TYPE OF CONTRACT: Non-Personal Service (Non-Appropriated)

1. Environment:

1.1 The RAF Mildenhall (RAFM) Chaplain Corps mission is tailored around the 100 ARW Wing’s mission. We provide for the religious and spiritual needs of the Air Force personnel and their families. We support individual rights under the First Amendment to the US Constitution and provide an opportunity for everyone to freely exercise their religious beliefs.

1.2 The environment in which this contract is implemented is a religiously pluralistic and sensitive arena of numerous faith groups and denominational traditions. The contractor accomplishes the terms of the contract in compliance with the mission requirements and standards set forth and defined by the Department of the Air Force and the Air Force Chaplain Corps. Activities and behavior to the contrary are viewed as grounds for contract termination.

1.3 The contractor shall provide an atmosphere of religious sensitivity to all who participate in the program. This sensitivity does not limit the contractor from sharing his/her faith, moral values, religion, or views on any other issues. However, no form of coercion will be employed by the contractor or their employees to cause DoD personnel or their family members to conform to a particular religious denomination or faith group either directly or indirectly. The contractor shall ensure that all key leaders and volunteers comply with this no coercion policy. Respect for other religious faith groups shall be maintained.

2. Scope of Responsibility:

The SAM is a relational ministry concept based upon a model that has proved successful on college campuses across our nation. Contractor will be required to act as facilitator, and mentor to the dormitory residents, engage and build relational ministry with the airmen and equip emerging leaders to serve as productive team members of the United States Air Force (USAF).

3.Qualifications:

3.1 Associates degree is desired but not essential. The contractor must have a minimum of three

(3) years of experience working directly with single adults in established ministry organizations.

3.2 The contractor must submit the following items with proposal: Resume, letters of recommendation and documentation of training.

3.3 The contractor must have an in-depth working knowledge of American ethical values, social issues and American life styles.

3.4 Must have an understanding of, and be able to work within a military structure and environment.

3.5 Contractor shall submit completed Installation Records Check (IRC) within five (5) days of award of contract. A criminal history background check on individuals in Child Care Service (Care National Agencies Check with Inquiries (CNACI)) application will be turned in within twenty (20) working days after award date to the chapel background check program manager for processing. It is mandatory that all contractors working with children under the age of eighteen (18) have a favorable background investigation. This is in compliance with Public Law 101-64, Section 231 (Crime Control Act of 1990 and Department of Defense Instruction (DoDI) 1402.5, Criminal History Background Checks on Individuals in Child Care Service Position, 19 Jan 93). An adverse finding from this investigation at any time will result in termination of services.

4. Duties and Responsibilities:

4.1 The contractor will submit in writing all planning items no later than the 5th day of the second month after the effective date of the contract. No advertisement of proposed event or activity, either printed or verbal, will be released until approved.

4.2 The contractor will file an after-action report for all off-site retreats, special events and training. After action reports are required within ten (10) days following the completion of the event. Continuity folders will be maintained by the contractor for all events and will ensure these reports are available upon request.

4.2.1 The contractor will maintain copies of all written reports submitted to any organizations with which he/she is affiliated.

4.3 The contractor will attend weekly chapel staff meetings and planning days and Parish Council meetings for the RAFM Chapel.

4.4 The contractor will order and maintain a stock of inspirational literature and multimedia products that seek to build character and leadership, inspire faith, support the Air Force core values, and address current issues that are of a concern to single airmen.

4.5 The contractor will establish a Single's Leadership Team composed of dormitory residents and meet with this leadership team at least monthly to develop programs, plan event calendars, event support, and build ministry. The Singles Leadership Team will try to identify and understand the current issues and concerns impacting the single airmen at RAFM and will seek solutions to the issues identified.

4.6 The contractor will maintain the SAM ministry office that will provide an informal setting for airmen enabling one-on-one, small group discussions, and providing airmen with an opportunity to socialize, seek advice, and build mentoring relationships. Hours of operations will depend upon the needs of the airmen.

4.7 The contractor will properly screen and recruit a volunteer staff to assist in mentoring, facilitating discussions, leading events and activities, leading small group ministries, and managing and organizing the SAM ministry.

