Statement of Work (SOW) For

CONSTRUCTION SERVICES

REPAIR OVERHEAD DOOR, BUILDING 4606

Project Number: CZQZ16-0014

Cannon Air Force Base, New Mexico

08 February 2016

1. BACKGROUND: Hangar/Building 4606 is located in the southeast region of Cannon Air Force Base (CAFB), New Mexico. The building is approximately 3 years old and houses the Aircraft Fuels System Maintenance

Squadron. Currently, the existing aluminum grille overhead door, on the northeast face of the structure, is insufficient in its ability to adequately insulate the Canopy Room [as referenced in the original As-built

Drawings] fire suppression system. This became apparent during the 26 th

- 27 th

December 2015 snow storm.

During the storm, wind gusts in an excess of 80 mph, caused snow to penetrate the grille door, leading to a significant temperature drop within the Canopy Room. To further intensify the matter, the building experienced a power outage, leaving the attic space Canopy Room without heat. This ultimately led to the freezing of the fire suppression system, and subsequently contributed to a breakage in the system. While the breakage has since been fixed, this section of the fire suppression system still remains down, as the fire suppression pipes continue to be greatly susceptible to freezing during this winter seasons if the building experiences another power outage.

Additionally, during this storm, strong wind gusts also caused the top panel covering of the galvanized enclosure jacket to the exterior HVAC ductwork, on the northwest side of the building, to partially pry open, exposing the insulation inside (currently the top panels are being held down with sand bags) . Lastly, at some point during the storm, a double pane window, on the northeast of the building was damaged, causing the exterior pane to completely shatter.

2. OBJECTIVES: The objectives of this Work include but are not limited to:

2.1. Repair storm damage and improve components to prevent future damage in B4606.

2.1.1. Complete removal of the existing 20’x16’ aluminum grille overhead door on the northeast side of the building. This includes all auxiliary components associated with the door’s current function. Door will be disposed of by the construction contractor, unless specified otherwise by the Government.

2.1.2. Installation of a new Wayne Dalton Thermotite 800C ‘or equal’, equipment necessary to make the door mechanically operative (i.e. electric door operator) and any other auxiliary components necessary to have a fully functional door (i.e. mounting brackets, overhead drum, hood etc.) on the northeast side of the building.

2.1.2.1. Door must be able to withstand a wind load of at least 120 mph per IBC 2012 and ASCE 7-10 requirements.

2.1.2.2. Because colors vary from manufacturer to manufacturer, contractor shall provide color samples for Government to approve. Ideally, the new roll-up door color should match the roll-up doors in building 4607 to the best extent possible

2.1.2.3. A 2-year manufacturer warranty shall be included.

2.1.2.4. Operation and Maintenance Data shall be proved to the Government upon completion of the project.

2.1.3. Modify current electrical 3-phase heater circuit. Electrical heater is located in the Attic space of the

Canopy Room. The circuit for this heater needs to be rerouted to the emergency direct panel labeled

‘DPEM’ in room 103 “Electrical Room’ (see Attachment 4 for location of heater and panel).

2.1.3.1. Contractor shall account for all new electrical conductors, conduit, safety switch and circuit breaker required to connect the existing heat to the ‘DPEM’ panel

2.1.4. Clean and re-lamp light fixture to meet UFC 3-530-01 Interior and Exterior Lighting and Controls

(issued date 01 April 2015) performance requirements for a Storage Room. The Canopy Room

Statement of Work (SOW) CZQZ 16-0014 Repair Overhead Door Page 2 of 9 Total Pages currently has exterior lighting. With the installation of new door, the room would become fully enclosed and would require interior lighting.

2.1.4.1. The existing Lighting Fixture is as follows:

2.1.5. Remove the loose top covering of the current galvanized HVAC jacket enclosure on the northwest side of the building (two different ducts approximately 20’ high).

2.1.5.1. Replace place missing and damaged 2” insulation. The amount of new insulation required is approximately 400 SF. However, this quantity should be field verified by contractor during pre-contract award site visit.

2.1.5.2. Reattach top covering utilizing the necessary amount of rivets and tie downs. If it is determined during pre-contract award site visit that certain section of the galvanized jacket are no longer usable, they will be replaced as directed by the Government.

2.1.6. Replace 4’x4’ double pane exterior window on the northeast side of the building, insuring that new window matches existing window to the best extent possible.

