Performance Work Statement for
Elkay Bottle Filler and Drinking Fountains
1.0. Description of Services. The contractor shall provide all supplies, materials, tools, equipment, transportation, engineering, personnel, training, supervision, and management, to provide and deliver the ELKAY bottle filler/drinking fountains to various buildings for 355th Security Forces Squadron (355 SFS), building 250, 38, 56, 165, 173, 2720, and 83, on Davis-Monthan AFB (DMAFB), AZ.
1.1. Elkay Drinking Fountain Replacements. The contractor shall replace current older drinking fountains and replace with new ELKAY drinking fountains that can support water filters to the following specifications and must ensure drinking fountains will be powered by 110V power outage at each location:
a. ELKAY (or equal) Bottle Fountain. Provide one (1) drinking fountain with full installation at each location that supports a water filter. Part # LZS8WSLK or Equal
b. ELKAY (or equal) Filter for Fountain: Water fountain must have ability to install water filter easily for users. Filter Part# 51300C or Equal
c. Water Fountain Parts Required. The fountains also require condensate pumps
115V, straight stops, and SS Lav Flex as required per location.
d. Drinking Fountain Replacement Buildings. Bldg 38 Wilmot Entry Control Point, Building 56 Craycroft Entry Control Point, Building 2720 Visitor Center, Combat Arms Range 165 and 173.
e. Drinking Fountain Color. Silver in color.
1.2. Minor Plumbing Work. Any plumbing work that will be required at each location is the responsibility of the contractor. The contractor must ensure once any plumbing work is completed the areas are left dry, clean, and any painting required will match existing wall color.
a. Any sink waste will be the responsibility of the contactor for proper disposal.
1.3. New Drinking Fountain Installation (not replacing). The contractor shall provide plumbing services and any electrical requirements to full install new ELKAY drinking fountains that can support water filters.
a. New Drinking Fountain Installation. These two (2) locations will be new installations and will require plumbing work and possibly minor electrical work depending on the feaseable location of the drinking fountain placement.
Building 250 Kennel Admin and Building 83 Swan Search.
b. ELKAY (or equal) Bottle Fountain. Provide one (1) drinking fountain with full installation at each location that supports a water filter. Part # LZS8WSLK or Equal.
c. ELKAY (or equal) Filter for Fountain: Water fountain must have ability to install water filter easily for users. Filter Part# 51300C or Equal.
d. Water Fountain Parts Required. The fountains also require condensate pumps 115V, straight stops, and SS Lav Flex as required per location.
1.4. Minor Plumbing and Electrical Work. Any plumbing and electrical work that will be required at each location is the responsibility of the contractor. The contractor must ensure once any plumbing work is completed the areas are left dry, clean, and any painting required will match existing wall color. Any electrical work that is required must ensure the power outlet installed can support 110V.
a. Any sink waste will be the responsibility of the contactor for proper disposal.
2. General Information
2.1. Work Hours. The contractor shall perform services Monday through Friday, 8:00 a.m. to 4:00 p.m., excluding federal holidays. If the contractor wants to perform services outside of these days and hours, the contractor shall obtain authorization in advance from the 355 SFS point of contact.
2.2. Warranty Response. The contractor shall provide next business day on-site service for any maintenance or repair services that are covered under warranty.
2.3. Contractor Qualifications. All contractor employees shall be certified and qualified to perform equipment training for the equipment to be procured. The contractor shall provide proof of employee certification/qualification within one (1) business day upon the Government’s request.
2.4. Contracting Officer Authority. The Contracting Officer is the only Government official that can make any changes to the contract. If the contractor is asked to make changes or deviate from the contract, the contractor shall not make the changes and notify the Contracting Officer immediately.
2.5. Contractor Travel on DMAFB. Contractor employees shall not loiter in any working area before or after performing services. All contractor employees shall limit their travel on DMAFB only to the specific areas required for performance of services or in travel directly to and from these locations. Upon completion of contract work, contractor employees shall promptly depart DMAFB.
2.6. Contractor Employee Conduct. The Government reserves the right to require removal from the installation any contractor employee who endangers persons or property, whose actions are inconsistent with professional conduct, whose continued employment is inconsistent with the interest of military security or who is found to be incapacitated or under the influence of alcohol, drugs, or other substances. Removal of employees for any reason does not relieve the contractor of the requirement to perform services specified herein.
2.7. Appearance. Employees shall present a neat appearance and be easily recognized as contractor employees by company uniforms, or name tags having the company logo affixed.
All contractor employees must have in their possession at all times, Company ID badges and any Government Base access passes issued to them.
