1. DESCRIPTION OF SERVICES.
To procure moving services to relocate USAF Regional Mortuary from building number 115 on
Camp Kinser Okinawa to occupy spaces in building number 4069 Yokota Air Base. The relocation service will include packing, loading, shipping, unloading, delivering and unpacking. The relocation is scheduled to occur on 10 Sep 2014 with a completion date no later than 30 Sep 2014. The items to be moved are office equipment, files, books, some office furniture, appliances, special equipment items such as steel tables, toolboxes, steel cabinets with glass, caskets and other specialty equipment.
1.1. BASIC SERVICES: Contractor shall provide all packaging materials (boxes, labels, tape, etc.), equipment, tools, tracks, forklift, supervision, and labor necessary to pack, label, and relocate for all office equipment, office furniture, appliances, special equipment items such as table-kitchen, steel tables, toolboxes, caskets, chest-ice, boxes and other specialty equipment to assigned space in building number 4069 at Yokota Air Base. The relocation service is included for packing, loading, shipping (from the Okinawa Aja Port to the Tokyo Ariake Port), unloading, delivering and unpacking.
1.1.1. The period of performance for this project shall commence on 10 Sep 2014 with a completion date no later than 30 Sep 2014.
1.2. SPECIAL REQUIREMENTS:
1.2.1. Move Plan
1.2.1.1. A Move Plan including a comprehensive relocation schedule will be provided by the
Government.
1.2.1.2. The Government will provide a detailed day-by-day move plan consisting of date, building number, office number and position number to be vacated and to be occupied on a given date. Daily moves will be confirmed prior to start performance.
1.2.1.3. The contractor will not be responsible to pack and/or move any personal items. The
Contractor shall comply with the Government’s Move Plan.
1.2.2. Contractor shall be responsible for any damage caused to Government property.
1.2.3. The Contractor shall provide all equipment and labor to accomplish the moves described herein.
1.2.4. The Contractor shall arrive at Building 115, Camp Kinser Okinawa on the assigned move date to pack, load, and move the items to the final destination of assigned space in building number 4069 at Yokota Air Base.
1.2.5. Computers and associated peripheral devices will be moved to the designated location(s).
Designated computer workstations, individual computers, and laptops shall be moved by the
Contractor.

1.2.5.1. Computer peripherals (i.e. printers, scanners, monitors) and microfiche equipment shall be appropriately packaged in accordance with commercial industry standards by the Contractor in order to secure and protect the equipment during transportation.
1.2.5.2. Disconnecting and subsequent reconnection of computer and telephone equipment is not in the Contractor’s scope of work.
1.2.6. Specialty equipment will be also moved to the designated location(s) by the contractor. The specialty equipment shall be appropriately packed and unpacked at the designated location(s) according to instruction by the Government.
1.2.7. The contractor shall be allowed to use the loading docks at each facility to access to all necessary floors within building number 4069 at Yokota Air Base.
1.2.8. The contractor shall be required to disassemble and reassemble some executive furniture in order to move it. All required disassembly/reassembly of furniture will be accomplished on the same day as the facility’s move.
1.2.9. The contractor shall be required to inventory boxes prior to being loaded on moving vehicles.
1.2.10. The contractor shall be responsible for ensuring all items have been moved to the new location.
1.2.11. The contractor shall document any damage done.
1.2.12. The contractor will NOT be required to move classified materials.
1.2.13. The contractor is able to re-use boxes to the maximum extent possible if those are in good condition.
1.2.14. The contractor shall field verify all existing conditions at the move sites, noting dock and building access, clearance concerns, and any other issues relating to the move. There will be a joint inspection including the Government and Contractor, of the move sites both before and after the move to determine condition of the buildings and identify any incurred damages.
1.2.15. The contractor shall assume that moves may require multiple tasks within the move period.
1.2.16. The contractor is responsible for the pick-up of all used packing materials, boxes and other debris from building number 4069 at Yokota Air Base.
1.2.17. The contractor is to provide all equipment and supplies necessary to complete the move including but not limited to; machine carts, blankets, four wheelers, shrink wrap, assembly tools, and all other equipment and supplies necessary to complete the move as required.
1.2.18. The contractor is responsible for providing and installing all necessary walls and floor protection at all move sites.