2.1.6.1. Because colors vary from manufacturer to manufacturer, contractor shall provide color samples for Government to approve. Ideally, the new window color should match the existing adjacent window to the best extent possible

2.1.6.2. Contractor shall repair exterior and interior wall damage that occurs in the removal and installation process.

3. PROJECT LOCATION: Cannon Air Force Base (CAFB) is located in Curry County, approximately six miles west of Clovis, NM. The project location is in the southeast region of CAFB. The site is accessible through either the Main Gate or the Portales Gate via Aderholt Rd, turning into Turboprop Rd followed by right turn into South Dagger Rd.

4. PERIOD OF PERFORMANCE: This Period of Performance for construction shall be 50 working days or less. This time frame excludes Weekends and Federal Holidays.

5. MEETINGS: The contractor shall attend one project Pre-Construction meeting within one (1) week of Notice to Proceed and one post-survey briefing following the site visit. The contractor shall prepare and distribute meeting minutes from each meeting.

6. GOVERNMENT FURNISHED PROPERTY: Upon the Contractor’s request, if available, the Government will provide electronic and/or printed copies documents and correspondences pertinent to the Work. It is the contractor’s responsibility to verify/validate the current existing conditions. Any deviations from provided AS-

BUILTS shall be reported. In addition, the Government will provide assistance with base passes, identification badges, and registrations.

7. GREEN PROCUREMENT PROGRAM: In performance of this contract, DBE shall use Environmental

Protection Agency (EPA) designated recycled content products, Information Technology (IT) Energy Star products/appliances, Federal Energy Management Program (FEMP) Designated Energy Efficient Low Stand By

Power products/appliances, U.S. Department of Agriculture (USDA) Bio based/Bio preferred products, Environmentally preferable products, Electronic Product Environmental Assessment Tool (EPEAT) registered products, Water Sense or other water efficient products, non- or Low Ozone depleting substances under the

Significant New Alternatives Policy (SNAP), non or Low toxic or hazardous constituents (e.g. non-VOC paint) and any other environmentally sustainable product/method, to the greatest extent possible.

8. BASE ACCESS AND SECURITY: The Contractor shall ensure that all employees comply with all security requirements imposed by Cannon Air Force Base (CAFB) when entering, occupying and departing the base. All contractor personnel shall be screened for security clearances as determined by CAFB.

9. PROTECTION OF GOVERNMENT PROPERTY: The Contractor shall use reasonable care to avoid damaging government-owned property including buildings, equipment and vegetation at CAFB. If the

Contractor causes damage to any government-owned property, the Contractor shall replace or repair the damage at no expense to the government. If the Contractor fails or refuses to make such repair or replacement, the cost may be deducted from the contract price.

10. REMOVAL OR DISPOSAL OF GOVERNMENT PROPERTY: Unauthorized removal or disposal of government property is a crime. Government property includes all materials and equipment purchased directly or indirectly by any agency of the federal government for any purpose. No government property is to be removed or disposed without written permission from the AF Contract Officer Representative (COR) and

CAFB Police Department listing each item and quantity of materials and equipment to be removed or disposed.

11. SAFETY: The Contractor shall comply with UFGS 01 35 26 Governmental Safety Requirements and the

Occupational Safety and Health Administration (OSHA).

11.1. Contracts are subject to job site inspections by OSHA and Air Force representatives. In the event of a conflict between the OSHA standards and Air Force requirements, the most stringent shall apply.

Resolution of OSHA citations for violations of OSHA standards is a Contractor responsibility and shall provide for no basis of a claim against the Government.

11.2. All construction materials and equipment shall not block facility egress points and emergency vehicle access.

12. DAILY WORK SCHEDULE: Workdays shall be from 0700 to 1700, Monday to Friday. Weekend and

Federal Holiday work is not permitted unless otherwise approved by the Contracting Officer (CO). The

Contractor shall provide written notification to the Contracting Officer (CO) three (3) calendar days lead time for proposed work schedule changes. The Prime Contractor superintendent shall remain on-site at all times during construction when workers are present.

13. CONSTRUCTION WASTE MANAGEMENT: The Contractor shall recycle and/or salvage nonhazardous construction and demolition debris. Construction debris shall be contained on-site, collected and disposed off-site each day or as often as conditions dictate. The Contractor shall develop and implement a construction waste management plan that, at a minimum, identifies materials to be diverted from disposal and whether the materials will be sorted on-site or off-site. Submit the Waste Management Plan to the COR for the project record.

14. CONSTRUCTION SITE MANAGEMENT: The Contractor may limit access to the work area to contractor personnel and essential, authorized Air Force military and civilian personnel. All construction activities shall be conducted in a safe manner to minimize risk of injury or property damage.

15. WORKMANSHIP: The work for this project shall be executed in the best and most workmanlike manner by qualified and efficient tradesmen who are skilled in their respective trades. Certified journeymen in each respective trade, or apprentices under the direct supervision of certified journeymen, are required for the performance of the work of each trade.

