PERFORMANCE WORK STATEMENT (PWS)
For
MULTI-MEDIA MONITORING, SAMPLING, AND ANALYSIS
At
NIAGARA FALLS AIR RESERVE STATION, NEW YORK
26 Jan 2015
1.0 GENERAL REQUIREMENTS
1.1 GENERAL
The Contractor shall furnish the personnel, services, equipment, tools, materials, vehicles, and other requirements necessary for, or incidental to, the performance of work set forth herein. The Contractor shall be immediately capable of addressing and interpreting all aspects of environmental law and regulation.
This service is considered to be a non- mission essential service in accordance with (IAW) AFI 63.124 Paragraph 2.7.1, and DODI 3020.37.
1.2 APPLICABLE DOCUMENTS
The Contractor shall comply with all applicable federal, state, and local environmental statutes, instructions, manuals, handbooks, regulations, guidance, policy letters, and rules (including all changes and amendments in effect on the date of issuance of this contract). It is the Contractor's responsibility to identify all applicable federal, state, and local environmental statutes, instructions, manuals, handbooks, regulations, guidance, policy letters, and rules for all work performed and apply the procedures and protocols required.
1.3 ADMINISTRATIVE AND MANAGERIAL REQUIREMENTS
The Contractor shall perform management and planning functions, as well as performance measurement and cost status reporting, during the course of this effort.
1.4 SPECIAL NOTIFICATION
1.4.1 Health Risks
The Contractor is required to notify the Contracting Officer (CO) and 914 MSG/CEV of critical issues that may affect the contract performance and/or human health and the environment. The types of issues that require notification include, but are not limited to, health risks, spills, and changes in critical personnel. On critical issues, verbal notification should be made immediately, followed by written notification as soon as possible.
1.4.2 Spills and Spill Reporting
The Contractor shall be responsible for cleaning up any spills caused by the Contractor or its agents. Spill clean-up shall meet all requirements (Federal, state, local, base, etc.). The 914th AW is responsible for the disposal of any spill waste generated during field operations, and the Contractor shall reimburse the 914th AW for disposal costs of such waste. The Contractor shall maintain, at the sampling sites, spill equipment and materials sufficient to handle the containing and cleaning up of spills in the capacity of the proposed projects.
The Contractor shall immediately report any and all spills or leaks to the Base Fire Department at extension 911, or by cell phone at (716) 236-2117. In the event of a spill, the following information shall be reported:
· Description and amount of material spilled
· Location of the spill (i.e. bldg. number, street name, room number, etc.)
· Name of the person reporting the spill (with location and telephone number where they can be reached)
· How the spill occurred
· The estimated time of the spill and the time the spill was discovered
· What has been done to stop the spill and have any personnel been evacuated
· Description of affected natural resources
· Amount of spill and time of entry into storm water drains or sanitary sewers
Significant quality assurance/quality control (QA/QC) problems in the field or in the laboratory shall be reported in writing within 48 hours of notification.
The Contractor shall immediately report to the CO, verbally by telephone or e-mail pursuant to this contract, any issues or incidents which may indicate potential imminent risk to contracted, federal, or local personnel, the public at large or the environment. Following the verbal or e-mail notification, a written notice with supporting documentation, to include photographic documentation whenever appropriate, shall be prepared and delivered within three (3) working days to the CO. Upon request of the CO, the Contractor shall provide pertinent raw laboratory data immediately (not to exceed two (2) weeks) via tele-facsimile, and then provide final results and laboratory quality data via standard mail as soon as possible. Contractor shall coordinate all requirements for photographs with 914 MSG/CEV via the CO. The Contractor shall not contact any regulatory agency while working under this PWS unless authorized by 914 MSG/CEV.
1.4.3 Identification and Change of Critical Contractor Personnel
The Contractor shall submit an organizational chart displaying key personnel involved in the effort and their respective labor categories. The Contractor shall immediately notify the CO of any changes in critical contractor personnel. The Contractor shall obtain CO approval of any proposed changes in project personnel along with the steps taken/proposed to ensure there are no impacts to the schedule or costs associated with individual tasks. The Contractor shall identify to the CO all subcontractors to be used under this contract, prior to signing any contract or initiating any work. The Contractor shall provide subcontractor qualifications to the CO prior to subcontractor utilization.
1.5 WORK SITE REQUIREMENTS
1.5.1 Safety Requirements
The Contractor shall coordinate work site activities to ensure the protection of human health and the environment; the prevention of damage to property, utilities, materials, supplies, and equipment; the avoidance of work interruptions; and compliance with Occupational Safety and Health Administration (OSHA) regulations (including Hazardous Waste Operations and Emergency Response Standard [HAZWOPER] requirements) and Base safety office requirements. The Contractor shall perform all operations in a prudent, conscientious, safe and professional manner and conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work. The Contractor shall maintain documentation supporting training records on site and have a written Health and Safety Plan (HSP) on site and available for workers and/or regulatory review. The Contractor shall provide the CO copies of any OSHA reports, regarding work at the project site, submitted during the duration of this contract.
