For

Dive Locker Maintenance Contract

Initial Execution Date: 27 August 2012

1.0. INTRODUCTION/BACKGROUND: Headquarters Air Combat Command is the lead command for Guardian Angel (GA) units across the Air Force. The mission of Air Combat Command (ACC) is to organize, train, equip, and sustain Guardian Angel Weapon System (Combat Rescue Officer; Pararescue; Survival, Evasion, Resistance and Escape (SERE) specialists; and mission essential equipment) to provide Combat Air Forces (CAF) with worldwide Personnel Recovery/Recovery Operations and Special Operations mission capability across the full range of Military operations. Guardian Angel forces are required to rapidly deploy in support of CAF & joint operations in all physical environments, both day and night, and to provide the war fighting commanders the ability to project both air and ground forces to any region of the world in support of our national objectives. GA Rescue Squadrons (RQS) provide unique and special skills required for support of these joint operations and are equipped and tailored to support specific missions.

1.1 Per DOD Instruction 3224.04, the US Navy is the DoD single point manager and proponent for all military diving matters. The Navy Safety Center or Command designated agent will inspect each of the RQS dive lockers every 12 months and not to exceed 18 months to ensure mission success and personnel safety. Therefore, it is imperative that RQS dive lockers and associated mission support equipment are compliant and maintained at peak operational readiness levels IAW Navy guidelines. If US Navy guidelines are not available for certain types of equipment; manufacturer’s specifications will be adhered to.

1.2. To maintain compliance and insure operational readiness, each RQS unit must have highly qualified and experienced dive locker personnel with extensive and practical knowledge of: US Navy Maintenance and Material Management (3M) system; maintenance and repair of SCUBA and oxygen diving systems; maintenance and repair of assigned boats and outboard motors; and working knowledge of general Dive Locker/Maritime operations training procedures IAW with US Navy/MAJCOM standards. The Contractor shall be responsible for performing all services and meeting all directives noted in this PWS by imperative verbs regardless of whether such services and directives are preceded by the words “the contractor shall.”

2.0. OBJECTIVE: The objective of this Performance Work Statement (PWS) is to provide each GA RQS Commander (CC) with Dive Locker Maintenance support in order to fulfill their mission in an efficient and cost-effective manner. The Contractor shall be responsible for providing on-site Dive Locker maintenance support to Air Combat Command (ACC), Pacific Air forces (PACAF), United States Air Force in Europe (USAFE), Air Force Reserve Command (AFRC) , and Air Education and Training Command (AETC) as they assist in providing technical expertise in the operation and maintenance of the dive locker program, support development, and implementation of waterborne operations procedures designed to rapidly enhance the combat readiness and collective skills of Combat Rescue Officers and Pararescuemen. Specific requirements under this objective will actively support Dive Locker Maintenance support of Major Commands (MAJCOMS), Headquarters Air Force (HAF), System Program Offices (SPO), Joint Services, and Coalition partners as required/directed.

3.0. REQUIREMENTS: The Contractor shall provide Dive Locker Maintenance support to various locations supporting; Air Combat Command (ACC), Pacific Air forces (PACAF), United States Air Force in Europe (USAFE), Air Force Reserve Command (AFRC), and Air Education and Training Command (AETC) (optional).

The support shall include but not be limited to; Assist GA RQS by providing technical experience in the operation and maintenance of assigned diving equipment, boats and outboard motors IAW Manufacturer Specifications, and operation of boats during unit dive operations. In addition, Contractor dive locker technicians shall support development of waterborne operations procedures designed to enhance the combat readiness and collective skills of Combat Rescue Officers and Pararescuemen. Military diving experience and credibility in the DoD dive community are essential to successful delivery of the required service. Research available and emerging technologies and work with the lead command ACC and MAJCOM diving program manager to identify areas where technology advances in Dive Locker/Maritime operations can benefit the command. Specifically, the contractor should look for solutions to technology gaps, resource protection, and enhanced equipment training methodology that will save training time, funding, or enhance combat survivability of Pararescuemen and Combat Rescue Officers during water operations. The Contractor Dive Locker Technician may also be called upon to supervise and/or provide instruction to Combat Rescue Officers and Pararescuemen in all aspects of diving operations to include but not limited to Dive Supervisor training, Dive search Techniques, and high altitude diving restrictions at the discretion of the RQS Commander.

The term instruction refers to training, coaching, and professional development. The term Supervise refers strictly to the specific duties associated with coordinating, managing, and controlling hazardous activities for operators’ instruction and safety IAW applicable military directives; it does not imply day-to-day supervision and discipline of military personnel. The term IAW Manufacturer Specifications is definitive to the dive locker equipment that requires additional upgrade training / refresher

Assist GA RQS by providing technical training in the operation and maintenance of assigned diving equipment, boats, outboard motors, advanced rescue craft and all-terrain vehicles. Operate boats during unit Dive Locker/Maritime operations at the discretion of the GA RQS/CC. The contractor Training Instructors shall provide training in their respective competencies. Military diving experience and credibility in the DoD dive community are essential to successful delivery of the required service.

