Performance Work Statement

FOR

Cranes and Hoist

MAINTENANCE, REPAIR, AND CERTIFICATION

EIELSON AFB ALASKA

17 January 2018

PERFORMANCE WORK STATEMENT

CRANES AND HOISTS

1. DESCRIPTION OF SERVICES. Period of performance: 20 August 2018 – 19 August 2019 (base plus four).

Perform evaluations, inspections, testing, maintenance, repair, and certification of cranes and hoist, lifting slings and straps, and components of this equipment. Includes mobile, rail-mounted, and fixed-location cranes and hoists.

Includes both powered and manually-operated units. Provide tags/stenciling and written documentation. Note 1: The equipment may vary throughout the life of this contract.

2. Note 2: Lifting slings and straps shall require inspection and certification only, not repair. If not certifiable, these devices shall not be repaired by the contractor.

2.1. SCOPE. The contractor shall provide all personnel, equipment, tools, parts, lubricants, weights, transportation, and supervision necessary to evaluate, inspect, load test, service, and repair cranes, hoists, and other related lifting devices at Eielson AFB Alaska. Contractor employees shall be trained and uniquely qualified to perform all necessary service of cranes and hoists.

2.2. INSPECTION, CERTIFICATION, AND REPAIR. All cranes and hoists shall be inspected, maintained, repaired, and certified in accordance with 29 Code of Federal Regulations (CFR) 1910.179. This CFR can be found at http://www.osha.gov/pls/oshaweb/owadisp.show_document?p_table=FEDERAL_REGISTER&p_id=18080. Periodic inspection frequency for cranes and hoists shall be determined by activity, severity of service, and environment.

Cranes and hoists are listed in appendix 5A. The government reserves the right to add or remove cranes and hoists from this list.

2.2.1. Inspection. The contractor shall inspect each unit at least annually and shall provide written results of these inspections to the government. Inspections and reports shall include: make, type, model number, tonnage, specific location, and condition. Provide cost estimate, if any, to repair each unit to certifiable condition, IAW par 2.2.7 and

2.2.8. For inspections the Designated Government Technical Representative will be the main point of contact.

2.2.2. Inspection of Slings. The contractor shall inspect and certify slings, straps, and other assemblies used for connecting the crane or hoist to the load, as required. This inspection and certification shall be accomplished annually at the same time as the certification occurs for the cranes and hoists. The using organization (identified in Appendix 5B) shall be responsible for presenting items for inspection to the contractor. Items not certified or not presented for inspection shall be removed from service by the using organization (identified in Appendix 5B). Items failing inspection or certification shall be tagged as such by the contractor and removed from service by the using organization.

2.2.3. Certification. The contractor shall test and certify each crane or hoist in accordance with 29 CFR 1910.179.

Upon certification/recertification, each unit shall be labeled with the appropriate tag or stenciling. Written documentation shall be provided to the government within 10 business days after certification.

2.2.4. Certification Due Date. All inspections, repairs, and certification shall be accomplished prior to the expiration date of the previous certification. For the first year of this contract only, all cranes and hoists shall be inspected, repaired, and certified within 90 calendar days of award, regardless of previous certification date.

2.2.5. Certification and Retesting. If necessary after repairs or maintenance, the contractor shall retest and recertify cranes and hoists.

2.2.6. Certificates and Reports. Upon completion of inspection and certification of each crane and hoist, the contractor shall tag or stencil the unit with the new date of certification. Tags and stencils shall be in place not later than 48 hours after certification. Inspection reports and load test certificates shall be duplicated with originals to http://www.osha.gov/pls/oshaweb/owadisp.show_document?p_table=FEDERAL_REGISTER&amp%3Bp_id=18080 using agency (identified in appendix 5B) and copies to 354 CES/CEOSS, 2310 Central Ave, Eielson AFB AK 99702, within 14 calendar days after certification

2.2.7. Repair. The contractor shall repair cranes and hoists as necessary to keep them fully operational and to maintain their certification. Repairs (including parts) costing $250 or less shall be approved in writing by the designated Government Technical Representative prior to accomplishment. Repairs costing over $250 shall be approved in writing by the designated Government Technical Representative and Contracting Officer prior to accomplishment. All copies of all records shall be retained by the Contractor and shall be made available to the government upon request. If necessary after repairs, cranes and hoists shall be retested for load and recertified. The contractor may be reimbursed for rental equipment up to the amount awarded in CLIN 0002 Maintenance and may charge labor hours not to exceed the amount awarded in CLIN 0003 Labor Hours.