1.5.2 Scheduling
The Contractor shall schedule all field activities with the 914th MSG/CEV. Whenever on site, the Contractor shall sign in at Bldg. 403, prior to performing work. Work can be performed between the hours of 7:15 a.m. and 4:00 p.m., Monday through Friday, excluding Federal holidays unless prior arrangements are made with 914th MSG/CEV, the CO and security police. Prior to accomplishment of any fieldwork, Contractor personnel must receive a safety and security briefing. The contractor is also responsible for ensuring their personnel and any subcontractors have sufficient documents filed with Security Police to enable personnel to receive contractor’s badges prior to work.
1.5.3 Maintenance of Work Premises
The Contractor shall keep the project sites free from waste materials and debris at all times. The Contractor shall dispose of all debris, off the installation, that is generated under this contract. The Contractor shall leave the work premises in a neat and clean condition upon completion of the fieldwork to the satisfaction of the 914th MSG/CEV.
The Contractor shall record and report promptly (within 1 hour) by telephone, facsimile or other direct means to the CO, the Base POC, and to the military installation’s Safety Office, all available facts relating to each instance of damage to Government property or injury to either contractor or Government personnel. In the event of an accident/mishap, the Contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, preserve evidence until released by the accident/mishap investigative authority through the CO, and immediately report the incident to the Fire Department at 911 or by cell phone at (716) 236-2117. If the Government elects to conduct an investigation of the accident/mishap, the Contractor shall cooperate fully and assist Government personnel in the conduct of investigation until the investigation is completed.
The Contractor’s personnel and equipment are subject to safety inspections by Government personnel while on federal property.
1.5.4 Global Positioning System (GPS)
The Contractor shall use Global Positioning System (GPS) at field locations to mark sampling points, if required by the 914 MSG/CEV. GPS data will be recorded in a format compatible with the installation’s Geographical Information System. The installation POC and the CO shall be consulted to properly position sampling locations (wells) with respect to site locations, to minimize the disruption of installation activities, to minimize disruption of natural and cultural resources, and to avoid penetrating underground utilities. The Contractor shall coordinate all field survey operations with installation personnel to attain these objectives. The Contractor shall provide for the detection of underground utilities utilizing geophysical or other techniques if necessary. All necessary permits and coordination shall be completed prior to commencement of individual sampling operations. Frequent communication and coordination with installation personnel shall be necessary to accomplish these goals.
1.5.5 Storage
Temporary on-base storage of equipment and materials may be allowed at the discretion of the installation POC and the CO. The Contractor shall request prior approval for any on-base storage of equipment and materials. The Contractor shall be responsible for security and weatherproofing of stored material and equipment. Equipment or materials used in the work, requiring storage on the installation, shall be placed at the approved sites designated by the installation POC. At the completion of the work, the Contractor shall remove all temporary fences and structures (used to protect materials and equipment) from the installation unless otherwise directed by the CO. The Contractor shall clean the storage area of all debris and material and perform repairs as required to return the site to its pre-use condition. All hazardous materials shall be handled, stored, labeled, and transferred in accordance with OSHA regulations.
1.5.6 Site Access
The Contractor shall verify that identification credentials presented by all Contractor and subcontractor employees to obtain security badges, passes, cards, or decals used during the duration of this contract are current and correct. As a minimum, verification should be conducted with the Social Security Administration (http://www.ssa.gov/employer/ssnv.htm), the FBI Criminal Justice Information Services, Identification & Investigative Services Section (304 625-5590) and the Form I-9 Employment Eligibility Verification (http://www.bcis.gov/graphics/howdoi/faqeev.htm). The Contractor shall monitor assigned badges (used by his/her own staff) to ensure all information remains current and to ensure all security badges or passes are returned to the base POC upon expiration of the badge, upon completion of the project, or when possession of the badge is no longer necessary (e.g., upon removal of contracted personnel from specific projects).
The Contractor shall obtain the necessary training and certificates to be able to access the flightline in order to perform sampling tasks independently of base personnel, in particular stormwater monitoring. Contractor personnel will be required to receive flightline driver training (approximately 8 hours) to access the airfield. Annual refresher training (approximately 4 hours) is also required. The Contractor is responsible for ensuring that all training and certificates are up to date and valid for flightline access. When accessing the flightline, the Contractor and its agents shall coordinate with the security police and operations squadrons, and airfield management in advance of working near the flightline or airfield to obtain any necessary clearances and permits in accordance with base policy.
1.5.7 Permits
The Contractor shall be responsible for coordinating and obtaining any required permits to implement a site-specific project.
2.0 MANAGEMENT, PLANNING, AND REPORTING REQUIREMENTS
The Contractor shall plan project activities, including the development, implementation, and maintenance of project schedules, events, status of resources, report(s) on the activities and progress toward accomplishing project objectives, and document for Government review and approval the results of the project efforts for this contract.
