Joan K. Mendel Elementary School Buildings 4306 and 4315 Pre-Abatement Planning Report

='; H$./"*52.6"$%

There is a planned abatement/renovation project for Buildings 4306 and 4315 of Joan K.

Mendel Elementary School, Yokota Air Base, Japan. The project would entail the removal of all carpet and resilient flooring (except gymnasium flooring) in the buildings and replacement with new resilient floor tiles.

ALPHA Facilities Solutions, LLC (ALPHA) conducted an abatement planning survey, on 28 and 29 July 2011. It had been determined some asbestos containing materials (ACM) need to be abated prior to the renovation. This abatement survey report contains the results of material sampling within the specifically designated areas of Buildings 4306 and 4315. The purpose of this survey was to assure:

• Materials that will be removed or that the renovation may disturb are identified and adequately sampled for asbestos prior to the renovation process.

• If needed, destructive investigation sampling would be performed.

• The potential for enhancement sampling of previously identified materials would be studied and performed, if needed for clarification.

• Quantities of materials to be removed or abated would be clarified.

• Non-ACM materials or portions of non-ACM materials that need to be abated as

ACM were identified and quantified.

The abatement recommendations are made in accordance with U.S. regulations, Japan Environmental Governing Standards (JEGS); previous Asbestos Hazardous Emergency Response Act (AHERA) inspection reports; and a project-specific conducted by ALPHA.

The procedures specified herein are guidelines for minimum performance. The Contractor is responsible for his own methods of operations and conformance to regulatory codes, rules and guidelines. The Contractor is required to obtain all approvals to perform the work, including any rights to use patented systems.

It is recommended that the asbestos abatement contractor shall have a minimum of three

(3) years experience performing asbestos abatement and shall have successfully completed three (3) projects on U.S. bases using U.S. Environmental Protection Agency (EPA) regulations. The abatement contractor shall submit documentation of these projects to show of experience in similar size and scope of work.

Mr. Mark F. Spence, of ALPHA, an AHERA Building Inspector, Management Planner, Contractor Supervisor and Project Designer, conducted the abatement survey inspection and prepared this report. The proposed renovation is understood to include:

Joan K Mendel Elementary School

! 3!

• Removal and Replacement of all resilient floor tile, resilient floor sheeting (except gymnasium flooring), carpet, and cove base in Buildings 4306 and 4315 of Joan K.

Mendel Elementary School.

• Abatement and replacement of any ACM or portion of ACM that would be disturbed in the process of the planned renovation.

Previous AHERA Reports were reviewed to identify ACM that has the potential to be disturbed during this project.

In addition to pre-abatement planning efforts for Buildings 4306 and 4319, a floor tile off-gassing study was previously conducted for Building 4319. For convenience, background information and results of that study are provided in Appendix E.

='=% ADD+62#B+)%C)75+#.6"$8%#$*%1.#$*#/*8%

U.S. and Japan asbestos abatement regulations differ significantly. Asbestos abatement contractors bidding on this project shall need to be knowledgeable of the requirements of both regulations including but not limited to:

• EPA, Toxic Substances Control Act (TSCA)

• EPA AHERA 40 CFR 763 Subpart E

• EPA National Emission Standards for Hazardous Air Pollutants (NESHAP)

Regulations for Controlling Visible Emissions, National Emission Standard for Asbestos, Title 40, Part 61, Subpart M of the Code of Federal Regulations

• EPA Guidelines for Disposal of Solid Waste, Title 40, Part 241 of the Code of Federal Regulations

• Occupational Safety and Health Administration (OSHA) Occupational Exposure to Asbestos, Title 29, Part 1910, Section 1001 and Part 1926, Section 1101 of the Code of Federal Regulations

• OSHA Respiratory Protection, Title 29, Part 1910, Section 134 of the Code of Federal Regulations

• OSHA Training and Work Practices, Title 29, Part 1926, Section 26 of the Code of Federal Regulations

• OSHA Employee Exposure and Medical Records, Title 29, Part 1910, Section 20 of the Code of Federal Regulations

• OSHA Specifications for Accident Prevention Signage and Labeling, Title 29, Part 1910, Section 145 of the Code of Federal Regulations

• Japan Environmental Governing Standard (JEGS) Section 15

Note: Government of Japan (GOJ) and U.S. regulations both apply, however the stricter of the two shall be enforced. When procedures are different from each other, such as in decontamination procedures, the U.S. process shall apply. GOJ and local prefectural regulations shall include landfill and transportation regulations when material is transported off base.