2.2.8. Repair Service Calls. There are no crane or hoist emergency service calls. Upon notification of a crane or hoist service requirement, the contractor shall respond on-site, with appropriate materials, tools, equipment, and personnel, within 14 calendar days of notification. Cranes and hoists shall be repaired and returned to service, including certification if necessary, within 21 calendar days of notification.

2.3. LOAD LIMIT TEST. The contractor shall be responsible for performing and providing weights for the load limit tests.

2.4. PARTS AND MATERIALS. The contractor shall furnish and use only original manufacturer parts or approved equal. Replacement parts (including repairs) exceeding $250 shall be submitted to the designated Government Technical Representative and approved by the contracting officer, who will determine the approval of the proposal.

3. SERVICE DELIVERY SUMMARY. The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

SS-4: VPP

Requirements:

The contractor shall perform work in a safe manner as required by Occupational Safety and Health Administration (OSHA) Safety and Health Requirements. Provide a verbal report to the CO as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the CO within five calendar days

4.4. 100% of the time;

No more than 1 valid defect per month.

Notify Contracting Officer (CO) ASAP after occurrence.

Provide a completed copy of required Accident Investigation Reports to the CO within five calendar days of each occurrence.

4. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The government will provide transportation to remote sites through aircraft or government vehicles, from Eielson AFB. This does not include transportation to the main base.

5. GENERAL INFORMATION.

5.1. GOVERNMENT QUALITY ASSURANCE. The government will evaluate the contractor’s performance after completion of services. Cranes and hoists not fully operational, not certified, or other services not performed IAW 29 CFR 1910.179 or other applicable laws and commercial practices shall immediately be reperformed until satisfactorily completed.

5.2. GOVERNMENT REMEDIES. The contracting officer shall follow Federal Acquisition Regulation (FAR) 52.212-4, Contract Terms and Conditions-Commercial Items (May 1997), and services summary for service provider’s failure to perform satisfactory services or failure to correct non-conforming services.

5.3. HOURS OF OPERATION. The contractor shall provide services Monday through Friday, 0730 to 1630, except for recognized federal holidays.

5.3.1. Federal Holidays

4.4. VOLUNTARY PROTECTION PROGRAM: The contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements. Provide a verbal report to the CO as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the CO within five calendar days of each occurrence. 4.5. SAFETY. The contractor shall provide and place signage as needed to safely cordon areas and clear zones where cranes and hoists are being serviced. These clear zones shall be completely void of people and equipment not directly involved in service of the cranes and hoists. During inspection, service, maintenance, or repairs, each affected crane and hoist shall be suitably identified as being out of service. This placard placement shall also include switches, controls, breakers, etc.

4.6. SECURITY REQUIREMENTS. The contractor shall be responsible for and shall pay for obtaining all required passes, identification media, etc, for both contractor personnel and contractor vehicles, to gain access to Eielson AFB and to facilities where cranes and hoists are located. Base access information is available from 354 Security Forces Squadron Pass and Identification office located at 2677 Wabash Ave (bldg 3125). For servicing of cranes and hoists located in restricted-access areas, the using organization shall provide suitable escort personnel for the contractor.

4.6.1. The contractor shall maintain a current listing of employees. The list will include employee name(s) and date(s) of birth. This list shall be validated by the contract manager and provided to the contracting officer and servicing security forces organization at the contract start date. An updated listing shall be provided upon change of employees.

4.7. WARRANTY. The contractor shall warrant all contractor-installed parts and all contractor workmanship for a period of one year from the date of acceptance by the government. The contractor shall repair or replace, at his expense, any materials that become defective during the warranty period. Warranties shall not apply where the damage or failed materials results from abuse or neglect by the government.

4.8. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL COMMAND

AUTHORITY OR OVERSEAS COMBATANT COMMANDER. The contractor must maintain annual inspections and certifications regardless of times of crisis or national emergency.

4.9. MISCELLANEOUS. Use of cell phones while operating any motor vehicle on base is strictly prohibited.

Driving privileges will be suspended for 30 days for the first violation. The use of seatbelts is mandatory.

4.9.1. The on-base speed limit, while passing military troops performing physical training, is 10 MPH and is strictly enforced.
