SECTION 1

DESCRIPTION OF SERVICES / GENERAL INFORMATION

1. DESCRIPTION OF SERVICES. The Contractor shall provide all management, supervision, tools, supplies, parts, equipment, and labor necessary to provide maintenance for the elevator at Building 100, Kure Pier 6 to commercial standards to ensure continuous and safe operation in accordance with the Japanese Building Standard Law and Japanese Industrial

Standards. The following identifies the specification and location of the elevator:

1.1. Inspections & Maintenance. All inspections & maintenance work shall be in accordance with commercial practices meeting the manufacturer’s specifications and is intended to maintain the elevator in safe and reliable operating condition until the next scheduled maintenance.

Minimum requirements of maintenance work and frequency are described in APPENDIX A, para 4.2.

1.1.2. Repair/Replacement. The contractor shall perform repairs and replacement work as required during the scheduled inspection and maintenance. The defective parts, discrepancies and needed adjustments that are found during inspection shall be repaired, replaced or corrected by the contractor. Minimum parts and materials that the contractor is required to furnish and install under maintenance are shown in Appendix A, para 4.1.

If repairs and replacement work cannot be accomplished immediately, repairs and replacement work shall be accomplished within 3 days upon identification of defective parts, discrepancies and needed adjustments.

Exceptions are noted in Appendix A, paragraph 4.1.6. Any repairs that are beyond the scope of this contract shall be brought to the immediate attention of the Government Representative (GR).

1.1.3. Report. The contractor shall prepare and submit written reports both in English and

Japanese within 10 days after completion of all inspection and maintenance. The report shall identify each elevator, by location, the maintenance work performed the repairs needed, the date of inspection, the name of inspector, the overall condition of the elevator and information concerning any repairs needed that are beyond the scope of full maintenance. The contractor shall undertake detailed coordination with GR for the use of blank forms provided for maintenance and inspection reports inclusive repair services. Blank forms are attached in

Appendix B for the contractor’s information and may be reproduced in similar format unless the contractor's alternative format is accepted by the U.S. Government.

1.1.4. Schedule. The contractor shall provide the first inspection & maintenance schedule no later than (NLT) 5 days prior to contract start date. The contractor shall provide an annual schedule NLT 15 days prior to the end of the current contract year to be used for the following contract year. The schedule will show the building, time and day of the inspection and maintenance service. The GR will evaluate the contractor’s schedule and will send recommendations to the GR for approval. The contractor shall not deviate from the approved schedule without prior approval of the contracting officer or the GR. The contractor shall notify the date and time of the next scheduled inspection and maintenance service to each building manager when the site work is completed.

1.1.5. On-Call Repair/Replacement Service. The contractor shall provide On-Call

Repair/Replacement Services when requested by the GR. The contractor shall respond to on-call

Repair/Replacement services within 1 hour after notification 24 hours a day, 7 days a week including holidays throughout the contract period. The On-Call Repair/Replacement Service is to fix minor malfunctions that can be accomplished at the time of on-call Repair/Replacement

Service. If repairs are unable to be made at the time of the response, the contractor shall take temporary measures for safety and minimize damage at the time of the response. The contractor shall report to the GR the details of the on-call service after its completion. If

Repair/Replacement is unable to be performed at the time, the contractor will provide an estimated time to complete the repair to the GR and post appropriate warning signs. The contractor shall prepare and submit a written report to the Akizuki Sub Facility Engineer, Directorate of Public Work (DPW), ATTN: IMJN-PWS-A, within two business days after the on-call service. The report shall include the date and time of the GR call, the location of the elevator, the repairs and the name of the technician performing the repairs. Estimated number of on-call service is 2 times a year.

1.1.6. OTHER REPAIRS. For those repairs not covered under this contract, the contractor shall commence repair work only after approval from the Contracting Officer (KO). These

“other repairs” are not included in the basic unit price of the contract. The KO will determine whether these repairs can be accomplished by a contract modification or by a separate contract action. All repair work shall be in accordance with normal commercial practices using parts specified by the manufacturer or items of equal specifications. The contractor shall provide the

GR and the KO an estimated cost to include a breakdown of material and labor costs, all projected downtime and estimated time associated with the repair. Each of these occurrences shall be a matter of negotiation between the contractor and the KO.