2.1 Post Award Meeting/Teleconference
After the issuance of this contract, the Contractor shall attend a post award meeting/teleconference at the location specified by the CO. The purpose of the meeting shall be to become familiar with the work requirements, information, and/or site-specific data addressed under this contract. The Contractor shall prepare and submit minutes of all conferences and meetings within 10 days of the meeting.
2.2 Progress Meetings
The Contractor shall attend approximately four Multi-Functional Team (MFT) progress meetings annually with the installation representative(s) as required. The Contractor shall prepare and submit minutes of all conferences and meetings within 10 days of the meeting.
2.3 Project Planning Chart
The Contractor shall prepare and submit a Project Planning Chart (PPC) for approval. The PPC shall detail the project schedule and status through the use of Gantt charts, which shall depict percent complete for each task.
2.4 Plans
The Contractor shall prepare the following plans or plan addendums, as applicable, for use during this project prior to work:
A Chemical Data Acquisition Plan (CDAP) consisting of an installation-specific Sampling and Analysis Plan (SAP) and a site-specific Health and Safety Plan (HSP) shall be developed in accordance with current Federal and New York State guidelines for the execution of environmental investigation activities. The SAP should identify the technical scope of services for field investigations and how the Contractor proposes to execute the specific fieldwork. It must also include a Quality Assurance Program Plan (QAPP), which identifies QA/QC for the project. It shall be prepared in accordance with USEPA “Requirements for Quality Assurance Project Plans for Environmental Data Operations” (EPA QA/R-5 1999) and the AFCEE Uniform Federal Policy Quality Assurance Project Plan (UFP-QAPP). The QAPP should also include the laboratories’ QA/QC plans.
The CDAP shall also contain copies of all applicable certificates such as laboratory Environmental Laboratory Approval Program (ELAP) and National Voluntary Laboratory Accreditation Program (NVLAP) certificates and asbestos/lead inspector certificates for personnel conducting sampling. Any certificates updated during the course of the contract shall be submitted to the 914th MSG/CEV via the CO (3 copies) within 10 business days.
2.5 Technical Project Reports
The Contractor shall be required to prepare reports summarizing various sampling events as required by each task. These letter reports shall identify pertinent information such as the location of the sampling event, media sampled, and number and type of samples and analyses. At a minimum, reports will include the following:
· Date, time, and weather conditions during sampling;
· Personnel who collected the sample;
· Detailed description of sampling methods and shipping/handling;
· Concentration of the substance sampled;
· Laboratory quantification or detection limits;
· Type and number of QA/QC samples;
· Identification of any sample results that are associated with out-of-control laboratory control samples;
· Discussion of any laboratory data quality issues;
· Comparison of results against applicable state and federal regulatory standards;
· Results from control samples to assure data quality;
· Chain-of-custody records;
· Laboratory data packages;
· Data summary tables, figures, maps, photos as applicable;
· Professional conclusions and recommendations.
Reports are assumed final for this contract unless comments are received from 914 MSG/CEV within 10 business days.
2.6 Quality Control Program
The Contractor shall adhere to the project QAPP for implementing quality assurance controls.
The Contractor’s QAPP should cover all of the QA/QC procedures outlined in the latest versions of the following references:
· 40 CFR 136;
· Environmental Protection Agency (EPA), 1986, Test Methods for Evaluation Solid Waste, Physical/Chemical Methods, SW-846, 3rd edition, Washington DC. EPA has finalized Updates I, II, IIA, IIB, III, and IIIA, IIIB & IV and,
· New York State, most recent version, Title 6 of the New York Codes, Rules and Regulations, Parts 360,364,370,371, and 372 with amendments.
3.0 MINIMUM QUALIFICATIONS
Due to the specialized nature of the project tasks, the following factors must be met. The contractor shall submit documentation of minimum qualifications within five (5) days after award.
3.1 Technical and Management Approach
The Contractor must provide resumes of primary project team members. The proposal should discuss the experience of the project team relevant to the tasks in this PWS. The proposal should discuss how the project team will be organized and how its members will interact with NFARS personnel.
3.2 Proximity to Site
One of the time-sensitive tasks in this project requires on-site monitoring at multiple locations at the installation to be completed within 1 hour (including travel time) of real-time weather conditions. The Contractor must provide details on how they will be able to access to the site in a timely manner for this and other tasks as specified by this PWS and all other project requirements.
3.3 Availability of Labor and Resources
One project task (see Section 4) requires on-site monitoring to be performed at multiple locations inside and outside of restricted areas within 1 hour (including travel time) based on real-time weather conditions. Other tasks will be needed on an on-demand basis and the Contractor must be prepared to fulfill these tasks within the requested time-frame as identified in Section 4 of this PWS. The Contractor must provide details on how they will be able to staff this project with qualified personnel and available resources to ensure the tasks are performed as described in this PWS. Qualified personnel availability in a timely manner is one of the critical factors in maintaining environmental regulatory compliance at NFARS. The Contractor must have the following qualified personnel available for accomplishing the various tasks:
· Geologist
· Chemist
· New York State-certified Asbestos Inspector, Project Monitor, and Air Sampling Technicians
· EPA-certified Lead-based Paint Inspector
3.4 Past Performance
The Contractor must demonstrate acceptable past performance on all tasks in this PWS within the previous 10 years.
