PERFORMANCE WORK STATEMENT

FOR

HEMATOLOGY ANALYZER

Mountain Home AFB, ID

6 March 2017

1. SCOPE: The government estimates the performance of 15 cost-per-tests per day, 7 days a week, Monday - Friday, on a 24-hour basis (or when dictated by mission requirements). The contractor shall provide a hematology analyzer that has a self-contained waste management, does not require water or plumbing. The analyzer will report, at a minimum the following parameters: WBC, RBC, HGB, HCT, MCV, MCH, MCHC, PLT, neutrophils (# and %), lymphocytes (# and %), monocytes (# and %), eosinophils (# and %), basophils (# and %), RDW-SD, RDW-CV, and MPV. This analyzer will be capable of processing both adult and pediatric samples (microsample capability) and require no more than

110 uL for minimum specimen volume (open or closed). The contractor shall maintain, replace, repair, provide supplies, warranty and replacement as required, in order to ensure the equipment is fully operational and within manufacturer specifications.

Room: Laboratory, 366th Medical Group, Mountain Home AFB, ID 83648

2. REQUIRED SERVICES:

a. The contractor shall provide to the government a hematology analyzer and all necessary materials, accessories and reagents to conduct hematology testing on a cost per test /per result basis. The contract will consist of a 12 month base period and four one-year option periods, the contractor will provide a cost schedule based upon the estimated volume of tests/results estimated by the government.

b. The contract shal l include: 1) Equipment, 2) 24/7 Technical/Specialist Support,

3) Equipment Maintenance and Repair. All items and services shall be provided in accordance with all federal, state, local laws, and regulations, manufacturer’s recommended standards, commercial requirements, and this PWS.

c. Preventative Maintenance Inspections (PMIs). The contractor shall provide scheduled PMIs, calibrations, and on-site emergency services for the equipment listed below. Service shall include travel, labor, parts, materials, updates, technical data, and required tools and test equipment. Services shall be performed during normal duty hours of 0730 to 1630, Monday thru Friday. Response time shall not exceed 24 hours after request for service call has been placed. These inspections shall include quality assurance checks for proper operation, detection and correction of incipient failures, actions pertaining to periodic technical inspections, lubrications, adjustments, calibrations, controls, cleaning, and replacement of worn parts.

(1) Two (2) PMIs shall be performed biannually (6-months) per contract year with the optimal schedule as follows:

(a) During the month of February

(b) During the month of August

(2) PMIs should include the following:

(a) Consulting with equipment operators regarding equipment performance

(b) Performing operational checks on all systems and correcting noted deficiencies

(c) Replacing worn or defective parts, clean and lubricate IAW manufacturer’s service manual.

(d) Six (6) month calibration of analytes IAW College of American

Pathologists (CAP) standards using manufacturer Calibrators

(3) Scheduled PMIs shall be coordinated with the Mountain Home AFB

Laboratory, 208-828-7830 no less than one business week in advance

d. Emergency Service Calls. The contractor shall provide on-site emergency repair to correct any system malfunctions. Travel and overtime will be included at no additional cost. All tools, test equipment, parts, and supplies will be the responsibility of the contractor.

(1) Response time to emergency service requests shall not exceed 24 hours, excluding weekends and holidays.

(2) The equipment shall be returned to operational use within 24 hours after contractor responds to service request. Any parts that need to be obtained shall be done in the most expedient manner, at no additional cost to the government.

(3) Use of a service hotline. The service hotline should be available for questions, concerns, and troubleshooting 24 hours per day, 7 days a week.

e. Equipment Updates. The contractor shall furnish and install all software updates and equipment modifications provided by the manufacturer. These updates are provided at no additional cost and installed during the contractor’s next service call (PMI or emergency)

3. ADMINISTRATION:

a. The contractor shall receive PMI and on-site emergency service requests from Medical

Equipment Repair (MERC). Upon arrival, the contractor will contact MERC, Room

N106, prior to performing any work.

b. The service technician shall prepare a service report for each visit and obtain the signature of the shift supervisor or lead technician within the laboratory certifying acceptance of services performed. The service report shall describe the service performed and includes but is not limited to the following:

(1) System name, model, serial number, location, and site number if applicable

(2) Purpose of the visit or description of the malfunction

(3) Start/Stop times and dates

(4) Description of all services rendered – including calibration validation & linearity verification data for CAP evidence of compliance.

(5) Complete list of replacement parts used

(6) Copy of test records or checklist used to prove completion of PMI

(7) Signatures of the person performing the service and of the person accepting the service.

c. The contractor shall provide one (1) copy of the service report to MERC located in room N106.

d. Replacement Parts. The contractor shall only utilize new original or factory refurbished replacement parts. The contractor shall maintain a manufacturer’s

“recommended stock” of parts for the system readily available.

e. Training/Experience. Contractor shall provide to the contracting officer no later than

10 business days following contract award a copy of the training certificates for service technicians.

f. Test Equipment. The contractor shall ensure that all test equipment used has been calibrated according to manufacturer’s specification against a traceable standard.

g. Technical Data. The contractor’s shall ensure that all service technicians maintain a complete file of technical documentation for the system serviced to include schematics, service manuals, update memos, and service history records.

