STATEMENT OF WORK
Relocation of Government Property
389th Fighter Squadron, Building 278 Mountain Home AFB ID 83648
21 February 2018
1.0 SCOPE OF WORK: The contractor shall furnish all personnel, equipment, supplies, services and materials for the packing, moving, and storage of office furniture, equipment, and supplies from building 278. A portion of items will be moved to building 210 and Hangar 211 (2nd Floor). The remainder of the items will be packed and stored at a building 1326 designated by CE/Real Property. All buildings are located on Mountain Home AFB, ID.
2.0 General Description:
2.1. Services shall consist of a full range of packing, storing, moving, and set-up to remove existing office furniture, equipment and related supplies from building 278 and transport the items to the three locations outlined below in paragraph 3.1. The Contracting Officer (CO) will approve final placement of all items for each specific building.
2.2. It is the contractor’s responsibility to coordinate the container size and configuration for each mode of shipment prior to packing containers.
2.3. A CO and/or Contract Specialist (CS) will be present at origin and each destination during the performance of the SOW. Only changes to the SOW shall be approved by the CO in writing prior to acceptance by the contractor.
3.0 LOCATION AND DESCRIPTION
3.1. Current location is 389th Fighter Squadron, 180 Thunderbolt Street, Bldg. 278, Mountain Home AFB, ID 83648. Move locations are Building 210: 725 Alpine Street, Mountain Home AFB, ID and Hangar 211, 2d Floor: 815 Alpine St, Mountain Home AFB, ID. The distance between building 278 and the relocation sites is approximately half (1/2) mile on paved road.
Additional packed/stored items will be relocated to Building 1326.
3.2. Building 278, 210, 211, and 1326 do not contain loading docks.
3.3. Parking/loading areas at all locations are capable of supporting a semi-truck.
3.4. The project shall be completed in three (3) phases as outlined below:
3.5. Phase One: Select items at Bldg 278 and Bldg 210, identified by the customer and marked for storage, will be boxed and placed in storage containers. Boxes will be sequentially numbered and annotated on an inventory list. One copy of the inventory list shall be provided to the CO and the customer at Bldg. 278.
3.5.1 Storage containers shall be provided for government use for a period of up to, but not to exceed, 24 months. Contractor shall identify any cost associated with retrieving storage containers once government has removed stored items.
3.5.2. Placement of Storage Containers. Contractor will transport storage containers to Bldg. 1326 in advance of moving packed items from Bldg. 278/210.
3.6. Phase Two: Select items at Bldg. 278, identified by the customer and marked for building 210, shall be packed and moved to Building 210, 366th Operations Group, 725 Alpine Street (single story). This location does not contain a loading dock.
3.7. Phase Three: Select items at Bldg. 278, identified by customer and marked for storage, shall be packed and moved to Hangar 211, 2d Floor, 366th Maintenance Group, 815 Alpine Street. This location does not contain a loading dock and will require ramps or other equipment unloading furniture/equipment/etc. Elevators are not available at this location.
3.8. Government personnel shall be responsible for packing and moving all computer related equipment and classified equipment to the new locations.
4.0 CONTENTS DESCRIPTION AND LISTING:
Building 278 contents include, but is not limited to the following:
Freestanding Desks and Furniture Refrigerators, Washer and Dryer, and Dishwasher Office Equipment File Cabinets, Storage Cabinets, Lockers, and Safes Large screen televisions and monitors Various heritage memorabilia Miscellaneous office equipment/supplies Fitness Equipment (Bldg. 210 & 278) Miscellaneous Wall Hangings and Pictures Wooden Aircrew Lockers (Bldg. 210)
4.1 ESTIMATED WEIGHT: 55,710 lbs. Inventory lists at Appendix 1.
4.1.1 Approximate weight to crate: 37,500
4.1.2 Approximate weight to pack and move: 18,210
4.1.3. Contractor is responsible for verifying the weight totals.
5.0. TAGGING, INVENTORY, AND PACKING LISTS
5.1. The contractor shall prepare an accurate, legible, and descriptive inventory. The contractor shall count and examine all goods tendered for packing and make written exception for any goods not in apparent good order. All tagging shall be accomplished prior to packing any crate for storage or moving any furniture or equipment from Building 278.
5.2. Each crate/carton/box shall be marked with sequential numbers and then indicate general contents with an inventory list attached in a secure pouch to the crate, after inspected by contractor and CO/CS. The crate/carton shall be sealed in a way to prevent theft, rodent infestation, etc.
5.3. Removal of Debris. The contractor is required to remove any excess containers, packing materials, and other debris accumulated during the performance of this SOW from all facilities and surrounding area.
5.4. Contractor shall furnish padding and/or other protective material for furniture and equipment during move.
5.5. Contractor shall furnish plywood, vinyl, heavy gauge plastic or Masonite runners as required to provide protection to the floors and carpet.
5.6. Disassemble and/or reassemble items when necessary for the safe and efficient transportation to the new location, as identified above.