3.5 Laboratories
The Contractor must submit the names and certifications/qualifications of laboratories to be utilized for this project. The Contractor shall select off-site laboratories with analytical capabilities sufficient for the methods specified in the Sampling and Analysis Plan (SAP), and adequate throughput capacity to handle the project’s analytical workload during all field activities. The Contractor shall ensure that the selected laboratories meet all state and Federal requirements including state certification where appropriate.
Sampling shall be consistent with ASTM standards, Federal EPA and New York State Department of Environmental Conservation (NYSDEC) requirements. Unless otherwise specified, analyses shall be performed by a laboratory currently certified by the New York State Department of Health’s (NYSDOH) Environmental Laboratory Approval Program (ELAP) and maintain certification throughout the length of the contract. The Contractor must provide proof of certification prior to sampling or collection being initiated.
Laboratory analysis shall use equipment and methods described in the USEPA regulations and American Society for Testing and Materials (ASTM) manuals. The Contractor shall furnish QA/QC data for field and laboratory procedures. Laboratories shall maintain chain-of-custody records for all samples.
All samples collected on site shall be maintained, preserved, and transported per ASTM and EPA lab standards. The Contractor shall furnish all preservatives and containers for sampling and shall be responsible for transportation of samples to the laboratories within the required timeframes. When compositing is required the Contractor shall use procedural compositing rather than mathematical compositing unless procedural compositing may result in loss of analyte.
Any proposed deviations from the above requirements shall be submitted in writing to the CO for concurrence prior to proceeding with the affected work.
4.0 TECHNICAL SCOPE
4.1 General
The Contractor shall sample and analyze solid and hazardous waste, non-potable water (including but not limited to sanitary wastewater, storm water, groundwater, surface water, base operational waste streams, etc.), asbestos, lead-based paint, air, and other media as required by 914 MSG/CEV. All work shall be coordinated through the 914 MSG/CEV POC. Requests to perform services shall be made by telephone, e-mail, or fax. The Contractor shall collect samples only in the presence of the 914th MSG/CEV, unless directed otherwise by 914 MSG/CEV. Certified laboratory results shall be provided. Strict adherence to QA/QC procedures is mandatory. All samples shall be representative samples of the environmental media. The Contractor shall follow the approved SAP for this contract at all times.
The Contractor shall analyze and test environmental samples using standardized methods as prescribed in the "Test Methods for Evaluating Solid Wastes, Physical/Chemical Methods," current edition. Analysis for each sampling event and media shall be defined at the time of sampling by the 914 MSG/CEV. The Contractor shall ensure the defensibility of test and analytical results through execution of scientifically sound activities as outlined in the appropriate portion of the approved QAPP.
The Contractor shall ensure that all hard copy and electronic data deliverables supplied by the laboratories are complete and adequate to support the quality and usability of the data. Raw data packages shall be submitted to the CO upon request. Data packages shall include all information required to recreate the analysis, including correspondence with the laboratory regarding QA/QC exceedances and documentation of corrective actions.
Sample holding times used shall be the shortest of those specified in the method or as required by an applicable regulation. Per the New York State Department of Environmental Conservation (NYSDEC), sample holding time begins when the sample is first received at the laboratory. Extraction holding time ends when the final extraction step is completed. Samples exceeding holding times shall be re-sampled at the Contractor’s expense or the cost of the sample analysis will be credited to the 914th MSG/CEV, at the Government’s option.
Limits of detection or quantification limits shall be those that are specified in the analytical method or as stated herein and shall be reported with the analytical results. The Contractor shall report all analyses for each sample as detected concentration or as less than the specific limits of quantification. Guidance values shall be displayed along with analytical results.
Any samples and wastes being shipped shall comply with New York State Department of Transportation (NYSDOT), the United States Department of Transportation (USDOT), and any other applicable shipping requirements.
The Contractor shall be responsible for the proper handling and disposal of all sample materials and any wastes generated during sampling in accordance with all applicable Federal, state, and local laws and regulations. Sampling materials and waste generated by the Contractor shall not be stored anywhere on the installation without the consent of 914th MSG/CEV. The Contractor is required to obtain all the necessary licenses and permits for complying with any and all applicable Federal, state, and local laws, codes, and regulations, in regard to the work described herein. If necessary, the Contractor shall prepare waste manifests and any other necessary shipping documentation and notification and provide copies for review by the 914 MGS/CEV.
At a minimum, internal QC samples shall be analyzed at a rate specified in the laboratory’s Quality Management Plan (QMP) or the specific analytical methods, whichever is greater. Control sample frequency shall be the greater of the references given below or the rate as specified in the laboratory’s QMP.
