Addendum to 52.212-1(k), Central Contractor Registration. The paragraph is tailored as follows:
(k) System for Award Management Registration.
(1) Definitions. As used in this section-
"Central Contractor Registration (CCR) database" means the retired primary Government repository for Contractor information required for the conduct of business with the Government.
"Commercial and Government Entity (CAGE) code" means-
(i) A code assigned by the Defense Logistics Agency (DLA) Logistics Information Service to identify a commercial or Government entity; or
(ii) A code assigned by a member of the North Atlantic Treaty Organization that DLA records and maintains in the CAGE master file. This type of code is known as an "NCAGE code."
"Data Universal Numbering System (DUNS) number" means the 9-digit number assigned by Dun and Bradstreet, Inc. (D&B) to identify unique business entities.
"Data Universal Numbering System+4 (DUNS+4) number" means the DUNS number means the number assigned by D&B plus a 4-character suffix that may be assigned by a business concern. (D&B has no affiliation with this
4-character suffix.) This 4-character suffix may be assigned at the discretion of the business concern to establish additional SAM records for identifying alternative Electronic Funds Transfer (EFT) accounts (see the FAR at Subpart 32.11) for the same concern.
"Registered in the SAM database" means that-
(i) The Contractor has entered all mandatory information, including the DUNS number or the DUNS+4 number, into the SAM database;
(ii) The Contractor's CAGE code is in the SAM database; and
(iii) The Government has validated all mandatory data fields, to include validation of the Taxpayer Identification Number (TIN) with the Internal Revenue Service (IRS), and has marked the record "Active". The Contractor will be required to provide consent for TIN Attachment, Page 1 of 4 validations to the Government as a part of the SAM registration process.
"System for Award Management (SAM)" means the primary Government repository for prospective federal awardee information and the centralized Government system for certain contracting, grants, and other assistance related processes. It includes-
(i) Data collected from prospective federal awardees required for the conduct of business with the Government;
(ii) Prospective contractor submitted annual representations and certifications in accordance with FAR Subpart 4.12; and
(iii) The list of all parties suspended, proposed for debarment, debarred, declared ineligible, or excluded or disqualified under the nonprocurement common rule by agencies, Government corporations , or by the Government
Accountability Office.
(2)
(i) The Contractor shall be registered in the SAM database prior to submitting an invoice and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation.
(ii) The SAM registration shall be for the same name and address identified on the contract, with its associated CAGE code and DUNS or DUNS+4.
(iii) If indicated by the Government during performance, registration in an alternate system may be required in lieu of SAM.

(End of provision)
Addendum to FAR 52.212-2:
This is a competitive Lowest Price Technically Acceptable (LPTA) source selection in accordance with the Federal Acquisition Regulation (FAR) Part 12 and Part 15. By submission of its offer in accordance with the instructions provided in clause FAR 52.212-1, Instructions To Offerors (and its addendum), the offeror accedes to the terms of this model contract. All technically acceptable offers will be treated equally except for price and past performance record. Failure to meet a requirement may result in an offer being determined “Unacceptable.” The evaluation process will proceed as follows:
A. Technical Acceptability. Initially, the Government technical evaluation team will evaluate the technical proposals by assigning an Acceptable or Unacceptable rating (see Table 1 below). The offeror is required to submit one (1) copy of proof (Diploma, License/Certificate, or Transcript) for each applicant at each location for assessment of technical acceptability. License must be current on the date of receipt of proposal(s), and maintained throughout the life of the contract. The proposals will be evaluated against the following subfactors:
Subfactor 1 - Education:
(a) Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN), Utilization Manager: Must have graduated from a State Board of Nursing approved LPN/LVN training program or completion of equivalent United States military training that permits sitting for a state licensure examination.
(b) Registered Nurse Case Manager: Must have graduated from an associate degree (AD) or a baccalaureate degree program in nursing (BSN) accredited by a national nursing accrediting agency recognized by the US Department of Education.
(c) Social Worker Case Manager: Master of Social Work (MSW) degree from a Council on Social Work Education (CSWE) accredited school.
Subfactor 2 – License/Certification:
(a) Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN), Utilization Manager: Nurse applicants must be a current U.S. LPN/LVN. Active, current, valid and unrestricted license to practice nursing in accordance with State Board requirements. License cannot be under investigation nor have any adverse action pending from a Nursing State Board or national licensing/certification agency.
(b) Registered Nurse Case Manager: Nurse applicants must be a current U.S. licensed Registered Nurse. Active, current, valid and unrestricted license to practice nursing in accordance with State Board requirements. License cannot be under investigation nor have any adverse action pending from a Nursing State Board or national licensing/certification agency.

