Contract Title: Medical Linen Services

1.0. DESCRIPTION OF SERVICES.

1.1. General Description. The contractor shall provide all personnel, equipment, tools, materials, supervision, other items and services necessary to perform Linen Services as defined in this performance work statement (PWS), except where specified as government-furnished property and services at Moody Air Force Base, GA. The Contractor shall perform to the standards in this contract. The estimated quantities of work are listed in Technical Exhibit 3, Workload Estimates. The Government has the right to inspect the facility prior to acceptance/award to ensure it meets the standards outlined in this PWS and the Inspection of Laundry Services Checklist.

The contractor is encouraged and expected to use innovative approaches to efficiently and effectively accomplish PWS requirements in a timely manner and in a way that fosters pride and ownership in the work performed. Contractor will pick-up soiled linen, clean and return linen to the 23d Medical Group Clinic on Moody AFB.

The lack of a properly prepared invoice will either delay or be grounds for non-payment for services. The Contractor shall provide the Government representative with a record of quantities picked up. The contractor will bill for the total quantity of company provided linen listed in Appendix B, para 1.1.10. Customer owned items will be billed by piece when turned in and cleaned.

1.2. Specific Task Requirements.

1.2.1. Pickup and Delivery Schedules.

1.2.1.1. Pickup and Delivery Points and Time. Linen Exchange, Building 900, 3278 Mitchell Blvd, Moody AFB, GA, 8:30 am or a time that is agreed upon by both parties.

1.2.2. Pickup. Weighing/counting of articles, including the laundry bags, shall be accomplished by government personnel prior to pick-up. The laundry that the Contractor shall pickup will be dirty/wet and will be in yellow impervious bags. All items will be accounted for by the contractor and weighed/counted either on site or at the facility.

1.2.3. Delivery. (Weight of Articles). The contractor shall record the pounds delivered on the delivery ticket and the government representative shall verify this amount. The Contractor shall furnish the Government with the original pickup/delivery ticket. Difference in bulk weight for laundry returned shall not exceed 10% from bulk weight recorded on the prior pickup.

1.2.4. Delivery. (Count of Articles). The contractor shall record the number of pieces delivered on the delivery ticket, and a government representative shall verify this amount. The laundry shall be clean and dry when delivered. The Contractor shall furnish the Government with the original pickup/delivery ticket. Any article(s) not delivered as scheduled shall be counted as defective. Delivery of those defective articles shall be on the next regularly scheduled delivery.

If government owned articles are lost/missing the contractor shall be responsible for replacement of those articles (replace with matching item) or reimbursement to the Government at the current replacement cost and at no additional cost to the Government.

1.2.5. Unserviceable Government Owned Articles. After laundering, unserviceable articles will be separated by the Contractor. These articles shall be bundled, wrapped in plastic, and marked unserviceable on the outside of the plastic wrapper and returned to the 23d Medical Group.

1.2.6. Damaged Government Owned Items.

1.2.6.1. Items that are identified as damaged due to the negligence of the contractor shall be repaired or replaced by the contractor. The replacement(s) or repair(s) must be acceptable to the contracting officer (CO) or contracting officer representative (COR).

1.2.7. Workmanship and Sanitation

1.2.7.1. The contractor delivered finished articles shall conform to the generally accepted industry standards of quality for cleanliness, finish and appearance.

1.2.8. Packaging.

1.2.8.1. All wrapping shall be accomplished with plastic film, unless otherwise specified in this PWS. All items on hangers shall be covered with plastic film bags. The packaging shall be as indicated in Technical Exhibit 3.

1.2.9. Vehicle Carrier Compartment. Vehicle carrier compartment requirements shall include linen carts. Vehicle carrier compartments shall be white Kemlite or other acceptable material for proper sanitation that must be performed on daily basis. Carts used for transporting linens must be sanitized on a daily basis. The aseptic technique required for daily sanitizing is a pressurized steam apparatus or manual cleaning with an Environmental Protection Agency (EPA) approved detergent germicidal.

1.2.9.1. Soiled items and clean items shall not be transported in the same vehicle at the same time. All clean items are to be covered during transit to prevent contamination. After transporting dirty/soiled laundry, the vehicle carrier compartment is to be vacuumed and sanitized with an EPA detergent germicidal agent before loading the clean articles.

