PERFORMANCE-BASED WORK STATEMENT

FOR

PREVENTIVE MAINTENANCE OF

PITNEY BOWES MAIL PROCESSOR AT BLDG 519

1. DESCRIPTION OF SERVICES/GENERAL INFORMATION. The Contractor shall provide all personnel, equipment, tools, materials, calibration, vehicles, supervision, and other items and services necessary to provide semi-annual preventive maintenance and emergency maintenance/repair to the

Pitney Bowes Mail Processor, described below in accordance with the manufacturer’s specifications and commercial practices.

Annual usage volume limit:

12,000 parcels per year

1,000 parcels per month

Parts covered under this contract: All Inclusive

1.1. Preventive Maintenance. Preventive maintenance services shall be performed on a normal routine manner to care for wear and tear resulting from daily usage, and is not intended to include damages caused by careless abnormal usage or by any other mishandling of the equipment. In case of abnormal damages, the Contractor shall stop his work and report his findings to the Contracting Officer immediately. Preventive maintenance shall include semi-annual inspection, cleaning, lubricating adjusting testing, replacement of parts and all other services deemed necessary by the Contractor to keep the equipment in good operating condition and correction of all dust, scale and the other parts of the equipment for deterioration, excessive wear, misalignment, loose joints improper adjustment or any other defects that affect serviceability in accordance with the manufacturer’s specifications and commercial practices.

1.1.1. Maintenance/Inspection Limitation. Maintenance services do not include maintenance of accessories, attachments machines like Meter and Printer, or other devices not furnished by the

Contractor, or the painting or refinishing of the equipment nor does it include the replacement or the furnishing of accessories or consumable supplies not elsewhere listed. Only a functional inspection is required of the printer to ensure it is operational after maintenance of the mail processor.

1.1.2. Assessment. The Contractor shall perform initial test upon completion of preventive maintenance to all the equipment listed in paragraph 1 above to ensure equipments are in operational condition.

1.1.3. Work Area. Task shall be performed in a restricted area. The Contractor shall not enter any section in the Post Office without an escort.

1.1.4. Upgrade. Includes upgrade postage disks for any postage rates and fees changes.

1.2. Emergency Call (Maintenance/Repair). Emergency calls shall be responded to within 24 hours after receipt. Emergency calls placed on Fridays or the day prior to holidays shall be responded to no later than 1300 hours, the first working day following. Such remedial maintenance shall be performed at no additional cost to the Government. Emergency maintenance/repair covers only a scope of preventive maintenance. If the work does not fall in the above scope, yet necessary to be done for normal operation, the Contractor shall submit a written report with cost estimate to the Contracting Officer (CO), and gain written approval from the CO, prior to commencement of any out-of-scope work.

1.3. Consumables. The contractor shall provide the following consumables during the time of contract as required by the Government:

1.3.1.1. One (1) Package of 766-8 Ink Cartridge

1.3.1.2. One (1) Package 627-2 Gummed Tape Rolls

1.4. Hours of Performance. Work site will be made available to the Contractor from 1000 – 1600 hours

Monday through Friday including Japanese but excluding US legal holidays.

1.5. U.S. Holidays. The designated US legal holidays are follows:

1.6. Work Schedule. Preventive maintenance services shall be scheduled so that each service may be performed at equal intervals. The Contractor shall prepare and submit GR a practical work schedule indicating the month of services within 14 calendar days after award.

1.7. Warranty. The Contractor agrees to provide the most favorable commercial warranties that the manufacturer or supplier provides to any customer for the types of items furnished under this contract.

The Government reserves the right to reject any unserviceable or substandard item to require replacement with a serviceable like item or acceptable quality item or to require full credit for the item.

1.8. Government Remedies. The CO shall follow FAR 52.212.4, Contract Terms and Conditions-

Commercial Items for Contractor’s failure to perform satisfactory services or failure to correct non-conforming services.

1.9. Disposal of Materials. All generated materials from the work performed shall be removed from the job site by the Contractor and disposed of off base in accordance with the laws of Japan.

1.10. Performance of Services during Crisis Declared by Secretary of Defense of the Oversees

Combatant Commander. This service contract is not classified as “mission-essential” during a crisis as declared by the National Command Authority or Overseas Combatant Commander. In the event of such a crisis, the contracting officer will direct the Contractor personnel to discontinue the service due to base closure. Until such a notification, the Contractor personnel are required to continue the performance. There shall be no costs paid by the Government to the

Contractor during a crisis except those costs incurred for work performed under this contract and PWS.

2. GOVERNMENT FURNISHED PROPERTY/MATERIALS. The US Government at no cost to the

Contractor will furnish water and electric utilities.

3. GENERAL INFORMATION

3.1. PERSONNEL. The Contractor is responsible for the performance and appearance of their employees.

a. Appearance and Attire: The contractor personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing distinctive clothing bearing the name of company or by wearing appropriate badges that contain the company name and the employee name.

b. Authorization of base entry: To request the base entry authorization, prior to the start date of on-site work, the Contractor shall submit applications (listed below) for the base passes to the Contracting

Officer. Ensure to request for all employees including subcontractor employees, and vehicles requiring access to Misawa AB in order to perform this contract.

(1) 5 AF Form 99EJ Application for Base Pass for employees

(2) 35 FW Form 54, Application for Vehicle Registration for vehicles

Upon completion of this contract, the Contractor shall return all employee and vehicle passes to the

Visitor Control Section (35 SFS/SFAP), Bldg 301 and obtain a certification of receipt. Final payment will be withheld until the Contractor accomplishes required clearances.

3.2. Physical Security/Safety

All contractor employees shall be instructed on accident prevention and safety.

3.5. Misawa Air Base Fire Prevention

The Contractor shall be responsible for fire prevention in all contracted area and be free from any potential fire hazards. The contractor shall comply with the following fire prevention guidance.

3.5.1. FIRE REPORTING

(1) The primary emergency reporting number on Misawa Air Base is 911 or 0176-53-1911 from off base. Fire Department operations take place out of Bldg 947 and 3003 for main base, and

Bldg 1940 for the north housing area.

(2) On-base telephones if any, in the on-base warehouse(s) shall have “911” stickers.

(3) All contractor employed personnel shall be familiar with fire reporting numbers and procedures and shall put the number “0176-53-1911” into his/her mobile phone.

3.5.2. VEHICLES

(1) Access for fire fighting vehicles must be maintained at all working sites.

(2) Contractor’s vehicles shall not be parked within 15 feet of any fire hydrant of Fire

Department connection.

(3) Contractor’s vehicles shall give the right-of-way to responding fire/emergency vehicles.

3.5.3. ELECTRICAL HAZARDS

(1) All the contractor’s electrical tools and equipment shall be in good (original) working condition, and its maintenance shall be done properly in compliance with manufacturer’s maintenance instructions.

(2) All extension cords used shall be equipped with circuit breakers and plugged directly into the outlets (not other extension cords).

(3) Building utility panels shall not be blocked by contractor tools/supplies.

3.5.4. STORAGE

(1) Flammable liquids shall not to be stored inside the warehouses, next to the buildings or near exit ways.

(2) Fire protection system shall not be obstructed at any time. Carpet storage shall be neat in a manner which does not present fire/safety hazards and not to block the access to where fire extinguishers are mounted.
