GENERAL INFORMATION

This is a combined synopsis/solicitation (combo) for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested.

SOLICITATION DATE: 27 June 2019 TYPE OF SOLICITATION: Request for Quote QUOTES DUE: Quotes for this solicitation are due to this office by 16:00 Central European Time (CET) on Thursday, 11 July 2019.

**NOTE: Contractor must be willing to provide an on-site technician on an as-needed basis for either general or warranty maintenance. Maintenance not covered by warranty will be priced separately, at the time of need.

This solicitation document and the incorporated provisions and clauses are those in effect through the most current Federal Acquisition Circular (FAC) 2019-03 Effective 12 June 2019, Defense Federal Acquisition Regulation Supplement (DFARS) DPN 20190531 Effective 31 May 2018, and Air Force Acquisition Circular 2018-0525 Effective 25 May 2018.

The following NAICS code applies to this solicitation: 423720. Due to the overseas location of this solicitation, no small business set-aside will be used.

The provision at 52.212-1, Instructions to Offerors -- Commercial Items (Jan 2017), applies to this acquisition. The following addenda are made to the provision:

• Basis for Contract Award The Government will select the offer of the responsible vendor whose quote represents the lowest priced offer, after evaluation in accordance with the factors outlined under Submission of Quotations below.

Aviano Air Base

• Number of Contracts to be awarded The Government intends to award one contract for the supply of a Contrast Bath for use at Aviano Air Base, Italy

• Solicitation Requirements, Terms and Conditions Quotations may be hand delivered (no receipt released), or e-mailed. Hand delivered quotations must include a digital copy of the quote on CD. Offerors are required to meet all solicitation requirements. Failure to comply with the terms and conditions of the solicitation may result in the offeror being ineligible for award.

• Submission of Quotations Quotations shall contain the following information AT A MINIMUM: Company name, POC name, phone, and e-mail, and provide

Factor 1: PRICE: complete Pricing Schedule (pg. 1), to include quantity, unit of measure, unit price (EUR), shipping costs (if applicable, as outlined in Pricing Schedule), total price, and discount terms.

Factor 2: TECHNICAL CAPABILITY: provide product information, to brand name and model, part number, product description, copy of the relevant technical information of the items offered (catalog data), information about delivery time and warranty information.

The provision at 52.212-2, Evaluation -- Commercial Items, applies to this acquisition. The following addenda are made to the provision:

The Government will evaluate each offer according to the following factors:

Factor 1: PRICE After ensuring that the Pricing Schedule has been completed as outlined in 52.212-1 above, a determination will be made that the proposed price is reasonable and affordable based upon the total price offered. Partial quotes will not be considered.

Offerors whose price cannot be determined to be reasonable and affordable may not be considered for award.

Factor 2: TECHNICAL CAPABILITY This factor evaluates offeror’s capability to provide items that meet the Government requirements. This requirement is met when the offeror submits a list of proposed items, as outlined in 52.212-1 above, in sufficient detail to allow the Government evaluation team to ascertain compliance with the item description/requirements outlined in the Pricing Schedule.

The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. The following addenda are made to this clause. Paragraph (a) Inspection/Acceptance is added to read: The contractor shall use the Wide Area Workflow web page at https://wawf.eb.mil/index.html to submit invoices for acceptance. Paragraph (o) Warranty is added to read: The contractor shall provide all standard commercial (including manufacturer) warranties to the United States Government.
