REQUEST FOR QUOTATION:

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FA520517QB001; this solicitation is a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-95, DPN 20161222, and AFAC 2017- 0127. This acquisition will be full and open competition.

Offerors may use another format for submission of a response to this RFQ. All responses must contain the minimum information required as listed below.

Award shall be made in the aggregate, all or none.

This requirement is subject to the availability of funds. This solicitation may be canceled at any time by the governement. No award will be made unless funds become available.

INSTRUCTIONS TO OFFERORS:

To assure timely and equitable evaluation of quotes, offerors must follow the instructions contained herein. Only offerors who submitted the items in para. (a) and (b) in accordance with para. (c) are eligible to attend Music Demonstration.

(a) Submit offeror information and the price as specified on page one of this RFQ.

(b) Prepare a resume and a copy of PCS orders (if applicable) to demonstrate qualification requirements per section

1.7. of the PWS.

(c) Submit above items to below POC by 7 Mar 17 @ 16:00 local time by Email.

(d) Music Demonstration per section 1.7. of the PWS shall take place as specified below:

Date/Time: 7 Mar 17 at 0930 hours Location: Misawa Air Base Chapel The title of the music is attached to this RFQ.

EVALUATION CRITERIA:

Evaluation of quotations will be completed in accordance with FAR 13.106-2. The Government will award the contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation and represents the best value to the Government.

Evaluation Factors: (1) Technical and (2) Price.

Technical capability will be evaluated with a Music Demonstration per section 1.7. of the PWS.

Contractors must be actively registered with the System Award Management (SAM). Lack of registration in the SAM database will make an offeror ineligible for award. The Contractor is required to confirm on an annual basis that its information in the SAM database is accurate and complete at https://www.sam.gov/. (IAW FAR 52.232-33 Payment by Electronic Funds Transfer- System Award Management, Jul 13)

Contractors must be registered in Wide Area Work Flow (WAWF). The WAWF system is the method to electronically process vendor payment requests and receiving reports, at http://wawf.eb.mil/. ADDENDA TO FAR 52.212-2

The following paragraphs are added.

(d) Award will be made to the Lowest Priced offer that meets the Technical Acceptability.

Technical Acceptability. The government will evaluate technical acceptability of each offer through a music demonstration and review of resume per section 1.7. of the PWS, on a pass/fail basis and assign an Acceptable or Unacceptable rating as described in Table 1 below.

Price. The government will rank and evaluate and all quotes by total price including all option prices. Offerors are reminded that evaluating the option prices does not obligate the government to exercise the Options.

ADDENDA TO FAR 52.212-4

FAR 52.212-4 is tailored as follows:

Paragraph (k), Taxes, is changed to read "Exemption from Japanese customs duties and taxes. Resulting contract shall be exempt from customs duties and taxes of Japan, in accordance with article XI and XII of the status of Forces Agreement between Japan and the United States.

This is applicable to Japanese contractors only.

AFFARS 5352.201-9101 -- OMBUDSMAN (APR 2014)

(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.

(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

(c) If resolution cannot be made by the contracting officer, the interested party may contact the ombudsmen, Air Force Installation Contracting Agency (AFICA)/KP Director, 1940 Allbrook Drive, Building 1, Wright- Patterson AFB OH 45433, Phone: (937)257-5529, Fax: (937)656-0919. Concerns, issues, disagreements, and recommendations that cannot be resolved at the Center/MAJCOM/DRU/HQ AFICA/AFISRA/SMC ombudsman level, may be brought by the interested party for further consideration to the Air Force ombudsman, Associate Deputy Assistant Secretary (ADAS) (Contracting), SAF/AQC, 1060 Air Force Pentagon, Washington DC 20330- 1060, phone number (571) 256-2395, facsimile number (571) 256-2431.

(d) The ombudsman has no authority to render a decision that binds the agency.