16. COORDINATION: The Contractor shall coordinate the work of all trades in a manner that will result in a professional and expeditiously completed project. The work shall be in strict accordance with prevailing industry standards and manufacturer’s instructions.

17. ADMINISTRATION: The Contractor shall prepare submittals as required by the project specifications, the contract for construction or as requested by the COR. Submittals may include but are not limited to product data, installation and maintenance instructions, shop drawings, calculations, samples and test reports. Submittals shall be presented to the COR for review and approval at least two weeks prior to purchase or installation.

Failure to present submittals to the COR in a timely manner will not justify any claims for a schedule delay or added costs.

18. VERIFICATION OF DIMENSIONS AND CONDITIONS: The Contractor may visit the premises to become thoroughly familiar with details of the work and working conditions, verify dimensions in the field, and shall advise the COR of any discrepancies before starting the work.

19. DELIVERY AND STORAGE: All equipment and materials delivered and placed in storage shall be stored with protection from the weather, humidity and temperature variation, dirt and dust, and any other contaminants. Store all materials in a secure, clean and dry location. Contractor shall submit a location with dimensions for government approval.

20. TEMPORARY FENCE: When job site fencing is required, the Contractor shall provide temporary screened fencing around the site perimeter and site signage as determined by the Air Force and the COR.

21. CONSTRUCTION DELIVERABLES:

21.1. PROJECT SCHEDULE: Construction (AF 3064 and Gantt) – no later than one (1) week of the Notice to

Proceed for construction, the DBE shall provide a comprehensive, legible project schedule in PDF format indicating all major milestones, submittals, critical path items and the anticipated completion date for all construction activities. The Construction Schedule project schedule must be approved by the government prior to the start of construction activities, and shall be updated monthly or as events require. The schedule must be in the form of an AF 3064 and Gantt chart, and sufficiently detailed to enable tracking of construction progress and the anticipation of critical tasks.

21.2. CONSTRUCTION QUALITY CONTROL (QC) PLAN: Submit no later than one (1) week of the Notice to Proceed for construction. The quality control plan shall meet all requirement specified in UFGS 01 45

00.00 10 Quality Control.

21.3. AS-BUILT CONSTRUCTION DRAWINGS AND SPECIFICATIONS: Upon completion of construction, provide as-built drawings and specifications incorporating any departures from the original contract documents. Additionally, each page of the as-builts shall be labeled “As-Built.”

21.3.1. Electronic As-built Drawings and Specifications: Provide 2 CDs of electronic drawing files in a format compatible with AutoCAD 2010 including all external references required to view and print the drawing files properly. Include all necessary *.ctb, *.pc3 or similar files needed to properly set line weights for printing. Drawing files should be organized in general accordance with the

Department of Defense A/E/C CAD Standard (ERDC/ITL TR-12-X) available in PDF format on many federal websites. In addition, provide drawings in PDF format for on-screen viewing and reproduction. Provide final project specifications in PDF and Microsoft WORD *.doc or *.rtf format.

21.3.2. The file naming convention and file structure shown in the As-built Submittal Guidelines shall be used in lieu of what is found in the Department of Defense A/E/C CAD Standard.

21.3.3. Hardcopy As-built Drawings and Specifications: Provide two (2) full size and two (2) half size printed copies of the as-built drawings.

21.4. OPERATION AND MANTAINANCE MANUALS: Provide O&M Manuals(two (2) hard copies and two

(2) electronic copies on 2 CDs) including all warranties, vendor contacts, relevant product data including proper maintenance and operation, and pertinent shop drawings describing all installed materials, systems and equipment in the project. Manuals must be organized by CSI specification sections.

21.5. GEOGRAPHIC INFORMATION SYSTEMS (GIS): All new facilities and utilities shall be located by the

Contractor in an electronic format that is compatible with the current base GIS software and includes descriptive information required by the Air Force. When the Contractor is unable to provide GIS information as described, the Contractor shall notify the PM so that other arrangements can be made. In such cases, the Contractor is required to assist Air Force agents or personnel in the generation of GIS information if requested by the PM. Subject to change, the following are the basic requirements for GIS information to be delivered to the Air Force on CD:

21.5.1. DATUM – WGS84

21.5.2. Coordinate System – UTM

21.5.3. Units – Meters

21.5.4. Data in attribute Table will be SDSFIE version 2.6 Compliant

21.5.5. Shapefile Geodatabase format

21.6. REAL PROPERTY RECORDS: Prior to completion of the project, provide a 1354 with as much detail as possible. This document will assist the government in capturing all Real Property items into our base inventory.
