PERFORMANCE WORK STATEMENT

FOR

36 LRS WRM CONCRETE DISPOSAL

1.0 DESCRIPTION OF SERVICES The contractor shall provide all management, supervision, qualified personnel, supplies, materials, equipment, vehicles, and services necessary for the removal and disposal of a total of 800 (each), 3,000 lb (each) super sacks of concrete located in the Civil Engineer Lower Yard, Andersen Air Force Base, Guam.

1.1 SCOPE Services to be performed include the removal and disposal of 800 (each), 3,000 lb.

(each) concrete super sacks being stored inside containers located in the Civil Engineer

Lower Yard, AAFB Guam. Concrete super sacks that are situated on metal racks within the containers shall be removed by the contractor and metal racks shall be placed back into the container. If concrete super sacks are stuck to the rack, the metal rack can be disposed of with the super sacks. The contractor shall dispose all material to designated disposal site located off base.

1.2 PERIOD OF PERFORMANCE The period of performance shall be for 90 days after award of contract.

1.3 LIABILITY:

The contractor shall be liable for any damages resulting from any carelessness or failure to follow proper safety, installation, engineering, or maintenance procedures and standards.

1.4 GOVERNMENT FURNISHED SERVICES:

The Government will provide emergency medical transportation and treatment if required. The contractor shall reimburse the Government for the cost of these medical services at current rates.

2.0 GENERAL INFORMATION

2.1 HOURS OF OPERATION:

The Contractor shall perform work required under this contract during the hours specified.

Normal duty hours are 7:30 a.m. to 4:30 p.m. on Monday through Friday except on recognized

US holidays or when the Government facility/installation is closed due to local or national emergencies, administrative closings, or similar Government-directed facility/installation closings. The contractor shall at all times maintain an adequate work force for the uninterrupted performance of all tasks defined within this PWS when the Government facility/installation is not closed for the above reasons. When hiring personnel, the contractor shall keep in mind that the stability and continuity of the work force are essential.

2.2 CONTRACTOR PERSONNEL:

The contractor shall provide point of contact (POC) for all contractual matters during all hours of operation. The POC shall respond on-site within 1 hour during normal duty hours of operation and within 2 hours during non-duty hours to discuss concerns with Government personnel designated by the CO to discuss problem areas.

2.3 CONTRACTING OFFICER REPRESENTATIVE (COR):

The (COR) will be identified by separate letter. The COR monitors all technical aspects of the contract and assists in contract administration The COR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract: perform inspections necessary in connection with contract performance: maintain written and oral communications with the Contractor concerning technical aspects of the contract: issue written interpretations of technical requirements, including Government drawings, designs, specifications: monitor Contractor's performance and notifies both the Contracting Officer and

Contractor of any deficiencies; coordinate availability of government furnished property, and provide site entry of Contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting order.

3.0 DEFINITIONS AND ACRONYMS:

3.1 DEFINITIONS:

CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.

CONTRACTING OFFICER. A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.

CONTRACTING OFFICER'S REPRESENTATIVE (COR). An employee of the U.S.

Government appointed by the contracting officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.

DEFECTIVE SERVICE. A service output that does not meet the standard of performance associated with the Performance Work Statement.

DELIVERABLE. Anything that can be physically delivered but may include non-manufactured things such as meeting minutes or reports.

KEY PERSONNEL. Contractor personnel that are evaluated in a source selection process and that may be required to be used in the performance of a contract by the Key Personnel listed in the PWS. When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal.

PHYSICAL SECURITY. Actions that prevent the loss or damage of Government property.

QUALITY ASSURANCE. The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.

QUALITY ASSURANCE Surveillance Plan (QASP). An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance.

QUALITY CONTROL. All necessary measures taken by the Contractor to assure that the quality of an end product or service shall meet contract requirements.

SUBCONTRACTOR. One that enters into a contract with a prime contractor. The Government does not have privity of contract with the subcontractor.

WORK DAY. The number of hours per day the Contractor provides services in accordance with the contract.

WORK WEEK. Monday through Friday, unless specified otherwise.

4.0 SECURITY

4.1 PASS AND IDENTIFICATION ITEMS:

The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, names of employees, and level of security clearance needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The CO will endorse the request and the contractor will submit it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, and valid vehicle insurance certificate to obtain a vehicle pass.

4.2 RETRIEVING IDENTIFICATION MEDIA:

The contractor shall return all identification media and/or visitor passes from employees who depart for any reason before the contract expires; i.e. terminated for cause, reassignment, retirement, etc.

4.3 TRAFFIC LAWS:

The contractor and its employees shall comply with base traffic regulations. Traffic regulations include, but are not limited to:

· Employees shall wear seatbelts when operating or riding in any vehicle (military, company or private owned) while on installation.

· Speed limits on Andersen AFB are 25 MPH unless otherwise posted. Be aware of reduced speed limits and exercise caution when driving in base housing and school zones.

· All vehicle operators on the installation shall not use cell phones unless the vehicle is safely parked or they are using a 100% hands free device. The wearing of any other portable headphones, earphones or other listening devices while operating a motor vehicle is prohibited.

4.4 WEAPONS, FIREARMS, AND AMMUNITION:

Employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within vehicles (military, contractor-owned, or privately owned) while on Andersen AFB.

4.5 REPORTING REQUIREMENTS:

Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified/unclassified defense information. Employees shall be briefed by their immediate supervisor upon initial on-base assignment.

4.6 PHYSICAL SECURITY:

The contractor shall be responsible for safeguarding all Government property and controlled forms provided for contractor use. At the end of each work period, all Government facilities, equipment and materials shall be secured.

4.7 CONTROLLED/RESTRICTED AREAS:

The contractor shall implement local base procedures for entry to AF Controlled/Restricted areas where contractor personnel will work. An AF Form 2586, Unescorted Entry Authorization

Certificate, must be completed and signed by 36 LRS security manager before restricted area badges (RAB) will be issued. Employees must have a completed and favorable NACI investigation before receiving a RAB. Interim access can be granted in accordance with AFI 31-

501, Personnel Security Program Management.

4.8 MILITARY EXERCISES:

No compensation will be claimed for any delays resulting from re-entry procedures due to exercises.