In addition, contractor Training Instructors shall support development of waterborne operations procedures designed to enhance the combat readiness and collective skills of Combat Rescue Officers and Pararescuemen. Research available and emerging technologies and work with the lead command ACC and MAJCOM diving program manager to identify areas where technology advances in Dive Locker/Maritime operations can benefit the command.

3.1. Task 1: ACC/A8R–Guardian Angel Master Diver (Core) –The government requires Master Diver support for each of the following locations: ( ACC) - Moody AFB, Georgia; Davis-Monthan, Arizona; Nellis AFB, Nevada; (PACAF) - Kadena AB, Japan; (USAFE) - RAF Lakenheath, United Kingdom; (AFRC) - Davis-Monthan, Arizona; Portland, Oregon and Air Education and Training Command (AETC) (optional). Subtasks are required at all locations unless otherwise specified. The table below identifies the number of military divers, and estimated number of contingencies at each location.

The following are the required sub-tasks associated with Guardian Angel Master Diver Support:

3.1.1. Subtask: Assist GA RQS Commander/ Director of Operations as members of the Military Technical Acceptance Board (MTAB) and/or Technical Training Acceptance Board (TTAB) IAW DoDI 3224.04. Requires 1 person for 1 week annually on average.

3.1.2. Subtask: Assist with establishing qualification and re-qualification criteria and administrative procedures for documentation in training records and diving logs for Air Force diving personnel, in coordination with GA Leadership.

3.1.3. Subtask: Make recommendations to ACC/A3J on Air Force diving operations, equipment and training matters that affect GA.

3.1.4. Subtask: Participate in or assist with joint and sister service diving program conferences, as requested. On average, all contract personnel attend 2 weeks annually.

3.1.5. Subtask: Review all reported diving incidents In order to recommend/direct coordinated changes to training and procedures, where appropriate.

3.1.6 Subtask: Maintain Dive Master list (for messages, e-mail, FAX numbers, etc) of all Diving Program Manager and disseminate information periodically to promote communication. Estimated 1 person, 3-5 hours monthly.

3.1.7. Subtask: GA RQS Master Divers shall provide training on all dive equipment (See attachment C) and procedures utilized by Guardian Angel to include the following:

3.1.7.1. Diver upgrade training for Mission Qualification Training, DIDSON & side scan SONAR, swift water search and recovery, enclosed space diving and Dive Supervisor IAW current Task Condition Standards.

3.1.7.2. Use, Operation, & Maintenance procedures for all SCUBA diving equipment to include open circuit SCUBA regulators, buoyancy compensators, life preservers, High Pressure Air Cylinders, diving Air Compressors, Full Face Mask systems, Dry Suits, and Single Para Scuba Deployment Systems (SPUDS).

3.1.7.3. Use and operation of the Navy Dive Computer (3M system).

3.1.7.4. Training for Oxygen Handling and Transfer procedures.

3.1.7.5. Confined Space SCBA Maintenance/Training.

3.1.7.6. Train the trainer capability for swift water rescue procedures.

3.1.7.7. The Rapid Deployment Search, Rescue and Recovery (RDSRR) Course Of Instruction to the train the trainer. (1 person from each location each year)

3.1.8. Subtask: Conduct formal and informal training on the maintenance of assigned equipment/systems and ancillary equipment.

3.1.9. Subtask: The Contractor will, when requested (1-2 times/monthly), assist the squadron training and scheduling section with planning, coordinating, and conducting training in open-water diving, Dive Locker/Maritime operations, rough water operations, and low visibility diving/underwater mechanical dexterity training. Required training may include, but is not limited to tactical diving, waterborne operations, and recovery fixed wing and helicopter deployed inflatable airdrop bundles.

3.1.10. Subtask: The contractor may conduct other training to include; HAZWASTE/HAZMAT program training, Tag out/Lockout procedures and the Planned Maintenance System.

3.1.11. Subtask: The Contractor is responsible for maintaining a stable workforce and providing personnel with expertise in the following areas:

3.1.11.1. The contractor shall be a DoD certified Master Diver

3.1.11.2. The contractor shall have Three (3) years experience knowledge of US Navy recompression chamber operations and have a thorough familiarity with recognition and treatment of diving emergencies as outlined in the US Navy Diving Manual.

3.1.11.3. The contractor shall have Three (3) years experience knowledge in and demonstrate familiarity in certification and maintenance of high pressure air systems and US Navy approved under water breathing equipment and support systems.

