Question 1:
May we bid on some of the disciplines? Or, must we bid on all?
Response 1:
Offerors must bid on ALL as outlined in Addendum to 52.212-1, L.9. Part IV Price ..."The pricing information provided in the offer shall be complete. Complete means that the offeror proposed a Firm-Fixed Price for ALL item numbers on the Position Description Pricing Worksheet, Attachment 4 and ALL items on the Overtime Pricing Worksheet, Attachment 5 for the base period, the four (4) option periods, and the six (6) month extension."...
Question 2a:
Per this FOIA request, please advise if there is an incumbent contractor (s), if this is an existing contract as such or one to which you are adding positions, what incumbent staff are currently on-board by position, what vacancies you have by position, what pending vacancies you are aware of by position, names and contact information for all current and recent (within 90 days) healthcare personnel working on this contract, off-duty contact numbers for all and former incumbent staff members, and facility contact numbers that we might use to set up off work hour conversations. Also, please share what fully loaded hourly rates the government currently is paying by labor category, and to what those positions conform to by corresponding labor category level in the prevailing WD (e.g., Occ Health Nurses conforms to an RNII position, rather than just Registered Nurse). Thank you in advance for your assistance.

If I simply asked these questions without them being a FOIA request, would I have fared any better?
Response 2b:
No, most of the information you are requesting is FOIA information that may or may not be releasable.
In this case, I can answer there is an incumbent contractor, who upon a FOIA request will have to be notified and give consent for releasing some of the information you are requesting. In addition some of your questions are not solicitation questions. Our office does not maintain contract employee’s private information nor do we have a facility contact number for employees. Incumbent information is only passed on to contractor who is awarded the contract.

Question 3:
In order to move forward with our teaming arrangement for the above referenced RFP, please respond to the following question regarding section M3.1.1.1. which mentions "formal written documentation that a teaming/subcontracting arrangement was established with one (1) firm."
Is there a limit to the number of subcontractors an offeror may team with?

Reference: Pargarph L7.2 Technical Organization, pg 91 states: "The offeror’s name, logo, or any other identifiable reference to the offeror shall be provided ONLY on the cover page (L6.1.)."
Does this statement only apply to Part II and Part Technical or to the entire proposal, Parts I - V?
Response 5b: Yes, applies to entire proposal. See L5.7 Binding and Labeling, …“A cover sheet shall be used in each binder, clearly marked with the part, title, copy number, solicitation number, and the offeror’s name. The same information shall be placed on the spine of each binder.”
Question 5c:
Paragraph L.7.3 Quality Control Plan, pg 91 Request the Quality Control Plan be excluded from the page count to ensure adequate space and coverage of the plan and its details.

This will not be excluded, falls under Technical with a 30 page limit.

Paragraph L8 - Part II Past Performance, pg 92 states:
"When addressing the past performance evaluation criteria in Section M3.2 of the solicitation, the offeror shall provide past performance data on at least three (3) but not more than seven (7) of the most recent and relevant contracts performed for Federal, State, Local Government and private industry within the past three (3) years from the issue date of this solicitation.”
However, paragraph 8.3.1 Past Perforamnce Questionnaires (Attachment 6), pg 93 states:
" The offeror is to have no more than five (5) past performance questionnaires submitted" Please clarify whether we are to submit up to 7 or up to 5 past performances.
Response 5e:
Section L8 and paragraph L8.3.1 are two separate conditions (past performance data/past performance questionnaire). The past performance data is submitted by the contractor. The contractor must provide information on at least three (3) contracts, but no more than seven (7). The questionnaires, (L8.3.1, Attachment 6) are submitted by the contractor’s customers and cannot have more than (5) five submitted.