1.3. Linen Services Qualifications.

1.3.1. State or Local Department of Health Requirements

1.3.1.2. Compliance with any certifications or labeling requirements arising from Federal, State, or local department of health regulations is the responsibility of the contractor. Healthcare Laundry Accreditation Council (HLAC) or Textile Rental Services Association (TRSA) Hygienically Clean Certification is mandatory.

1.3.1.3. Contaminated Linens

1.3.1.3.1. The contactor shall comply with all local health regulations and state laws pertaining to the transporting and handling of linens contaminated with contagious disease.

1.3.1.3.2. All linens will be placed in “yellow impervious bags” and handled as potentially contaminated by 23d Medical Group personnel and will be processed without individual handling by Contractor personnel when picked up.

1.3.1.3.3. The contractor shall have facilities available where goods contaminated with infectious disease can be stored and received separately from other goods in the main plant and working area of the plant. Means shall be available and utilized to remove or completely destroy all contaminants in linens during the laundering process.

1.3.1.4. Inventory Levels

1.3.1.4.1. The 23d Medical Group orders linens as needed due to storage space and will place the order at least the day prior to the scheduled delivery day. Since there are limited quantities stored in the facility, it is critical that the ordered quantities are provided on time and in the amounts requested. All dirty scrubs and customer owned articles are turned in weekly if used and will be returned clean the following week.

2.0. DELIVERY SUMMARY.

2.1. General. The Services Summary identifies the performance objectives and performance thresholds for all critical tasks associated with providing the services outlined in this PWS for the 23d Medical Group. This section contains only those items considered most important for mission accomplishment. The performance objectives are those services/tasks required in the PWS. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. The thresholds are critical to mission success. The Government expects the Contractor to perform at the acceptable levels throughout the life of the contract.

3.0. GOVERNMENT FURNISHED FACILITY, EQUIPMENT, SERVICES,

PUBLICATIONS/FORMS, AND TRAINING.

3.1. General Information. The Government will provide the following facilities, equipment, materials, services, technical order, publications and forms. If facilities, equipment, materials or services are not listed as government- furnished or government-shared, they shall be Contractor-furnished.

3.2. Government-Furnished Facility. The Government will provide the facility-- Linen Exchange, Building 900, 3278 Mitchell Blvd, Moody AFB.

3.2.1. The Government will provide adequate storage for all linen products at Building 900, 3278 Mitchell Blvd, Moody AFB GA.

3.3. Contractor-Furnished Equipment.

3.3.1. Except for those items or services specifically stated as government-furnished, the Contractor shall furnish everything needed to perform this contract according to all its terms.

3.3.1.1. Delivery Vehicle Capacity. Truck capacity for pickup and delivery must be of a size to hold at least the amount of laundry sent on a given trip.

3.3.1.2. Vehicle Interior Requirements. Vehicle interior requirements shall include vehicle interior and linen carts. Vehicle interior shall be of appropriate material for proper cleaning.

Carts used for transporting linens shall be sanitized. Soiled items and clean items shall not be transported in the same vehicle at the same time. The vehicle shall have enough hanging racks to hold finished laundry without causing wrinkling.

3.3.1.3. Laundry carts must be constructed of hard durable plastic and have large caster wheels that are lockable for proper control during transporting. Carts shall have covers for protection against contamination. They shall also be marked with their empty weight. The Contractor shall not be required to provide carts for laundry collection.

3.3.1.4. The Contractor's facility shall be equipped and ventilated to prevent dissemination of contaminants. The ventilation system should include adequate intake filtration, exchange rate, and exhaust in accordance with state and Federal requirements. The Contractor's facility shall be subject to inspection of sanitary conditions at any time by a representative( s) of the Government using the Inspection of Laundry Services Checklist.

3.3.1.5. Delivery Tickets. The tickets shall be provided with the delivery upon receipt.

3.4. Contractor Furnished Services.

3.4.1. The contractor will be responsible for pick up and drop off of clean, dry and serviceable linens to the 23d Medical Group. Articles will be free of holes, lint, odors and stains.

