PERFORMANCE WORK STATEMENT

FOR

LATRINE & PUMPING SERVICE

1. DESCRIPTION OF SERVICES. The contractor shall provide all labor, equipment, transportation, and other items and services necessary to provide chemical latrines and domestic wastewater tank pumping service in compliance with all Federal, State and local laws and regulations. The contractor shall furnish, maintain and service portable chemical latrines (Standard and Handicap Accessible) and hand wash stations at Cannon Air Force Base (CAFB), Melrose Air Force Range (MAFR) and remote sites throughout Eastern New Mexico, and to pump domestic wastewater holding and septic tanks at CAFB and MAFR. At the end of the performance period, the latrines and hand wash stations will not become the property of the Government.

1.1. LATRINES AND HAND WASH STATIONS.

1.1.1. Latrines shall be well-ventilated, with self-closing doors, inside lock/latch, tight-fitting door seals and screen material to prevent pest intrusion and accumulation of filth. Latrine unit quantities may range from 1 to over 50, depending upon the situation or event. Small quantities of latrines (static locations and remote sites) may be utilized long-term (e.g., 1-7 months). Large quantities of latrines (e.g., 30-60) and hand wash stations (e.g., 5-10) may be utilized for short-term events (e.g., 1-2 days), such as an Open House or Air Show and shall include servicing and replenishment of supplies after the first day’s event.

1.1.2. Latrines shall be furnished at static locations and remote sites (Attachments B and C-1).

Workload requirements may increase or decrease as needed to support mission requirements.

Each unit will be set up (to include shimming/leveling), maintained and serviced. The contractor shall use an environmentally safe chemical in the waste tank specifically designed for portable latrines during cold weather to prevent the freezing of waste in the tanks.

1.2. DELIVER AND SET UP LATRINES AND HAND WASH STATIONS. The contractor shall make timely delivery and set up of units. Delivery shall be performed in accordance with the work order issued by the Contracting Officer (CO). Delivery includes completed placement and set up actions to make the unit ready for customer use at the specified location. The contractor is responsible for ensuring latrines and hand wash stations stay upright and in place for anticipated wind conditions.

1.2.1. Sandbags, or equivalent materials, provided by the contractor, are acceptable for use when arranged in a neat and professional manner.

1.2.2. Stakes may be used to secure units, however, may not exceed 4” in length. If the contractor intends to drive stakes into the ground on CAFB, a completed 27 SOCES Form 103, Base Civil Engineering Work Clearance Request (also known as a Dig Permit) is required before breaking ground (Attachment E-2). 27 SOCES conducts Work Clearance Request briefings with contractors to process Work Clearance Requests. Briefings are held in the 27 SOCES Conference Room, Building 355, 506 N Air Commando Way, every Thursday morning at

7:30am, excluding holidays. The contractor shall attend a briefing if their Work Clearance Request is being processed.

1.3. SERVICE LATRINES AND HAND WASH STATIONS. Latrines shall be kept clean and odor free. Services shall consist of cleaning the inside and outside of the latrines, disinfecting the interior including the toilet seat and urinal, providing air freshener, hand sanitizer and a minimum of two full rolls of toilet paper for each latrine; pumping waste materials and replenishing the chemical effectiveness of the liquid chamber for each latrine; and sanitizing and replenishing fresh water, paper products and hand sanitizer (soap) for each hand wash station. The contractor shall avoid waste transfer onto surfaces outside of container opening, and is responsible for applicable cleanup of waste transfer from surfaces outside of container opening. The contractor is responsible for cleanup of any chemical product or waste spills. The contractor shall dispose of all waste, materials, empty containers, etc., off of CAFB and MAFR property at an off-base permitted site in accordance with all applicable Federal, State, and local laws and regulations. The contractor shall ensure all latches, hinges, vents, hoses and attached accessories are maintained and in working order at no extra cost to the Government.

1.3.1. Inaccessible Latrines. The contractor shall attempt to service each latrine. If the latrine is inaccessible due to weather or access road conditions, the contractor will notify the COR within 24 hours or upon the next duty day. Government personnel will not be sent to clear snow/ice from around the latrine nor is the contractor expected to clear snow/ice from around the latrine.

The Government will not be responsible if a contractor vehicle becomes stuck attempting to access impassible roads.