Paragraph L8. - Part II Past Performance, pg 92 states: "Each offeror shall submit past performance that contains resent and relevant past performance information. Past performance information is required on the offeror and all teaming partners, subcontractors, and on joint venture partners who are proposed to perform on this requirement."
Are past performances required from each prime, subcontractor or joint venture partner or is the total number of past performances a combination of all of the above?
Response 5f:
Yes, a combination of all of the above.
Question 5g:
Reference: Attachment 4 and Attachment 5, Attachment 6 pg 97 – 124 Can the Government provide an excel sheet or MS Word document for these attachments? The only version currently available is as a PDF within the overall RFP making it difficult to extract the information and keep it in the same required formats.

Reference: Paragraph L.9.1 Price Proposal Information, pg 94 states: "All information pertaining to the proposed price, including all worksheets and supporting documentation must be included in the section of the proposal designated as the Price part.”
Please clarify/specify what is meant by "supporting documentation".
Response 5h:
L.9.1 also states in second paragraph: “The offeror shall only submit price information as instructed, unless the offeror considers it essential to document or support price position.” Supporting documentation is if an offeror considers it essential to substantiate a portion of the proposal, if it is not essential then it shouldn’t be included.
Question 6a:
Page 92-93, Section L, Paragraphs L8.3 and L8.3.1
Paragraph L8.3.1 states that “The offeror is to have no more than five (5) past performance questionnaires submitted to 355th Contracting Squadron by their references.” This appears to be in discrepancy with paragraph L8.3 which states that “The offeror shall provide past performance data on at least three (3) but not more than seven (7) of the most recent and relevant contracts.” Assuming that the offeror is expected to send a Past Performance Questionnaire to each customer used as a reference, please clarify the maximum number of past performance submissions allowed.

These paragraphs state that “The offeror’s name, logo, or any other identifiable reference to the offeror shall be provided ONLY on the cover page (L6.1.).” Please clarify that the intent is to receive a sanitized copy of Parts II, III & IV. Does the Government consider the use of CAGE codes instead of the names of the offeror, teaming partners, and any other contractors in Parts II, III and IV to be identifiable information?
Response 6b:
Yes the intent is to have a sanitized copy for evaluation purposes. See L5.7 Binding and Labeling, …“A cover sheet shall be used in each binder, clearly marked with the part, title, copy number, solicitation number, and the offeror’s name. The same information shall be placed on the spine of each binder.” Yes, the main purpose of a cage code is to identify a business, so yes, it is considered identifiable information.

Page 93, Section L, Paragraph L8.1 & L8.3
May the offeror include the use of information that identifies teaming partners and other individuals not affiliated with the offeror? Also, does the Government require that the Subcontractor Consent Letter (L8.1) not contain any information that could identify the offeror, the subcontractor or any other teaming partners?
Response 6c:
The content of the offerors proposal is at the discretion of the contractor.
Yes, the letter requires the offeror to identify the arrangement and needs to identify the business arrangements.

Regarding Part III, please confirm that the Subcontractor Authorization Letter(s), Commercial Customer Authorization Letters, and list of references requested by the offeror to provide a past performance questionnaire do not count against the 20 page limit.

No, these are not included in the 20 page limit.
Question 6e:
Page 137, Section SI, Paragraph SI.4
The paragraph states “…pursuant to DODI 6025.5, date 6 January 1995, para. 4.7., the total compensation paid to an individual in any year under a personal service contract cannot exceed the full time equivalent rate of $200,000 dollars each year.” Yet, page 69, FAR 52.237-7 (a) states, “It is expressly agreed and understood that this is a nonpersonal services contract...” Can you please clarify these seemingly conflicting paragraphs?
Response 6e:
This effort will result in both a personal and nonpersonal IDIQ Contract. Positions will be identified as personal or nonpersonal in subsequent task orders when awarded.
Question 6f:
Who is the incumbent contractor? If possible, could you provide previous rates for that contract?
Response 6f: There is an incumbent contractor, Kuhana Associates, LLC which is information already publized. Previous rate information or other contract specifics must be requested through a FOIA request and may or may not be releasable information per the contractor.