3.1.11.4. The contractor shall have Three (3) years experience knowledge in implementation and employment the US Navy Dive/Jump Reporting System, U.S. Navy Planned Maintenance System, and the 3M system as it applies to diving for near, mid, and long-term equipment requirements.

3.1.11.5. The contractor shall have Three (3) years technical background and communication skills to advise and/or assist the GA RQS subject matter experts and be able to analyze various equipment tests.

3.1.11.6. The contractor shall have Three (3) years experience knowledge in reading blueprints and schematic drawings, understand specifications, and interpret illustrated parts breakdowns and other publications used in the field of Dive Locker/Maritime equipment and dive related specialization.

3.1.11.7. The contractor shall have Three (3) years experience knowledge of US Navy dive regulations, MAJCOM and USAF regulatory guidelines and instructions to maximize the commands capability to provide Guardian Angel with interoperability with Special Operations Forces, Joint Services and/or coalition services.

3.1.11.8. The contractor shall have, and maintain a Secret security clearance upon assignment to an RQS.

3.1.11.9. The contractor shall have experience and knowledge of preparing and presenting high level briefings, Bullet Background Papers, and Technical Reports.

3.1.11.10. The contractor must be capable of producing electronic correspondence and proficient in Microsoft Office (Word, PowerPoint, and Excel) and with excellent analytical, speaking and writing skills.

3.1.11.11. In addition to US Navy dive regulations; the contractor shall be knowledgeable of USAF and MAJCOM regulatory guidelines and instructions to maximize the commands capability to provide Guardian Angel with interoperability with Joint and/or coalition forces.

3.1.12. Subtask: The contractor shall perform the following activities:

3.1.12.1. Schedule, direct, and monitor 3M system while keeping abreast of changes/directives issued by the US Navy.

3.1.12.2. Administer all daily, weekly, monthly, quarterly, and annually or greater 3M maintenance requirements for assigned diving related equipment.

3.1.12.3. Conduct inventory and maintenance of GA RQS (Attachment C) diving related equipment to include hydrostatic testing of SCUBA, SCBA bottles and other compressed cylinders IAW Department of Transportation regulations.

3.1.12.4. Maintain master files for Government Furnished Equipment (GFE) inventory and dive related programs.

3.1.12.5. The contractor shall maintain and update government furnished regulations, publications, and directives required by MAJCOMS, Naval Sea Systems Command (NAVSEA) and the Naval Safety Center pertaining to waterborne operations.

3.1.13. Subtask: The contractor shall advise users concerning equipment capabilities and maintenance requirements, andwork to resolve maintenance problems and advise users of problems outside the contractor’s ability to overcome.

3.1.14. Subtask: When requested, the contractor shall advise/assist in Air Force investigations and analysis of accidents, incidents, and failures. Provide technical expertise and assistance in report writing.

3.1.15. Subtask: The contractor shall Maintain the Naval Safety Center Dive/Jump Reporting System, provide quality assurance inspection of dive related information, make entries into the system and provide GA RQS divers with copies of Personal Dive Logs.

3.1.15.1. The Contractor shall maintain the Dive/ Jump Reporting System (DJRS) of all types of dives performed, equipment used, and note any dive related injuries/accidents to be entered into the Command database.

3.1.16. Subtask: The contractor shall provide operational support to GA RQS as follows:

3.1.16.1. The contractor shall provide boat driver support (on a non-interference with primary duties basis) for any Guardian Angel unit training (8 times/year)

3.1.16.2. The contractor shall provide Dive Supervisor or Diving Medical Technician support during training dives conducted by the Guardian Angel unit at home station and TDY locations. (4 times/year)

3.1.16.3. Upon successful completion of local qualification procedures, perform duties in accordance with local directives for Drop Zone Controller (DZCO). Not to interfere with primary duty capacity, 6 times annually.

3.1.16.4. The contractor shall ensure the SCUBA tanks are within hydrostatic test and annual visual inspection dates, filled and serviceable for employment.

3.1.16.5. The contractor shall function as squadron liaison between MAJCOM and GA RQS operators to ensure career field standards are applied to all contracted Dive Locker/Maritime operations.

3.1.16.6. The contractor shall provide recommendations and assistance with updating the MAJCOM Training and Standards/Evaluations Sections in the development of diver training policies, directives, and regulations for review and approval by HQ ACC A3J.

3.1.16.7. The contractor shall keep the Functional Area Chief (FAC) ,Contracting Officer’s Representative (COR), Unit COR-Field (formally QAP) ,and others as designated by the Contracting Officer, informed of task status and progress, as a minimum, by monthly correspondence or meetings. (listed in 3.3 Deliverables)

3.1.16.8. The contractor shall perform oxygen handler/oxygen transfer duties as required for aerial oxygen support.

3.1.16.9. The contractor shall manage and maintain SCBA’s associated with Technical Rescue/Recovery IAW and in conjunction with the manufactures maintenance programs.

