments).
· Prepare and maintain attendance rosters and statistical data for individuals who received the team’s services of all varieties.
· Prepare PowerPoint slide presentations as directed.
· Assists in developing program operating instructions and works with providers, to uncover inefficiencies and makes recommendations for corrective action.
· Manages and monitors inspection checklists
· Makes comparison reviews, to include office time sheets, to ensure program operations are managed within budget.
· The Mental Health Technician is responsible for the development and implementation of appointment templates and schedules for clinical providers.
· The Mental Health Technician also places physical maintenance work orders and maintains hand receipts for physical property and inventory.
· Duties also involve assisting or independently gathering data for various presentations such as process improvements initiatives, longitudinal statistical tracking, hospitalizations, and discharges. Manipulates data and presents it using off the shelf software applications.
· Provides information via telephone and in person to the members of the Command, the professional staff, civilian and military personnel in response to inquiries regarding appointments and services.
· Interfaces frequently with base helping agencies and other organizations.
· Maintains competency folders in accordance with program policy.
· Coordinates attendance and agenda for meetings as directed.
· Coordinates and reserves meeting locations, verifies attendance, and prepares agendas.
· Prepares open and closed case reports for required special handling of outpatient medical records in accordance with regulations.
· Orders and annotates case reviews in outpatient medical records
· Maintains a resource library of virtual and physical items for use by professional staff and customers.
· Library materials are selected with input from professional staff, command staff and customers.
· Makes recommendations to users of the resource library on materials that best meet their needs.
1.1.1.2.2 Licensed Clinical Social Worker (LCSW). The contractor shall function as a Behavioral Health Care Provider within the operational unit primarily and the mental health clinic occasionally. The contractor provides a wide range of behavioral health activities, including education, prevention, treatment and consultation to commanders, individuals, and groups. The contractor shall attend and participate in meetings during normal duty hours, professional staff conferences in the MTF and other appropriate professional activities such as, but not limited to the following: quality improvement meetings, professional staff meetings, commander's staff meetings, and other meetings required by applicable regulations or as directed by the unit Commander of his/her designee. The contractor shall conduct applied research and clinical investigations in clinical/behavioral health/organizational social work.
The contractor is responsible for the application of social work procedures and techniques, including interviewing, behavioral assessment, and evidenced-based therapies, in the evaluation, diagnosis, and treatment of psychological disorders using the following skills: individual, family and group psychotherapy, couples therapy, alcohol and drug treatment evaluations.
The contractor consults with medical personnel, legal authorities, military commanders, other helping agencies on the installation, and school districts as required. The contractor shall maintain accurate and current notes in the paper medical records (if created) and electronic medical records of all patients seen, as appropriate, and produces reports of evaluation and/or treatment as required. The contractor shall participate in military specific training/activities (e.g., security clearances, use of the DoD or service specific clinical practice guidelines, pre-post deployment screening, PTSD, and combat stress) and others as directed by the unit Commander or his/her designee. This position will include clinical supervision of the team mental health technician. The contractor may be required to perform the following some, or all, of the following activities:
· Participates in and successfully completes required DoD or service required training.
· Contact referred patients to assess treatment adherence.
· Use validated symptom inventories to assess current symptom severity.
· Assess barriers to treatment adherence and help patient problem-solve solutions to barriers.
· Provide patients with education regarding their mental health condition and treatment regimen.
· Maintain accurate and current notes in the electronic medical records of all patients
· Perform occupational assessments and provide written documentation as directed (e.g., PRP paperwork, medical profile reports, medical evaluation board reports, commander directed evaluations, etc.)
· Routinely staffs difficult cases with the referring provider, another mental health provider, the base Director of Psychological Health, or the base Chief of the Medical Staff (SGH).
· Performs case management functions.