ADDENDUM TO 52.212-1, INSTRUCTIONS TO OFFERORS -- COMMERCIAL ITEMS
L1. Objective
The resulting contract from this acquisition will be an IDIQ Award Contract. The Government will utilize Lowest Price Technically Acceptable (LPTA) source selection procedures to conduct a 100% 8(a) certified small business set-aside competition.
L2. Government Point of Contact
The Government individuals listed below are the primary and alternate points of contact for this acquisition.
Address all questions or concerns to both individuals. Written requests for clarifications may be sent to both individuals via fax or email. Email is the preferred method of communication. The Government points of contacts are as follows:

L3. Formal Communications
Offerors who determine the requirements of this solicitation require clarification(s) in order to enable submittal of a responsive proposal are requested to send all questions clearly stated with specific page, section, paragraph in writing within 10 business days from the solicitation issue date in block 6 of the SF 1449. Questions should include sufficient detail to enable the Government to respond without requesting further clarification. These questions shall be directed to the Contract Administrator, Ms. Roxanne Elfering, and either sent by mail to the address located in Block 9 of the SF 1449 or via email to roxanne.elfering@us.af.mil. Answers will be compiled and posted electronically to the Federal Business Opportunities website: https://www.fbo.gov.

L4. Funding Information
The maximum ceiling value is $48 million over a five (5) year ordering period. This contract is an IDIQ ordering vehicle only and does not guarantee that any funds above the minimum contract award amount of $100,000 will be awarded. Budget constraints may determine future task orders and task order option periods.
Funds are not presently available for this effort. No task orders will be awarded under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date.
In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs.
L5. General Instructions
To assure timely and equitable evaluation of the proposal, the offeror must follow the instructions contained herein.
The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation.
L5.1. General Guidance
This section provides general guidance for preparing proposals as well as specific instructions on the format and content of the proposal. Offeror’s are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those items identified as evaluation factors. Offeror’s must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.
The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of stated claims. The proposal should not simply rephrase or restate the Government’s requirements, but rather, shall provide convincing rationale to address how the offeror intends to meet these requirements.
Offerors shall assume that the Government has no prior knowledge of their facilities and capabilities and will base the evaluations on the information presented in the offeror’s proposal.
Elaborate brochures or documentation, binding, detailed art work, or other embellishments are unnecessary and are not desired or required.
The offeror shall establish the acceptance period for a minimum of 120 calendar days after the proposal due date specified in Block 8. The offeror shall make a clear statement in the proposal that the proposal is valid through this date.
In accordance with FAR Subpart 4.8 (Government Contract Files), the Government will retain one copy of all unsuccessful proposals. Unless the offeror requests otherwise, the Government will destroy all other copies of unsuccessful proposals. No destruction certificate will be issued to the offeror.
The offeror shall complete blocks 12, 17a, and 30a, b, and c of the Standard Form 1449. In doing so, the offeror accedes to the contract terms and conditions as written in the solicitation and all amendments, with attachments. The solicitation once awarded constitutes the contract.
L5.2. Format for proposal Part I-V
The offeror shall prepare the proposal in accordance with Table L1 – Proposal Order and Organization. Proposals shall be sectioned into five (5) separate parts with dividers and marked accordingly; Part I - Executive Summary, Part II - Technical, Part III - Past Performance and Part IV- Price, Part V - Contract Documentation.