3.1.17. Subtask: Monitor accountability of GA equipment, forecast for new equipment purchase, and research emerging technologies to support instruction. Provide up to date reports IAW Section Deliverables section 3.3.

3.1.18. Subtask: Maintain the Material Safety Data Sheet (MSDS) and hazardous material program for the Dive Locker/Maritime section.

3.1.19. Subtask: The contractor shall provide boat support during Dive Locker/Maritime training operations as boat operator.

3.1.20. Subtask: Contractor shall be capable of performing open-water swimming in support of Dive Locker/Maritime operations for emergency situations.

3.1.21. Subtask: The contractor shall schedule and perform routine preventive maintenance and minor repairs IAW 3M System (PMS or manufacturer’s specifications). Major repairs and overhauls are to be accomplished by a manufacturer’s authorized service facility or equivalent.

3.1.21.1. The contractor shall schedule calibration of all diving related gauges and special instruments tracked within the 3M system.

3.1.21.2. The contractor shall maintain, inspect, and report on the Diver’s Recall System.

3.1.21.3. The contractor shall maintain, inspect, and report on unit assigned breathing air compressors including quality assurance of divers breathing air samples.

3.1.21.4. The contractor shall maintain, inspect, and report on unit assigned search sonar system.

3.1.22. Subtask: A lead master diver shall be appointed to act as a focal point for all GA dive related issues. This individual shall be coordinated and approved by HQ ACC/A3J and HQ ACC/A5R.

3.2. Task 2 – ACC/A8R – Boat Operator/ Small Engine Technician support (Core) The government requires support for each of the following locations: ( ACC) - Moody AFB, Georgia; Davis-Monthan, Arizona; Nellis AFB, Nevada; (PACAF) - Kadena AB, Japan; (USAFE) - RAF Lakenheath, United Kingdom (AFRC) - Davis-Monthan, Arizona, Portland, Oregon and Air Education and Training Command (AETC) (optional). The actual number of personnel employed at each location will be dependent on the approval of funds.

The following are the required sub-tasks associated with Boat Operator/ Small Engine Technician support:

3.2.1. Subtask: GA RQS Boat operators/small engine technicians shall provide training in boat operation and maintenance to include the following:

3.2.1.1. Boat Master training IAW MAJCOM approved Qualification Training Plan (QTP), to include Training Platforms, inflatable Combat Rubber Raiding Craft (CRRC), Advanced Rescue Craft (ARC), Outboard Motors, Rules of the Road and the various delivery methods used when deploying these assets. The QTC and training platforms are located at the respective RQS Director of Operations Training (DOT) Office.

3.2.1.2. Develop and instruct use and maintenance courses for GA Watercraft to include but not limited to ARC, inflatable CRRC’s, and Outboard Engines.

3.2.2. Subtask: The Boat Operator/Small Engine Technician shall have appropriate military or civilian training and/or experience to support the training, technical and maintenance skills required for RQS operations.

3.2.3. Subtask: The contractor shall schedule, direct, and monitor 3M system while keeping abreast of changes/directives issued by the US Navy and other applicable agencies.

3.2.4. Subtask: The contractor shall administer all daily, weekly, monthly, quarterly, annually, or greater 3M maintenance requirements for assigned Dive Locker/Maritime related equipment.

3.2.5. Subtask: The contractor shall conduct inventory and maintenance of GA RQS watercraft, engines, and Dive Locker/Maritime related equipment.

3.2.6. Subtask: The contractor shall maintain master files for Government Furnished Equipment inventory and Dive Locker/Maritime related programs. Provide up to date reports IAW Section 4.3.

3.2.7. Subtask: The contractor shall maintain and update government furnished regulations, publications, and directives required by MAJCOM’s, Naval Sea Systems Command (NAVSEA), the Naval Safety Center, and other applicable agencies pertaining to waterborne operations.

3.2.8. Subtask: The contractor shall advise users concerning equipment capabilities and maintenance requirements. Work to resolve maintenance problems and advise users of problems outside the contractor’s ability to overcome.

3.2.9. Subtask: When requested, the contractor shall advise/assist in Air Force investigations and analysis of accidents, incidents, and failures. Provide technical expertise and assistance in report writing

3.2.10. Subtask: The contractor shall provide operational support to GA RQS as follows:

3.2.10.1. Provide boat driver support for unit training.

3.2.10.2. The contractor shall ensure that unit assigned boats and engines are serviceable and in operational condition, and the inflation systems are properly installed (as required).

3.2.10.3. Demonstrate experience and certification in small engine and watercraft operation.

3.2.10.4. Provide boat support during Dive Locker/Maritime training operations as boat operator and provide boat operator training.