1.1.1.3 Base Operational Support Team.
1.1.1.3.1 OST Clinical Psychologist. -The contractor shall provide clinical psychology services in the Medical Group Mental Health Clinic. The duties may include but are not limited to the following:
· Clinical/Counseling Psychology. Responsible for and applies psychological procedures and techniques, including interviewing, behavioral assessment, evidenced-based therapies, and psychological testing/psychodiagnostic testing, in the evaluation, diagnosis, and treatment of psychological and neuropsychological disorders. The following are desirable skills:
· Individual, family and group psychotherapy, and couples therapy, alcohol and drug treatment evaluations.
· Consults with medical personnel, legal authorities, military commanders and school districts, as required.
· Maintain accurate and current notes in both the Mental Health records and inpatient medical records of all patients seen, as appropriate, and produces reports of evaluation and/or treatment, as required.
· Participates in military specific training (e.g., training to perform Command Directed Evaluations, security clearances, Military Training Instructor clearances, pre-post deployment screening, PTSD, and combat stress)
· Attend and participate in meetings during normal duty hours and professional staff conferences and other appropriate professional activities such as, but not limited to the following: Quality Improvement meeting, professional staff meetings, Commander's staff meetings, Mental Health Flight meetings, and others required by applicable regulations, MTF guidance, or as directed by the Mental Health Flight Commander or his/her designated representative.
· Conduct applied research and clinical investigations in clinical/behavioral health psychology
1.1.1.3.2 OST Licensed Clinical Social Worker (LCSW) - The contractor shall provide clinical social work services in the Medical Group Mental Health Clinic. The duties and responsibilities include but are not limited to the following:
· Applying accepted social work procedures and techniques in the evaluation and treatment of psychological/psychiatric disorders. Activities may include, but are not limited to the following:
· Providing individual, family and group psychotherapy, and couples therapy.
· Consulting with medical personnel, legal authorities, military commanders and school districts, as required.
· Maintaining accurate and current notes in both the Mental Health records and patient medical records of all patients seen, as appropriate, and produce reports of evaluation and/or treatment, as required.
· Participating in professional peer review case conferences and other organized meetings.
· Perform occupational assessments and provide written documentation as directed (e.g., PRP paperwork, medical profile reports, medical evaluation board reports, commander directed evaluations, etc.)
1.1.1.3.3 Exercise Physiologist – The contractor shall provide workplace centered exercise physiology services aimed at improving musculoskeletal resilience, optimizing Airmen physical fitness and performance in the operational environment, and reducing risk of musculoskeletal injury. The contractor may be required to perform some, or all, of the following activities:
· The contractor will observe Airmen working in the operational environment to understand the mission, identify occupational musculoskeletal and/or cardiovascular stressors, and build rapport with unit members.
· Provide education which may include topics covering: nutrition, physical activity, sleep, and fatigue.
· Provide occupation-specific group instruction which may include techniques to enhance musculoskeletal performance and reduce risk of musculoskeletal injury.
· Recommend changes to work-center and work-center culture to mitigate identified risks for musculoskeletal injury (i.e. Air Force Specialty Code-specific fitness programs, routine duty-related musculoskeletal conditioning/”warm-up” techniques, etc.).
· Provide performance enhancement testing, functional movement evaluation, and exercise prescriptions, as needed.
· Advise commanders, supervisors and individual Airmen on techniques to elevate overall physical fitness levels, improve unit physical fitness testing performance and decease risks of occupational injuries.
· Collaborate with the OST Physical Therapist in the identification of unit-specific physical and/or ergonomic stressors that increase risk of mission related occupational injury.
· Coordinate with base OST Physical Therapist to facilitate appropriate healthcare services following injury and conduct monitoring and periodic reporting on indicators related to health and human performance.
· Assist with occupational health surveillance and trend monitoring during time limited unit interventions.
1.1.1.3.4 OST Physical Therapist - The contractor may be required to perform some, or all, of the following activities:
· Perform a full range of physical therapist services in accordance with the scope of clinical privileges granted by the Medical Treatment Facility (MTF).
· Provide appropriate therapeutic procedures and provide a full range of therapy services in support of referral patients.