L5.3. Page and Format
The page limitations established shall be treated as maximums. Pages exceeding the limitation set for in Table L1 – Proposal Order and Organization will not be read or evaluated and will be removed from the proposal. Each page shall be counted towards the maximum limit except the following: Cover page, Table of Contents, Tabs, Glossaries, Dividers, Blank pages, and Past Performance Questionnaires.
The proposals will be printed on 8 ½” x 11” paper, except for fold-outs used for charts, tables, or diagrams, which may not exceed 11” x 17” sheets. A page is defined as one face of a sheet of paper (white or ivory in color) containing information. (Charts, tables, or diagrams, shall be printed on 8 ½” x 11” sheets only), and each face will be counted as one page. This includes all appendices, charts, graphs, diagrams, tables, photographs, etc. Typed data shall not be less than 12 points using Microsoft Word, Times New Roman font. Pages shall be numbered sequentially by using Part and Page (e.g., Part I, Page 1).
L5.4. Pricing Information
Pricing information shall only be addressed in the Price Proposal Part.
L5.5. Table of Contents
Each part should contain a table of contents to outline the subparagraphs within that part. Tabs shall be used as breaks between each section.
L5.6. Glossary of Abbreviations and Acronyms
Part I shall contain a glossary of all abbreviations and acronyms for the entire proposal that explains each abbreviation and acronym.
L5.7. Binding and Labeling
Each copy of the four (4) hardcopies for each part of the proposal shall be bound separately in a three-ring, loose leaf binder allowing each part to lie flat when opened. Staples shall not be used. A cover sheet shall be used in each binder, clearly marked with the part, title, copy number, solicitation number, and the offeror’s name. The same information shall be placed on the spine of each binder.
L5.8. Marking of Contractor Proposal
Each hardcopy of the proposal shall be marked appropriately in accordance with FAR 52.215-1(e), Restriction on Disclosure and Use of Data, and FAR 3.104-4, Disclosure, Protection, and Marking of Contractor Bid and Proposal Information and Source Selection Information. One hardcopy shall be clearly marked as the “Original”.
L5.9. Electronic Proposals
Proposals submitted electronically ONLY will not be considered. An electronic disk may accompany the four (4) hardcopy proposals.
L5.10. Proposal Submission
The original proposal and copies as specified in this document shall be mailed or hand carried to the issuing office listed in Block 9 of Standard Form (SF) 1449 on or before the deadline specified in Block 8 of SF 1449. Please mark all packages as follows:

The sealed envelope or package used to submit your proposal must show the time and date specified for receipt, the Solicitation Number, and the name and address of the offeror.
Offerors are cautioned that this is a secure facility, so appropriate arrangements are required to gain access to the facility with enough time to meet the proposal deadline. The United States Air Force has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. DELAY SHOULD BE ANTICIPATED WHEN HAND CARRYING PROPOSALS. Offerors should allow sufficient time to obtain a visitor pass and arrive at the 355th Contracting Squadron PRIOR to the deadline. Offerors are cautioned to strictly comply with all instructions within this solicitation to ensure submission of a complete proposal. Failure to furnish a complete proposal at the time of proposal submission may result in the proposal being considered non-responsive to the Government and eliminated from consideration for award. Late proposals will be processed in accordance with
FAR 52.212-1(f) “Late submission, modifications, revisions, and withdrawals of offers.” If the proposal is received late and not considered the Government will destroy all late proposals. No destruction certificate will be issued to the offeror.
L5.11. Debriefings
The Contracting Officer will promptly notify offerors of any decision to exclude them from the competitive range;
so the offeror may request and receive a debriefing in accordance with FAR 15.505. Offerors excluded from the competitive range, or determined to be “Unacceptable” during the evaluation process, may request a pre-award debriefing or they may choose to wait after the source selection decision to request a post-award debriefing.
Offerors are entitled to no more than one (1) briefing.
L5.12. Discrepancies
If an offeror believes the requirement in these instructions contain an error, omission, or are otherwise unsound, the offeror must immediately notify the Contracting Officer in writing with supporting rationale as well as the remedies the offeror is asking the Contracting Officer to consider. The Government reserves the right to award this effort based on the initial proposal, without discussion.