The Contractor shall sample, analyze, and test environmental media in accordance with the established QAPP. The Contractor shall collect and process the analytical and test data in order to demonstrate regulatory compliance and establish that compliance and remediation goals are being achieved. The Contractor shall create and maintain written and electronic records of analytical data.
4.2 Line Items
The Contractor shall provide all necessary labor, tools, transportation, and materials to complete sampling, testing, analysis, and monitoring at Niagara Falls ARS for the Line Items specified below. Estimated yearly quantities for each line item are provided in the summary price list, Table 2. Reports shall be in accordance with Section 2.5, unless otherwise specified.
1. CDAP and other Plans: The Contractor shall prepare and deliver a Chemical Data Acquisition Plan (CDAP) for the sampling and analysis project described in this PWS per Section 2.4. This deliverable will be required before work begins. The CDAP will include information on the collection and analysis of environmental samples including groundwater, surface water, and all media required in this PWS. This plan will also include the Sampling and Analysis Plan, QA/QC manual, and necessary certifications.
SAMPLING LINE ITEMS (includes labor, tools, equipment for sample collection, preparation, shipping, data review, reporting, etc.; laboratory analysis is not included)
2. Multi-Media Sampling Event, up to 5 samples, including report: The Contractor shall collect and prepare samples for analysis. Generally, most media to be sampled will already be collected and stored in drums or containers. Occasionally, media will need to special preparation. Collection and/or preparation of some samples may be labor intensive and may require multiple personnel on-site. Analysis of samples is covered under separate line items. Examples of typical media to be sampled include:
· Solids/liquids already collected in drums, pails, or other containers;
· Liquids to be drained from equipment;
· Liquids to be collected from drains, ponds, storm water inlets, etc;
· Compositing various media collected within one drum or multiple drums;
· Preparing samples from waste solid end use items such as spent fuel filters, spent materials, or military items.
Sampling will be scheduled on an as needed basis. The Contractor shall collect requested samples within 5 working days of the sampling request, unless otherwise approved by the installation POC and Contracting Office. All reports shall be submitted electronically (via email) and in hardcopy (1 hardcopy) within 10 working days after the sample was collected (or within 30 workdays for TCLP analyses or analysis of non-standard sample matrices). Reports are assumed final for this contract unless comments are received from 914 MSG/CEV within 10 business days.
3. Multi-Media Sampling Event, up to 10 samples, including report: This line item is identical to Line Item #2, except that up to 10 samples will be collected under this line item.
4. Multi-Media Sampling Event, up to 5 samples, with Permit Confined Space Entry, including report: This line item is identical to Line Item #2, except that it includes permit confined space entry. The Contractor shall provide the necessary personnel, tools, and equipment to access a permit confined space and collect the requested sample (s). The Contractor shall adhere to applicable state, federal, and installation regulations regarding confined space entry. The Contractor will be required to coordinate with the installation’s Safety Office and Fire Department prior to the work, including providing a signed permit for the confined space work. The installation’s Fire Department is available to provide emergency and rescue services for this work.
5. Bulk Asbestos Survey and Sampling, up to 10 total samples, including report: The Contractor shall perform an inspection/survey for asbestos containing materials and collect up to 10 total bulk samples of suspected asbestos containing materials (ACM) as needed. Sample analysis will be covered under separate line items. All sampling, testing, and disposal shall be in accordance with local, state, and federal regulations, including New York State Department of Labor Compilation of Codes, Rules, and Regulations, 12 NYCRR Part 56; 29 CFR Part 1926; and 40 CFR Part 61. The Contractor shall be responsible for disposal of all samples collected.
The Contractor’s personnel shall be New York State certified as an asbestos inspector. The Contractor shall submit a copy of its New York State Asbestos Handling License, the laboratory’s NVLAP/ELAP accreditation, and asbestos inspector certificates for its personnel prior to work. The contractor is responsible for providing all equipment, training, and personal protection equipment (PPE) for its employees. The Contractor is also responsible for temporary repair/patching to any surfaces from which samples are collected unless otherwise notified by the 914 MSG/CEV POC.
Stratum within a sample shall consist of a single test. Friable samples shall be tested by Polarized Light Microscopy (PLM). Non-friable organically bound (NOB) materials are to be tested by Transmission Electron Microscopy (TEM) testing as necessary.
Unless otherwise specified, the Contractor shall be required to collect asbestos samples within 24 hours of the sampling request. Bulk asbestos sampling results shall be telephoned, emailed, or faxed to the primary POC within 24 hours of sampling (or 72 hours for TEM testing results), followed up by a written report within 5 days of receipt of hard copy of testing results, unless alternate arrangements are made with the POC.
At a minimum, the written report must include the following: a summary of the inspection and sampling event, a description of the area surveyed, a figure displaying the sample locations and building floor plan along with sampling results, a summary table displaying sample descriptions and sample results, laboratory data package, photos of all materials sampled, and a table summarizing the quantity, location, and condition of any and all asbestos containing materials identified.
