PERFORMANCE WORK STATEMENT

Upgrade lighting in power plant /warehouse to LED As of: 6/26/2019

PERFORMANCE WORK STATEMENT

1. DIVISION 1 - GENERAL REQUIREMENTS

a. PROJECT LOCATION: This Construction project shall be completed as a routine requirement at building 18224 Fairchild Ave, on Joint Base Elmendorf‐Richardson, AK. Project hours are standard work hours 0700-0500.

b. UNIQUE REQUIREMENTS: Work is located at building 18224 Fairchild Ave., therefore strict security guidelines, procedures and restrictions will apply. Contractors will be required to have escorts at all times during work hours. See AMOC Appendix A for further details regarding security guidelines.

c. PROJECT DESCRIPTION: Install LED lighting throughout bldg. 18224, in accordance with the attached drawings and associated specifications noted below. Note: This does not include the Generator/Switch Gear Room.

d. WORKMANSHIP: All work shall be accomplished in accordance with all applicable building and fire codes. Contractor shall field verify all measurements, quantities, or values stated by the Government. Liquidated damages shall be assessed for work that is not finished within the scheduled performance period.

e. WORKSITE: The contractor shall limit the work to the areas indicated by the government. The contractor shall make every effort not to disturb or obstruct portions of the facilities still occupied by the government. The contractor shall coordinate and schedule work with the Facility Management office prior to the start of any operation. The contractor shall dispose of all construction debris off base at an approved disposal facility.

f. SITE SECURITY AND PROTECTION: The contractor shall make every effort to secure the work site, allowing only authorized personnel to enter the work site. The contractor shall protect the site from damage from weather, vandalism, and/or theft.

g. DAMAGE TO EXISTING SITE CONDITIONS: The contractor shall be responsible to repair any and all damage incurred to existing Real Property/Grounds due to poor workmanship at no cost to the government. Contractor shall not park or drive on sidewalk or grassy/snowy areas. Damage incurred shall be repaired at no cost to the government.

h. HEALTH/SAFETY: The contractor shall be responsible for compliance with OSHA and AFOSH standards. The contractor shall maintain a clean and debris free site at all times. The contractor shall keep driveways and entrances into and around the work site clear at all times. The contractor shall not use driveways or entrances as employee parking or storage of materials unless otherwise stated. The contractor shall minimize storage of materials and equipment on site.

i. GREEN PROCUREMENT: Contractor must incorporate “green procurement” principles into the design of their projects. Specifically, consult the EPA’s web site at www.epa.gov/greenerproducts for a listing of the products that have been mandated as green procurement requirements for purchases.

2. DIVISION 2- DESCRIPTION OF WORK AND SPECIAL INSTRUCTIONS

a. Contact (insert name here) 7 days (or other) in advance of actual work start so that government can plan official escort detail.

b. All LED lighting must be completely installed within 60 days from contract award.

c. Contractor will install/retrofit 12 400w, high-bay and high-pressure sodium fixtures with LED lamps. (Contractor will replace 6 2'x2' fixtures in old racquetball court with new LED flat panels.

d. Contractor will replace 3 exterior canopy lights with modern LED fixtures.

e. Contractor will install new LED strip fixtures in bathroom and basement.

f. Contractor will retrofit 26 T8 fluorescent fixtures with new LED drivers, tombstones, and lamps. This portion will be labor only. The facility manager will provide materials for this portion of the project. Contractor to inspect material prior to contract start. Materials are on site.

g. Contractor to provide new LED lighting in all locations shown in facility diagram except for those areas hatched out. Contractor to walk through facility prior to contract to ensure that all lights are planned and accounted for.

3. DIVISION 3- ATTACHMENTS

AMOC APPENDIX A SITE SECURITY GUIDELINES

1. Vetting:

(U) In accordance with U.S. Government policy, this project shall be accomplished by U.S. companies using U.S. citizens. At no time will the use of illegal immigrants/workers be authorized for use on this project. The use of non‐U.S. citizens will be prohibited in all aspects of the project.

(U) Contractor personnel requiring access to the project site are subject to security vetting through 673 Security Forces Squadron, AMOC Security, and the U.S. Government. Contractor personnel requiring access to the project site must submit the following biographical information to AMOC Security at least 60 days in advance of the desired date access:

• Full name and any aliases

• Current address(es)

• Social Security Number

• Date and place (city, state/province, country) of birth

• Information pertaining to all citizenships held

• Any persons born outside the U.S. who are now U.S. citizens, must provide a copy of their proof of citizenship (U.S. Passport or naturalization certificate and alien registration number)

• Driver’s License Number or State Identification Number, and state of issue

• Inclusive dates of access required

(U) Proof of U.S. citizenship is required for access to the project site. One of the following identifications shall be provided to security, as proof of U.S. citizenship, on the first day that access is required:

• U.S. Passport (active only)

• State or U.S. Territory government issued birth certificate (original or certified copy)

• U.S. Naturalization Certificate or U.S. Certificate of Citizenship (original)

2. Badging and Site Access:

(U) All contractors performing work at the site must wear an AMOC produced security badge at all times. The badges will be signed‐out from the Entry Control Facility (ECF) and authorizes the wearer to enter the AMOC compound. The badge must be worn exposed and above the waist so as to be clearly visible at all times. The security badges are property of the U.S. government and will be returned to the ECF daily. In addition to the site badge, all workers must have valid photo identification in their possession at all times.

(U) Work hours of the contractors will typically be from 0700 to 1700 M‐F, excluding federal holidays. Any work intended to commence outside of these hours will require coordination and approval by the AMOC Chief of Staff, with coordination through the on‐site Project Manager and the AMOC Security Office, no later than the requested workday. Escorts will be provided at all times while contractors are present within the AMOC SCIF.

3. Other Guidelines:

A. The following items are prohibited on the work site: 1. Weapons, Explosives, Flammables, Hazmat

2. Alcohol or Illegal Narcotics

3. Computers and associated media

4. Recording Devices (CD‐R, RW, DVD‐R, RW, USB drives, etc.)

5. Photographic Equipment (cameras, cell phones w/camera, etc.)

6. Communications Equipment (cellular phones, two‐way radios, etc.)

B. Professional conduct is required at all times on the work site. Prohibited actions below will result in expulsion from the site.

1. Racial harassment or disparaging remarks

2. Sexual harassment or disparaging remarks

3. Religious harassment or disparaging remarks

4. Any behavior deemed unsafe or hazardous to other workers

C. All personnel must be ready to work when they report to the AMOC compound.

1. All workers must be sober. Anyone who reports for duty while intoxicated or under the influence of narcotics will be denied access to the work site.

2. All personnel must wear appropriate attire. This includes the proper safety gear.

D. All workers must immediately report any suspicious or unsafe incidents to the appropriate Security, Safety, or Supervisory official. Examples include:

1. Anyone seeking information about this project

2. Anyone seeking information about project personnel (e.g.: names, numbers, etc.)

3. Any unauthorized personnel watching the AMOC compound

4. Any suspicious vehicles near the site

5. Workers are required to guard all information in their possession pertaining to this project. Failure to protect information can result in removal from the work site.

E. All personnel are required to report all negative contact with law enforcement authorities to the AMOC SSO while assigned to this project.

(U) All personnel and any carry‐on items (e.g.: lunch boxes, backpacks, etc.) are subject to search by AMOC security at all times.