· Test and measure patients; strength, motor development, sensory perception, functional capacity, and/or respiratory and circulatory efficiency.
· Plan and prepare written treatment programs based on an evaluation of the patient.
· Record findings to revise treatment programs
· Administer manual exercises to improve and maintain function.
· Instruct motivate and assist patients in performing various physical activities, such as non-manual exercises, ambulatory functional activities, daily-living activities, and in use of assistive and supportive device, such as crutches, canes and prostheses.
· Administer treatments involving application of physical agents, using equipment such as a pulsed lavage unit and/or whirlpool bath, moist packs, ultraviolet and infrared lamps, and ultrasound machines. Evaluate effects at various stages and adjust treatments to achieve maximum benefit.
· Administer soft tissue mobilization, applying knowledge of mobilization techniques and body physiology.
· Administer traction to relieve pain, using traction equipment.
· Record treatment response in patients’ charts and/or automated systems.
· Instruct patients and families in treatment procedures to be continued at home.
· Evaluate, fit and adjust prosthetic and orthotic devices and recommend modification, as required.
· Coordinate treatment with physicians and other staff members to obtain additional patient information, suggest revisions in treatment programs and integrate physical therapy treatment with other aspects of the patients’ health care. Contact referring physicians regarding patient care concerns, as required.
· Provide documented treatment and discharge recommendations to members of the staff in routine, emergency, and special cases, as needed.
· Attend and provide input at rehabilitation team meetings, seminars, and quality assurance meetings.
· Participate in peer review activities.
· Provide clinical supervision to assistants, technicians, aides, and students. (note: “supervision”, as used in this PD, means clinically-related supervision only, and does not include inherently governmental supervisory duties, including, but not limited to, personnel and employment-related activities, i.e. timecard or leave approval).
· All patient contact and care shall be safe, timely, result in achievement of realistic and documented treatment goals, and comply with or satisfy the intent of the referring medical staff.
1.1.1.3.5 Human Performance Information Manager - The contractor may be required to perform some, or all, of the following activities:
· Conduct data collection and reporting to the 711th Human Performance Wing Operational Support Team (HOST) data analytics cell following guidance provided by HOST
· Provide required IT and administrative support for Base OST team
· Provide occupational health surveillance, limited epidemiologic support and trend monitoring
· Conduct monitoring and periodic reporting on indicators related to health and human performance
· Participate in initial and on-going training, continuing education, workshops, lectures, and seminars to maintain required certifications and program requirements
· Ensure compliance with applicable laws (HIPPA, Privacy Act, etc.) and all applicable AF instructions governing privacy and security of patient data.
· The contractor may be required to travel outside of local area for events that include trainings, seminars, conferences, briefings, interventions and Geographically Separated Units. Travel could include commercial or military aircraft or vehicle. The Information Manager may be required to work other than normal duty hours and normal duty days to support unique mission requirements and shift work. The Information Manager may also be required to work at various locations and environments to effectively address targeted population.
1.1.2. Professional Qualifications. The contractor shall ensure qualified professionals are placed at each location; the qualified professionals meet standards of practice as defined by the MTF or other source of clinical oversight and provide timely and effective solutions to problems employing evidenced-based practices (e.g., outlined by DoD/VA clinical practice guidelines) to ensure quality services are provided; conduct thorough and timely pre-performance screening; provide complete and timely credential/privileges and security packages; and manage recruitment and retention of personnel. The following qualifications are required for the contractor to support this effort:
1.1.2.1. Embedded Mental Health (MH) Teams. The contracted embedded MH teams will operate within specified units at the designated locations (identified in section 2.1). The team’s purpose is to provide limited mental health services within the operational unit to improve access to services, reduce barriers to seeking emotional help, and decrease stigma. Each team will be composed of four licensed clinical social workers and one MH technician. (NOTE: Installation leadership, in coordination with the Installation and Headquarters Air Force TFTN Program Managers, may move licensed clinical social workers between teams in order to increase or decrease the team size based on the needs of the unit(s) being supported.) The team’s activities will include unit circulation, education, command consultation, unit assessments, team building exercises, providing resource and referral information, and limited scope counseling (LSC). Team members will educate unit personnel on psychological factors that impact resiliency, emotional health, physical health, social functioning, healthy coping, and effective problem solving. Team members will assist commanders in identifying and developing solutions to issues affecting the resiliency of their units (e.g., operational stressors, unit cohesion and morale, organizational climate). Team members will collaborate with base helping agencies to obtain needed services for unit and assigned Airmen and to coordinate team activities with those of other agencies to avoid confusion or redundancy. Team members will provide LSC to individual active duty members inside the operational unit following parameters defined by the government. Team members will be credentialed in the Military Treatment Facility (MTF) and will occasionally provide care to unit personnel and their family members in the MTF.