2.8. Safety. The contractor shall comply with all local, state and federal laws and safety regulations. Compliance with OSHA and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor, and the Government will assume no liability or responsibility for the contractor’s compliance or noncompliance with such responsibilities. The contractor is responsible for furnishing contractor employees with all necessary personal protective equipment to perform services.
2.9. Work Areas. The contractor shall, at all times, keep work areas free from accumulations of waste material, rubbish, tools, equipment, and materials. Upon completion of the job or when away from the work site, the contractor shall leave the work area and premises in a clean, neat, safe and workmanlike condition.
2.10. Trash/Waste Removal. The contractor shall remove and dispose all trash and waste materials at a permitted site. The contractor shall not dispose of any trash or waste on DMAFB.
2.11. Smoking in AF Facilities. Contractors are advised that the AF has placed restrictions on the smoking of tobacco products in AF facilities. AFI 40-102, Tobacco Use in the Air Force, outlines the procedures used by the commander to control smoking in our facilities. Contractor employees and visitors are subject to the same restrictions as Government personnel. Smoking is permitted only in designated smoking areas.
2.12. Illegal Substances. Illegal substance possession and/or use will not be tolerated on any military installation, including DMAFB. This includes the use, possession, distribution, or purchasing of marijuana, even if it has been medically prescribed. The contractor shall ensure that contractor employees who have base access do not possess and/or use illegal substances. All contractors and subcontractors are required to adhere to all local, state and federal laws concerning possession and/or use of illegal substances. Contractors found to be in possession of illegal substances will be barred from the installation, and subject to arrest and prosecution by local authorities.
2.13. Federal Holidays. The following federal holidays are observed at Davis-Monthan AFB. The contractor is not required to perform services on federal holidays, or the day set-aside for observance or designated DMAFB Family days.

3. Security Requirements
3.1. List of Employees. Before installation begins, (minimum three weeks prior) the contractor shall submit to the 355th Security Forces point of contact, a list of contractor employee names, social security numbers, and driver’s license numbers and state of issue, or other means of positive identification of all employees performing services on DMAFB if they don’t already have the proper contractor credentials for base access. Foremen, supervisors, and superintendents must be identified on the list of employees. The contractor shall submit an updated list when an employee's status or information changes. The name of any contractor employee that is terminated during the period of the contract must be reported to the Government point of contact in writing within one workday of the employee’s termination.
3.2. Safeguarding of Classified and Other Information. The contractor shall ensure that contractor employees do not notify, repeat, comment upon, disclose, or use in any way information seen, observed, or derived from documents or conversations of a sensitive or confidential nature overheard by them. The contractor shall comply with, and assume responsibility for compliance of this provision by its employees.
3.3. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance on DMAFB are obtained for employees and non- Government owned vehicles.
3.4. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires; e.g. terminated for cause, retirement, etc, or upon contract completion.
The contractor shall notify the 355th Security Forces point of contact in writing when an employee departs, and return the employee’s identification to the office that issued it.
3.5. Traffic Laws. Contractor employees shall comply with all Davis-Monthan AFB traffic regulations and state traffic laws. Contractor employees are subject to random vehicle speed control checks. Failure to adhere to base traffic regulations may result in the loss of base driving privileges, debarment from the base, or other administrative action. The use of cell phones is strictly prohibited while driving on Davis-Monthan AFB, unless the phone is hands free. Seat belt use is mandatory for all drivers and vehicle passengers.
3.6. Random Personnel and Vehicle Searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base. Denial of entry to contractor personnel who refuse to be searched, or detention of personnel found to be in possession of contraband, may result in loss of base driving privileges, debarment from the base, or other administrative action.
Detention of contractor employees for any reason does not relieve the contractor of the requirement to perform contract services.
3.7. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor owned vehicle or privately owned vehicle while on Davis-Monthan AFB.
3.8. Reporting Requirements. Contractor employees shall immediately report to an appropriate DMAFB authority any information or circumstances of which they are aware that may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.
3.9. Physical Security. Contractor employees are responsible for safeguarding all Government property and controlled forms provided for the contractor’s use. At the end of each work period, the contractor shall secure all DMAFB facilities, equipment, and materials.
4.0. GOVERNMENT PROVIDED PROPERTY AND SERVICES
4.1. Utilities. A source of electricity will be available for the contractor’s use.
4.2. Emergency Services. The Government will provide security forces services (call 228- 4444, for emergencies only), and fire protection services (call 228-3333). There are no emergency medical services available on Davis-Monthan AFB, therefore the contractor must dial 911 for emergency medical services.
