STATEMENT OF WORK

Clean And Evaluate The Existing Condition Of Well #12 Luke Air Force Base, Arizona 24 April 2019

1. OVERVIEW

Well #12 is currently being used. This well is 972 feet deep. Screen level intervals are unknown. Other wells on the base range from 740 to1150 feet in depth.

2. DESCRIPTION OF WORK

2.1. This project includes all of the necessary tools, equipment, and manpower needed in order to clean, test, evaluate, and analyze the condition of Well #12.

2.2. Performance Period: All work shall be completed within 12 months after the Notice to Proceed (NTP) is issued.

2.3. Hazardous Material Abatement: No asbestos, lead or other hazardous substances are known to exist. The Contractor shall report any suspicious material immediately to the Contracting Officer

2.4. Design Drawings: This project does not require design drawings, but will require a simple one-line diagram indicating the locations for zonal sampling and keynotes regarding spinner flow meter logging is to be conducted.

2.5. Calculations: In order to identify where the zone specific samples should be collected, a spinner flowmeter tool will be lowered down the 3-inch diameter access tube into the well casing below the pump intake at three (3) different line speeds. Data collected will include the depth, line speed and impeller rotation rate of the spinner tool. With this data, the quantity of water entering the well casing, along with the well profile will be calculated. This information will then be used to identify the location of the specific zonal sampling points.

2.6. Survey: The contractor shall perform two (2) down-well video surveys; one before cleaning begins, and one after cleaning is complete. Details of how the survey shall be conducted are listed in paragraph 2.7.4 below.

2.7. General Construction:

2.7.1 Demolition: The Government does not expect or foresee any demolition in this project, but the Contractor shall contact the Contracting Officer (CO) or his/her designated representative if any demolition becomes necessary as a part of this project.

2.7.2 Well Evaluation: Pull existing pump equipment for access into the well. Inspect the pump column pipe, pump bowls, and ancillary equipment. Provide recommendations for repair, cleaning, and/or replacement.

2.7.3 Cleaning: Remove and clean the dedicated pumps. Mechanically clean the well casings/screens/perforations in order to remove scale and encrustation. Chemically clean the well casing in order to remove biocide buildup. The Contractor should assume the perforations are completely clogged due to very heavy encrustation and biocide buildup and plan for an increased level of mechanical cleaning to remove scale and encrustation and increased amount of chemicals to remove biocide buildup.

2.7.4 Video Survey/Log: Develop a video survey/log before and after the mechanical/chemical cleaning is completed. Perform Down Well Video Survey to:

2.7.4.1 Confirm total depth and diameter of the well.

2.7.4.2 Determine the static water level.

2.7.4.3 Evaluate the condition of the casing for breaks, tears and buckles.

2.7.4.4 Determine the location and conditions of the perforations.

2.7.4.5 Evaluate the amount and location of encrustation

2.7.5 Spinner Flowmeter Log: Conduct spinner flowmeter logging in order to identify distinct flow zones, their flow rates, and total contribution into the well.

2.7.6 Collect and analyze depth specific water samples from flow zones specified by the government after evaluating the results from the spinner flow meter log.

2.8 Zonal Sampling:

2.8.1 Install test pump equipment in order to collect down well zonal water samples below the pump intake. Existing pump-head equipment will not permit access of a spinner logging tool and sampling equipment.

2.8.2 Conduct a down well spinner log while pumping at a constant rate.

2.8.3 Conduct zonal sampling after 12 to 24 hours of constant pumping.

2.8.4 Analyze a minimum of five (5) dynamic zonal samples for arsenic, fluoride, nitrate, and Total Dissolved Solids (TDS)

2.8.5 Review the zonal sampling data in order to determine/recommend which zones might be sealed off in order to improve the pumped ground water quality.

2.9 Testing And Analysis:

2.9.1 The Contractor shall ensure the selected laboratory is certified by the Arizona Department of Health Services. The Contractor shall analyze and test environmental samples using standardized methods in current editions of approved testing references such as SW-846,”Test Methods of Evaluating Solid Wastes, Physical/Chemical Methods”; 40CFR 136, “Guidelines Establishing Test Procedures for the Analysis of Pollutants”; and ”Standard Examination of Water and Wastewater”. The Contractor shall ensure the defensibility of test and analytical results through execution and scientifically sound activities as outlined in the appropriate portion of the approved Sampling and Analysis Plan (SAP).

2.9.2 The Contractor shall perform services in support of the AF Safe Drinking Water Surveillance Program and in compliance with the Safe Drinking Water Act (SDWA) In addition, the Contractor shall also comply with all applicable federal, state and local laws/regulations. All DoD and AF directives including but not limited to AFI48-144, AFI32-1066, AFI32-1067, and AFI10-246 apply as well.

2.10 Schedule:

2.10.1 The contractor shall coordinate the start date with the base.

3. GOVERNMENT FURNISHED INFORMATION, PROPERTY, OR SERVICES

3.1. The Contractor is responsible for reviewing all available information and forming their independent, professional conclusions/interpretations of the equipment conditions and/or requirements that are necessary to meet the requirements in the Statement of Work.

3.2. Upon the Contractor’s request, the Government will provide all prior and relevant research, studies, documents, plans, drawings, monitoring reports, permit applications and permits connected with this project.

4. GENERAL INFORMATION

4.1. GENERAL

4.1.1 The contractor shall furnish all materials, equipment, and personnel necessary to design, manage, and accomplish the project. The Contractor shall thoroughly review the SOW and Request For Proposal (RFP) for constructability, then advise the contracting officer of potential problem areas prior to submitting a cost proposal. No alterations, additions or deletions shall be made unless prior written approval is given by the Contracting Officer. Under no circumstances is the Contractor to begin any work on the project until a Notice to Proceed (NTP) is issued by the Contracting Officer.

4.1.2 The Contractor shall identify and comply with applicable federal, state, and local environmental laws, statutes, and regulations, as applied to each installation through the appropriate EO’s AF/Military instructions, manuals, handbooks, regulations, guidance, and policy letters. This includes but is not limited to AFI 32-7001, as well as American Petroleum Institute Codes, National Association of Corrosion Engineers, National Fire Protection Agency, Steel Structures and Painting Counsel, Uniform Fire Code, and National Environmental Accreditation Conference. All work must meet or exceed acceptable standards of the building trades.

4.2. SITE VISIT

The Contractor shall inspect the site where services are to be performed. This site review shall satisfy the Contractor’s need to identify all general and local conditions that may affect the cost of the contract. Failure to inspect the site will not constitute grounds for claims after contract award.

4.3. SPECIAL DELIVERY/WARRANTY

4.3.1. N/A.