3.2.10.5. At the discretion of unit commander, the contractor shall provide boat driver support (on a non-interference with primary duties basis) for any Guardian Angel unit training at home or TDY locations.

3.2.11. Subtask: The contractor shall demonstrate experience and qualifications in the following:

3.2.11.1. Inflatable CRRC bundles such as RAMZ, Soft Duck, T-Duck, and Hard Duck along with ARC package deployment and recovery procedures, parachute and swimmer recovery.

3.2.11.2. Waterborne Inflatable CRRC package pallet recovery and towing.

3.2.11.3. Small craft, Inflatable CRRC and outboard engine maintenance as outlined in the US Navy 3M systems.

3.2.11.4. Small Craft operation, Navigation and Rules of the Road, Seamanship, and Dive Locker/Maritime Communications.

3.2.12. Subtask: As required, be capable of performing open-water swimming in support of amphibious operations for emergency situations.

3.2.13. Subtask: The contractor shall be certified and experienced in the maintenance and repairs of outboard marine engines.

3.2.14. Subtask: The contractor shall maintain and repair RAMZ bundles, assigned boats, outboard engines, and ARC in the most economical way possible based on available facilities and extent of repairs required.

3.2.14.1. Schedule and complete routine preventive maintenance and minor repairs as necessary.

3.2.14.2. Major repairs and overhauls are to be accomplished by a manufacturer’s authorized service facility or equivalent. See Appendix C for list of government equipment to be maintained.

3.2.15. Subtask: The contractor shall ensure Dive Locker/Maritime equipment such as Inflatable CRRC’s, Outboard Motors, Whalers, ARC, Rigged Inflatable Boats (RIBs) Watercraft and other associated equipment is maintained in a serviceable condition.

3.2.16. Subtask: The contractor shall have manufacturer certification to be qualified to repair and overhaul outboard engines (Attachment C). For ARC, RIB and Whalers that require specific qualification (beyond the standard outboard engine repair), the contractor will schedule an off base repair by an authorized mechanic.

3.2.17. Subtask: The Contractor, when requested at the discretion of GA RQS/CC shall maintain other small engines not supported by base vehicle support flight (on a non interference with primary duties basis). The costs for additional training and certification will be paid for by the supported RQS.

3.2.18. Subtask: The Contractor shall maintain the Dive Locker/Maritime Material Safety Data Sheets (MSDS) and hazardous material program.

3.2.19. Subtask: The contractor shall monitor accountability for GA equipment, forecast for new equipment purchase, and research emerging technologies to support instruction.

3.2.20. Subtask: The contractor shall have experience and knowledge of preparing and presenting high level briefings, Bullet Background Papers, and Technical Reports.

3.2.21. Subtask: The Contractor is responsible for maintaining a stable workforce and providing personnel with expertise in the following areas:

3.2.21.1. Must have three (3) years experience and current certification in the maintenance and repairs of marine outboard engines and demonstrate experience and certification in small engine and watercraft operation.

3.2.21.2. Must have three (3) years experience and current qualification in Small craft, Zodiac and outboard engine maintenance as outlined in the US Navy 3M systems.

3.2.21.3. Must have three (3) years experience knowledge in the maintenance and use of High Pressure air compressors.

3.2.21.4. Must have three (3) years experience knowledge in Small Craft operation, Navigation and Rules of the Road, Seamanship, and Dive Locker/Maritime Communications course.

3.2.21.5. Must have three (3) years experience knowledge of the supply system and material ordering procedures and demonstrated writing ability with sufficient technical background to analyze data from various equipment tests.

3.2.21.6. Must have three(3) years experience knowledge in maintaining and repairing unit assigned watercraft, inboard/outboard engines, and associated equipment in the most economical way possible based on available facilities and extent of repairs required. Scheduling and completing routine preventive maintenance and minor repairs as necessary.

3.2.21.7. The contractor shall have, and maintain a Secret security clearance upon assignment to an RQS.

3.2.21.8. Must be capable of producing electronic correspondence and proficient in Microsoft Office (Word, PowerPoint, and Excel) and with excellent analytical, speaking and writing skills.

3.2.22. Subtask: Participate in or assist with joint and sister service exercises/conferences, as requested.

3.3. Deliverable(s): The Contractor shall provide deliverable(s) in a format mutually agreed upon by the Government and the Contractor. The ACC AMIC/PKB does not require copies of listed deliverables.

3.3.1. Monthly Status Report. The Contractor will submit monthly status reports due the 10th of each month. The report will include a summary (for each employee, listed separately) the number of trips completed; Number of Dive Supervisor Hours, work performed or completed status of training currently in progress and projected activities, events and trips for the next quarter. Specific delivery instructions for monthly progress reports will be as follows: Provide one electronic copy of the report to the COR ACC/A5R and ACC/AMIC Contracting Officer & Manager.