1.1.2.1.1 Mental Health (MH) Technician. The contractor shall possess an associate degree or commensurate level of training for a psychiatric technician or mental health program assistant. Education requirement may be met by military or comparable training. If military trained, the individual’s specialty must have been a mental health technician / specialist. In addition, all MH Technicians must have the following specialized training, education, and qualifications that prepare them for their duties:
· All mental health technician training includes a sound foundation / basis to psychology and/or social work which will be necessary to address the types of individuals and potential illnesses they will likely encounter.
· This training will also address the basics of conflict management and how to de-escalate crisis situations and individuals who are agitated, violent or potentially violent and / or suicidal.
· The contractor shall have no less than 3 years’ experience as a psychiatric technician.
· The contractor shall demonstrate military cultural competency, including the ability to assess and understand the occupation of the member and provide services relevant to the military member in the context of his/her job.
1.1.2.1.2 Licensed Clinical Social Worker (LCSW). In addition to required master’s level training and licensure, all LCSWs must have the following specialized training, education, and qualifications that prepare them for their duties:
· Must hold a Master of Social Work (M.S.W.) or equivalent (MSSW, MSSA) degree from a graduate school of social work fully accredited by the Council on Social Work Education (CSWE).
· Maintain a current, unrestricted clinical license to independently practice social work in any one of the 50 states, the District of Columbia, Puerto Rico, or the U.S. Virgin Islands.
· Have a minimum of two (2) years within the past five (5) years in the independent practice of clinical social work in a mental health setting. Experience working in a Government setting such a DOD or Department of Veterans Affairs (VA) MTF is preferable.
· Demonstrate military cultural competency, including the ability to assess and understand the occupation of the member and provide services relevant to the military member in the context of his/her job.
· Demonstrate a working knowledge of professional standards and ethics regarding the delivery of clinical social work services.
· Must have experience in brief behavioral interventions.
· The contractor is required to receive and maintain clinical practice privileges.
1.1.2.2. Base Operational Support Team.
The Base Operational Support Team (OST) is the Air Force Medical Service resource focused on improving mission performance of a squadron/unit by employing unit-level psychological and/or physical resilience enhancement and military occupational injury prevention interventions, with priority given to those units identified as having the highest risk. The base OST is a multidisciplinary team of medical personnel that will formulate and deliver time-limited, squadron-level health-related interventions to help maximize unit mission capability. The team is dedicated to sequential integration in targeted squadrons to enhance airman resiliency and squadron mission execution through evidence-based solutions to operational human factors problems within musculoskeletal and psychological domains. The contractor will provide personnel who will: 1) serve at the Military Treatment Facility (MTF) to enable designated uniformed personnel to leave the MTF and be members of the OST; or 2) be members of the OST.
1.1.2.2.1 Clinical Psychologist. (MTF backfill)
Qualifications
· Have a Doctor of Philosophy (Ph.D.) or (Psy.D.) degree in clinical or counseling psychology from an APA accredited psychology program.
· Shall have completed an APA accredited internship/residency in professional psychology.
· Have and maintain a current license to practice psychology in any one of the 50 states, the District of Columbia, Puerto Rico, or the U.S. Virgin Islands.