(a) Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN), Utilization Manager: Four (4) years of clinical nursing experience is required. LPN/LVN shall have held full time employment in a nursing field for twelve (12) out of the last twenty-four (24) months (mandatory) from the date of receipt of proposal. Equivalent combinations of education and experience may be qualifying if the experience is directly related to utilization management. Equivalency combinations must be acceptable to the Medical Treatment Facility and approved by the Contracting Officer.
(b) Registered Nurse Case Manager: RN must have at least five (5) years of licensed nursing practice. Two years’ experience in Discharge Planning or Case Management is required. Certification in case management from the American Case Management Association, the American Academy of Case Management or the American Nurses Credentialing Center issued within the last two years from the date of receipt of proposal may be substituted for direct case management experience.
(c) Social Worker Case Manager: A minimum of two years of full-time post-Master’s Degree experience within the last four (4) years from the date of receipt of proposal working a professional case management or discharge planning program with adults, children, families, seniors and groups.

(d) The Letter of Intent must be received no later than 15 Jul 2013, 12:00 pm Japan Standard Time (JST). LATE LETTERS OF INTENT WITH THEIR ADDITIONAL MATERIALS WILL BE REJECTED REGARDLESS OF THE REASON.
Table 1, Technical Acceptability Ratings
Rating
Description
Acceptable
Proposal meets the requirements of the solicitation.
Unacceptable
Proposal does not meet the requirements of the solicitation.
Note: The technical proposal must “Pass” all of the subfactors to be determined “Acceptable.” There will be no point scores assigned, only “Pass” or “Fail.” In order to be considered “Acceptable,” there must be a “Pass” rating in every subfactor. Even if a proposal fails a subfactor, the evaluators will continue to assess the remaining portions of the proposal; the entire proposal must be evaluated. In order to be considered awardable, there must be a “Pass” rating in every subfactor. Only those proposals determined to be technically “Acceptable,” either initially or as a result of discussions, will be considered for award.
B. Past Performance. Next, using questionnaires, the Government will seek recent and relevant performance information on all technically acceptable offerors based on the references provided by the offeror and data independently obtained from other Government or commercial sources.
(i) Recent performance information includes relevant contracts performed for the Department of Defense, or Government of Japan, or US Government agencies, or commercial customers within the last three (3) years from the issuance date of the solicitation.
(ii) Relevant performance includes performance of efforts that is similar in scope, magnitude, and complexity described in this solicitation. The Government will use the relevancy definitions and ratings in Table 2 below when assessing for relevancy:
Table 2. Past Performance Relevancy Ratings
Rating Definition
Rating Definition
Very Relevant
Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation
Relevant
Present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires.
Somewhat Relevant
Present/past performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires.
Not Relevant
Present/past performance effort involved little or none of the scope and magnitude of effort and complexities this solicitation requires.
(iii) Performance Quality Assessment: The Government will consider the quality of recent and relevant efforts. For each of the recent and relevant past performance information reviewed, the performance quality of work will be assessed and assigned ratings based on the Quality Assessment Criteria evaluated under Paragraph 4 of Present/Past Performance Survey (Solicitation Attachment 3)
The Past Performance Evaluation results in an assessment of the offeror’s probability of meeting the solicitation requirements. Offerors must receive a past performance rating of “Acceptable” to be eligible for award. Past performance information based on the experience of proposed key personnel and/or any subcontractors will not be considered as past performance information for the principle offeror. For joint ventures, the government may consider the relevant past performance history of the individual joint venture partner of the prime contractor. If at least one partner receives an “acceptable” rating, the teaming partners will receive an “acceptable” past performance rating overall.
The Past Performance factor will receive one of the ratings in Table 3 below:
Table 3, Past Performance Evaluation Ratings
Rating
Description
Acceptable
Based on the offeror’s performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown.
Unacceptable
Based on the offeror’s performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort.
Note: In the case of an offeror without record of relevant past performance or for whom information on past performance is not available or so sparse that meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305(a)(2)(iv)). Therefore, the offeror shall be determined to have an unknown past performance. In the context of acceptability/unacceptability, “Unknown” shall be considered as “Acceptable.”
C. Price Evaluation. Lastly, the Government will evaluate all offers with acceptable technical and past performance ratings by price, including all option prices. An offeror’s proposed prices will be determined by multiplying the quantities identified in the Price Schedule by the proposed unit price for proposed unit for each Contract Line Item Number to confirm the extended amount for each. The price evaluation will document the reasonableness and completeness of the proposed total evaluated price. The Government reserves the right to perform price realism analysis.
In order to meet the requirements of FAR Part 6 and FAR Part 17, the price equivalent to the six (6) months extension of services authorized by FAR 52.217-8 will be evaluated separately, and will be factored as part of the total evaluated price for award. The Government will calculate prices for a quantity equivalent to 6 months by prorating prices offered for CLIN 0001 thru CLIN 0006 and all corresponding CLINs under Options of FAR 52.217-9 in order to evaluate the extension of services authorized by FAR 52.217-8.
D. Offerors are cautioned to submit sufficient information and in the format specified in the FAR 52.212-1 and/or its addendum. Offerors may be asked to clarify certain aspects of their proposal. Communication conducted to resolve minor or clerical errors will not constitute discussions and the Contracting Officer reserves the right to award a contract without the opportunity for proposal revision.
E. The Government intends to award a contract without discussions with respective offerors.
