PERFORMANCE WORK STATEMENT
FOR
SHOP RAGS RENTAL SERVICE
AT
YOKOTA AIR BASE AND
TOKOROZAWA SITE, JAPAN
30 June 2016
1. Description of Services. The contractor shall provide all management, tools, supplies, equipment, and labor necessary to perform the rental and laundry services for shop rags at Yokota Air Base and Tokorozawa site. The total estimated annual workload of each shop is listed in Appendix A. The Contractor shall comply with applicable laws and regulations of the Governments of Japan and the United States including Japan Environmental Governing Standards (JEGS) and AFI 32-7042, Waste Minimization
1.1. Delivery and Cleaning Rags. The contractor shall provide the rags and furnish containers owned by the contractor, and the service shall meet the following requirements:
1.1.1. The rags are provided in sets of 50 rags to different shops. When dropped-off, the rags shall be clean enough for shops to perform the intended job.
1.1.2. The rags shall be at a minimum 35 x 35 cm (13.8 x 13.8 inch) or 1,225 cm2 (1.32 ft2), and the weight shall be no less than 34 g (1.2 oz). The rag shall not exceed a maximum size of 41 x 76 cm (16 x 30 inch). The rag shall be at least 85% cotton and made of soft laid string.
1.1.3. The rags shall be appropriate to wipe light grease, oil, cleaning solvent, other maintenance chemicals and metal particles, and WILL NOT be used for Polychlorinated Biphenyl oil, paint, ink, glue, vegetable/animal oils, or any corrosive chemicals that may damage the rags under discretion of the contractor. Also, the rags WILL NOT be used for heavy metals, which exceed hazardous waste standards under Japan Environmental Governing Standard, Chapter 6,and Hazardous Waste.
1.1.4. Different grade of rags shall be provided for use of metal particles. Users at each shop are responsible to prevent metal contamination by mixing these rags with others, however the contractor shall coordinate with 374 CES/CEOES for any preventive method in order not to mix different rag types.
1.1.5. New rags shall be provided to Tokorozawa site in avoidance of damaging radio tuner coil insulation.
1.2. Containers for Soiled or Contaminated Rags. The contractor shall furnish metal containers for soiled rag collection. The containers shall have a lid and be clearly marked and separated for clean and used rags. The container lid shall be self-closing. Some shops will have more than one container.
1.2.1. A label indicating the rag service’s instruction and caution shall be placed on each container in English and Japanese. The contractor shall coordinate with 374 CES/CEOES for the label.

1.3. Work Schedule. The contractor shall pick up and drop off rental rags in one, two, four, eight week intervals according to shop’s request, which is listed in Appendix A. The contractor shall perform work during normal duty hours, from 0730hrs to 1630hrs Monday through Friday for Yokota Air Base and Tokorozawa site excluding U.S. and Japanese Holidays. Recognized US legal holidays are listed below:

* Holidays falling on a Saturday will be observed on the preceding Friday.

1.3.1. If the contractor cannot complete the delivery service on first available day, the Contracting Officer’s Representative (COR) shall coordinate and establish the new delivery date. If U.S. and Japanese holidays fall on a delivery day the contractor shall notify 374 CES/CEOES and 374 CES/CEIE with a new delivery date in advance. If difficulty to access shops is expected due to exercise or any other reasons, notification of change of delivery date to the contractor shall be made by 374 CES/CEOES in advance.
1.4. Requirement of Environmental Standards. The contractor shall comply with all applicable Japanese standards for the treatment and handling of potentially hazardous wastes generated from cleaning the shop rags and disposal of shop rags which are no longer useful. The standards are listed below:
Japanese Sewerage Law and local Sewerage Ordinance for wastewater treatment.
Japanese Waste Disposal and Public Cleansing Law for transporting, disposing or recycling waste oil and sludge from wastewater treatment, ash from incineration, etc.
Air Pollution Control Law for incinerating un-useful potentially hazardous shop rags, sludge from wastewater treatment, etc.
1.4.1. The contractor shall allow 374 CES/CEOES, 374 CES/CEIE and the Contracting Officer to observe its or its sub-contractor’s wastewater treatment and incinerating procedures to ensure compliance with Japanese standards after the contract is awarded.
1.5. Submittals. The contractor shall submit delivery receipts to each shop’s POC during the time of delivery and a copy of the receipts and an invoice to 374 CES/CEOES every month.
The Contracting Officer’s Representative (COR) will review all submittals
1.5.1 The contractor shall submit AF Form 3000 along with report its or its sub-contractor’s industrial waste transportation and disposal permits, waste disposal manifests and/or recycling documents, and wastewater and air emission sampling results to 374 CES/CEOES, 374 CES/CEIE and the Contracting Officer fourteen (14) calendar days prior to contract award and annually thereafter. Laboratory analyses shall be conducted in a laboratory certified by a Government of Japan authority.
1.6. Free Trial. The contractor shall provide a free trial for two weeks with requested rag amount if any shop is interesting and joining this contract. This trial shall be conducted for determination of delivery interval and rag amount.
1.7. Missing Rags. When the contractor finds any missing rags, the contractor shall submit a written letter including the name of shop (location), date of pick-up and number of missing rags to 374 CES/CEOES. If 374 CES/CEOES determines missing rags are caused by defaults of Government personnel at each shop, the Government will pay for any rags that are lost or missing during this contract performance. Charge of one lost rag will be negotiated and agreed by contract modification.
1.8. Change of Delivery Condition. The shop representative will notify 374 CES/CEOES and 374 CES/CEIE when any changes in the number of rags and in delivery schedule are required.
The number of rags at a shop could increase or decrease in accordance with mission requirement through coordination between 374 CES/CEOES, 374 CES/CEIE and the contractor. The contractor shall not make any changes without coordination with 374 CES/CEOES and 374 CES/CEIE. Substantial increase in quantities or delivery schedule in this contract will be implemented by a modification issued by the Contracting Officer (CO) for funding information.
2. GENERAL INFORMATION
2.1. Project Manager. The contractor shall designate a representative – hereafter referred to as the Project Manager (PM) – who shall be authorized and empowered to act fully on behalf of the Contractor on all matters to this contract, who shall serve as the main point of contact with the Government, and who shall conduct overall management coordination with the Government.
2.1.1. The contractor shall designate an Assistant Project Manager (APM) who shall be authorized to act on behalf of the contractor in the absence of the PM.
2.1.2. The PM and APM must be able to read, write, speak and understand English, or provide translator.
2.1.3. If the costumer or his designee is not available at the facility during the time of delivery, then the contractor shall notify the respective Contracting Officer’s Representative (COR) with the customer’s delivery receipt information.
2.2. Contractor Employee. The contractor shall not employ persons for work on this contract, if such employees are identified to the Contractor by the CO as a potential threat to the health, safety, security, general well-being, or operational mission of the installation.
2.2.1. The contractor’s personnel shall present a neat appearance and be easily recognized as the Contractor employee.
2.2.2. The contractor shall not employ any person who is an employee of the Government if employing that person would create a conflict of interest. Additionally, the contractor shall not employ any individual who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to The Joint Ethics Regulation.
3. GOVERNMENT FURNISHED PROPERTY AND SERVICES.
3.1. GOVERNMENT-FURNISHED UTILITIES. None provided for this contract.
3.2. EMERGENCY SERVICES: The Government will also provide, for emergency purposes only, the following services by dialing DSN: 911:
a. Security Police
b. Fire Department
c. Hospital Emergency (Reimbursable Basis)
3.2.1. Any time the Contractor uses one of the above services, the PM shall immediately report to the Contracting Officer's Representative (COR) with all of the details.
