DESCRIPTION OF SERVICES.

Davis Monthan AFB Fire & Emergency Services 4690 S. Phoenix Street Tucson, AZ 85707 is requesting leak detection testing, site evaluation screening, system inoculation, sampling, on-site analysis and final report at the Fire Department Training Grounds (POL Training Pit)

CONTRACTOR BASIC REQUIREMENTS

The contractor shall provide the following services: Proposal shall include leak detection, site evaluation screening, system inoculation, sampling, on-site analysis, final report, perdiem, and labor. Additional costs/rates shall be identified for manpower and testing/re-testing, slow repair, excessive leaks, or other issues outside of contractors control. All pricing shall be in USA currency.

ITEMS:

Scope:

• Conduct testing at the aircraft burn test site Vehicle Mobilization:

• Site Setup

• Man-day Travel Rate (including perdiem)

• Fuel Charge

Testing:

• Scope and record video of monitoring well

Test primary pipeline from test pit to separator tank (0.1gph MDL)

• Locate pipeline for testing

• Install test ports at each end of pipeline

• Inoculate the pipeline with a chemical marker H, Y or A

• Install sampling ports above pipeline

• Install leak simulation port

• Inoculate the primary pipeline

• Sample and analyze

Utilizing a ground penetrating radar

• Identify the HDP liner boundaries for primary liner

• Identify the HDP liner boundaries for secondary liner

Test Primary HDP containment to the height of the drain system with two test methods Method One:

• Fill containment with Rhodamine mix water solution (water by others)

• Monitor for detection at the monitoring well (by others)

Method Two:

• Monitor the water level within the primary containment over time

• Utilizing a evaporation compensating method for liquid loss

• Perform a hydrocarbon survey around area to determine if the information could be a useful before additional testing

Install a water monitoring system at the monitoring well and record all data (monitored by others)

• Install a flow meter to measure water from pump and record

• Install water storage container to maintain all water removed from well

• Inspect all water removed from well for Rhodamine

• Monitor and record all rainfall (monitored by others)

Evaluate primary pipeline secondary penetration for leakage to soil

Contractor shall submit all documents to the Owner/Prime to include testing results and dates

Backfill material is engineered pea gravel or sand

Concrete does not exceed 12" depth

MISCELLANEOUS REQUIREMENTS

The contractor shall advise all POC’s of all findings and request permission to conduct any testing outside of scope of work and final awarded contract.

GOVERNMENT FURNISHED PROPERTY AND SERVICES

Location. Fire Department Training Grounds (contractor to be escorted) from HQ Fire Station 1 4690 S. Phoenix Street Tucson, AZ 85707 (Bldg. 4838).

Map. Lay out of POL Pit attached (approximately 100 ft. diameter pit Area), piping is approximately 10 ft. below the surface. Blue Prints available on site. Map of the base layout will be made available upon contract award.

Emergency Services. The Government will provide security forces services (call 228-4444 for emergencies only), and fire protection services (call 228-3333). There are no emergency medical services available on Davis Monthan AFB; therefore, the contractor must dial 911 for emergency medical services.

Expenses. Shall be identified on quote

GENERAL INFORMATION

Traffic Laws. Contractor personnel shall comply with all traffic regulations and are subject to random vehicle speed control checks. Contractor personnel cited for speeding on the installation may suffer loss of base driving privileges and debarment from the base. The use of cell phones is prohibited while driving on Davis Monthan AFB, unless using it hands free.

Seat belt use is mandatory for all vehicle occupants.

Random Personnel and Vehicle Searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges and debarment from the base.

Weapons, Firearms, and Ammunition. Contractor personnel are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately owned vehicle while on Davis Monthan AFB.

Reporting Requirements. Contractor personnel shall immediately report to an appropriate Government authority any information or circumstances of which they are aware may pose a threat to the security of Department of Defense personnel, contractor employees, resources, and classified or unclassified defense information.

For Official Use Only (FOUO). The contractor shall comply with DoD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for disclosure of records to the public for marking, handling, transmitting, and safeguarding FOUO material.

Safety Requirements and Reports. The contractor shall perform work in a safe manner as required by Occupational Safety and Health Act (OSHA) Safety and Health Requirements. The contractor shall provide a verbal or written report to the Contracting Officer as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. The contractor shall also provide a completed copy of an (OSHA) Accident Investigation Report to the Contracting Officer within five calendar days of each occurrence.

QUALITY CONTROL

The contractor shall develop and maintain a quality control program to ensure each demonstration is satisfactorily performed and address any deficiencies in a timely manner.
