STATEMENT OF WORK
Gas Leakage Test Survey
Luke AFB, AZ 85309
1.0. Introduction:
The Contractor shall provide all labor, tools, equipment, hoses, gauges, test equipment, hearing protection, eye protection including prescription safety glasses, materials, supplies and parts, transportation and other incidentals necessary to travel to Luke AFB, AZ, to perform a Gas Leak Detection Survey on approximately 244,770 LF of natural gas distribution system. The Contractor shall identify, report and record all leak locations and perform work in a manner that will comply with Federal, State and local regulations for leak detection surveys and reporting services.
2.0. Location of Work:
All of Main Base Property (maps will be provided upon request).
3.0. Site Visit:
Contractors are urged and expected to inspect the site where services are to be performed. This will satisfy the contractor regarding all general and local conditions that may affect the cost of contract performance to the extent the information is reasonably attainable. In no event shall failure to inspect the site constitute grounds for claim after contract award.
4.0. Scope of Work:
The work will be covered by these specifications consisting of furnishing all labor, materials, transportation, and tools, necessary to conduct gas leakage test survey on main base property.
4.1. Inspection/Survey:
a) Survey all natural gas lines using a non-optical survey method
b) Survey master meter systems using guidelines from the Arizona Corporation Commission
c) Survey gas leakage on all above ground meter set assemblies in base property
d) Survey all underground piping in main base property
e) Pinpoint all underground leaks to the highest read in percent of gas with combustible gas indicator
f) Submit a leakage control report with address location of leak and description of items leaking
g) Below ground leaks should have a drawing showing the combustion gas indicator bar holes percentage, identification/location of leaks, the footage from building or street and recommendation of repair if needed.
h) The contractor is responsible for the storage and safe keeping of all materials used. Damage of any nature caused by this work shall be repaired or replaced by the contractor without any expense to the Government
i) All work shall be performed by a license contractor and comply with all federal and state codes
j) Submit a final written report of the survey with recommendations IAW CFR 49 192 in paper and electronic formats
k) Submit a final written report of the master meter systems survey with recommendations IAW the Arizona
Corporation Commission guidelines within thirty (30) calendar days after completion of the survey.
l) Upon completion of the survey, contractor shall arrange for a final inspection and acceptance with CE.
4.2. Clean Up:
Contractor must clean up debris, refuse and surplus material from job at the end of each working day. Remove stated items from premises upon completion of work, so that work location is left in clean condition.
5.0. Final Inspection:
The contractor shall conduct a final inspection and publish the findings in a Final Inspection Report in accordance with the format specified by 56th CES/CEOI. The inspection shall concentrate on the items identified in this SOW. The final inspection report shall also certify that all items of the SOW have been implemented and that the Service is complete. This Final Inspection Report shall be complete and submitted to the COR within thirty (30) calendar days after completion of the survey.
6.0. Performance Assessment:
The Contractor shall be aware that the absence of any contract requirement from the Service Summary does not detract from its enforceability nor limit the rights or remedies of the Government under any other provision of the contract. The Contractor shall ensure the following service summary requirements are accomplished.
6.1. Quality Assurance:
According to the contract’s inspection clause, the Government will evaluate the Contractor’s performance under this contract. The Contracting Officer’s Representative (COR) is a representative of the Contracting Officer (CO) and will participate in the administration of this contract. Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the CO. All services to be performed by the Contractor shall be subject to review by the CO or COR. The Contractor shall provide an answer to a submitted Corrective Action Report (CAR) and Customer Complaint Form from the COR within one (1) working day from receipt. The Contractor shall provide written response addressing the root cause, corrective action and preventive actions.
6.2.

7.0. Work Site Requirements:
7.1. Working Hours:
All tasks will be performed during normal work hours of regular workdays, from 0700 to 1600, Monday through Friday. Unless otherwise discussed with the Contracting Officer and the 56th CES/CEOI, PM. Contractor will notify the utilities manager prior of any utility outage by a minimum of 2 weeks notification.
7.2 Contractor Access:
Advance scheduling and coordination with the using organization is mandatory to avoid conflict with any activities having precedence.
8.0. Safety Requirements:
a) The contractor shall conduct operations in accordance with FAR Part 52.236-13 and Alternate 1.
b) Concerning possible asbestos; the contractor must take preventative measures. If the contractor suspect’s asbestos is present at any locations as specified in this project, please contact Mr. Carl Moser immediately at
(623) 856-8488.
c) The contractor must follow the Fire Prevention Guide set forth by Luke AFB, Fire Department. Particular attention must be focused to prevent setting off false fire alarms.
8.1. Storage:
The contractor shall conduct operations in accordance with FAR Part 52.236-10.
8.2. Security:
The contractor shall obtain and monitor security badges used by their own staff during the duration of this contract, in accordance with contract Clause L-727, entitled Contractor Access to Air Force Installations.
8.3. Period of Performance:
The period of performance will be for a base year plus four one-year option years. The survey shall be completed annually, between the months of November - January, and the Contractor shall provide the Government 30 days advanced notice with a proposed schedule of inspection/survey dates prior to start of the leak detection survey.