· Have a minimum of two (2) years within the past five (5) years in the independent practice of clinical psychology in a mental health setting working with an adult population.
1.1.2.2.2 Licensed Clinical Social Worker (LCSW). (MTF backfill) Qualifications
· Must hold a Master of Social Work (M.S.W.) degree from a school of social work fully accredited by the Council on Social Work Education (CSWE).
· Must possess and maintain a current, unrestricted clinical license to practice social work in any one of the 50 states, the District of Columbia, Puerto Rico, or the U.S. Virgin Islands. A clinical social work license is defined as a license that allows the Social Worker to practice independently without clinical supervision.
· Have a minimum of two (2) years within the past five (5) years in the independent practice of clinical social work in a mental health setting working with an adult population,.
· Must possess the knowledge and experience to use computer software applications for drafting documents, data management, and tracking, especially those programs in use by DoD.
· Demonstrate military cultural competency, including the ability to assess and understand the occupation of the member and provide services relevant to the military member in the context of his/her job.
1.1.2.2.3 Exercise Physiologist (OST member)
Qualifications
· Master’s degree from an accredited university in exercise physiology, kinesiology, exercise science or closely related field with curriculum focused on human performance, motor learning, biomechanics, exercise assessment, programming and evaluation.
· Certification in one the following: National Academy of Sports Medicine Corrective Exercise Specialist or Performance Enhancement Specialist, National Strength and Conditioning Association Certified Strength and Conditioning Specialist, American College of Sports Medicine Certified Exercise Physiologist, Certified Clinical Exercise Physiologist or Registered Clinical Exercise Physiologist.
· Ability to communicate effectively in both oral and written communication for the purpose of counseling, individual and group presentation and education, public speaking, briefings and trainings.
· Ability to use computer programs such as Microsoft Office in development of training materials and when preparing, evaluating and assessing on-going data collection efforts.
· Certification in Basic Cardiopulmonary Resuscitation with Automated External Defibrillator training.
1.1.2.2.4 Physical Therapist. (MTF backfill)
Qualifications
· A Masters or Doctoral degree in Physical Therapy from a physical therapy program accredited by the American Physical Therapy Association (APTA) and acceptable to the USAF Surgeon General.
· A current, unrestricted license to practice Physical Therapy in any one of the fifty states, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands.
· At least 24 months of active employment experience as a physical therapist within the past 36 months.
1.1.2.2.5 Human Performance Information Technician. (OST member) Qualifications
· Ability to communicate effectively in both oral and written communication for the purpose of counseling, individual and group presentation and education, public speaking, briefings and trainings.
· Ability to use computer programs such as Microsoft Office in data collection, communication, and report preparation.
· An understanding of military mission, operations, and culture
2.0. SPECIFIC REQUIREMENTS. The contractor shall provide the previously specified embedded mental health and medical operational support services to AF individuals and/or units and supporting staff and when appropriate to family members. The contractor shall ensure all health care professionals (HCPs) are certified and/or licensed by the appropriate professional level organizations and maintain professional competence through required continuing education courses as outlined within the PWS Attachment 1, Personnel and Qualification Listing.
2.0.1. In addition to the responsibilities and qualifications previously specified, the Contractor shall:
· Ensure that timely replacement of personnel is ensured to reduce any interruption of the requesting activity’s mission.