4.8 The contractor in consultation with the Single's Leadership Team and the Singles Ministry Chaplain, will develop a monthly calendar of Chapel sponsored trips, sporting events, retreats, recreational adventures and leadership/spiritual development opportunities. The contractor is encouraged to partner with the RAFM ITT Office and Outdoor Recreation services to sponsor the mentioned activities. The contractor will coordinate all advertising of events and activities prior to publicizing or obligating funds.

4.9 The contractor will plan a quarterly service project that encourages airmen to give of themselves to our community, and challenges them to make a lifelong commitment to public and faith-based volunteer service. All planning, advertising, and coordination for this project shall be at no additional cost to the government. Additional costs per participant may be funded through RAFM Chapel Tithes and Offerings Funds and must be approved prior to publicizing or obligating funds.

4.10 The contractor will plan, organize, and implement at least one annual weekend spiritual retreat for single airmen. The retreat should provide a forum for single airmen to fellowship, share experiences and struggles with other single airmen. This retreat will offer an opportunity for spiritual instruction, nurture and provide an opportunity for character and leadership development. All planning, advertising, and coordination for this retreat shall be at no additional cost to the government. Additional costs per participant may be funded through RAFM Chapel Tithes and Offerings Funds and must be approved prior to publicizing or obligating funds.

4.11 Contractor is responsible for ensuring that all money collected for programs/events have been handled in compliance with AF and local directives.

4.12 The contractor will be concerned for the spiritual health of all single airmen. Occasions for ministry of presence include but are not limited to: Eating in the dining facility, visitation in the dormitory common rooms, and visitation in authorized work-centers across the base and social activities or gatherings where single airmen assemble. The contractor will be intentional in seeking opportunities to lead informal group discussions, activities, provide guidance necessary to help the airmen improve their decision-making skills and grow in their faith.

4.13 The contractor will develop small group ministries to encourage single airmen to pursue matters of spirit and build nurturing and caring communities. The contractor will develop a core curriculum encompassing social, ethical and religious topics to develop life skills and spiritual growth.

4.14 The contractor will develop and submit an annual budget. All activities, planning, advertising, and coordination shall be at no additional cost to the government. Funding may be provided through RAFM Chapel Tithes and Offerings Funds and must be approved prior to obligation.

4.15 The contractor will plan and organize an annual two-day training retreat for adult volunteers of particular areas of single adult ministries. All planning, organizing and coordination for this shall be at no additional cost to the government. Additional costs per participant may be funded through RAFM Chapel Tithes and Offerings Funds and must be approved prior to publicizing or obligating funds.

4.16 Contractor is responsible for collecting and maintaining program/event figures. These figures must be turned in to the appropriate chapel support staff member by the last Monday of each month for inclusion in the Chapel AF Fm 1270 reporting.

4.17 The contractor will work in coordination with the RAFM Singles Ministry Chaplain to plan vital and dynamic interest-specific worship opportunities for single airmen.

4.18 Contractor will submit all fund, supply, and support requests for approval prior to purchase. Purchases will not be made without prior approval. Purchases made without approval may not be reimbursed.

4.19 Contractor will coordinate chapel availability, and turn in completed facility requests for approval. Programs must be approved and facilities must be determined available prior to publicizing any event or program.

4.19.1 If utilized, the contractor coordinates with outside sources/organizations to ensure required facilities/spaces are available.

4.20 Contractor is required to maintain a current equipment list of all non-expendable items purchased in support of the program. These items include but are not limited to games, videos, game systems and computer equipment.

4.21 The contractor will comply with all applicable Air Force and local instructions for financial management, facility use, maintenance, transportation, etc., and will coordinate all support requirements with the appropriate chapel staff member(s).

5. Equipment and Supplies:

5.1 As available, the contractor shall be provided with all equipment needs to fulfill this statement of work by RAFM Chapel. The contractor shall be provided appropriate office space, and logistical support including but not limited to a computer and printer, phone, and e-mail/LAN account and access.

5.2 The contractor will safeguard all government/chapel equipment and supplies provided to fulfill this position. The contractor will be held accountable for the proper use of all Government equipment used in any program event, activity or location. Contractor may be held financially responsible for any damage or replacement costs.