L5.14. Oral Presentations
Oral presentation will not be considered for this requirement.
L5.15. Competitive Range Determination
Based on the rating of each “Acceptable” proposal against the evaluation criteria, the Source Selection Authority may establish a competitive range for purposes of efficiency. Offerors are advised that only proposals determined to have a reasonable chance for contract award will be included in the competitive range.
L6. Part I – Executive Summary
The executive summary part shall consist of the following:
L6.1. Cover Page
Titled with the solicitation number and Clinical Health Services. Include the offeror’s name, logo, and contact information.
L6.2. Master Table of Contents (Tab 1)
Include a master table of contents for the entire proposal.
L6.3. Narrative Summary (Tab 2)
The offeror should provide a narrative summary that is concise, providing information on any key or unique feature, excluding price. The narrative should also address any significant risks. The material presented in the narrative summary shall not be considered for meeting the requirements for other required parts of the proposal.

Include a glossary of ALL abbreviations and acronyms used throughout the entire proposal.
L7. Part II Technical
The purpose of this section is to allow the offeror to set forth in the most comprehensive manner its understanding of the Clinical Healthcare Services requirements and to demonstrate its ability to meet the evaluation requirements of Section M, Evaluation Factors for Award. The technical information provided in the offer shall be specific and complete. Legibility, clarity, and coherence are very important. The offeror’s responses will be evaluated against the Technical evaluation criteria established in Section M, Evaluation Factors for Award. The offeror shall use the instructions provided below to provide the methodology the offeror would use to satisfying the factors and subfactors. Meeting all the requirements in the solicitation is mandatory. The offeror is representing, by the proposal submission, that the offeror will perform all the requirements of the solicitation. The offeror should not merely reiterate the requirements specified in the solicitation, but instead should describe how the offeror plans to accomplish each of the requirements.
L7.1. Technical Evaluation Factors and Subfactors
Again, to facilitate the evaluation, the technical portion of the proposal should be specific, detailed, and complete to clearly and fully demonstrate that the offeror has a thorough understanding of the requirements for the accomplishment of the effort. The offeror shall describe the performance standards that have been established in each area and explain how performance standards will be met or exceeded. This section shall not contain any pricing data. The technical factors and subfactors consist of the following:
L7.1.1. Factor 1- Management Approach.
The offeror shall provide their approach to meeting the Government’s requirements to properly complete the Clinical Healthcare Services requirement.
L7.1.1.1 Subfactor 1- Management Plan.
L7.1.1.2 Subfactor 2- Corporate Experience. The offeror shall possess all the required core capabilities and qualifications to fulfill the requirement.
L7.1.1.3 Subfactor 3- Quality Control Plan.
L7.1.2 Factor 2- Recruitment / Retention / Transition Plan.
L7.2. Technical Organization
All information that the offeror deems appropriate to address for this factor and its subfactors must be included in the proposal in an orderly format. The technical part shall address all the technical factors and subfactors. The offeror’s name, logo, or any other identifiable reference to the offeror shall be provided ONLY on the cover page (L6.1.). The technical part shall consist of the following:
L7.2.1. Table of Contents (Tab 1)
Include a table of contents for this part.
L7.2.2. Management Plan (Tab 2)
The offeror shall provide a management plan of the offeror’s overall objectives, methodology, and planned procedures to fulfill the requirement.
L7.2.2.1. Teaming / Subcontracting Documentation (Tab 2a)
The offeror shall submit written documentation of teaming / subcontracting arrangements if provided under this contract.
L7.2.3. Quality Control Plan (Tab 3)
The offeror shall provide a Quality Control Plan of the offeror’s procedures to control quality output and to ensure standards of medical care.
L7.2.4. Vacation / Leave Plan (Tab 4)
The offeror shall provide a plan of the offeror’s strategy to effectively manage healthcare workers absences to ensure contract coverage and to monitor billable hours, as not to exceed the allotted hours on the contract.
L7.2.5. Recruitment / Retention / Transition Plan (Tab 5)