1.3.2. Blown/Knocked Over Latrines. If tipping occurs, the contractor shall set the latrine upright at no additional cost to the Government. The contractor shall clean any spillage resulting from blown/knocked over latrines. The contractor shall not dig deeper than 2” to clean any spilled chemicals.

1.3.3. Vandalism. The contractor shall report, in writing, acts of vandalism, theft, or apparent willful act of damage of latrines to the CO. The contractor shall cooperate with Government personnel and provide a written statement of facts for Security Forces personnel, when requested.

1.4. REMOVE LATRINES AND HAND WASH STATIONS. The contractor shall pick up and remove all latrines and hand wash stations at the end of the period of performance of the contract or in accordance with the instructions issued on the work order. Removal includes any sanitary service actions required for the contractor’s retrieval of the unit. The contractor shall ensure the equipment, service products and related items/debris are removed from the area.

1.5. PUMP HOLDING AND SEPTIC TANKS. Services shall include pumping waste materials, only leaving minimal residue in the tank. The contractor shall dispose of all waste materials, empty containers, etc. off of CAFB and MAFR property in accordance with all

The contractor shall avoid waste transfer onto surfaces outside of tank opening, and is responsible for applicable cleanup of waste transfer from surfaces outside of tank opening. The contractor shall securely close the tank opening upon completion of pumping. The contractor is responsible for cleanup of any chemical product or waste spills. The contractor shall dispose of all waste at an off-base permitted site.

1.6. WORK ORDERS. All work orders will be approved by the CO. Work orders shall specify CLIN item(s), description, quantities, locations, date of service and removal, and other applicable information (Attachment E-1).

1.6.1. CAFB and Cannon Permanent Exercise Facility (CPEF). On occasion, Out of Schedule services may be required to support a short-notice emergency or requirement (e.g., facility water outage, emergency cleaning, emergency pumping). The contractor shall meet the short-notice requirement within four (4) hours.

1.6.2. MAFR. On occasion, Out of Schedule services may be required to support a short-notice activation, exercise or contingency. The contractor shall meet the short-notice requirement within one business day.

2. SERVICES SUMMARY (SS). The contract service requirements are summarized in Performance Objectives (PO) that relate directly to essential mission needs. The Performance Threshold briefly describes the minimally acceptable levels of service for each requirement. The SS provides information on contract requirements, the expected level of Contractor’s performance and the expected method of Government validation and confirmation of services provided.

2.1. Quality Control. The contractor shall provide adequate quality control to ensure that all required service tasks are performed in accordance with commonly accepted commercial practices.

2.2. Quality Assurance. The Government will inspect and evaluate the contractor's performance to ensure services are received in accordance with the requirements set forth in this PWS. The COR shall inspect by validating actual work performance, physically an attribute of the completed task, checking a management information report, investigating customer complaints, conferring with facility managers or otherwise inspecting the task or its results to determine whether or not performance meets the standards contained in this PWS. Results of the validation then become the official Air Force record of the contractor’s performance. When a performance threshold has not been accomplished, the COR will initiate and provide the CO a DD Form 2772, Contract Discrepancy Report (CDR), for issuance to the contractor. The contractor shall respond to the CDR IAW the instructions provided and return it to the CO within three (3) calendar days of receipt. (Attachments D and E-3)

2.3. Re-Performance of Service. When service performed does not conform with contract requirements, the contractor shall re-perform at no additional cost to the Government.

Performance or re-performance of these services shall commence immediately upon notification by the CO. See Section 2, Services Summary.

2.4. Progress Meetings. The contractor may be required to meet at least weekly with the CO and the COR during the first month of the contract. Subsequent meetings will be on an as needed basis. However, the contractor may request a meeting through the CO.

3. GOVERNMENT-FURNISHED PROPERTY. None

4. GENERAL INFORMATION.

4.1. Personnel. Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. All employees performing on this contract must be clearly able to read, write, speak, and understand English.

4.1.1. Contract Manager. The contractor shall provide a Contract Manager who shall be responsible for the performance of the work. The name of this person and an alternate(s), who shall act for the contractor when the Contract Manager is absent, shall be designated in writing, with daytime contact information (email address, work and cell phone numbers), to the CO at least 2 days prior to contract performance. An email address is required for submission of work orders from the COR.

4.1.2. Employees. The Government has the right to restrict the employment of any contractor employee, or prospective contractor employee, who is identified as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.