· Successfully implement the transition/recruitment plans/processes as proposed in the contract level Program Management proposal
· Provide accurate, current and complete data and reports that meet the required timeline
· Maintain and adequate accounting and purchasing systems
· Effectively recruit qualified personnel within the proposed labor category ceiling rates
· Contractor personnel will exhibit reasonable and cooperative behavior
· Ensure proper integration and coordination of all activities needed to execute the contract adequately - specifically the timeliness, completeness and quality of problem identification, corrective action plans, proposal submittals, the Contractor’s history of reasonable and cooperative behavior (to include timely identification of issues in controversy), customer satisfaction, timely award and management (for example: Be customer oriented; interaction between the Contractor and the Government will be satisfactory; be responsive; minimal Government technical direction required to solve problems that arise during performance; the quality of problem identification and corrective action plans, if applicable, will be focused on customer satisfaction
· Demonstrate understanding of all MTF standards, rules, and procedures including requirements for any licensure, credentialing and quality assurance requirements
· Effectively recruit, retain, manage, and ensure continuous training of employees, and replace, when necessary, contract personnel throughout the period of performance as required (for example: ensure all personnel have met all pre-employment requirements and are prepared to begin performance at the location, time, and date stated in the task order; proactively monitor personnel fill rates and vacancies and ensure timely replacement of personnel to reduce any interruption of the requesting activities mission, etc.)
· Provide an efficient process for Government review of candidate qualification and resume screening
· Facilitate efficient and timely coordination for personal interviews of candidates as required by the Government
· Remain vigilant for any performance related issues and take appropriate actions to ensure quick resolution
2.1. PLACE OF PERFORMANCE. For the purposes of this performance work statement (PWS) when support to the AF is specified this shall include Minot AFB, ND; Beale AFB, CA; Joint Base Elmendorf-Richardson, AK; and Whiteman AFB, MO.

2.2. HOURS OF OPERATION
Contractor shall work 40 hours per week on average across the year. The work day will generally be 8 hours. However, there will be days and weeks that require additional hours, resulting in fewer hours on other days or weeks. Work hours will be scheduled by the government with occasional variation from scheduled plans. Work hours will be scheduled by the government to cover duty operations as needed including day, evening and night hours. Contractors will attend meetings, trainings and events as scheduled by the government.
2.2.1. Government Holidays. The following Government holidays are normally observed by Government personnel: New Year’s Day, Martin Luther King’s Birthday, President’s Day, Memorial Day, Independence Day, Columbus Day, Veteran’s Day, Thanksgiving Day, Christmas Day, and any other day designated by Federal Statute, Executive Order, and/or Presidential Proclamation.
2.3. ORIENTATION AND TRAINING. When commencing work under this contract, the contractor personnel shall obtain an in-processing checklist(s) from the installation program manager. All personnel are required to complete the applicable items on the checklist and turn it in to the designated office. Upon completion of the duty assignment, the personnel shall complete an out-processing checklist and return it to the designated office. Personnel will be required to accomplish government provided newcomer’s orientation. Personnel will be required to accomplish training required by the Unit. Personnel will be required to accomplish orientation and training required by the MTF for affiliate providers. Orientation and initial training shall be completed within 21 calendar days.
2.3.1. The contractor shall ensure contractor personnel complete all mandatory annual training requirements necessary as part of licensure or credentialing renewal. All Health Care Providers (HCP) shall meet continuing education (CE) requirements in accordance with AFI 44-117, Chapter 5. The government may elect to provide unique government training to HCPs who are performing services under this contract. If the government elects to provide such training, the government will provide such training at no additional expense to the contractor.
2.3.2. The contractor shall provide transportation, lodging, and expenses for contract personnel required to attend the AF PRP training course at the USAF School of Aerospace Medicine at Wright-Patterson AFB in Dayton, OH. The training itself will be provided by the government at no cost.
2.4. PERSONNEL AND MINIMUM SKILL REQUIREMENTS. Appendix 1 provides the labor categories and credentials and licenses that the Government requires the contractor personnel to possess.
2.4.1. Contractor’s Quality Control Plan (QCP). A contractor's internal quality control measures, in the form of a QCP, are to be written to satisfy this contract for written procedures or to provide direction on how contractor employees are to perform. The COR will review the QCP IAW requirements of the solicitation and subsequent contract award. The COR will report any deficiencies/non-conformances to the Quality Assurance Program Coordinator (QAPC) and CM/CO for appropriate action. The contractor shall submit a monthly Quality Control Inspection Report to the Government.
