PERFORMANCE WORK STATEMENT (PWS)

GYM EQUIPMENT MAINTENANCE SERVICES

MOUNTAIN HOME AFB, ID

1.0 DESCRIPTION OF SERVICES. The Contractor shall provide all personnel, tools, parts, equipment, materials, supervision, transportation, and other items necessary to ensure that gym equipment maintenance services are performed at Mountain Home AFB in a manner that will ensure proper inspection, diagnostics, repairs, modification, and preventative maintenance of all exercise equipment located at the Gunfighter Fitness Center. The contractor shall accomplish scheduled inspections, necessary diagnostics, repairs and modifications necessary, and all preventative maintenance on exercise equipment at Gunfighter Fitness Center. Equipment inventory listing can be found in Appendix A, Equipment Assets and Frequency Schedule. All work performed by the contractor shall be performed in accordance with all applicable laws, regulations, and commercial practices.

1.1. PREVENTATIVE MAINTENANCE. The contractor shall provide on-site preventative maintenance and equipment repair services, and ensure that all maintenance and repairs are documented in a maintenance repair log, as described in paragraph 1.1.6. All preventative maintenance shall ensure all equipment remains serviceable, to reduce the frequency of equipment repairs, and to extend life of the equipment housed at the Gunfighter Fitness Center in accordance with the manufacturer’s specifications as listed in technical literature for each piece of equipment. The contractor shall take every measure within the scope of this agreement to ensure all known and/or identified discrepancies are corrected or repaired during the quarterly preventive maintenance visit. The only exception for any delay would be based on the availability of replacement parts.

1.1.1. LEVEL 1, MONTHLY MAINTENANCE. For equipment listed in Appendix A Maintenance Schedule as monthly, the contractor shall perform preventive maintenance on the first Monday of each month. If the first Monday falls on a federal holiday, the contractor shall perform on the following business day.

1.1.2. LEVEL 2, QUARTERLY MAINTENANCE. For equipment listed in Appendix A Maintenance Schedule as quarterly, the contractor shall perform preventive maintenance on the first Monday of March, June, September, and December. If the first Monday falls on a federal holiday, the contractor shall perform on the following business day.

1.1.3. TESTING. The Contractor shall perform on-site electronic tests and inspections.

Maintenance includes but is not limited to inspections, lubrication, adjustment, calibration, interior cleaning, pad repair, and replacement of worn and/or deteriorated parts. The inspections shall include, but are not limited to, checking for proper operation, detection and correction of machine failures before they develop into major problems, and equipment hazards. All testing shall be accomplished within the manufacturer’s prescribed or recommended time frame.

1.1.4. UNSCHEDULED SERVICE CALLS. On-site unscheduled repair service calls will be placed by the Fitness Center Director or the Fitness Center Section Chief. The contractor shall respond to unscheduled service calls within 24 hours of notification. The contractor shall take every measure within the scope of this contract to ensure all repairs are made within two (2) business days.

1.1.5. REPLACEMENT PARTS. The contractor shall supply the required replacement parts except for parts under warranty. Before purchasing and replacing parts, the contractor shall provide the Contracting Officer or designated Contracting Officer Representative (COR) a list of parts required for the job and the cost of each part. All replacement parts shall be ordered within 24 hours after receiving written approval. Repairs identified during preventive maintenance, which are awaiting replacement parts, shall be completed within two (2) business days after receipt of parts.

1.1.6. MAINTENANCE REPAIR LOG. The contractor shall provide and maintain a detailed listing of any and all service performed on each individual piece of equipment covered under this contract. The log must include the following information: manufacturer, model number, serial number, unit number, date work performed, description of work performed, date of service call, date repaired, cost of repair, and parts installed. The contractor shall submit a copy of the maintenance log to the Contracting Officer or designated COR ten (10) days after the first preventive maintenance service call. Updates to the maintenance log shall be submitted five (5) business days after each subsequent preventive maintenance service call.

1.1.7. REPORTS. The contractor shall prepare a service report for each service call (scheduled or unscheduled) and obtain a signature from the using activity to certify that the service was completed and accepted.

1.2. EQUIPMENT REMOVAL. If necessary, the Contracting Officer will modify the basic contract to delete equipment which no longer requires servicing via a bilateral modification to the contract. Removal of equipment items will decrease the contract price for preventive maintenance services as determined from a flat rate calculation based upon the total amount of the maintenance divided by the total amount of equipment for each correlating level of service.

The removal will decrease the associated contract line item by the set rate as appropriate.

1.3. EQUIPMENT ADDITION. If necessary, the Contracting Officer will modify the basic contract to add equipment via a bilateral modifcation to the contract. Additional equipment items will increase the contract price for preventive maintenance services as determined from a flat rate calculation based upon the total amount of the maintenance divided by the total amount of equipment for each correlating level of service. Once it has been determined which level of service the additional equipment falls under, the addition will increase the associated contract line item by the set rate as appropriate.

1.4. CONTRACTOR FURNISHED PARTS.

1.4.1. PARTS. The contractor shall obtain approval from the Contracting Officer or designated COR and furnish all replacement parts unless parts are under warranty or available from the using activity. All replacement parts shall be new factory warranty parts; the Contracting Officer or designated COR must approve any rebuilt parts in advance, in writing.

1.4.2. UPHOLSTERY. The contractor shall provide all materials and equipment necessary to replace or repair any covering on equipment as necessary. The covering must be within the manufacturer specifications and match the color of the existing upholstery or be approved by the Contracting Officer or designated COR.

2.0. SERVICES SUMMARY. The contract service requirements are summarized in performance standards that relate directly to mission success. The performance threshold briefly describes the minimally acceptable levels of service required for each requirement. The services summary provides information on contract requirements, the expected level of contractor performance (performance threshold) and the expected method of assessment by the government to ensure services have been provided. These thresholds are a key element to mission success.

Procedures as set forth in the FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies.

2.1 QUALITY ASSURANCE. The COR will evaluate the contractor’s performance under this contract. Government personnel will record all surveillance observations. The COR will require the contract manager or representative at the site to initial all evaluation results. The initialing of the observation does not necessarily constitute concurrence with the observation, only acknowledgment that he or she has been made aware of the defective performance. Any action taken by the Contracting Officer as a result of surveillance will be according to the terms of this contract.

3.0. GOVERNMENT PROVIDED SERVICES.

3.1. Government Furnished Utilities. The Government will furnish electricity, water and sewage service as necessary for accomplishment of work in accordance with this contract.

3.1.1 Utility Conservation. The Contractor shall adhere to all base level utility conservation practices or requirements. The Contractor shall be responsible for operating under conditions that prevent waste of utilities.

3.2. Security, Fire and Medical Services. The Government will provide police and fire protection. In the event of a medical emergency, base ambulance service for transporting an injured employee to a local hospital is available on a cost reimbursement basis.

3.3. Refuse Collection and Disposal. The Contractor shall use existing bulk containers to dispose of trash or refuse generated from accomplishment of services detailed in this PWS. The contractor shall adhere to all base level recycling programs.

4.0 General Information:

4.1 Hours of Operation. The contractor shall perform the services required under this contract during the following hours. The contractor is not required to perform preventative services on Federal Holidays.

4.1.1 Normal Base Hours. Base hours of operation are 0730 – 1630 M-F. The Contractor may find it necessary to deviate from the normal base hours of operation, to ensure timely completion of work under this PWS at no additional cost to the government.