6. Bulk Asbestos Survey and Sampling, up to 25 total samples, including report: This line item is identical to Line Item #5, except that a total of up to 25 samples will be collected. Additionally, the Contractor shall have 5 business days from the sampling request to perform the inspection and collect samples.
7. Bulk Asbestos Survey and Sampling, up to 50 total samples, including report: This line item is identical to Line Item #6, except that a total of up to 50 samples will be collected.
8. Lead-based Paint Survey and Sampling, up to 2 samples, including report: The Contractor shall perform an inspection/survey for lead-based paint, and collect up to 2 samples for laboratory analysis. Analytical tests are covered under separate line items. Paint requiring sampling will be removed and sent off-site to a certified lab for analysis. All lead paints samples shall be full depth (i.e. to the substrate) unless otherwise requested. Samples shall be collected within 24 hours following the sampling request unless otherwise specified.
Personnel performing the survey and collecting samples shall be EPA-certified lead inspectors. The Contractor shall submit copies of all applicable certifications prior to starting work. All sampling, testing, and disposal shall be in accordance with local, state, and federal regulations. The Contractor shall be responsible for providing appropriate PPE for its employees. The Contractor is also responsible for temporary repair/patching to any surfaces from which samples are collected unless otherwise notified by the 914 MSG/CEV POC.
Sample results are to be communicated to the 914 MSG/CEV POC within 24 hours of analysis. A written report must be submitted to the 914 MSG/CEV POC within 5 business days of receipt of hard copy of testing results. At a minimum, the written report must include the following: a summary of the inspection and sampling event, a description of the area surveyed, a figure displaying the sample locations and building floor plan along with sampling results, a summary table displaying sample descriptions and sample results, laboratory data package, photos of all materials sampled, and a table summarizing the quantity, location, and condition of any and all lead-based paint identified.
9. Lead-based Paint Survey and Sampling, up to 10 samples, including report: This line item is identical to Line Item #8, except that up to 10 samples will be collected. Samples are to be collected within 5 business days of the sampling request.
10. Lead-based Paint Survey and Sampling, up to 20 samples, including report: This line item is identical to Line Item #9, except that up to 20 samples will be collected.
11. Asbestos Project Monitoring, Visual Inspection Only, including report: Under this line item, the Contractor shall provide asbestos project monitoring services for an asbestos abatement project, consistent with the requirements of 12 NYCRR 56. For this task, only visual inspection will be performed; no air monitoring. The Contractor shall assume the visual inspection will be 4 hours or less in duration.
Project monitoring duties include observation of abatement work to ensure compliance with applicable regulations in accordance with 12 NYCRR 56. The Contractor is responsible for providing all equipment, training, and personal protection equipment (PPE) for its employees. Contractor personnel shall be New York State-certified asbestos project monitors and shall submit copies of their certificates to the 914 MSG/CEV POC prior to work. The Contractor shall immediately notify the 914 MSG/CEV POC if any instances of non-compliance are observed during asbestos abatement. A written report must be submitted within 10 business days following completion of the project monitoring. The report must include: personnel on-site, location and description of abatement activities, any instances of non-compliance, and corrective actions. Reports are assumed final for unless comments are received from 914 MSG/CEV within 10 business days.
12. Asbestos Project Monitoring Background or Clearance Event, including report: The Contractor shall provide asbestos project monitoring and air sampling services for a background or clearance air sampling event (Phase IB or IIC) in accordance with the requirements of 12 NYCRR 56. The number and location of air samples to be collected will be in accordance with 12 NYCRR 56 for the size and type of abatement project being performed (i.e. small or large). The contractor is responsible for providing all equipment, training, and personal protection equipment (PPE) for its employees. Contractor personnel shall be New York State-certified asbestos project monitors and air sampling technicians and shall provide copies of their certificates to the 914 MSG/CEV POC prior to work.
Air samples shall be tested using Phase Contrast Microscopy (PCM) or TEM. Turnaround time for air samples shall be 24 hours. Air sampling results shall be telephoned, emailed, or faxed to the primary POC within 24 hours of sampling and communicated to the asbestos abatement contractor as needed. Laboratory analysis of air samples is covered under separate line item(s).
A written report for project monitoring/air sampling tasks shall meet the requirements of 12 NYCRR 56 and should include at a minimum: description of abatement activities and any observations of non-compliance, sample locations, air sampling log, sample number and type (background, daily, clearance), analytical method used, sampling pump data (manufacturer, model, serial number, beginning flow rate, end flow rate, average flow rate (L/min), calibration data (date, time, method, name, signature), sample period, total air volume sampled, laboratory sample results, and laboratory QA data and confidence level. Reports shall be submitted within 10 business days following completion of the sampling event. Reports are assumed final for unless comments are received from 914 MSG/CEV within 10 business days.
