Performance Work Statement For

The Repair of Fuel Tank Monitoring System and UDC Sump Penetration Boots

1.0. DESCRIPTION OF SERVICES

1. Scope of Work. The contractor shall provide all equipment, tools, parts, transportation, personnel, labor, supervision, and management to provide and install one new TLS450 Veeder Root tanking Monitoring System with printer and system components at building / site 4885.

The contractor will install and provide new penetration repair fitting for one transition sump and two dispenser UDC pans at building / site 4885. The contractor will provide and install new penetration repair fitting for two dispenser UDC pans at building / site 4712.

1.2 Install Tank Monitoring System Bldg. 4885

1.2.0 The contractor will furnish equipment, material and labor to barricade the construction site; and remove and dispose of the old existing tank monitoring system.

1.2.0.1. The contractor will provide and install (1) new TLS450 Veeder Root Tank Monitoring System, (1) New Veeder Root TLS450 console / touchscreen with software and printer (1) new Veeder Root Tank Monitoring probe, (1) new Veeder Root Interstitial Sensor, (1) new Veeder Root Transition Sump Sensor, (1) new Veeder Root overfill Alarm with Acknowledgement Switch for existing above ground fuel tank.

1.2.0.2 The contractor will pull out all of the existing Tank Monitoring (T/M) wiring out of the existing conduits and reinstall new Veeder Root Cable for the new Veeder Root TLS 450 system.

1.2.0.3 The contractor will reuse most of the existing conduits on site for the new installation.

The contractor may have to install new conduits as required.

1.2.0.4 The contractor will program and start-up the new system, test the system to ensure it functions properly, and provide necessary system overview to Air Force fuels technicians..

1.2.0.5 The contractor will provide all materials and labor necessary to put the TLS450 Veeder Root Tank Monitoring System into service.

1.3 Install New Sump and Dispenser UDC Sump Penetration Boots Bldg. 4885

1.3.0 The contractor will furnish equipment, material and labor to barricade the construction site; and remove all the fuel dispensers off the existing fuel island.

1.3.1 The contractor will provide and install new tank sump penetration repair fittings for (1) transition sump and (2) dispenser UDC pans.

1.3.1.1 The contractor is to remove the existing old damaged UDC sump penetration fittings out the (2) UDC pans.

1.3.2 The contractor will provide and install the new repair UDC sump fitting into the existing

(2) UDC dispenser pans and hydrostatic test each UDC sump.

1.3.3 The contractor will re-install the (2) existing dispensers and perform restart operations.

1.3.4 The contractor will barricade the transition sump manway area, remove the old existing damaged transition sump penetration fittings out of the (1) transition sump.

1.3.5 The contractor will provide and install a new repair transition sump fitting into the existing (1) fuel transition sump and conduct hydrostatic testing.

1.4 Install new UDC penetration repair kits for the (2) UDC dispenser pans for site / Bldg.

1.4.0 The contractor will furnish equipment, material and labor to barricade the construction site; and remove all the fuel dispensers off the existing fuel island.

1.4.1 The contractor will remove existing old damaged UDC sump penetration fittings out of the (2) UDC pans.

1.4.2 The contractor will provide and install new repair UDC sump fitting into the existing (2) UDC pans. Contractor will provide and install new repair UDC sump fitting into the existing (2) UDC dispenser pans and hydrostatic test each UDC sump.

1.4.3 The contractor will hydrostatic test each UDC sump, ensure that each passes, re-install the

(2) existing dispensers and restart-up.

Note: Project must comply with Davis-Monthan Base Design Guide, High Performance Green Building (HPGB) and Sustainable Development requirements established in Executive Order 13423, Public Laws, and Federal Regulations

1.5 The Civil Engineer Squadron can provide hard copy as-built drawings of the fuel tank systems for the contractor’s use upon request.

2.0. Emergency Services. The Government will provide security forces services (call (520) 228-4444 for emergencies only), and fire protection services (call (520) 228-3333). There are no emergency medical services available on DMAFB; therefore the contractor must dial 911 for emergency medical services. IF the contractor calls 911 from a base phone, they must tell the 911 operator that they are calling from DMAFB.