3.5. Government-Furnished Publications and Forms.

3.5.1. Publications. Applicable publications and forms that apply to this PWS are listed below in paragraph 3.5.3.1. The publications and forms have been coded as mandatory or advisory. The Contractor is obligated to follow those publications and use those forms coded as "mandatory" to the extent specified in other sections of this PWS. The Contractor shall be guided by those publications or use those forms coded "advisory" to the extent necessary to accomplish requirements in this PWS. All publications and forms listed shall be provided by the Government at the start of the contract. The Government will provide follow-on requirements to the Contractor when changes occur. Supplements or amendments to listed publications from any organizational level may be issued during the life of the contract.

3.5.2. The Contractor shall immediately implement those changes that result in a decrease in the contract price and notify the Contracting Officer (CO) in writing of such change. Should a decrease in contract price result, the Contractor shall provide a proposal for a reduction in the contract price to the CO. Before implementing any change that will result in an increase in contract price, the Contractor shall submit to the CO a price proposal within thirty (30) calendar days following receipt of the change by the Contractor. The CO and the Contractor will negotiate the change into the contract under the provisions of the contract clause entitled "Changes."

3.5.4. Operating Instructions. Operating instructions and all forms/publications will be available to contract personnel. Initial familiarization with the local installation/organization will be provided.

3.6. Contractor Training.

3.6.1. Employee Training. Training shall be accomplished for all individuals working with or around medical linens. This training shall consist of Infection Control, Safety, proper handling of hospital linens, and contamination of linens. This training shall be documented covering the following topics:

duration of training, names of trainers, and names of attendees (signatures required).

4.0. GENERAL INFORMATION.

4.1. Contractor Personnel.

4.1.1. Contract Manager. The Contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of the contract manager and who shall act for the Contractor when the manager is absent, shall be designated in writing to the Contracting Officer (CO) prior to contract start date. The contract manager may also serve as a Contractor employee.

4.1.1.1. The contract manager shall have full authority to act for the Contractor on all contract matters relating to the daily operation of this contract.

4.1.1.2. The contract manager, alternate(s) and employee performing services shall be able to read, write, speak, and understand English.

4.1.2. Contractor Employees. The CO may restrict the employment under the contract of any contractor employee or prospective contract employee who is identified as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.

4.1.2.1. Contract personnel shall present a neat and professional appearance.

4.1.2.2. The Contractor shall ensure that employees have current and valid professional certifications before starting work.

4.1.2.3. The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person could create a conflict of interest, nor shall the contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with DOD 5500.7-R Joint Ethics Regulation (JER), Chapter 2. In addition, the Contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in AFI 64-106, Air Force Industrial Labor Relations Activities, paragraph 3.

4.1.2.4. The Contractor is cautioned that off-duty active military personnel hired under this contract may be subject to permanent change of station (PCS), change in duty hours, or deployment. Military Reservists and National Guard members may be subject to recall or active duty. The abrupt absence of these personnel could adversely affect the contractor’s ability to perform. Their absence at any time shall not constitute an excuse for nonperformance under this contract.

4.1.2.5. The Government reserves the right to conduct site visits in all Contractor-operated facilities in conjunction with the solicitation of offers for any follow-on contract. In the event the follow-on contract is awarded to other than the incumbent, the successor or Contractor is responsible for obtaining all information from the incumbent Contractor, necessary to permit a smooth transition without an interruption in service during the transition period. Additionally, the incumbent Contractor shall cooperate to the extent required to permit an orderly changeover to the successor Contractor. With regard to the successor Contractor's access to incumbent employees, a recruitment notice may be placed in each facility.

4.2. Quality Control.

4.2.1. The Contractor shall provide a comprehensive Quality Control Plan (QCP) for all work required in this PWS. The written QCP shall be submitted to the CO for validation no later than the pre-performance conference. The CO will notify the Contractor of validation or of any modifications that may be required to be made to the plan.

4.2.1.1. The plan shall include, at a minimum:

4.2.1.1.1. A description of the inspection system to cover all services listed on the Services Summary (SS). The description shall include specifics as to the areas to be inspected on a

Contract Title: Medical Linen Services scheduled and unscheduled basis, frequency of inspections, submission of QC schedule and the title and organizational placement of the quality control inspectors (QCIs). Additionally, control procedures for any Government-provided keys or lock combinations shall be included.

4.2.1.2. A description of the methods to be used for identifying and preventing defects in the quality of service performed.