4.1.3. The contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest. The contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with Department of the Air Force (DAF) Standards of Ethical Conduct Rules.

4.1.4. Listing of Employees. The contractor shall provide a list to the CO of all personnel who will require access to CAFB or MAFR for performance IAW this PWS (see Attachment D). The contractor shall ensure the pass and identification items required for contract performance are obtained for employees. The Pass and Identification Section of 27th Special Operations Security Forces Squadron (27 SOSFS) will provide base entry authorizations/passes to each authorized contractor employee. In addition, contractor personnel wishing to operate a privately-owned vehicle on CAFB must comply with all applicable state and base requirements. Entry authorizations must be renewed as required by the 27 SOSFS Pass and Identification Section.

4.1.5. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes, from employees who depart for any reason before the contract expires (e.g. terminated for cause, retirement, etc.).

4.2. Security Requirements.

4.2.1. CAFB Access. Contractor personnel shall not enter any restricted or controlled areas without proper security escort. Anticipate delays in getting commercial vehicles on base and allow time for commercial vehicles to reach their destination by driving designated routes at posted speed limits throughout the base. Procedures for commercial vehicle access to the base are subject to change.

4.2.1.1. Airfield Drivers Training. A special event (i.e., Air Show, Open House) may require the delivery of latrines and hand wash stations to facilities adjacent to the airfield. Airfield drivers training must be completed prior to operating vehicles on the airfield (Attachment D).

Airfield Management conducts training in a classroom setting, and employees are required to attend the course and pass a written test. The training session and testing may take 1-3 hours to complete. Upon successful completion, employees will be issued an AF IMT 483, Certificate of Competency, for airfield driving. All employees driving on the airfield are required to have a valid driver’s license and AF IMT 483 with them at all times. The contractor shall contact the COR to schedule training appointments through the Deputy Airfield Manager.

4.2.1.2. Privately Owned Vehicle (POV) Passes. Vehicles must have a valid POV pass issued by Airfield Management prior to operating on the airfield. The contractor shall provide Airfield Management with the following information for each vehicle that will be operated on the airfield: Year, make, model, and color of each vehicle, license plate number, proof of insurance and vehicle registration (Attachment D). At no time will equipment be left unattended while

Contract: Pending LATRINE & PUMPING SERVICE Attachment 1 working in these areas. The contractor shall be responsible for ensuring that all foreign objects and debris produced from this contract is removed before leaving the work area.

4.2.2. MAFR Access. The contractor may be required to deliver, relocate and provide cleaning service on short notice throughout MAFR and at remote sites. The contractor shall ensure entry authorizations are completed as required by MAFR officials. All drivers on MAFR must receive Range Safety Officer training. Training is offered each Tuesday by appointment only. The contractor should be prepared to experience delays entering MAFR. The contractor shall agree to comply with MAFR Standard Operating Procedures (SOP’s) before performance start (Attachment F).

4.2.2.1. The Government will provide a radio upon entry to the MAFR. The contractor shall maintain radio contact with the Range Control Officer (RCO) and monitor radios at all times while on the MAFR. The contractor shall ask for approval for all movement on the MAFR from the RCO (“Ground Control”). At no time should the contractor move on the MAFR without Ground Control approval. All equipment issued to contractor employees must be returned when leaving the MAFR each day. It is not required to monitor range frequencies when servicing remote sites not on the MAFR.

4.2.3. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, Contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.

4.2.4. Force Protection Condition (FPCON). Levels may change for real-world events or for exercise scenarios which may prevent contractor employees from entering or leaving CAFB until the situation has lessened. Depending on the situation, contractor employees shall react to FPCON requirements and guidance provided through the CO or COR.

4.3. Vehicles. The contractor shall provide and maintain all vehicles necessary to perform the requirements of this contract. The contractor may be required to place latrines in many areas that have rough or sandy terrain. Appropriate vehicles/equipment to access these areas are required.

Contractor vehicles shall have the company name prominently displayed on both sides of the vehicles. Vehicles shall be clean and always have a neat, professional appearance. All vehicles shall be in operable condition and meet New Mexico safety requirements. Vehicle repairs shall not be accomplished on CAFB or MAFR. Equipment failure shall not alleviate the contractor from performing any requirement contained in this contract.

4.3.1. Only qualified operators shall be permitted to operate the vehicles or equipment required by this contract. Vehicle/equipment operators shall possess a valid driver’s license.