4. INSTALLATION: Placement and installation of the hematology analyzer shall include the following:

a. The contractor shall provide an Uninterrupted Power Supply (UPS) for the analyzer and which shall be positioned adjacent to the analyzer. The contractor shall ensure that the

UPS provides enough power, during a commercial power outage to ensure that the hematology analyzer system remains operational and continue to producing accurate patient results. All contractor’s shall include with their quote the specific type of UPS

(make, model) required for their equipment.

b. Complete daily clean-up and a final (upon installation completion) clean-up of the work area. To include any need for cosmetic repairs on walls, floorings, ceiling, etc.

c. The contractor shall provide a project manager to perform/oversee system installation, correlation, validation. The contractor shall provide all validation reagent/materials needed to perform these tasks.

d. The contractor is responsible for the removal of the clinical hematology analyzers at the end of the contract period of performance.

5. TRAINING: The Contractor shall provide on-site training for government personnel following installation. The contractor shall provide training for at least two government users at contract start and one additional user with each contract option period.

GENERAL

6.1 HOURS OF OPERATION

6.1.2 Recognized Federal Holidays. The contractor is not required to maintain normal customer service hours on Federal Holidays:

Washington’s Birthday Third Monday in February

Memorial Day Last Monday in May

Independence Day 4 July

Labor Day First Monday in September

6.2 PERSONNEL

6.2.1 Contract Manager and Alternate. Contractor shall appoint in writing to the CO within 10 business days following contract start date a contract manager and alternate(s). Any changes to this letter of appointment shall be provided to the CO within three (3) business days. These individual(s) will be responsible for contract performance IAW the PWS. The contract manager or alternate will have full authority to act for the contractor on all contract matters relating to daily operations. The name of this person, and an alternate or alternates, will act for the contractor when the primary is absent.

6.2.2 All contractor personnel must present a neat appearance and be easily recognized as contractor employees. This may be accomplished by wearing contractor-furnished clothing bearing the company name and/or logo. Dress of office workers will be in good taste and conducive to working with the general public. The contractor will identify himself as a contractor in meetings, telephone conversations, and e-mails and in any correspondence with the

Government.

6.2.3 All employees shall fluently read, write, speak, and understand the English language.

6.2.4. The following information needs to be incorporated into the PWS:

Contractor Manpower Reporting. The contractor shall report ALL contractor labor hours

(including subcontractor labor hours) required for performance of services provided under this contract for the COCESS via a secure data collection site. The contractor is required to completely fill in all required data fields using the following internet address:

http://www.ecmra.mil/. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1st through

September 30th. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2016. Contractors may direct questions to the help desk at: http://www.ecmra.mil.

6.3 SECURITY REQUIREMENTS

6.3.1. The contractor must possess or obtain a National Agency Check with Inquiries (NACI).

The contractor shall apply for all personnel security clearances within 15 calendar days following contract award. On the contract start date, if no NACI are approved, the government may be required to provide an escort until favorable checks are returned. The approximate time for processing a NACI is 30 to 90 days.

6.3.2. Pass and Identification Items: Obtain pass and identification items required for contract performance for employees IAW AFFARS Clause 5352.242-9000, Contractor access to Air

Force Installations, and AFI 31-113, Installation Perimeter Access Control. The sponsoring AF organization will ensure that contractors undergo a National Crime Information Center and

National Sex Offender Registry check prior to being issued any installation access media. All contractors requiring access for a period of six months or more will obtain a common access card

(CAC) regardless of network access requirements. The CAC must be in their possession at all times while performing contractor duties on a government installation. Upon termination of the contract , individual employment, or otherwise directd, the contractor shall return their CAC to the security manager.

6.3.3 Listing of Employees: The contractor shall maintain a current listing of employees. The list shall include the employee’s name, social security number and type of investigation if contract work involves unescorted entry to Air Force restricted areas or other sensitive areas designated by the installation commander. The list shall be provided to the CO and sponsoring agency’s security manager. An updated listing shall be provided when an employee’s status or information changes.

6.3.4. Reporting Requirements: Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware that may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.

6.3.5. Weapons, Firearms, and Ammunition: Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor-owned vehicle or privately-owned vehicle while on Mountain Home AFB.

6.3.6. Physical Security: The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

6.3.7. Access, General Protection and Security Policy, and Procedures: Contractor and all associated subcontractor employees shall comply with applicable installation, facility, and area commander installation and/or facility access and local security policies and procedures

(provided by government representative). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by installation Security Forces. Contractor workforce must comply with all personnel identity verification requirements as directed by DoD, Air Force, and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection

Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.

6.3.8. Traffic Laws. The contractor and its employees shall comply with base traffic regulations.

6.3.9. The Contractor agrees to utilize only experienced, responsible and capable individuals in the performance of the work. The CO may terminate the contract or require that the contractor remove employees who endanger persons or property, or whose continuity under this contract is inconsistent with the interests of military security.

7.0. Records Management: All records, files, and documents, regardless of media (e.g., paper, electronic, etc), as described in this PWS, which are the responsibility of the Contractor are the property of the Government and shall remain so upon termination or completion of the contract.

7.1. The contractor shall keep these item(s) current and maintain and dispose of them in accordance with the requirements established in AFI 33-322, Records Management Program, paragraph 10, AFMAN 33-363, paragraph 6.4, AFMAN 33-364, paragraph 2.10 and the Air

Force Records Disposition Schedule which may be accessed from the following web address:

7.2. Records shall be turned over to the government upon completion of the contract unless otherwise stated. All records are subject to the Freedom of Information and Privacy Act.