4.2.1.3. A description of the records will be kept to document inspections and corrective or preventive actions taken. This includes the use and distribution of required forms and reports.

4.2.1.4. The records of inspections shall be kept and made available to the Government throughout the contract performance period and for the period after contract completion until final settlement of any claims under this contract.

4.3. Quality Assurance Program.

4.3.1. The Government’s COR may evaluate the Contractor’s performance at any time during the duration of this contract. Inspections will be conducted with the intent of verifying compliance with contract requirements and performance within the performance thresholds included in the SS. The COR will record all surveillance/assessment observations. When observation indicates defective performance, the COR will require the contractor to initial the observation. This initialing of the observation(s) does not necessarily constitute concurrence with the observation, only acknowledgment that the Contractor’s representative has been made aware of the defective performance. If performance defects are identified and validated by the COR, the Contractor shall be provided the opportunity to re-perform.

Disputes between COR and Contractor personnel over performance standards shall be referred to the CO for final determination. The Government reserves the right to inspect any tasks on the contract, even if not shown on the SS.

4.4. Reperformance.

4.4.1. Reperformance of Service. When service performed does not conform to contract requirements, the Contractor shall reperform the service in conformity with the contract specifications at no additional cost to the Government. Reperformance of the service(s) shall commence immediately upon notification by the COR, and the Contractor shall have a sufficient workforce dedicated to ensure corrections are accomplished by noon the next day without degradation to standard services.

4.4.2. Acceptable Performance. Acceptable performance is achieved when the Contractor’s performance meets the minimum the contract standard(s).

4.4.3. Unacceptable Performance. Unacceptable performance exists when the Contractor does not meet the minimum contract performance standard(s).

4.4.3.1. When the Contractor’s performance is unacceptable, the COR will determine the cause

Contract Title: Medical Linen Services of the unacceptable performance. If any Government action, or lack of action, caused or contributed to the unacceptable performance, the unacceptable performance will not be counted against the Contractor. The COR will take action to ensure Government action, or lack of action, does not interfere with the Contractor’s performance in the future. The COR will completely document the circumstance accordingly.

4.4.3.2. When the unacceptable performance is not the result of Government action, or lack of action, COR shall promptly notify the Contractor and may issue a *Corrective Action Report (CAR) to the Contractor to assure corrective action is taken. By initialing and dating the CAR, the Contractor representative is acknowledging that he/she has been informed of the unacceptable performance and is not necessarily agreeing with the performance assessment. The COR shall direct the Contractor to re-perform the service, if possible, without additional cost to the Government. If the Contractor challenges the validity of the COR unacceptable assessment finding, and the COR and Contractor cannot come to an agreement, the matter shall be referred to the CO for resolution. The Contractor shall be required to return all completed CARs to the COR no later than the suspense date indicated on the CAR.

*Note: The COR should discuss with the CO before issuing a CAR.

4.4.3.3. Unacceptable performance re-performed by the Contractor and acceptable to the COR shall not be considered a defect. However, unacceptable performance not corrected or re-performed in the timeframe required and still considered unacceptable by the COR will be considered “a defect”.

4.4.3.4. Procedures for performance that does not meet contract standards shall be as cited in accordance with FAR 52.212-4, Contract Terms and Conditions—Commercial Items (a) Inspection/Acceptance. The Contractor shall only tender for acceptance those items that conform to the requirements of the contract. The Government reserves the right to inspect or test any supplies or services that have been tendered for acceptance. The Government may require repair or replacement of nonconforming supplies or reperformance of nonconforming services at no increase in contract price. If repair/replacement or reperformance will not correct the defect(s) or is not possible, the Government may seek an equitable price reduction or adequate consideration for acceptance of nonconforming supplies or services.

4.5. Hours of Operation. The Contractor shall perform the services required under this contract as follows:

4.5.1. Standard Base Hours. Base hours of operation are 7:30 am-4:30 pm (0730-1630) , Monday through Friday, excluding those holidays, as listed below. The contractor will perform services 1 day a week. The day will be established and agreed upon by both parties. Linens must be delivered between 7:30 am-11:00 am (0730-1100) during normal operating hours.

4.5.2. Place of Performance. Medical Treatment Facility, Bldg 900, 3278 Mitchell Blvd, Moody

AFB, GA, 31699.