1.2.19. The contractor shall attend a pre-performance meeting and all required move meetings when the Government conducts.
2. GOVERNMENT FURNISHED PROPERTY AND SERVICES. - Not Applicable
3. GENERAL INFORMATION.
3.1. BASE ACCESS: Contractor shall coordinate with the Government personnel to obtain necessary base entry pass prior to commencing work.
3.1.2. The contractor shall notify the government in advance to arrange for escorts into and out of the facilities.
3.2. SECURITY: All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the installation, shall abide by all security instructions and directives of Camp Kinser Okinawa and Yokota Air Base.
3.3. HOURS OF OPERATION: On-site activities involving base access during the move will be scheduled during business hours between 0730 and 1630 Monday through Friday.
3.3.1. Recognized Holidays: The contractor is not required to provide service on Labor Day (first
Monday in September).
3.4. SAFETY: Contractor shall comply with all federal, state, and local safety requirements.
Contractor employees shall wear seatbelts when operating motor vehicles on Camp Kinser Okinawa and Yokota Air Base. The use of cell phones is prohibited unless hand free devices are used.
3.4.1. If at any time during the course of this project the Government has evidence the contractor has put anyone at risk relative to his or her safety and health, the Government will require cease work until the particular situation is resolved.
3.5. SERVICES DURING CRISIS:
3.5.1. The performance of these services is not considered to be mission essential during times of crisis. Should a crisis be declared, the Contracting Officer (CO) or their representative, will verbally advise the Contractor if temporary suspension of work is required, followed by a written notification.
A representative of the CO will further notify the Contractor verbally when the crisis has ended with written notification to follow. Any price changes resulting from this crisis will be negotiated in accordance with FAR clauses for changes.
4. CONTRACTOR PERSONNEL.
4.1. CONTRACTOR MANAGER: The Contractor shall provide a Contract Manager and alternate
Contract Manager (responsible in the absence of the contract manager) who will be the focal point for any Government interface during this contract. The Contract Manager’s responsibilities shall include serving as project liaison with the Government, assisting with the strategic and detailed move planning, and attendance at all required meetings including the pre-performance meeting, project
Attachment 1 status meetings, and move walk-through with the Government. The contract manager and alternate contract manager must be able to read, write, speak, and understand English.
4.1.1. The contract manager and alternate contract manager shall have full authority to act for the contractor on all contract matters relating to daily operations of this contract.
4.1.2. The contract manager or alternate contract manager shall be available during normal duty hours to meet on the installation with government personnel to discuss problems. After normal duty hours the manager or alternate contract manager shall be available within four (4) hours.
4.2. CONTRACTOR EMPLOYEES: The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the CO as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.
4.2.1. Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee name in
English.
5. JAPANESE ROAD LAWS.
5.1. COMPLIANCE: The contractor shall comply with Japanese Road Laws while on US
Military installations to specifically include: Article 43 The Road Law (Japan) (prohibited acts relative to roads). No person shall commit the following described acts;
5.1.2. Damaging the road, causing roads to be littered with debris, polluting the roads with debris or fallout from motor vehicles or otherwise defacing roads unreasonably or without due cause.
5.1.3. Article 43-2 (Measures to be taken to prevent cargo or things loaded on motor vehicles from falling out)
5.1.4. The agency managing a road may when there is reasonable cause for him to feel that items or cargo being carried in or on a motor vehicle may fall out and damage, pollute, or otherwise deface the road in such manner as to hinder or obstruct the traffic, order the operator of the vehicle in question to take steps or measures necessary to prevent occurrence of such hindrance by stopping operation of the vehicle, or correcting the method of loading or traveling or operation of the vehicle.
5.2. REQUIRED INSURANCE BY JAPANESE LAW:
5.2.1. The Contractor shall procure and maintain during the entire period of the performance under this contract the following minimum insurance on each vehicle used by the Contractor at the work site.
5.2.1.1. Type: Automobile Property Damage Insurance Amount: ¥3,000,000 or Dollar equivalent
Bodily Injury Insurance Amount: ¥30,000,000 or Dollar equivalent

5.2.1.2. All vehicles must be properly inspected/insured in accordance with Japanese Compulsory
Insurance (JCI) requirements.
5.2.1.3. Prior to the commencement of work hereunder, the Contractor shall furnish to the CO a certificate or written statement of the above required insurance.