13. Asbestos Project Daily Monitoring Event, including report: This line item is identical to Line Item #12 except, that the air sampling and monitoring shall be performed for daily work area preparation or asbestos handling for an abatement project (Phase IIA or IIB), in accordance with 12 NYCRR 56. The Contractor shall assume that the work day is at most 8 hours in duration. The number and location of samples shall be per 12 NYCRR 56 for the type and size of project specified. Analysis of samples is covered under separate line items.
14. Lead Wipe Sampling Event, up to 25 wipes, including report: The Contractor shall perform wipe sampling in accordance with EPA, OSHA, and US Dept. of Housing and Urban Development (HUD) standards, up to 25 wipe samples. Analytical testing of samples is covered under a separate line item. Sample results are to be communicated to the 914 MSG/CEV POC within 24 hours of analysis. A written report must be submitted to the 914 MSG/CEV POC within 5 business days of receipt of hard copy of testing results.
15. Lead Air Sampling Event, including report: The Contractor shall perform air sampling for lead in accordance with EPA, OSHA, and US Dept. of Housing and Urban Development (HUD) standards. The number and location of air samples will be determined on a project-by-project basis and in accordance with EPA and HUD standards. Analytical testing of samples is covered under a separate line item. Sample results are to be communicated to the 914 MSG/CEV POC within 24 hours of analysis. A written report must be submitted to the 914 MSG/CEV POC within 5 business days of receipt of hard copy of testing results.
16. Stormwater Annual Dry Weather Monitoring Event, up to 9 outfalls, including report: The Contractor shall conduct monitoring to comply with the NYSDEC SPDES Multi-Sector General Permit (GP-0-06-002) for Stormwater Discharges Associated with Industrial Activities (or most recent version) at the Niagara Falls IAP-ARS (NFARS). All monitoring shall be conducted in accordance with the Permit and Contractor’s CDAP and HSP. The Contractor shall provide this monitoring during “normal business hours” or from 0715 to 1600 Monday through Friday, excluding federal holidays. This task is two-part. First, the Contractor shall independently monitor weather conditions at NFARS to determine when an Annual Dry Weather Flow Monitoring Event can be performed. It is the Contractor’s responsibility to monitor the weather conditions and gather all data for verification of weather criteria for all monitoring events under this task.
Second, when the first weather conditions are met for the Annual Dry Weather Flow Monitoring Event, the Contractor shall conduct a dry weather field inspection of up to nine outfall locations (identified in the 2014 Niagara Falls IAP-ARS SWPPP, or most recent version). Per the Stormwater Pollution Prevention Plan (SWPPP) for NFARS, the Contractor shall conduct the single dry weather flow inspection to determine the presence of non-stormwater discharges to the NFARS's stormwater drainage system and to provide a baseline for comparison during storm discharges. Existing forms from the SWPPP shall be used to collect observations made during the monitoring event. If a non-stormwater discharge is discovered, the Contractor’s inspection shall include observations of the color, odor, clarity, floating solids, settled solids, suspended solids, foam, oil sheen, and any indications of pollution in accordance with SPDES General Permit GP-0-06-002. Additionally the Contractor shall provide weather documentation verifying that weather criteria were met, as described in the SPDES Multi-Sector General Permit GP-0-06-002, prior to the annual dry weather flow monitoring being performed.
If non-stormwater discharges are identified by the Contractor, they will be reported to the 914th MSG/CEV POC immediately and recommend a verbal plan to identify the source of the non-stormwater discharges.
The Contractor shall submit the following information in report format. All reports will be submitted electronically (via email) and in hardcopy (1 hardcopy) within 10 working days after completion of the monitoring event. Reports are assumed final for this contract unless comments received from 914 MSG/CEV within 10 business days.
· Map of outfall locations,
· Inspection date and time,
· Inspection personnel,
· Description of any discharges identified,
· Copies of the field inspection reports;
· Source of the discharges, if identified at the time of the inspection, and
· Record of (3) consecutive days of no precipitation
17. Stormwater Quarterly Wet Weather Flow Monitoring Event, up to 9 outfalls, including report: This line item is similar to Line Item #16, except that the Contractor shall perform the monitoring for Quarterly Wet Weather Flow events to comply with the NYSDEC SPDES Multi-Sector General Permit (GP-0-06-002) for Stormwater Discharges Associated with Industrial Activities (or most recent version) at the Niagara Falls IAP-ARS (NFARS). All monitoring shall be conducted in accordance with the Permit and Contractor’s CDAP and HSP. The Contractor shall provide this monitoring during “normal business hours” or from 0715 to 1600 Monday through Friday, excluding federal holidays. This task is two-part. First, the Contractor shall independently monitor weather conditions at NFARS to determine if/when there is a qualifying Wet Weather Flow Monitoring Event per the SPDES Permit and SWPPP. It is the Contractor’s responsibility to monitor the weather conditions and gather all data for verification of weather criteria for all monitoring events under this task. A qualifying storm event is one with at least 0.1 inches of precipitation preceded by 3 consecutive days of no measurable precipitation.
