USAF PROJECT:
LED Light Modernization
Finance Building 7328 Davis Monthan AFB
Tucson, AZ
DATE: 19 September 2017
Project includes, but is not limited to, the following work elements. All plant, labor, equipment, materials, services, and incidentals shall be furnished by the Contractor. All work must be in accordance with Unified Facilities Criteria 3-530-01 (www.wbdg.org) unless express written permission is received from the Contracting Officer for any deviations. All work must meet or exceed acceptable standards and codes of the building trades; and shall be installed in a neat, substantial, and workmanlike manner.
1. Scope of Work. Contractor shall remove 135 each 2 X 4 recessed troffer light fixtures, and supply and install 135 LED 2 X 4 recessed troffer fixtures, 9 of which shall include emergency battery back-up function. Contractor shall remove 19 each 1 X 4 recessed troffer light fixtures, and supply and install 19 LED 1 X 4 light fixtures. Contractor shall remove 15 exterior recessed downlights (can lights), and supply and install 15, exterior use, LED recessed downlights. Install all new LED recessed troffer lights and recessed downlights in accordance with the specifications listed in section 1 of the Scope of Work.

2. Execution.
2.1 Construction Delays. If for any reason the contractor is unable to perform above work on the scheduled project, the contractor shall immediately notify 355 Contracting Squadron who then will notify the 309 Aerospace Maintenance and Regeneration Group Facilities Flight Office, in writing, and provide the reason why the work cannot be made (e.g. awaiting parts), and an estimated date that work shall be resumed.
2.2 Schedule of Construction. The contractor shall develop, implement, and maintain a schedule of services for all projects. We expect this project to be accomplished within in 75 days from time of notice to proceed. The contractor shall submit a copy of the schedule to the Contracting Officer and AMARG Facilities Flight Office for review, no later than ten days prior to commencement of work, Page 3 of and submit an updated schedule when changes occur.
3. GOVERNMENT PROVIDED PROPERTY AND SERVICES.
3.1. Government Provided Property. None.
3.2. Government Provided Facilities. None.
3.3. Government Provided Utilities. The government will provide the contractor with a source of electricity.
3.4. Government Provided Services. The government shall provide the contractor with escorts within the restricted area surrounding AMARG.
3.5. Emergency Services. The Government will provide security forces services (call 228-4444, for emergencies only), and fire protection services (call 228-3333). There are no emergency medical services available on Davis-Monthan AFB, therefore the contractor must dial 911 for emergency medical services.
4 GENERAL INFORMATION
4.1. On-Site Forman. The contractor shall provide an on-site foreman who shall be responsible for contract performance and can act for the contractor on all matters relating to daily operation of the contract. The contractor shall submit the written name and phone number of the on-site foreman to the CO and AMARG Facilities Flight Office.
4.2. Working Hours. The contractor shall perform services between the hours of 5:30 a.m. and 4:30 p.m. Monday through Friday, excluding Saturdays, Sundays and Federal Holidays. If the contractor desires to work after 4:30 p.m., weekends, or during Federal Holidays, the contractor must submit the request to the host unit. Each request will be considered on a case-by-case basis.
Host unit operations sometimes require no contractors are allowed access to the site. The host unit will make every effort to provide the contractor a forecast of time spans where access will not be permitted. Additionally, although rare, operations can dictate that the contractor be requested to cease work immediately and vacate the job site.
4.3. Federal Holidays. The following Federal Holidays are observed by the host unit: New Year’s Day, Martin Luther King's Birthday, Presidents’ Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans’ Day, Thanksgiving Day, Christmas Day
4.4. Smoking in Air Force Facilities. Contractors are advised that the Air Force has placed restrictions on the smoking of tobacco products in AF facilities. AFI 40-102, Tobacco Use in the Air Force, outlines the procedures used by the commander to control smoking in our facilities.
Contractor employees and visitors are subject to the same restrictions as Government personnel.
Smoking is permitted only in designated smoking areas.