5.3 Contractor will maintain assigned work and storage areas in a neat, clean and safe fashion.

6. Security/Safety:

6.1 The contractor will need to have access to the UK-AD NIPERnet. A National Agency Check (NAC) will be required for access, and will be provided by the US Government. The contractor will comply with all Air Force (AF) network training and security requirements.

6.2 Contractor will be familiar with and implement fire and safety practices in accordance with AF and local Operating Instructions to prevent accidents and preserve the life and health of him/herself and any personnel performing or in any way affected by the performance of this contract.

6.3 Contractor will ensure that all personnel under their control are aware of, and implement local fire and safety procedures.

6.4 Contractor will be familiar with and comply with all security procedures to include: ensuring all chapel facilities are properly secured, doors and windows locked, and lights turned off when leaving the building, unless pre-arranged with chapel personnel or programs that are physically in the building.

6.5 Contractor shall be responsible for keys provided and keys shall not be duplicated. All keys will be returned on revocation of this contract or upon request of authorized personnel.

6.6 Contractor shall immediately report any duplicated, lost, or misplaced keys upon discovery of the occurrence.

7. Logistic Support (DFARS Clause, SUBPART 225.74—DEFENSE CONTRACTORS OUTSIDE THE UNITED STATES is hereby incorporated by reference.):

7.1 Logistic support will be provided to an authorized contractor. Authorized contractors are defined as a US citizen who has been hired as a consequence of this contract and is employed at least 40 hours per week on this contract.

7.2 Logistics support as defined below will be provided to authorized contractor employees and their dependents to the extent these services are available at the location where the contract is to be performed and as authorized by current applicable Air Force and European theater regulations, by current applicable international agreements and arrangements, by current policies, and the local Installation Commander. Lack of availability of any of these services shall not serve as a basis for claims by a contractor against the Government for increased cost of contract performance.

7.3 Abuse of Privileges. The contractor will include a provision in his employment agreement with his employees to provide for disciplinary action, or discharge for cause, of the employee for any abuse of privileges authorized to herein. The US government retains the right to withdraw privileges as a result of contractor employee abuse at no additional cost to the US government. This provision in no way will prohibit disciplinary action or legal prosecution by either the US government or the host country government. Services or privileges may be denied on an individual basis at the discretion of the Installation Commander.

7.4 The contractor shall assure that upon termination or transfer of any employee who is granted logistic support, action is taken simultaneous with the termination of employment to assure that said employee ceases to have access to the services granted under logistic support. The contractor shall assure that identification passes or other documents pertinent to or peculiar to the contract or privileges are turned over to the issuing office upon termination or transfer of any employee. The contractor shall require a written receipt of such return and shall immediately forward a copy to the contracting officer.

7.5 The following listed services will be made available, to the extent possible, to authorized contractor's employees and authorized dependents of a contractor employee.

7.5.1 Commissary (includes rationed items)

7.5.2 AAFES facilities (military exchange, theater, food concessions, shoppette)

7.5.3 Class Six (alcoholic beverages, includes rationed items)

7.5.4 Local government transportation for official government business (as available)

7.5.5 Local Services (morale, welfare, and recreation)

7.5.6 Military banking services, accounting and finance services, credit union

7.5.7 Military Postal Service. DoD 4523.6-M, Volume I, DoD Postal Manual, specifically outlines procedures required prior to approval of military postal service privileges for US contractors, In addition to contracting agencies limiting the extent of postal support to that which meets the approval of the appropriate major overseas command, the military departments concerned, and the MPSA, the provisions for postal support in contracts must be reviewed and approved by USAFE Air Postal Squadron (USAFE AIRPS/CC) and MPSA prior to the signing of the contract. When approved, a copy of the contract must be provided to the military post office by the contracting officer or a designated representative

7.5.8 Mortuary Service. In the event of death of such employee or authorized dependents, the US government shall provide such assistance as is necessary, including, but not limited to, embalming preparation of remains for shipment, and furnishing of transfer care of the expeditious handling of such remains. The cost incurred through the assist rendered by the US government in handling of such remains shall be reimbursed to the US government by the contractor in accordance with applicable regulations and directives

7.5.9 Officer and noncommissioned officer clubs

Dependent schools are not authorized on a space-required basis for contractor personnel

7.5.16 Emergency medical and dental services on a reimbursable basis

7.5.17 Transportation incidental to medical and dental treatment may be provided by the

US Air Force under the following conditions:

7.5.17.1 In emergencies, transportation in US government ambulances may be furnished by the US government at no cost

7.5.17.2 Transportation by air for medical reasons may be furnished by the US government. The cost of such transportation to the contractor will be at the rate provided in the international rate book if the service is on a regular (routine flight schedule). The contractor will not be reimbursed by this contract for this transportation. This paragraph shall also apply in the event of the death of a US employee or dependent and where transport of remains will be involved

7.5.18 Immunization and physical examination required by the US government of the contractor US citizen employees and dependents will be US government furnished in US government facilities, where and when available

7.5.19 Pet registration and control

7.5.20 Law enforcement (limited to accident investigation and on-scene interpreters (if required)

8. Period of Service/Number of Units:

The duration of the contract is from 1 October 2010 to 30 September 2011 and will require no more than 12 Units. A unit consists of one month.

9. Payment and Extended Period of Absence:

9.1 The contractor shall submit a monthly invoice to the Account Manager.

9.2 The contractor is being hired as an “independent contractor” and is accountable for the paying of any US or British Social Security taxes, federal or local income taxes, and state unemployment insurance premiums for themselves, as well as any individual employed or subcontracted by them to provide services under this contract.

9.3 In the event of illness or holiday this contract may be sub-contracted. The contractor is required to provide a two week notice of any extended period of absence. An extended period of absence is defined as absence for more than one week.

9.4 Substitutes must be pre-approved. All subcontractors must meet the same qualifications as the contractor and be approved as suitable by the Wing Chaplain.

9.5 Substitute MUST possess a current, favorable IRC and CNACI. A copy of these documents must be maintained on file with the chapel Background Check Program Manager.

9.6 The contractor will provide and pay a suitable substitute for each service if he/she is unable to fulfill this commitment himself/herself. In emergency situations, notice should be given at least 24 hours before such a substitution is made. The Wing Chaplain or his/her designated representative reserves the right to refuse the services of such a substitute he/she deems unqualified, per terms of this SOW.

10. Exercise of Option:

The Fund may extend this contract for an additional 12 months by issuing written notice to the contractor within 30 days of contract expiration. The preliminary notice does not commit the Fund to an extension. If the Fund exercises the option, the extended contract is considered to include this option provision and at the applicable prices set forth in the bid schedule.

11. CTOF Contract Monitor:

11.1 The CTOF Contract Monitor for this contract is the Account Manager. The Superintendant, Chapel Operations will serve as the alternate Contract Monitor. Any grievance by the Contractor should be resolved at the lowest level. The chain of command is as follows: CTOF Contract Monitor, Chaplain Advisor, Senior Protestant Chaplain, and then the Wing Chaplain.

11.2 The CTOF Contract Monitor will monitor and inspect the contractor's performance but will maintain no supervisory control over the contractor. The contractor will be notified in writing of all noncompliance work, and given an opportunity to correct nonconforming work. The contractor will be given an opportunity to respond in writing to any notice of noncompliance.

12. Termination Procedures:

This contract may be terminated by either the RAFM Chapel or the contractor with 30 calendar days advance notice in writing. Any notice required to be given by either party under any provisions of this contract will be sufficient for all purposes when sent to the other party at the contractor’s address of record.

52.212-5 CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE ORDERS--COMMERCIAL ITEMS (JUL 2010) (DEVIATION)

(a) Comptroller General Examination of Record. The Contractor agrees to comply with the provisions of this paragraph (a) if the contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records-Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to the right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times, the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(b)(1) Notwithstanding the requirements of any other clause in this contract, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (b)(i) in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause-

(i) 52.203-13, Contractor Code of Business Ethics and Conduct (APR 2010) (Pub. L. 110-252, Title VI, Chapter 1 (41 U.S.C. 251 note).

(ii) 52.219-8, Utilization of Small Business Concerns (May 2004) (15 U.S.C. 637(d)(2) and (3)), in all subcontracts that offer further subcontracting opportunities. If the subcontract (except subcontracts to small business concerns) exceeds $550,000 ($1,000,000 for construction of any public facility), the subcontractor must include 52.219-8 in lower tier subcontracts that offer subcontracting opportunities.