2.5. TRAVEL REQUIREMENTS. The contractor may be required to travel within the Continental United States (CONUS) in performance of this contract. The Contractor will be required to coordinate travel with the COR as stated in paragraph 2.5.2.below. The contractor shall be responsible for obtaining all passenger transportation, lodging and subsistence. Reimbursement of travel costs will be IAW FAR 31.205-46, Travel Cost. All costs shall be agreed to in advance and reimbursed on a cost basis upon receipt of the invoice in Wide Area Workflow and copies of receipts turned in to the COR. No General and Administrative (G&A) charges and profit will be paid on these actual costs.
2.5.1. Local Travel. Local travel within the designated installation area (50-mile radius from the duty station) is considered a cost of doing business and shall not typically be reimbursed under this contract.
2.5.2. Travel and Trip Reports. The contractor shall present official business travel requests in the form of a Travel Cost Estimate (TCE) to the COR at least five (5) days before trip when possible. COR will approve dates of travel and purpose. Upon the COR approval, the contractor may take the business trip. The contractor shall prepare and deliver a Trip Report following each trip to the COR within 10 calendar days after each trip. The Trip Report is not required for local travel as defined in the Federal Travel Regulations. The Trip Report shall include the following details:

3.0. MONTHLY REPORT REQUIREMENTS. Contractors shall complete monthly reports of activities as directed by the government and using a format provided by the government. The contractor will collate information from all contract personnel into one consolidated report to the government. Reports must be completed no later than the fifth day of the month following the month covered by the report.
3.1. SECURITY REQUIREMENTS. Performance under this PWS shall require a Tier 2 background investigation. The Contractor shall comply with the provisions of the National Industrial Security Operating Manual (DOD 5220.22-M). Security will be in accordance with the DD Form 254, Department of Defense Contract Security Classification Specification. The contractor will use Non-Secure Internet Protocol Router Network (NIPRNet) accounts at government facilities. In addition to normal security requirements, the Government may perform an independent security determination to potential contractor personnel both before and during employment under this contract. The contractor will have security clearance in-place prior to being hired and reporting to the installation.
Tier 2 investigations for the purposes of the work set forth herein involve moderate-risk positions and non-sensitive positions. An Automated Records Check for information regarding criminal history, employment conduct, military discharge, and Selective Service registration as appropriate is reviewed as well as information on financial history, foreign associates, business interests, and other foreign connections are investigated.
The contracting officer or requesting government activity shall notify the servicing Information Protection Office (IPO) of Contract award within thirty (30) days prior to contract award/modification. The notification shall include:
Name, address, and telephone number of company representatives
The TO number and Contracting agency
The highest level of classified information to which the Contractor employees require access
3.1.1. Physical Security. The Contractor will safeguard all Government property and control forms provided for Contractor use and adhere to the Government furnished property requirements contained in this Contract. At the end of each work day, all Government facilities, equipment and materials will be secured.
3.1.2. Security Training. The contractor employees will receive the training required by DoD 5200.1-R, the Information Security Program Regulation and AFI 16-1404, and the Information Security Program Management. The contractor employees will receive initial and follow-on training for the contractors who work in AF controlled/restricted areas IAW AFPD 31-101, and the Air Force Installation Security Program. This training shall be provided by an appointed Government security manager for the on-base Visitor Group.
3.1.3. Pass and Identification. The Contractor shall ensure the pass and identification items required for Contract performance are obtained for employees and non-Government owned vehicles prior to the TO start date.
3.1.4. Retrieving Identification Media. The Contractor shall retrieve all identification media, including vehicle passes, when the Contract expires or from employees who depart for any reason before the Contract expires, e.g., terminated for cause, retirement, etc. and returned to the COR.
3.1.5. Traffic Laws. The Contractor shall comply with the Government installation traffic regulations.
3.1.6. Weapons, Firearms, and Ammunition. The Contractor shall not possess weapons, firearms, or ammunition, on themselves or within their Contractor-owned vehicles or privately-owned vehicles while on any installation or any office, and/or working location covered under this Contract.