3.3.2. Dive Report. IAW OPNAV 3150.27B, as required, submit all dive reports using the computerized Dive/Jump Reporting System to the Naval Safety Center.

3.3.3. Maintenance Report. IAW OPNAV 4790, as required, prepare oral/written reports/recommendations to correct deficiencies in all aspects of waterborne equipment maintenance and provide one electronic copy of the report to the RQS/DO and RQS/ Tech Manager.

3.3.4. Trip Report. Upon return of all TDY’s or conferences the contracted employee at each RQS will submit a Trip Report within ten (10) working days after completion of any travel and will address the purpose of the trip, the results of any meetings that occurred and provide a summary of events that took place during the trip and the actual cost. Specific delivery instructions for trip reports will be as follows: Provide one electronic copy of the report to the ACC A5R COR and RQSTechnical Manager.

3.3.5. Mishap Report. When requested, advise/assist in Air Force and Navy investigations and analysis of accidents, incidents, and failures. As required, report incidents/mishaps in as prescribed in AFI 10-3501 within 72 hours of the event. As required, provide inputs in written form. As required, develop briefings and information papers related to maintenance and operational problems for dissemination to the GA RQS personnel. Provide one electronic copy of the report to RQS Safety POC.

3.3.6. Training Policy. When requested, assist the GA RQS Training and Standards/Evaluations Sections in the development and drafting of diver training policies, directives, and regulations for review and approval by the MAJCOM. Provide one electronic copy of the report to RQS/Technical Manager and RQS Director of Training Office (DOT).

3.3.7. Cost Breakdown Report. The contractor will provide, a monthly report detailing all expenditures of Government purchases incurred to include all forecasted expenses to include providing a detailed cost comparison of expended costs vs. programmed costs. Report shall be submitted electronically and be readable by MS Excel. Provide one electronic copy of the report to the ACC A5R COR, and RQS/Technical Manager by the 10th of each month.

3.3.8. Quality Control Plan (QCP). The contractor will provide a Quality Control Plan within 30 days of contract award. This plan should detail how the contractor intends to monitor their performance and correct major and minor deficiencies.

5.0. GOVERNMENT FURNISHED PROPERTY

5.1. TITLE OF DOCUMENTATION:

The Government will furnish or make available to the Contractor any documentation deemed necessary by the Government to accomplish the applicable objective, to include mission briefs, organization charts, existing database and website files, locally established guides and operating instructions. The Government will provide all required equipment for Dive Locker/Maritime operations to include dive apparatus and boats. See Attachment C for listing of inventory items to be included under this contract.

5.2. GOVERNMENT PROVIDED EQUIPMENT/OFFICE:

The government will provide desks, workspace, utilities, electrical power hookups, personal computers, printer, fax machine, telephone access, vehicles, administrative supplies for on-site personnel, and all audiovisual projection equipment. Contractor personnel must follow the standard facility rules and regulations required of government personnel. The Government will provide electronic mail and Internet access to the Contractor. The Contractor shall use these Government furnished services only in matters related to the performance of this contract. The Government will provide all Local Area Network (LAN) remote access. The Contractor shall not establish any external network connectivity. The Government may limit or restrict these services at any time.

5.3. GOVERNMENT PROVIDED EQUIPMENT/DIVE LOCKER:

A list of all equipment is provided in Attachment C. All equipment and parts required to perform this contract are provided by the Government and maintenance space is provided at each GA RQS dive locker location.

5.4 GOVERNMENT-FURNISHED PUBLICATIONS, SYSTEMS AND FORMS:

Publications and forms that apply to the Contractor UTM are listed below. The contractor shall maintain current copies of forms and publications via the Internet at http://www.e-publishing.af.mil/ for the documents required to provide a squadron Unit Education and Training Program.

5.4.1. Supplements or amendments to listed publications from any organization level may be issued during the life of the contract. The contractor shall immediately implement those changes that result in a decrease or no change in contract price and immediately notify the contracting officer in writing. Should a decrease in contract price result, the contractor shall provide a cost proposal for the downward adjustment to the contracting officer within 30 days of the change. In addition, any change that will result in an increase in price must be coordinated in writing with the contracting officer prior to execution, to ensure the change is in scope of the contract requirements. Before implementing any change that will result in an increase in price, the contractor shall submit a cost proposal to the contracting officer within 30 days for negotiation and approval. The contractor must be notified in writing by the contracting officer, prior to executing. Failure of the contractor to submit a cost proposal within 30 days following receipt of the change entitles the government to performance according to such change at no increase in contract price. Upon completion of the contract, the contractor shall return to the government all issued publications and forms.

6.0 DATA: The Government has unlimited rights to all deliverables of this contract to include intellectual property rights.