The offeror shall provide a plan of the offeror’s practices for attracting, developing, retaining and transitioning a quality workforce capable of fulfilling the requirement.
L8. Part III Past Performance
Each offeror shall submit past performance that contains recent and relevant past performance information. Past performance information is required on the offeror and all teaming partners, subcontractors, and on joint venture partners who are proposed to perform on this requirement.
The government will evaluate the quality and extent of offeror's performance deemed recent and relevant to the requirements of this solicitation. Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) which demonstrate customer satisfaction with overall job performance and quality of service for relevant past efforts/contracts (similar in scope and magnitude of effort and complexities that this solicitation requires). In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods, customer complaints, etc.
The Government will use information submitted by the offeror and any other sources of information available to the Government to evaluate past performance. When addressing the past performance evaluation criteria in Section M3.2 of the solicitation, the offeror shall provide past performance data on at least three (3) but not more than seven
(7) of the most recent and relevant contracts performed for Federal, State, Local Government and private industry within the past three (3) years from the issue date of this solicitation. There must have been a minimum of a 6 month period of performance. Only references for relevant contracts are desired. Furnish the following information for each contract listed:
- Company/Division name
- Services
- Contracting Agency/Customer
- Contract Number or comparable identifier
- Contract Dollar Value and period of performance
- Verified, current name, address, email address, & telephone number for each company point of contact and reference.
- Brief comments, if desired (Should offerors wish to provide information on problems encountered on the identified contracts and their corrective actions in accordance with FAR 15.305(2), they shall submit such information at the same time references are submitted.)
- If subcontractor, describe portion of the work performed.
Note: Do not leave any of the above information blank. If not applicable please state, “Not Applicable”. If the information is available through PPIRS the offeror can advise of contract number.
L8.1. Subcontractor Consent
Past performance information pertaining to any teaming partner, subcontractor, or joint venture cannot be disclosed to the primary offeror without the subcontractor’s consent. Provide with the proposal a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor. The offeror shall also provide a commercial customer authorization letter, which allows release of information to the Government. Past performance information pertaining to a teaming partner or subcontractor cannot be disclosed without the subcontractor’s consent.
If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past relevant and recent contracts.
L8.2. Table of Contents (Tab 1)
Include a table of contents for this part.
L8.3. Past Performance Information (Tab 2)
The offeror shall provide past performance data on at least three (3) but not more than seven (7) of the most recent and relevant contracts. Submit original and three (3) copies. The offeror’s name, logo, or any other identifiable reference to the offeror shall be provided ONLY on the cover page (L6.1.). Again, only references for recent and relevant contracts are desired.
L8.3.1. Past Performance Questionnaires (Attachment 6).
The offeror is responsible for sending the past performance questionnaire in Attachment 6 to their recent and relevant reference(s), with instructions to fax or email the completed questionnaire to the Contracting Office. The offeror is to have no more than five (5) past performance questionnaires submitted to 355th Contracting Squadron by their references. The questionnaire shall be sent “directly” by either fax or e-mail to 355th Contracting Squadron, Roxanne Elfering at 520-228-5284 or roxanne.elfering@us.af.mil. IAW Federal Acquisition Regulation, any questionnaires provided to the Contracting Squadron, directly from an offeror, will not be considered.
Questionnaires are due to the Contracting Office by the established offer due date in Block 8 on the Standard Form 1449, however the Government requests the past performance questionnaires be sent ten (10) calendar days prior to the proposal submission date to ensure all questionnaires are received for a complete evaluation. Instruction are also printed on the first page of the past performance questionnaire.
L8.3.1.1 Past Performance Questionnaire Early Submission
