PERFORMANCE WORK STATEMENT

For

AVGAS

Homestead Air Reserve Base

Homestead FL Wings over Homestead 2012

PERFORMANCE WORK STATEMENT FOR AVGAS

1. DESCRIPTION OF SERVICES. Aviation Fuel for airshow, 2-4 Nov 2012.

2. CONTRACTOR REQUIREMENTS.

2.1 Contractor-Services. The contractor will provide the following services:

1.) Total for all 3 days: 7000 gallons Avgas 100LL 2.) Days fuel truck needed at Homestead Air Reserve Base are 2-4 November 2012 3.) Approximately 2250 gallons per day 4.) Times truck must be available to fuel: 7am to 7pm each day 5.) Driver must be supplied for fuel truck and capable of transacting individual sales as needed 6.) Require 2 Avgas trucks at HARB in the event that there is a mechanical failure with one 7.) In the event that 2 trucks are not available - please advise how Avgas will be replenished when onsite truck is depleted.

8.) Vendor will accept for return and credit the government for any unused fuel after the event.

2.2 Equipment. Equipment failure shall not alleviate the contractor from performing any requirement contained in this contract.

2.3 Miscellaneous Requirements

1. Report fire hazards, unsafe conditions, and items in need of repair to the contract Point Of

Contact (POC)

2. . Notify Security Forces when unauthorized or suspicious persons are seen on the premises.

3. Government Furnished Trash Removal. Limited trash removal will be provided by the

Government.

4. Utility Conservation. The Contractor shall adhere to all base level utility conservation practices or requirements. The Contractor shall be responsible for operating under conditions that prevent waste of utilities.

5. Security, Fire and Medical Services. The Government will provide police and fire protection. In the event of a medical emergency, base ambulance service for transporting an injured employee to a local hospital is available on a cost reimbursement basis.

3. GENERAL INFORMATION

3.1 Traffic Laws. The Contractor and its employees shall comply with base traffic regulations.

3.2 Safety Requirements and Reports. The Contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements. Provide a verbal report to the CO as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the CO within five calendar days of each occurrence.

4. QUALITY CONTROL. The contractor shall develop and maintain a quality control program to ensure each performance is satisfactorily performed and address any deficiencies communicated by the POC in a timely manner.