1.2. Contractor Manpower Reporting: The Office of the Assistant Secretary of the Army

(Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor will report ALL contractor manpower (including subcontractor manpower) required for the performance of this contract. The contractor is required to completely fill in all the information in the format using the following web address:

https://contractormanpower.army.pentagon.mil. The required information includes: (1)

Contracting Office, Contracting Officer, Contracting Officer’s Technical Representative; (2) https://contractormanpower.army.pentagon.mil/

Contract number, including task and delivery order number; (3) Beginning and ending dates covered by reporting period; (4) Contractor name, address, phone number, e-mail address, identity of contractor employee entering data; (5) Estimated direct labor hours (including sub-contractors); (6) Estimated direct labor dollars paid this reporting period (including sub-contractors); (7) Total payments (including sub-contractors); (8) Predominant Federal Service

Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each sub-contractor if different); (9) Estimated data collection cost; (10) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army

Requiring Activity is responsible for providing the contractor with its UIC for the purposes of reporting this information); (11) Locations where contractor and sub-contractors perform the work (specified by zip code in the United States and nearest city, country, when in an overseas location, using standardized nomenclature provided on website); (12) Presence of deployment or contingency contract language; and (13) Number of contractor and sub-contractor employees deployed in theater this reporting period (by country). As part of its submission, the contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. Reporting period will be the period of performance not to exceed 12 months ending 30 September of each government fiscal year and must be reported by 31 October of each calendar year. Contractors may use a direct XML data transfer to the database server or fill in the fields on the website. The XML direct transfer is a format for transferring files from a contractor’s systems to the secure web site without the need for separate data entries for each required data element at the web site. The specific formats for the XML direct transfer may be downloaded from the web site.

1.3. GENERAL INFORMATION.

1.3.1. GOVERNMENT REMEDIES. The KO shall follow FAR 52.212-4, Contract Terms and

Conditions-Commercial Items for the contractor’s failure to perform satisfactory services or failure to correct non-conforming services.

1.3.2. AVAILABILITY OF FACILITIES.

1.3.2.1. Hours of Services. The contractor’s personnel will perform scheduled work on week days, Monday through Friday (exclusive of the U.S. Federal and Japanese holidays) shown below, during normal duty hours, 0800 – 1600 hours unless prior arrangements have been made with the GR.

And in the case of on-call services, the contractor will be required to remain available 7 days a week, 24 hour a day.

1.3.3. INSTALLATION/BASE PASSES. The U.S. Government will issue all required

Installation/Base Passes and Controlled Area Badges to contractor personnel at no cost to the contractor. The prime contractor shall be responsible for all passes issued to his own or his subcontractor’s employees under this contract. The contractor shall return all applicable passes upon termination of employment or completion of this contract to the Japanese Pass Section, and a certification of receipt shall be obtained. Final payment will be withheld until the contractor has been relieved of all accountability and/or obtained the necessary pass clearance certificate for

Government issued installation/base passes used in the performance of this contract.

1.3.4. SECURITY REQUIREMENTS.

1.3.4.1. Security Danger Warning, Accident Prevention, Fire Prevention and Fire

Protection Requirements. The contractor shall comply with all pertinent requirements of the

U.S. Army and USARJ Fire Protection and Safety Regulations, and insure compliance by all subcontractors. Fire protection and safety standards shall be made available to the contractor.

All temporary wiring at the construction site shall be installed with ground fault protection.

1.3.4.2. The contractor shall not commence work at the job site until a standard commercial

Accident Prevention Plan and a Job Hazard Analysis are submitted for review and approved by the Safety Officer through and GR. The contractor is responsible for reviewing section 1 of EM

385-1-1, the US Army Corps of Engineers’ Safety and Health Requirements Manual. The

Government shall provide appropriate manuals to the contractor.

1.3.4.3. The contractor shall provide warning signs and barricades while elevators are being serviced. The contractor shall control persons desiring access into the elevator when work is in progress.

1.3.5. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY

SECRETARY OF DEFENSE OR THE OVERSEAS COMBATANT COMMANDER.

This provision is not applicable since no services are required during the crisis. The CO or GR shall notify the contractor, by any means of communication, of a crisis. There shall be no costs paid by the Government to the contractor during a crisis except those costs incurred for work performed under this contract and PWS.

1.3.6. SPECIAL QUALIFICATIONS.

1.3.6.1. Contract Manager: The contractor shall provide a contract manager who shall be responsible for the work. The name of this person, and an alternate(s) who shall act for the contractor when the manager is absent, shall be designed in writing to the KO.

(1). The contract manager or alternate(s) shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract.

(2). The contract manager or alternate(s) shall be available during normal duty hours within 2 hours to meet on the installation with Government personnel (designated by the KO) to discuss problem areas. After normal duty hours, the manager or alternate(s) shall be available within 4 hours.

(3). The contract manager and alternate(s) must be able to read, write, speak, and understand

English.

(4). The contractor personnel shall be certified in accordance with the appropriate GOJ regulatory agency to meet the certification requirements in the maintenance, inspection and repair services of elevators.

1.3.7. RELATIONSHIP WITH MANUFACTURERS. The contractor will need specific and pertinent understanding of the elevators covered in this contract to perform the work. The contractor must have all necessary documents, a thorough comprehension, access to the related technology, data, drawings, parts lists and parts from the manufacturers which are needed for

100% satisfactory performance; therefore, it is necessary that the contractor establish and/or maintain relationships with manufacturers of the specified elevators and have access to such information and parts.