7.0 TRAVEL

7.1 The Contractor shall be required to travel in support of tasks listed above to fulfill PWS requirements. All trips will be in accordance with regulatory requirements and actual travel shall be coordinated and pre-approved by the Government COR in writing. Trips scheduled below are subject to change. The Government will only reimburse allowable costs in accordance with FAR

Clause 31.205-46. The Contractor shall be required to provide travel receipts to the COR for all allowable costs. Travel projections are subject to change based on Government requirements.

7.2 Local travel to attend meetings within the local city or base of assignment commuting vicinity shall be considered a cost of doing business and shall not be separately reimbursed. The Contractor will fund all travel and expenses for employee proficiency requirements.

8.0 SECURITY INSTRUCTIONS: A DD-254 will be required for this TO requirement.

8.1. Security Clearances. Certain objectives will require the contractor to work with documents with security classifications up to, and including, SECRET. All contractor personnel assigned to work on the contract shall hold a SECRET or higher clearance prior to being assigned work compatible with the level of clearance. The contractor shall provide a copy of the applicable security clearances, if not already provided, to both the Contracting Officer, ACC/A5RG COR and the RQS/Technical Manager personnel before performance is scheduled to begin. Refer to the DD Form 254 prepared for this requirement for additional security requirements.

8.2. Document Accountability. The contractor shall maintain accountability records for contractor generated and received classified material in accordance with DoD 5220.22M.

8.3. Contractor classified accountability record/receipts, or AF Form 310, Document Record and Receipts, shall be used for accountability of classified material, to include transfer and destruction. Contractor facilities for the storage of classified material shall meet the requirements of DoD and Air Force industrial security policies and procedures.

8.4. Notifications to Security Forces. The contractor shall notify the security forces at individual RQS units or any installation on which work is being performed under this contract, as to the details listed in AFFARs Clause 5352.242-9000. The contractor is required to notify the security forces at least 30 days before operations are to begin on base.

8.5. Facility Security Clearances. When the contractor is required to work on classified material but not required to have a facility security clearance, the contractor shall enter a security agreement or understanding with the installation commander to ensure that its’ security procedures are properly integrated with those of the installation.

8.6. Performance under this PWS shall require SECRET clearance. As and when government deems that any FTEs require NATO access, government will notify contractors and government will process applicable justification documentation.

9.0. ADMINISTRATION CONSIDERATIONS:

9.1. Performance Schedule: The Contractor shall perform this task from 1 Oct 2013 to 30 Sep 2018

9.2. Place of Performance: Place of Performance. Support will be provided at the following locations: See Attachment D for current locations.

9.3. Duty Hours: Contractor shall normally be scheduled 5 days a week; however, contractor shall be available 7 days a week. Determination of weekly hours and usage is dependent upon each individual GA RQS/CC and/or delegated personnel to provide sufficient support of mission operations and/or world-wide Dive Locker operations. The hours set forth in this contract are NTE 1880 hours a year per Dive Locker personnel per location. Overtime is not authorized.

9.3.1. Supervisor labor hours for on-site training will be documented against the appropriate MAJCOM contract line number. The preparation of the dive and travel to the site will be documented as normal labor.

9.4. Recognized Federal Holidays. The contractor will not normally be required to provide service on the following holidays recognized by the Federal Government. Holidays in addition to these are reflected in the current U.S. Department of Labor Wage Determination Agreement.

Normally, if the holiday falls on a Saturday, it is observed on the preceding Friday. If the holiday falls on a Sunday, it is observed on the following Monday.

9.5. ACCESS CARDS. Each employee performing under this contract is required to carry a Common Access Card (CAC) and will be issued one in accordance with applicable regulations.

9.6. PASS AND IDENTIFICATION ITEMS. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees and non-government owned vehicles. Base identification badges and/or car decals will be furnished by the installation security office for each contractor employee upon presentation of evidence of assignment.

9.7. RETRIEVING IDENTIFICATION MEDIA. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires; e.g. terminated for cause, retirement, etc.

9.8. TRAFFIC LAWS. The contractor and its employees shall comply with base traffic regulations.

9.9. WEAPONS, FIREARMS, AND AMMUNITION. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on any Air Force base.

9.10. FOR OFFICIAL USE ONLY (FOUO). The contractor shall comply with DOD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

9.11. REPORTING REQUIREMENTS. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware that is: illegal; may pose a threat to the security of DOD personnel, contractor personnel, or resources, or violates official regulations, directives or instructions and/or classified or unclassified defense information. Contractor employees shall be briefed by the RQS Technical Manager upon initial on-base assignment.

9.12. PHYSICAL SECURITY. The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

9.13. PRIVACY ACT. Work on this project may require that the contractor have access to Privacy Information. The contractor shall adhere to the Privacy Act, Title 5 of the U.S. Code, Section 552a and applicable agency rules and regulations.