5 SECURITY
5.1. Security Requirements. The construction site is in a restricted area and will require the Contractor and subcontractors to have the requisite badging and identification to access this project site. The Contractor shall consider in the contract documents (e.g., the construction cost estimate) the reasonable costs and effects of daily, short delays in accessing the project site, as required by the restrictive access requirements or by local alert conditions.
5.2. Security Training. All contractor employees shall receive security education training from the host unit security manager. Training must be conducted in accordance with DoD 5200.1-R, Information Security Program, and AFI 31-401, Information Security Program Management.
Contractor personnel who work in Air Force controlled/restricted areas must be trained IAW AFI 31- 101, The Air Force Installation Security Program.
5.3. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees and non-government owned vehicles.
5.4. Traffic Laws. Contractor employees shall comply with all Davis-Monthan AFB traffic regulations and state traffic laws. Contractor employees are subject to random vehicle speed control checks. Failure to adhere to base traffic regulations may result in the loss of base driving privileges, debarment from the base, or other administrative action. The use of cell phones is strictly prohibited while driving on Davis-Monthan AFB, unless the phone is hands free. Seat belt use is mandatory for all drivers and vehicle passengers.
5.5. Random Personnel and Vehicle Searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges, debarment from the base, or other administrative action.
5.6. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor owned vehicle or privately owned vehicle while on Davis-Monthan AFB.
5.7. For Official Use Only (FOUO). The contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.
5.8. Reporting Requirements. Contractor personnel shall immediately report to an appropriate government authority any information or circumstances of which they are aware may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.
5.9. Physical Security. The Contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

5.10. Base Entry Requirements. The contractor shall obtain Government identification documents for all employees, by submitting an application to the customer. The contractor must provide the full name, social security account number (SSAN), driver’s license number (including state), and hours of required access, for each contractor employee. Any employee who does not have a driver’s license must provide another official form of picture ID, such as INS work permit or an Arizona ID (available through the Arizona Department of Transportation). Any employee desiring to bring a privately owned vehicle on Davis-Monthan AFB must provide proof of automobile registration and at least the minimum required liability insurance, as well as a valid driver’s license. Contractor ID cards or temporary base pass will be available for pick up within seven working days after application is received by the customer.
5.11. Contractor Vehicle Requirements. All vehicles used in the performance of the contract shall be properly registered and meet local, state, and federal safety and emissions requirements.
Each contractor vehicle shall have the Contractor Logo neatly exhibited on each side of the cab.
The sign must use at least 2" high letters. Vehicles shall be in satisfactory mechanical condition and present a neat and clean appearance. Any vehicle(s) not so maintained may be ordered removed from the base by and at the sole discretion of the CO, until satisfactory repairs are made. The contractor shall operate vehicles to comply with base traffic regulations. Vehicle-mounted communication equipment shall conform to all applicable base and federal frequency regulations and avoid conflict with existing frequencies.
6. Work Area and Finishes. The contractor shall, at all times, keep work areas free from accumulations of waste material, rubbish, tools, scaffolding, equipment, and materials. Upon completion of the job or when away from the work site, the contractor shall leave the work area and premises in a clean, neat, safe and workmanlike condition. The contractor is responsible for removal and proper disposal of all job related waste material at a permitted site. Cleanup site and haul off-base all construction debris from the site daily.
6.1. Access to the areas adjacent to the work site shall be maintained at all times. Work site shall remain secure after work hours.
6.2. No asbestos-containing materials may be used or installed in this work
7. Safety. The contractor shall comply with all applicable laws and safety regulations. Compliance with OSHA and other applicable laws and regulations for the protection of employees is exclusively the obligation of the contractor, and the government will assume no liability or responsibility for the contractor’s compliance or noncompliance with such responsibilities.
8. Environmental Control.
a. The Contractor shall be required to meet or exceed the requirements and compliances of the most current 355 CES/CEIE, Davis-Monthan Contractor Environmental Guide in effect at the time of project award. To access the Davis-Monthan Contractor Environmental Guide:

b. Additionally, the Contractor shall be held responsible for complying with all federal, state, and local environmental regulations included as part of the processes in performing the work included in this project.
c. Hazardous material (HAZMAT) and/or regulated wastes may be encountered during construction. Contractor shall plan and submit a HAZMAT worksheet to the gov’ts contract representative prior to construction - see Appendix A and Attachment(s) A-1 to A-3 of the Environmental Guide. Contractor shall provide the Contracting Officer with a chemical inventory and an estimated quantity to be used, prior to bringing hazardous materials on the base. The chemical inventory and appropriate SDS or MSDS’s shall be provided as indicated in Appendix A of the Contractor Environmental Guide. These requirements shall be included as a submittal on the AF Form 66, Schedule of Material Submittals.
d. Obtain permits from the Pima County Department of Environmental Quality (PDEQ), Air
Quality Division, for installation of new equipment or use of Contractor equipment which generate air pollution, for earth-moving activities related to new construction, and for asbestos removal or demolition. These permits shall be included as a submittal on the AF Form 66, Schedule of Material Submittals. Submit copies of permits to the Contracting Officer prior to beginning construction. All permit costs shall be considered as having been included as a factor in the Contractor's bid price.
e. The Contractor shall also submit a Construction and Demolition (C&D) Plan (Attach B-1, Contractor Environmental Guide) 3 weeks prior to starting construction. If the C&D Plan indicates potential regulated wastes or hazardous wastes, the contractor shall coordinate any disposal requirements through the contractor’s representative prior to disposal or manifesting
– see Appendix C of the Contractor Environmental Guide. For projects involving asbestos abatement, complete the standard "Notification of Demolition or Renovation" forms and submit to PDEQ and other required agencies. These forms shall be included as a submittal on the AF Form 66, Schedule of Material Submittals. Submit copies of the forms to the Contracting Officer prior to beginning construction.
f. Contractors shall comply with all regulatory requirements/considerations if the project requires soil disturbance. These requirements may include Storm Water Pollution Prevention Plans (SWPPP), Air Activity Permits (see para d above), and Natural and Cultural Resource protections. Area disturbance requiring permits is set forth by Arizona Department of Environmental Quality (ADEQ) and PDEQ. Treat areas subject to dust producing activities with liquid palliatives which will not harm regrowth of vegetation, or other methods of dust
Page 9 of control. Also, ensure methods to protect native plants, birds and wildlife are included in projects where required.
g. No open burning of trash, brush, or refuse will be permitted.
h. The Contractor shall be responsible for the proper management of both hazardous and nonhazardous waste and the required submittals IAW Section 3 and Appendix C of the Contractor Environmental Guide. The Contractor shall segregate hazardous or regulated materials and non-hazardous or non-regulated materials during demolition; and handle, transport, store, and dispose of these hazardous and non-hazardous materials in accordance with applicable federal, state, and local laws, ordinances, or other rulings having the effect of law, including but not limited to the items listed below. In addition, the Contractor shall obtain proper disposal manifests for both hazardous and regulated materials, and for non-hazardous or non-regulated materials, and submit these manifests to the Contracting Officer. These manifests shall be included as a submittal on the AF Form 66, Schedule of material Submittals. Submit original manifest-with original signature of the Contractors superintendent or authorized representative in block 14 attesting to the validity of the manifest-to the Contracting Officer within 3 calendar days of disposal. For wastes regulated as PCB’s or RCRA, the Contractor must use DM’s Environmental Management Office at the time of shipment. All handling, transporting and disposal costs shall be considered as having been included as a factor in the Contractor’s bid price. Any Questions regarding proper handling or disposal of hazardous or non-hazardous materials may be directed through the Contracting Officer to the Installation Management Flight at 520-228-4840.

i. All radioactive material or equipment using radioactive material must be approved for use prior to bringing it on the base. The written request for approval, along with a copy of the permit for the source and a brief description of the intended activities, shall be submitted to the base Bioenvironmental Engineering flight through the Contracting Officer 30 calendar days prior to the Contractors use. No work involving radioactive materials may proceed until receipt of the approved request. This requirement shall be included as a submittal on the AF Form 66 Schedule of Materials Submittals.
j. Any items containing radioactive material, especially radio-luminescent exit signs, discovered while performing maintenance, repair, alteration or renovation activities, shall be turned in by the Contractor through the Contracting Officer to 355 CES/CEO for proper disposal.
k. Liquid paints or coatings with lead shall be restrictive to no more than 0.06 percent lead by weight of the non-volatile solids for use in or on all base facilities [Part 35 of 24 CFR]