The offeror is encouraged to have the past performance questionnaires submitted ten (10) calendar days prior to the established offer due date in Block 8 on the Standard Form 1449.
L8.3.1.2. Past Performance Questionnaire References
The offeror should provide the list of references requested by the offeror to provide a past performance questionnaire. This list will aid the government in obtaining any questionnaires not submitted. However, it is solely the offerors responsibly to ensure references receive, complete, and return the questionnaire on time. The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the acquisition.
L8.3.2. Recent Past Performance
The offeror shall submit past performance that is recent. To be recent, the current effort must be ongoing or must have been performed for a minimum of six (6) months during the past three (3) years from the date of issuance of this solicitation. Any past performance information submitted that does not meet this condition will be evaluated in accordance with M3.2.3. Recency Assessment, Table 3 - Past Performance Recency Rating and shall receive a rating of “Not Recent” or “Unacceptable”.
L8.3.3. Relevant Past Performance
The offeror shall submit past performance that is relevant. To be relevant, the performance information provided must resemble the scope, magnitude and complexity of this solicitation. The offeror’s submissions shall be recent and demonstrate the ability to perform the proposed effort. The offeror’s proposing past performance as the Prime Contractor will be weighted more heavily than past performance as a subcontractor. Any past performance information submitted that does not meet this condition will be evaluated in accordance with M3.2.4. Relevancy Assessment, Table 4 - Past Performance Relevancy Rating and shall receive a rating of “Not Relevant” or “Unacceptable”.
L8.3.4. Content Aspects
The offeror is responsible for explaining past performance recency and relevancy submissions to requirement and how the efforts relate. This includes management efforts to correct and improve any performance problems.
L9. Part IV Price
The purpose of this section is to ensure the offeror understands the Government will evaluate on the total price.
Compliance with these requirements is mandatory and failure to comply may result in rejection of the proposal. The pricing information provided in the offer shall be complete. Complete means that the offeror proposed a Firm-Fixed Price for ALL item numbers on the Position Description Pricing Worksheet, Attachment 4 and ALL items on the Overtime Pricing Worksheet, Attachment 5 for the base period, the four (4) option periods, and the six (6) month extension. The offeror has the responsibly to formulate a fully burdened unit price for each item number. Price proposals will NOT be rated or scored. The six (6) month extension will NOT be part of the evaluation, but will be assessed by the Government to determine the price fair and reasonable.
L9.1. Price Proposal Information
Offerors are to complete the Position Description Pricing Worksheet, Attachment 4 and the Overtime Pricing Worksheet, Attachment 5 by providing a fully burdened hourly unit price for ALL item numbers / position descriptions. The offeror shall then calculate the extended amount on the worksheet by multiplying the estimated quantity (EST QTY) of hours listed by the proposed fully burdened unit price. That information shall then be populated in the extended amount of the worksheet for ALL item numbers / position descriptions. The offeror shall then calculate the extended amount to provide the extended total for each item number / position description on the worksheet. That information shall then be populated in the extended total of the worksheet for ALL items / position descriptions. All dollar amounts shall be in whole dollars and whole cents. Each item number / position description shall contain a fully burdened unit price, the extended amount and the extended total for each worksheet to be considered complete and considered for contract award.
The offeror shall only submit price information as instructed, unless the offeror considers it essential to document or support price position. All information pertaining to the proposed price, including all worksheets and supporting documentation must be included in the section of the proposal designated as the Price part. The offeror’s name, logo, or any other identifiable reference to the offeror shall be provided ONLY on the cover page (L6.1.). UNDER
NO CIRCUMSTANCES SHALL PRICE INFORMATION AND DOCUMENTATION BE INCLUDED
ELSEWHERE IN THE PROPOSAL.
L9.2. Price Proposal Information Reasonableness