3.0. Contract Manager. The contractor shall designate a contract manager who shall be responsible for contract performance and can act for the contractor on all matters relating to daily operation of the contract. The Contract Manager may be a contractor employee performing services. Upon contract award, the contractor shall provide the name and phone number of the Contract Manager, in writing, to the Contracting Officer. The Contract Manager shall be available by telephone 24 hours a day, seven days per week, including holidays. The Contract Manager must be able to read, write, speak, and understand English.

3.1. Performance Location. The contractor shall perform services primarily at sites / near buildings 4885 and 4712 on Davis-Monthan Air Force Base.

3.2. Contractor Duty Hours. The contractor shall perform services during between the hours of 7:30 a.m. to 4:30 p.m., Monday through Friday. Duty hours may be adjusted, upon mutual agreement between the Government and the contractor. The contractor shall provide the names and telephone numbers of contractor employees to the Government section supervisor, in the event the contractor employees must be contacted outside of their duty hours.

3.3. Federal Holidays. The following Federal Holidays are observed at Davis-Monthan Air Force Base. The contractor is not required to perform services on federal holidays, or the actual day of observance.

3.4. Warranty Replacement. The contractor shall be responsible for honoring manufacturers’ and contract warranties and shall maintain on-site records of equipment warranties. The contractor shall provide support documentation for any warranty claims.

3.5. Listing Of Employees. The contractor shall maintain a current listing of employees. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees and non-Government owned vehicles.

3.5.1. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires;

e.g. terminated for cause, retirement, etc., and turn them in to the issuing office.

3.5.2. Traffic Laws. Contractor employees shall comply with all base traffic regulations.

Contractor employees are subject to random vehicle speed control checks. Contractor personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action.

3.5.3. Random Personnel and Vehicle Searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges, debarment from the base, or other administrative action.

3.5.4. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor owned vehicle or privately owned vehicle while on Davis-Monthan AFB.

3.5.5. For Official Use Only (FOUO). The contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

3.5.6. Reporting Requirements. Contractor personnel shall immediately report to an appropriate Government authority any information or circumstances of which they are aware may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.

3.5.7. Physical Security. Contractor employees shall be responsible for safeguarding all Government property and controlled forms provided for contractor use. At the end of each work period, all Government facilities, equipment, and materials shall be secured.

3.5.8. Controlled Areas. The contractor shall implement local base procedures for entry to Air Force controlled/restricted areas where contractor personnel will work. An AF Form 2586, Unescorted Entry Authorization Certificate, must be completed and signed by the sponsoring agency’s Security Manager before a Restricted Area Badge will be issued. Contractor employees must have a completed National Agency Check (NAC), investigation before receiving a Restricted Area Badge. Interim access can be granted in accordance with AFI 31- 501, Personnel Security Program Management.

3.5. Safety. The contractor shall comply with all applicable laws and safety regulations.

Compliance with OSHA and other applicable laws and regulations for the protection of employees is exclusively the obligation of the contractor, and the Government will assume no liability or responsibility for the contractor’s compliance or noncompliance with such responsibilities.

3.6. Work Area. The contractor shall, at all times, keep work areas free from accumulations of waste material, rubbish, tools, scaffolding, equipment, and materials. Upon completion of the job or when away from the work site, the contractor shall leave the work area and premises in a clean, neat, safe and workmanlike condition. The contractor is responsible for removal and proper disposal of all job related waste material at a permitted site.

3.7. Contractor Vehicle Requirements. All vehicles used in the performance of the contract shall be properly registered and meet local, state, and federal safety and emissions requirements.

Each contractor vehicle shall have the contractor logo neatly exhibited on each side of the cab.

Vehicles shall be in satisfactory mechanical condition and present a neat and clean appearance.

Any vehicle(s) not so maintained may be ordered removed from the base by and at the sole discretion of the CO, until satisfactory repairs are made. The contractor shall operate vehicles to comply with base traffic regulations. Vehicle-mounted communication equipment shall conform to all applicable base and federal frequency regulations and avoid conflict with existing frequencies.
