1. DECRIPTION OF SERVICES. This is a non-personal services. The contract shall provide all labor, supervision, transportation and other items and services necessary to perform Night Facility Monitor (NFM) services at Base Training and Education Services (BTES) office, Cannon AFB, New Mexico in accordance with all federal, state and local laws and regulations.

1.1. Basic Services. The contractor shall provide NFM services for the BTES office to include opening and closing BTES facilities for early morning, evening and weekend use; monitoring audio-visual, computer and support equipment. The contractor shall ensure facilities and classrooms are neat, clean and ready for use.

1.2. The contractor shall provide access to classroom services in accordance with a schedule provided by the COR based upon the following criteria. Classes may be scheduled from 0530-0730 hours and 1630- 2100 hours Monday through Friday. Classes may be scheduled Saturday and Sunday. Class times depend on the education office classes and school class schedules which will be finalized at the beginning of each term/semester. Contractor NFM services involve opening, closing and securing all facilities used by BTES customers and are linked to class schedules. The contractor shall have classrooms ready for instruction at least 15 minutes prior to class start times. The initial schedule is provided as appendix A. As program needs change the COR will give notification at least 7 days prior to the change of schedule.

1.3. A neat, clean and ready for use classroom is defined as removing/emptying trash, cleaning white boards, dusting, and vacuuming, adding, removing, arranging and wiping/cleaning furniture, as needed.

The contractor shall ensure all equipment is turned off and stored securely. Trash will be disposed of in designated dumpsters daily or after the last class for the day.

1.4. The contractor shall issue learning aids to BTES customers as required and inspect learning aid equipment for serviceability prior to and after use. The contractor shall check for proper use and safeguard equipment to prevent damage and/or malfunctions. The contractor shall report any abuse or damage to the

COR.

1.5. The contractor shall monitor the BTES facility and equipment during each class session. The contractor shall secure/lock classrooms and office area after last class. The Contractor shall notify Security Police and/or Fire Department in the event of an emergency.

1.6. The contractor may be called upon to notify faculty members and student of emergency messages, official phone calls, mission alert, or recall activities during class sessions.

1.7. During class sessions the contractor shall direct students and faculty to assigned classrooms, monitor hallways, break areas, classrooms, and surrounding grounds. The contractor shall provide guidance to instructors, students and visitor concerning BTES rules regarding personal conduct/behavior, and telephone and copier usage.

1.8. The contractor shall clean the BTES office and common areas at least weekly or more often if needed, to include removing and emptying trash, dusting and vacuuming and wiping/cleaning furniture.

2. SERVICE SUMMARY (SS). The contract service requirements are summarized in Performance Objectives that relate directly to essential mission needs. The Performance Threshold briefly describes the minimally acceptable levels of service for each objective. The SS provides information on contract requirements, the expected level of Contractor’s performance and the expected method of Government validation and confirmation of services provided.

2.1. Contracting Officer (CO) and Contracting Officer Representative (COR). The CO will appoint a representative for management of the day-to-day activities of the contract. The identity, title and authority of this representative will be provided in writing to the contractor after contract award.

2.2. Quality Assurance During the period of the contract the services performed by the contractor shall at all times be subject to review by the Contracting Officer or the COR. The COR will inform the contractor/CM when discrepancies occur and will request corrective action.

3. GOVERNMENT FURNISHED PROPERTY OR EQUIPMENT (GFP/GFE) The government does not anticipate providing GFP/GFE in performance of this contract.

3.1 Conservation of Utilities. The contractor shall practice utility conservation. The contractor shall operate under conditions that prevent the waste of utilities.

4. GENERAL INFORMATION

4.1 Contractor Employees. The Contractor shall not employ persons for work on this contract if such employee is identified to the Contractor by the CO as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.

4.1.1 Contractor employee shall present a neat and professional appearance and be easily recognized as contracted employee. This shall be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee name in English.

4.1.2 Conflict of Interest. The Contractor shall not employ any person who is an employee of the U.S. Government if employing that person would create a conflict of interest.

4.2 The contractor shall provide a contract manager (CM). The contractor shall submit the name, address and telephone number of the CM to the CO within 2 days of contract award. This contact information shall be kept current throughout the life of the contract. The CM shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract.

The CM shall be available to meet with the CO or COR by telephone to discuss contract issues during 0730-1630 M-F. The CM shall be able to read, write, and speak English.

4.3. Minimal Qualifications. Contractor shall be capable of performing light physical duties such as moving and setting up audiovisual equipment, moving tables and chairs, and other routine daily maintenance functions. The contractor shall be available to work early morning, evenings and weekends. The contractor shall be at least eighteen (18) years old and be able to effectively communicate and deal with professional educators, students and base officials. The contractor must communicate clearly and concisely with the BTES customers in English. This requirement is strictly for the purpose of efficiently performing the contract tasks. The contractor must meet Cannon base access requirements.

4.4. Work Schedule. See paragraph 1.2. The contractor is responsible for conducting business during each scheduled class session, as a minimum four (4) hours a week to support classroom and common area maintenance between school terms, but no more than 8 hours in one day for a maximum of 20 hours per week. Program fluctuations will be scheduled at least 7 days prior to the change of schedule by the COR.

4.4.1 Recognized Holidays: The contractor will NOT be required to work on the following recognized holidays.

4.4.2. BTES office is scheduled to be closed for business on AFSOC Family or Safety days.

(The above dates are subject to final approval by AFSOC. Any changes to the above dates will be provided to the contractor within 1 business day of the change being made.)

4.5. Physical Security. The contractor shall safeguard all Government keys provided. At the close of each work period Government facilities, property, and materials shall be secured. The contractor shall ensure all keys/key cards issued to the contractor by the Government are not lost, misplaced, duplicated, or used by unauthorized persons.

4.5.1. In the event keys are lost or duplicated, the contractor shall, notify the COR and Contracting Officer. The Government may replace the affected lock or locks or perform re-keying. When the replacement of locks or re-keying is performed by the Government, the total cost of re-keying or the replacement of the locks is the contractor’s responsibility. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the Government and the total cost is the responsibility of the contractor.

4.6 Safety Requirements and Reports. The contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements.

4.6.1 Mishap Notification. The contractor shall provide a verbal report to the CO as soon as possible after each occurrence of damage to Government property or a mishap resulting in death, injury, occupational disease or adverse environmental impact.

4.6.2. Reporting. The contractor shall provide a copy of all required Accident Investigation Reports to the CO within 5 calendar days of each occurrence. The contractor shall report contractor personnel occupational injuries, illnesses and other related activities in accordance with the Occupational Safety and Health Act (OSHA). The contractor shall maintain records as required by Federal, State, and local regulations.

4.6.3 Investigation. The contractor shall cooperate fully and assist Government personnel in any Government mishap investigation until released by the mishap investigation authority. Do not release mishap information outside of the official Government.

4.7 This service is determined to be non-essential for performance during a crisis.

4.8. Contractor Manpower Reporting. The contractor shall report all contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the “U.S. Air Force” via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address: https://www.ecmra.mil.

Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractor may direct questions to the electronic Contractor Manpower Reporting Application (eCMRA) help desk.