In accordance with FAR 15.403-1(b) and 15.403.3(a), information other than pricing data may be required to support price reasonableness. If, after receipt of proposals, the Contracting Officer determines there is insufficient information available to determine price reasonableness and none of the exceptions in FAR 15.403-1 apply, the offeror shall be required to submit pricing data.
L9.3. Price Rounding
All dollar amounts provided shall be in United States currency in whole dollars and whole cents rounded to the nearest penny.
L9.4. Table of Contents (Tab 1)
Include a table of contents for this part.
L9.5. Position Description Pricing Worksheet, Attachment 4 (Tab 2)
Attachment 4 was designed for offeror’s to use for the submission of pricing. This attachment will be used by the price evaluation team during the source selection process. Offerors must complete the attachment as designed. Do NOT make any formatting changes to Attachment 4. Any attempts to reformat or change the attachment may be reason to reject an offeror’s proposal. Again, the submission needs to be complete. If a proposal is received with incomplete pricing that proposal may be deemed unawardable.
L9.6. Overtime Pricing Worksheet, Attachment 5 (Tab 2a)
Attachment 5 was designed for offeror’s to use for the submission of overtime pricing. This attachment will be used by the price evaluation team during the source selection process. Offerors must complete the attachment as designed. Do NOT make any formatting changes to Attachment 5. Any attempts to reformat or change the attachment may be reason to reject an offeror’s proposal. Again, the submission needs to be complete. If a proposal is received with incomplete pricing that proposal may be deemed unawardable.
L9.7. Professional Compensation Plan
The offeror shall submit a professional compensation plan IAW FAR 52.222-46 that ensures uninterrupted services of incumbent healthcare workers from prior contracts.
L10. Part V Contract Documentation
Offeror’s shall provide the following information and items listed in this part which are required by the solicitation.
L10.1. Table of Contents (Tab 1)
Include a table of contents for this part.
L10.2. Standard Form 1449 (Tab 2)
The offeror shall complete blocks 12, 17a, and sign in 30a and complete blocks 30b and 30c of the Standard Form 1449. In doing so, the offeror accedes to the contract terms and conditions as written in the solicitation and all amendments, with attachments. The solicitation once awarded constitutes the contract.
L10.3. All Amendments (Tab 2a)
The offeror shall acknowledge ALL amendments issued against the solicitation by signing in block 15B and completing blocks 15A and 15C of the Standard Form 30.
L10.4. Representations and Certifications (Tab 3)
Complete Representation and Certification in SAM at https://www.sam.gov or complete the necessary fill-ins and certification in accordance with FAR Clause 52.212-3, and its Alt I. If FAR 52.212- 3 and its Alt I are completed in hard copy it shall be returned along with the contractor’s proposal.
L10.5. Any Exceptions to Solicitation Requirements (Tab 4)
Offerors must clearly identify any exceptions to the solicitation terms and conditions and provide complete accompanying rationale. Each exception shall be tied to a specific paragraph or specific part of the solicitation to which the exception is being taken. Explain the impact of the exception fully in terms of performance, schedule, cost, and any other requirement of the salutation.
L10.6. Financial Responsibility Letter (Tab 5)
A letter from the offeror’s financial institution shall be provided which authorizes the Contracting Officer to verify the offeror’s financial responsibility. Include financial institution’s name, address and primary point of contact and phone numbers for the financial institution.
L10.7. Authorized
Provide the name, title, email address, and telephone number of the offeror point of contact regarding decisions made with respect to the offeror’s proposal. The offeror shall provide the names, titles, email addresses, and telephone numbers for individuals authorized to negotiate on behalf of the offeror.
L10.8. Offeror Name, Address, Identifying Codes, and Applicable Designations
The offeror should provide the company name, street address, county and facility code; CAGE code; DUNS; TIN;
and Small Business designations.
L10.9. Mentor-Protégé Agreements and 8(a) Joint Venture Certifications
The offer shall provide a copy of the Small Business Administration approved Mentor-Protégé agreement or 8(a) Joint Venture Certification documentation, if either of these apply. If there will be any impacts on the requirements of the solicitation, that information shall be provided to address performance, cost, schedule, and other specific requirements of the solicitation.
L10.9. Proposal Reimbursement
The Government will not provide reimbursement to offeror’s for ANY proposal costs.