When the qualifying weather criteria are first met for the Quarterly Wet Weather Flow Monitoring, the Contractor shall field inspect up to nine outfalls (identified in the 2014 Niagara Falls IAP-ARS SWPPP, or most recent version) at Niagara Falls ARS during the first 30 minutes of when runoff or snowmelt begins discharging from the outfalls, as described in the SPDES General Permit GP-0-06-002, or most recent version. The Contractor must take into consideration the travel time to NFARS from their place of business and the time necessary to gain access to the outfall locations once arriving at NFARS. Several outfalls are located in the restricted area and require flightline driver training to be able to access those locations. The inability to obtain monitoring results within the specified SPDES timeframe due to these factors is not an acceptable excuse.
Existing forms from the SWPPP shall be used to collect observations made during each monitoring event. The Contractor shall follow the SPDES monitoring criteria and provide weather documentation verifying that wet weather criteria was met for the quarterly visual monitoring. The Contractor’s inspection shall include observations of the color, odor, clarity, floating solids, settled solids, suspended solids, foam, oil sheen, and any indications of pollution in accordance with SPDES General Permit GP-0-06-002, or most recent version.
The Contractor shall also collect a sample from the discharged stormwater at each location for visual and olfactory inspection as described above. The inspection information shall be noted on an inspection form. If contamination is discovered in the stormwater discharges (as identified by visual and olfactory observations) the Contractor shall immediately notify the 914th MSG/CEV and recommend a verbal plan to identify the source of the contamination. The Contractor should assume an additional 1-hour to investigate the presence of stormwater pollution.
The Contractor shall submit the following information in report format. All reports will be submitted electronically (via email) and in hardcopy (1 hardcopy) within 10 working days after completion of the inspection. Reports are assumed final for this contract unless comments received from 914 MSG/CEV within 10 business days.
· Map of outfall locations,
· Inspection date and time,
· Inspection personnel,
· Description of the discharges,
· Copies of the field inspection reports;
· Record of (3) consecutive days of no precipitation, and
· Data on the storm event sampled as outlined in Part IV A.2 c of SPDES Permit GP-0-06-002.
18. Stormwater No Qualifying Monitoring Event Report: This line item will be used if there is no qualifying monitoring event (either quarterly wet weather or annual dry weather). The Contractor shall independently monitor weather conditions at Niagara Falls ARS during “normal business hours” or from 0715 to 1600 Monday through Friday, excluding federal holidays. If weather conditions do not meet the qualifying event criteria, the Contractor shall submit a summary report with weather data records supporting the determination that no qualifying event occurred. The Contractor shall submit the following information in report format. All reports will be submitted electronically (via email) and in hardcopy (1 hardcopy) within 10 working days after request is issued. Reports are assumed final for this contract unless comments received from 914 MSG/CEV within 10 business days.
19. Oil Water Separator Inspection and Sampling Event, up to 14 Oil/Water Separators, including report: The Contractor shall provide all necessary labor, tools, transportation, and materials (including packaging and equipment) to inspect, monitor, and sample up to 14 oil/water separators at Niagara Falls ARS. All work shall be scheduled and coordinated through the 914 MSG/CEV POC at least 1 week prior to the planned inspection date(s). The Oil/Water Separators will be opened by on-base personnel for inspection by the Contractor. Laboratory Analysis of samples is covered under separate line items. The current list of 14 Oil/Water Separators to be inspected is identified below. This list is provided for informational purposes only and is subject to change.

The Contractor shall perform OWS inspections on an annual basis by collecting various measurements and observations. Inspection-specific criteria for each OWS are provided in the Oil/Water Separator and Grease Interceptor Management Plan (URS 2014), or most recent version, and should be summarized onto inspection forms. At a minimum, the following observations will be collected: observation of exterior condition, interior condition (as observed from the surface), oil sheen inside the OWS, and condition of floor/trench drains discharging to the OWS. Additionally, the Contractor shall measure the water and sludge level in each OWS. No confined space entry is required for this task unless specifically requested otherwise.
The Contractor shall also collect samples from oil/water separators. Samples should be collected utilizing bottom haulers or equivalent approved method. Samples should be representative of the OWS effluent. Efforts should be made to avoid sampling through the oil/water interface where possible. Safety precautions, including the use of a Photoionization Detector (PID), must be taken prior to collecting the sample. The Contractor shall verbally notify the 914th MSG/CEV within one working day from release of all analytical data, if any test results exceed the specific limits set below (not the Lower Detection Limits [LDLs]):

The Contractor shall provide a summary report of the OWS inspections and sampling event. The report should include copies of the inspection reports and summarize operation/maintenance recommendations in a table. Details should also include the date, on-site personnel who collected the samples, weather, description of work performed, summary of analysis performed, laboratory used, description of where sample was collected, PID readings, description of field observations, and text description of results exceeding action limits related to the facilities. All reports will be submitted electronically (via email) and in hardcopy (1 hardcopy) within 10 workdays after analytical results are released.