9.14. IN/OUT-PROCESSING REQUIREMENTS. Contractors will be required to complete in-processing requirements, which will include at a minimum, security paperwork, security badge issue, initial training requirements and network accounts creation. Contractor employees terminating work will be required to complete out-processing requirements, which will include at a minimum security badge revocation.

9.15. SMOKING, EATING AND DRINKING. The contractor shall comply with base smoking regulations and smoke, eat, and drink only in designated areas.

9.16. DRIVING LICENSE/ON-BASE REQUIREMENTS.

9.16. 1. DOD Contractor employees shall comply with base traffic regulation DOD 4500.36-R, para C8.2.5 and AFI 24-301, para 8.10. “Licensing Contractor Personnel: Contracts and agreements will require that operators comply with licensing requirements of the State and local motor vehicle laws. DoD contractor employees shall not be issued Optional Form (OF 346), ―U.S. Government Motor Vehicle Operator’s Identification Card or AF Forms 2293 or 2296. DoD contractor employees assigned to operate either government owned/leased equipment in performance of their contract shall be certified, by the contractor and at the contractor’s expense, as being fully qualified to operate the vehicles/equipment to which they are assigned. The prime contractor shall document all operator qualifications. This documentation shall be provided to the administrative contracting officer before any contract employee engages in any mode of equipment operation. This letter will be updated as necessary and remain on file with Operators Records and Licensing OR&L and the administrative contracting officer.

9.16.2. Forklift operator – The government will provide a government vehicle (to include forklift) to transport dive locker related equipment (e.g. storage containers, boats, RAMZ pkg)”. Government may provide training if host unit/base has equivalent forklift for training. If host base is unable to provide training then it is the responsibility of the prime contractor to fund training.

9.17. KEY/CIPHER LOCK CONTROL. The Contractor shall establish and implement methods of ensuring that keys/key cards issued to the Contractor by the Government are not lost or misplaced and are not used by unauthorized persons. All references to keys include key cards, cipher locks, CAC etc.

9.18. DISCLOSURE. The contractor shall not disclose or use proprietary information gained in the performance of this task.

10.0 ENVIRONMENTAL CONCERNS

10.1.. Recycling. The contractor shall comply with the Base Recycling Program as outlined in HFI 32-7002, paragraph 1.5.17.

10.2. Affirmative Procurement. The contractor shall practice environmentally preferable purchasing in accordance with Executive Order (E.O.) 131010 for all supplies and materials purchased for this contract.

10.3 Conservation of Utilities. The contractor and their employees shall operate under conditions that prevent the waste of utilities.

11.0. QUALITY ASSURANCE. The government will evaluate the contractor’s performance under this contract. For those tasks listed in the Service Delivery Summary, Section 2, the RQS Technical Advisor (formally QAP) will follow standard inspection procedures or other contract/order provisions. Any action taken by the contracting officer as a result of surveillance will be according to the terms of this contract.

12.0. ESSENTIAL SERVICES. The services under this contract are essential for performance during crisis according to DODI 3020.37. Specified hours of operation will be provided for continuation of services for each instance as required. The Contractor will be notified verbally and/or in writing by the on-site commander or the designated representative of the assigned unit when services are essential for performance during a crisis situation.

13.0. COMMAND SPONSORSHIP. The Government shall provide Contractor employees and authorized dependents, who have been determined by their USAFE and PACAF sponsoring agency to be command sponsored, and authorized full logistical support. This logistical support is sometimes referred to as "Package A", which includes twenty-three items or privileges to include Commissary and Military Exchange (AAFES) privileges. This package also includes medical/dental services on a reimbursable basis. However, some restrictions apply for certain items, such as access to government lodging and tuition-free access to DoD dependent schools. NATO Status of Forces Agreement (SOFA) is not granted for USAFE and PACAF, therefore the Prime Contractor will be responsible to cover the tax burdens. Prime Contractor will be responsible to ensure that their employee has a valid “Work Visa”.

13.1. ENTITLEMENTS: The 24 items considered to constitute full entitlement are listed below:

13.2. OTHER DIRECT COSTS: Living Quarter Allowance (LQA) shall be authorized as an ODC in support of positions located in Japan and the United Kingdom. LQA shall not exceed the annual cap as posted on the Department of State website at http://www.defensetravel.dod.mil/perdiem/ocform.htmlwithout the expressed approval of the Government Contracting Officer.

13.3. CONTRACTOR IDENTIFICATION: All contractor employees attending meetings, answering DoD telephones and working in other situations in which their contractor status is not obvious to third parties, to include answering the telephone, must identify themselves as contractor employees to avoid creating the impression that they are DoD employees. In particular contractors must wear a distinctive badge, nametag, or other signal as to their special status within the workplace.