Mendel ES, Building 4319, Non-ACM Floor Tile Removal

The non-asbestos containing material (Non-ACM), resilient floor tile Material number (MN) 199 located in Building 4319, was reported to have odorous off- gassing. A sample of this material was sent to an analytical laboratory specializing in volatile organic compounds and equipped with odorous sensing instrumentation. The analytical results were review by a certified industrial hygienist (CIH) and it was recommendation that this material should be removed. See “Odor Evaluation of A Flooring Product” (AQS Report 17007-01), by Air Quality Sciences, March 8, 2011 for off-gassing analytical results.

• A portion of MN198 lay over a section of ACM subflooring material (MN184). This material is an epoxy used as a filler material for cracks along the concrete floor of the second floor. The exact location of this epoxy material was measured and documented on a floor plan drawing during the 2009, Building 4319 abatement project. (See Attachment A - Location of that this material required Material 184 Drawing). Investigative testing during the 2010 abatement project indicated diamond tipped sanding equipment to remove the epoxy. (See2010 Abatement Oversight and Clearance Report, Mendel Elementary School, by ALPHA Facilities Solutions, LLC).

Therefore it was determined by an AHERA certified project designer Mark Spence (MFS) of ALPHA Facilities Solutions (ALPHA) that the flooring MN199 could be removed without disturbing MN184. A meeting was held to discuss how to proceed with this activity.

On 6/28/2011 Mark Spence, (an AHERA Project Manager, Project Designer, Supervisor and NIOSH 582 Abatement Monitor) met with Mr. Steve Dreskler of Japan DSO, Mr.

Nick Kulyk of SupplyCore Inc., Mr. Hisao Matsunnaga of Seikitokyu Kogto, and Mr.

Yugii Kubota of Sublett-Japan Inc., (an asbestos abatement contractor), to discuss what was needed to remove resilient floor tiles (MN198) in areas of Building 4319, Second Floor that were located above ACM Epoxy crack sealant (MN184).

Assumptions:

All of the floor tile except above the floor above the epoxy that fills cracks on the second floor of the second floor will be removed. Below is list of problems with leaving the tiles in place:

• The possibility of lingering odor from old tiles

• Mismatch of coloring from old tiles to new

• Incomplete abatement of odorous tiles

• Old tiles collect dirt and do not clean.

Assumptions:

• This is not an asbestos abatement.

• The removal of non-ACM flooring (MN198) with heavy off-gassing odors is considered an emergency project. See “Odor Evaluation of A Flooring Product”

(AQS Report 17007-01), by Air Quality Sciences, March 8, 2011 for off-gassing analytical results.

• Non-ACM floor adhesive (MN199) will not be separately removed after tile removal (M198). The existing adhesive in questionable area will remain in place.

• The non-ACM tile (MN198) removal is to be done by certified asbestos workers.

• The area of work will be taped off with asbestos marking tape. (The entire Second

Floor). The entire building will be empty during the work.

• Area sampling will be conducted and analyzed on site to assure fiber release is kept below the PEL.

• Since this work does not plan to disturb asbestos, clearance sampling will not be required unless fiber release problems are encountered.

• Test removal of the epoxy material following the 2010 asbestos abatement showed that the material was non-friable, and that it required diamond tipped grinders to remove the material.

• On-site results of the floor removal air monitoring will be included as an appendix to the 2011 Mendel ES Pre-Planning Report for Buildings 4306 and 4315.

• This work will be performed the same day as clearance sampling takes place at Yokota West ES, which is one day before flooring removal is scheduled for the rest of Building 4319.

It was decided that the tiles in question could be removed prior to start of removal of existing floor tiles with cooperation between the Contractor Supervisor/Project Designer/ NIOSH 582 Asbestos Air Monitor for ALPHA and the Sublett–Japan, the abatement company. This work would take place on 7/6/2011.

7/6/11 – Sublet –Japan, an asbestos abatement-contracting firm removed the non-ACM Floor tile to insure the floor removal did not disturb the epoxy below. As a precaution, certified abatement workers were used to remove the non-ACM tile. The area was roped off, the building was empty during abatement and until on-site analysis indicated asbestos was not disturbed beyond the asbestos action level. Air monitoring was conducted during the procedure. The samples were analyzed on site and the analytical results indicate that the procedure never exceeded the action level for asbestos. Since no asbestos was removed or disturbed clearance samples were not required.