5.7. The contractor shall provide closed-type vehicle(s) of sufficient size to accommodate the shipment and properly licensed personnel to operate such vehicle(s). The vehicle(s) shall be in good repair and in compliance with all federal, state, and local laws, and regulations for operation.
5.8. All vehicles used in relocations shall be equipped with clean pads, covers, and other protective equipment to ensure safe transit and delivery of property. Protective covering shall be used during the loading and unloading operations in the event of inclement weather.
6.0 DAMAGES
6.1. The contractor shall be responsible for damages to flooring, furniture, and equipment resulting from the physical transfer of the property between the three locations.
6.2. The contractor and CO/CS will visually inspect for any damage to items listed on the inventory. The CO will maintain this listing and coordinate with the contractor for the repair and/or replacement of the damaged item(s). Except when loss and/or damage arises out of the causes beyond the control and without the fault or negligence of the contractor, the contractor shall assume full liability for any and all goods and/or damage in the relocation covered by the contract.
6.3. The contractor shall report all incidents of damage or destruction to property, personal injury or death to any person, arising out of and during the performance of this contract. For the purpose of this statement, the term “property” includes equipment, freight, residence or property of any type whether Government owned or not and the term “Contractor” includes the contractor, its employees, agents, or subcontractors.
7.0. General Information:
7.1. Installation normal operating hours are 7:30 a.m. through 4:30 p.m. Monday through Friday, except Federal holidays or when the Government facility is closed due to 366 FW Family or Down Days or during adverse weather delays/closures. Local or national emergencies may also affect access to base facilities. Any work requested or performed outside of normal business hours shall be coordinated and approved with the CO PRIOR to work being performed.
7.2. Recognized Federal Holidays. A minimum of ten paid holidays per year:

If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the contractor will be responsible for rescheduling services for the first day after the holiday observance.
7.3. Pre-Performance Conference. A pre-performance will be held approximately one (1) week after contract award (ARO).
7.4. Contract Manager and Alternate. Contractor shall appoint in writing to the CO within 10 business days following contract award a contract manager and alternate(s). Any changes to this letter of appointment shall be provided to the CO within three (3) business days. These individual(s) shall be responsible for contract performance IAW the PWS. The contract manager or alternate will have full authority to act for the contractor on all contract matters. The name of this person, and an alternate or alternates, will act for the contractor when the primary is absent.
7.5. Contractor Employees. The contractor shall not employ persons for work on this contract if such employee is identified to the contractor, by the Contracting Officer, as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population. The contractor shall not employ any person who is an employee of the United States government if the employment of that person would create a conflict of interest, nor shall the contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with DOD
5500.7 and the Joint Ethics Regulation. If prior military, the employee must have been discharged under honorable conditions with no administrative discharge actions taken against them.
7.6. All contractor personnel must present a neat appearance and be easily recognized as contractor employees. This may be accomplished by wearing contractor-furnished clothing bearing the company name and/or logo. Dress of office workers will be in good taste and conducive to working with the general public. The contractor will identify himself as a contractor in meetings, telephone conversations, and e-mails and in any correspondence with the Government.
7.7. All employees shall fluently read, write, speak, and understand the English language.
7.8. Removal of Contractor Employees. The contractor shall utilize only experienced, responsible, and capable employees in the performance of this contract. The Contracting Officer may require the contractor to remove his/her employees who endanger persons or property, or whose continued employment under this contract is inconsistent with the interests of military security.
8.0 Security and Safety Requirements:
8.1. The contractor shall provide a completed 366 SFS Form 30 for each employee requiring installation access to the Contract Specialist (CS) within five (5) business days following contract award. After approval, the CS will notify the contractor that the employees may get their installation access ID at the Visitor’s Center.
8.2. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires i.e. terminated for cause, retirement, etc.
8.3. Traffic Laws. The contractor and its employees shall comply with base traffic regulations.
8.4. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor owned vehicle or privately owned vehicle while on MHAFB.
8.5. For Official Use Only (FOUO). The contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Action (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting and safeguarding FOUO material.
8.6. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware that may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.
8.7. Physical Security. The contractor shall be responsible for safeguarding all government property. At the end of each work period, all government facilities, equipment, and materials shall be secured.
8.8. Contractor shall comply with all applicable OSHA and AFOSH standards.
9.0. Records Management: All records, files, and documents, regardless of media (e.g., paper, electronic, etc), as described in this PWS, which are the responsibility of the Contractor are the property of the Government and shall remain so upon termination or completion of the contract.
9.1. The contractor shall keep these item(s) current and maintain and dispose of them in accordance with the requirements established in AFI 33-322, Records Management Program, paragraph 10, AFMAN 33-363, paragraph 6.4, AFMAN 33-364, paragraph 2.10 and the Air Force Records Disposition Schedule which may be accessed from the following web address:

9.2. Records shall be turned over to the government upon completion of the contract unless otherwise stated. All records are subject to the Freedom of Information and Privacy Act.
10.0 Contractor Manpower Reporting. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via a secure data collection site. The contractor is required to completely fill in all required data fields using the following internet address: http://www.ecmra.mil/. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2016.
