STATEMENT OF WORK (SOW)

FOR

HYDROSTATIC WATER JACKET

CYLINDER TEST STATION INSTALLATION

1st Battalion, 1st Special Forces Group (A)

DESCRIPTION OF TRAINING / INSTALLATION.

The contractor must provide all personnel, equipment, tools, materials, supervision, transportation, safety equipment, any other items and services necessary to perform installation of the Hydrostatic Water Jacket Cylinder Test Station as defined in this Statement of Work (SOW) in a Maritime Operations Facility on Torii Station, Japan.

1.1. CONTRACTOR RESPONSIBILITIES

1.1.1. Basic Services: The contractor shall provide all labor, material and equipment to accomplish the scope of work.

1.1.2. Liability for damages – The contractor will be held responsible for any damages that occur by the contractor and/or any personnel that work under this contract to Government facilities, Government owned property and Government purchase furniture.

1.1.3. Contractor is responsible for delivery to Tracy, California where the US Government will forward the Hydrostatic Water jacket Cylinder Test Station to Okinawa, Japan. After the Hydrostatic Water jacket Cylinder Test Station’s arrival to Okinawa via US Government means, the contractor shall be responsible for the unpacking of equipment from its shipping crating and installation of Hydrostatic Water jacket Cylinder Test Station in the designated area, and removal/disposal of rubbish at a coordinated date IAW para 1.1.4.

1.1.4. Contractor will coordinate schedule with 1-1 SFG (A) for installation and beneficial training date. Government will provide storage for Hydrostatic Water Jacket Test Station should beneficial training date change.

1.2. CLEANING AND PROTECTION

1.2.1. Maintain Walk Through – The contractor must maintain common walk through areas to allow proper evacuation of the building in the case of emergencies.

1.2.2. Remove Rubbish – The contractor must remove rubbish, wrapping paper, salvages and scrap. The contractor must not leave waste material inside or outside of the facility overnight, or for a prolonged period of time, when work is not done. Remove and dispose of waste materials only at authorized off base locations, daily.

1.3. TRAINING

1.3.1. The contractor is responsible for providing the following on-site training for up to six 1-1 SFG (A) personnel over the course of two days at the Maritime Operations Facility on Torii Station, Okinawa, Japan once the system is available for use:

1.3.2. Setup and Storage Procedures: Demonstrate proper setup, calibration, tear down, sanitation and long-term storage procedures. Fully test all equipment and hardware for normal operation.

1.3.3. Standard Operating Procedures to include the full live demonstration of how to properly perform a hydrostatic test on a cylinder provided by the user, verification of proper setup of equipment, operational training on daily operations, operational training on testing cylinders, trouble shooting of potential problems, review of proper record keeping, and DOT Hazmat training for cylinder re-testers.

1.3.4. Maintenance Procedures that demonstrate daily, monthly, and annual maintenance requirements.

1.3.5. Training Seminar for the operation of a D.O.T. hydrostatic water jacket cylinder test station, qualification of station operators as "CYLINDER RETESTER HAZMAT"

2. GENERAL INFORMATION.

2.1. SECURITY REQUIREMENTS.

The contractor employees will be required to obtain and display identification badges accordingly. Anticipate delays in getting commercial vehicles on base and allow time for commercial vehicles to reach their destination by driving designated routes at posted speed limits throughout the base. Procedures for commercial vehicle access to the base are subject to change without prior notice.

2.1.1. Access. Daily, in order to access Torii Station, contractor personnel must have government issued identification, those not native to Japan must have their passport. 1-1 SFG (A) will provide escorts to contractor for access to Torii Station, Okinawa, Japan.

2.2. HOURS OF OPERATION.

2.2.1. Normal Hours of Operation. The contractor shall perform the services required under this contract during between the hours of 0800 to 1700, Monday to Friday. The contractor may work, with prior approval of the Contracting Officer, extended hours to ensure timely completion of work at no additional cost to the government.

2.2.1.1.Recognized Holidays. The contractor is not required to provide service on the following days: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.

2.2.2. Emergency Services. On occasion, services may be required to support an activation or exercise of contingency plans outside the normal duty hours described above.

2.3. CONSERVATION OF UTILITIES.

The contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions which prevent the waste of utilities which include the following:

2.3.1. Lights shall be used only in areas where and when work is actually being performed.

2.3.2. Mechanical equipment controls for heating, ventilation, and air conditioning systems shall not be adjusted by the contractor or by contractor employees unless authorized.

2.3.3. Water faucets or valves shall be turned off after the required use has been accomplished.

2.3.4. Government telephones shall be used only for official government business.

2.4. SAFETY REQUIREMENTS

In performing work under this contract, the contractor shall:

2.4.1. Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work.

2.4.2. Perform work in a safe manner as required by OSHA 2206, General Industry, Occupational Safety and Health Standards (29 CFR 1910).

2.4.3. Develop and provide at the start of the orientation period or the start of the first operational performance period (if there is no orientation period) a safety plan for the protection of government facilities and property and to provide a safe work environment for contractor personnel.

2.4.4. Provide protection to government property to prevent damage during the period of time the property is under the control or in possession of the contractor.

2.4.5. Include a clause in all subcontracts to require subcontractors to comply with the safety provisions of this contract as applicable.

2.4.6. Record and report promptly (within one hour) to the contracting officer or designated government representative (GR), all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.

2.4.7. In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the contracting officer.

2.4.8. If the government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed.

2.4.9. Include a clause in each applicable subcontract requiring the subcontractor's cooperation and assistance in accident reporting and investigation.

2.5. CONTINUATION OF ESSENTIAL DOD CONTRACTOR SERVICES DURING

CRISIS IAW DoDI 3020.37. (MANDATORY)

IAW AFI 63-124, para 2.7.1, and DODI 3020.37 this requirement is not considered mission essential.

2.6. ROAD LAWS.

2.6.1. The contractor shall comply with appropriate road laws while on US Military installations.

No person shall commit the following described acts:

2.6.1.1.

Damaging the road, causing roads to be littered with debris, polluting the roads with debris or fall out from motor vehicles or otherwise defacing roads unreasonably or without due cause.

2.6.1.2.

Measures to be taken to prevent cargo or things loaded on motor vehicles from falling out)

2.6.2. The agency managing a road may when there is reasonable cause for him to feel that items or cargo being carried in or on a motor vehicle may fall out and damage, pollute, or otherwise deface the road in such manner as to hinder or obstruct the traffic, order the operator of the vehicle in question to take steps or measures necessary to prevent occurrence of such hindrance by stopping operation of the vehicle, or correcting the method of loading or traveling or operation of the vehicle.

2.7. CONTRACTOR INCURRED EXPENSES.

2.7.1. All expenses incurred by Contractor and employees while executing this contract to include, but not limited to: travel expenses (taxi, plane, rental vehicle, fuel, and public transportation), lodging, and dining will be the sole responsibility of the Contractor to procure. Any and all compensation for these expenses will be the responsibility of the Contractor, and thus considered as included in provided pricing quote.

2.7.2. Contractor shall arrange, book, and pay for all travel reservations and related expenditures for the Contractor and its employees while traveling to and from the vicinity of the Silencer install site and while working and residing in the vicinity of Torii Station.

2.7.3. Contractor shall procure and pay for all lodging and dining for contracted employees.

2.7.4. Contractor shall be responsible for all other miscellaneous expenses necessary to support the subsistence of contracted employees.