1.3.8. QUALIFICATION. Contractor personnel shall also possess Inspector for Elevators (昇

降機検査資格者) license at all times while performing work. The contractor shall submit a list with the employee’s name and licenses to the GR no later than pre-performance conference.

1.3.9. GOVERNMENT REPRESENTATIVE (GR). The contractor shall coordinate all aspects of this project with the GR. Completed work shall be subject to inspection and acceptance by GR. The GR shall NOT direct or redirect any contractor action(s). This can only be done by the Contracting Officer. The GR shall NOT Advise the contractor HOW to perform but rather WHAT is required per this PWS and contract.

1.3.10. CONTRACTING OFFICER AUTHORITY. Only the contracting officer (KO) has authority to enter into, administer, terminate or modify the terms and conditions herein. Any changes made by persons other than the KO will be considered an Unauthorized Commitment and subject to Ratification Actions in accordance with the Federal Acquisition Regulation part

1.602-3.

3.2. OTHER GOVERNMENT-FURNISHED ITEMS:

3.2.1. Utilities. The Government will furnish available water and electricity at existing outlets as may be required for the work to be performed under the contract at no cost to the contractor. The contractor shall provide and maintain, at his expense, the necessary service lines from existing

Government outlets to the site of work. All temporary installations and connections made by the contractor shall comply with prevailing safety, fire, sanitary regulations, and applicable building codes. Temporary installations and connections shall be subject to periodic inspection by authorized GR.

4.1.6.2. Complete replacement of electric motor, hydraulic cylinder, controller or cab. Construction work necessary for the replacement of component parts.

4.1.6.3. Improvement of the equipment or the addition of new accessories.

4.1.6.4. Repairs required due to carelessness, vandalism, unsuitable use, earthquake, fire, explosion or other accident.

O Check for wear and make sure the door rail and hanger rollers are lubricated. Check the condition of the ekisen (bearing of the hanger rollers) roller.

O Examine the interlock of car doors and landing doors, check for excessive heat from the resistor for the doors, make sure all bands and nuts are tight.

O Check that it is working properly, make sure the lead wire is in good condition and that the bolts are tight.

Device O Test the governor interlocking device and switch.

Inside Light O Test the light and make sure it is bright enough.

Connection O Make sure the interphone works and test the emergency buzzer.

Panel O Check the working condition of the buttons and if the lamps go on.

Safety Edge O Operation test.

Floor O Examine the floor for damage and make sure no tiles are peeling, correct level error by stopping at each floor.

Panels O Examine the panels, joints, plates, and paint condition and check for damage.

Hall Indicator

Button O Make sure it is working properly and that the lamps go on.

O Make sure the interlock switch is working properly and check the condition of the lock.

O Make sure the door rail and door hanger rollers are lubricated and that all bolts are tight.

O Check the condition of the door lever and door shoe and make sure the lever actuator is lubricated.

Landing

Doors O Check the condition of the ekisen (bearing of the hanger rollers) roller and make sure the terminal door closing device is working properly.

O Check the space between the doorknob and razor and make sure they interlock.

O Check the condition of the doors, remove dirt from the sill groove, and make sure the rubber doorstop is in place.

Hall Landing

Doors O Check for door interlocking rope wear and make sure all bolts are tight.

Hoistway Guide Rail O Check for damage, rust, and make sure the joint of the rail and the running surface are in good condition.

O Make sure all attaching bolts, clips, and brackets are tight.

O Make sure the up final and up limit switches are working and check the rotation of the roller.

Switch O Make sure the upper deceleration limit switch is working and check the rotation of the roller.

O Inspect the internal contact point of each limit switch and make sure all nuts are tight.

Rope O Adjust the tension balance and check for wear and rust on the ropes.

O Check the condition of the rope ends and their contact with other parts.

O Make sure it is installed properly, check the through bolts and the simple rod's double nut cotter pins.

Counter weight O Make sure there is enough oil in the lubricator and that the suspension sheaves bearing are lubricated.

O Examine the wire that holds the suspension sheaves, check for groove wear and for any abnormal noises.

O Make sure it is installed properly, check for deformation and make sure all bolts are tight.

O Inspect the internal contact point of each limit switch and make sure all nuts are tight.

Tension, etc. O Check the abnormal noises from bearings. Make sure the groove is in good condition and lubricated.

O Check the space of the pit floor and the condition of the balance chain and rope.

Safety

Device O Check the condition of the catch box, shoe, and rollers on the rail.

O Test operation and reset.

Counterweig ht O Measure spring buffer and runway.

Pit Travelling

Cable O Check the condition while the elevator is operating.

(swing, torsion, etc.)

O Check for scratches or deterioration of the coating and make sure the cable is installed properly (up or down).

Buffer O Check for corrosion and deformation.

O Oil circuit type: check the oil, examine for plunger damage and rusting.

Condition O Check for water leakage and oil splatter.

O Check for dirt and test the inspection lamp.