4.3.2. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicles or privately-owned vehicle while on CAFB or MAFR.

4.3.3. Traffic Laws. The contractor and contractor employees shall comply with all base traffic regulations. The contractor is not permitted to use cell phones on CAFB or MAFR while operating a moving vehicle, unless the device is hands-free.

4.4. Work Hours. Normal work hours at CAFB are 7:00 AM to 4:00 PM, Monday through Friday, except for Federal Holidays (listed below). Normal operations hours at the MAFR are 0800-0200, Monday through Friday. The contractor shall deliver/service latrines at the MAFR during daylight normal operations hours. The contractor may be required to perform requirements IAW this PWS outside of normal work hours, and 7 days per week.

4.5. Security Forces and Fire Protection Services.

4.5.1. CAFB. Contact the appropriate agency in case of emergency by dialing 911 from a base telephone. If calling from a cell phone or an off-base telephone, the following numbers apply:

Fire: 575-784-2578, Police: 575-784-4111, Command Post: 575-784-2253. If dialing 911 from a cell phone, the call may be automatically routed to an off-base agency, such as the city of Clovis, NM. Be clear about whom you are communicating with and what your situation requires.

4.5.2. MAFR. There is limited medical response on the MAFR and only civilian response outside of the MAFR for emergencies. In the event of a medical emergency on the MAFR, the contractor shall immediately notify the RCO by radio stating “Ground Control [call sign], I need to declare a medical emergency.” The contractor shall then follow the direction of the RCO and provide the information requested.

4.5.2.1. Reporting of Fires. The contractor shall immediately report any observed fire on the MAFR to the RCO.

4.6. Environmental Management System (EMS). All Contractor personnel shall complete EMS Awareness training prior to commencement of any activities. Awareness Training Certificates of Completion to the COR prior to any on-base performance under this contract (Attachment D).

4.7. Hazardous Materials (HM) Handling. HM must be approved prior to being brought on site by the contractor. The contractor may not bring HM onto CAFB or MAFR, nor use a HM, until the contractor receives all required authorizations.

4.7.1. The information on Cannon AFB & Melrose AFR Contractor Hazmat Worksheet (Attachment E-3) and Safety Data Sheets (SDS) must be submitted to the CO prior to usage. The contractor shall submit Cannon AFB & Melrose AFR Contractor HM Usage Worksheet (Attachment E-4) to the CO by the fifth business day after the completed month.

4.7.2. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws/regulations come into effect.

4.7.3. Notification of Environmental Spills. If the contractor spills or releases any substance contained in 40 CFR 302, Designation, Reportable Quantities, and Notification into the environment, the contractor or its agent shall immediately report the incident to CAFB Fire Department if on CAFB or according to MAFR instructions. The liability for the spill or release of such substances rests solely with the Contractor and its agent.

4.7.4. Permits. The contractor is responsible for obtaining all necessary permits to fulfill all requirements of this contract.

4.8. Continuation of Essential DoD Contractor Services During Crisis IAW DoDI 3020.41.

This service is determined to be non-essential for performance during crisis according to DODI 3020.41, Operational Contract Support, paragraph 2.c. In the event of a crisis, the contractor will be notified by the CO of the need to discontinue services due to contingency, base closure or other causes. If notification to discontinue services is not received, the contractor shall be expected to continue performance in accordance with this PWS.

4.9. Contractor Manpower Reporting Requirements. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address:

https://www.ecmra.mil. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. (Attachment D). 4.10. SAFETY REQUIREMENTS AND REPORTS. The contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements.

4.10.1. Mishap Notification. The contractor shall provide a verbal report to the CO as soon as possible of each occurrence of damage to Government property or a mishap resulting in death, injury, occupational disease or adverse environmental impact. (Attachment D).

4.10.2. Reporting. The contractor shall provide a copy of all required Accident Investigation Reports to the CO within 5 calendar days of each occurrence. The contractor shall report contractor personnel occupational injuries, illnesses and other related activities in accordance with the Occupational Safety and Health Act (OSHA). The contractor shall maintain records as required by Federal, State, and local regulations. (Attachment D)

4.10.3. Investigations. The contractor shall cooperate fully and assist Government personnel in any Government mishap investigation until released by the mishap investigation authority. Do not release mishap information outside official Government channels.