3.1.7. For Official Use Only. The Contractor shall comply with DoD 5400.7-R, Chapter 4, the DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets Policy and Procedures for the disclosure of records to the public and for marking, handling, transmitting and safeguarding FOUO material.
3.1.8. Reporting Requirements. The Contractor shall report to an appropriate authority (i.e., CO, PM, COR) any information or circumstances which they are aware of that may pose a threat to the security of DoD personnel, Contractor personnel, Government resources and classified or unclassified defense information. The Contractor employees will be briefed by their immediate supervisor upon initial on-base assignment.
3.1.9. Key Control. The Contractor shall establish and implement procedures to ensure all keys issued to the Contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The Contractor employees will not duplicate any keys issued by the Government.
The Contractor shall immediately report to the COR or PM any occurrences of lost or duplicated keys within two (2) days of occurrence.
In the event keys are lost or duplicated, the Contractor may be required, upon written direction of the CO, to re-key or replace the affected lock or locks with no additional cost to the Government. The Government may replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the Contractor.
The Contractor shall not loan issued keys to any other persons nor allow access by use of issued keys to other persons not associated with the performance of work at the Contract work site.
3.1.10. Listing of Employee(s). The Contractor shall maintain a current listing of all employees. The list shall include the employee's name, social security number and level of security clearance. The list shall be validated and signed by the company FSO and provided to the CO, and the PM. An updated listing will be provided when an employee's status or information changes.
3.2. TRANSITION PROCESSES/PLAN. The Contractor shall be required to provide a transition plan to be implemented following contract award. The following details are provided and shall be considered and used in the Contractors transition process:
The Contractor shall develop comprehensive procedures for phasing in Contractor performance to the level required within sixty (60) days from the date of award of this contract. The Contractor shall prepare to assume full responsibility for all areas of operation in accordance with the terms and conditions of this contract. Therefore, the Contractor should be completely established, have all processes/plans in place and ready to begin supporting contract requirements issued by this office within sixty (60) days of the date of award of this contract. The Contractor shall take all actions required for a smooth transition to support total contract operations as proposed and accepted. During the Contract phase-in period, the Contractor shall at a minimum: obtain all required certifications and clearances, obtain all required personnel security clearances, and attend post-award meetings as required.
Initial Operating Capability & Full Operating Capability. PWS Appendix 2 provides the Government’s projected timelines for both Initial Operating Capability and Full Operating Capability, and the estimated number of personnel per labor category required at each location. Additionally, Appendix 2 provides a chart outlining the overall enterprise contract transition plan for when current contract efforts are required to be phased in under this enterprise effort.
4.0. GOVERNMENT FURNISHED PROPERTY (GFP)/SPACE. The Government anticipates on-site support for this requirement for actions. The Government will provide necessary equipment and materials, and access to necessary networks and systems required to support the activities required under this contract. This includes medical equipment and testing materials to be used in the performance of this contract. The Government will provide furnished multi-use office spaces (workstations, office automation equipment, wired telephones, and furniture) and supplies at performance locations.
5.0. ENTERPRISE-WIDE CONTRACTOR MANPOWER REPORTING. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Department of the Air Force via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil/ Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2017. 5.1. Subcontractor Input in CMRA. Prime Contractors are responsible to ensure all subcontractor data is reported. Subcontractors will not be able to enter any data into CMRA, but will enter their information into a Bulk Loader spread sheet available from the AF CMRA helpdesk. Subcontractor shall fill in columns A-C then return it to the AF CMRA helpdesk after it's completed and a technician team will enter the information into CMRA.
5.2. Uses and Safeguarding of Information. Information from the secure website is considered proprietary when the contract number and the Contractor’s identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the Contractor name and contract number associated with the data.
5.3. User Manuals. Data for Air Force service requirements must be inputted at the Air Force CMRA link. However, user manuals for government personnel and contractors are available at the Army CMRA link at http://www.ecmra.mil
6.0. SERVICE DELIVERY SUMMARY (SDS)