4.5.3. Holidays.

4.5.3.1. If a holiday falls on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed.

4.5.3.2. If a holiday falls on a scheduled service day, the Contractor will be responsible for rescheduling services for the first work day following the holiday observance.

4.5.3.3. Base Down Days and/or Goal Days. The Contractor shall coordinate with the COR and CO regarding these days.

4.5.3.4. Base Closures. Work scheduled, but not accomplished, because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.

4.6. Security Requirements.

4.6.1. Pass and Identification. Pass and identification items, required for contract performance, shall be obtained for Contractor personnel.

4.6.2. The Contractor shall obtain personnel pass application forms from the Contracting Office’s Contract Administrator. The Contractor employee shall complete and carry the forms to the Security Forces, Pass and Registration Office.

4.6.3. Retrieving pass and identification items shall be accomplished by the contract manager and given to the Contractor Officer Representative upon the departure from employment, prior to contract expiration, of any Contractor employee and turned in to the Visitors’ Center, Pass and Identification Office.

4.6.4. Traffic Laws. The Contractor and its employees shall comply with base speed limits and other traffic laws.

4.6.5. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their Contractor-owned

4.6.6. Physical Security. The Contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

4.6.7. Controlled Areas. The Contractor shall implement local base procedures for entry to Air Force controlled areas where Contractor personnel shall work.

4.6.8. For Official Use Only (FOUO). The Contractor shall comply with DoD 5200.1R, Appendix C, Section 2, Information Security Program requirements. This regulation sets policy and procedures for marking, handling, transmitting, and safeguarding FOUO material.

4.6.9. Keys. The Contractor shall prohibit the use of keys issued by the Government by any persons other than the Contractor's employees and the opening of locked areas by Contractor employees to permit entrance of persons other than Contractor employees engaged in performance of contract work requirements in those areas.

4.7. Safety Requirements.

4.7.1. Safety of personnel and performance shall conform to Office of Safety and Health Administration (OSHA) requirements as specified in AFI 91-203, Air Force Consolidated Occupational Safety Instructions.

4.7.2. Cell Phone Usage. Cell phone use is prohibited while driving on base, unless a completely “hands-free” device is utilized.

4.7.3. Smoking Policy. All Contractor employees shall comply with Air Force smoking policy (AFI 40-102, Section 2), which states smoking is not permitted in any DoD facility.

4.8. Continuation of Essential Contractor Services During Crises.

4.8.1. Services.

4.8.2. All services under this PWS shall be performed during any wartime operations. Wartime operations are those actions, including contingency planning, which would be required to support current or future United States Air Force wartime requirements. The CO or the COR shall notify the Contractor of a crisis by any means of communication. There shall be no costs paid by the Government to the Contractor during a crisis except those incurred for work performed under this contract. Notwithstanding any other provision of this contract, the contractor agrees that, in the event of the occurrence of any wartime-related contingency, the Government shall have the unilateral right to extend performance of this contract as long as the wartime contingencies exist. Contract payment will be in accordance with the existing contract rate.

4.8.3. The Government will notify the Contractor within 48 hours prior to such extension of performance hours during a crisis.

4.8.4. Contract payment shall be in accordance with the existing contract rate.

4.8.5. The Contractor providing services designated as “essential” by Moody AFB is expected to use all means at their disposal to continue to provide such service in accordance with the terms and conditions of the contract during periods of crisis until appropriately released or evacuated by the CO.

4.8.6. The Contractor is responsible for providing the requested information as required in the DFARS Clauses 252.237-7023 and 252.237-7024 of this contract.

4.9. Listing of Employees.

4.9.1. Listing of Employees. The Contractor shall maintain and provide to the CO and COR a current listing of employees. The list shall include the employee's name, social security number, and type of investigation if contract work involves unescorted entry to Air Force restricted areas or other sensitive areas designated by the Installation Commander. An updated listing shall be provided when an employee's status or information changes.

4.9.2. Key Personnel. The Contractor shall replace personnel without interruption of services required by this contract.

4.10. Government Performance of Services During Labor Strikes.

4.10.1. The services called for under this PWS, and at the direction of the CO, the Contractor agrees to remove its non-striking force from the performance site and not to interfere in any way with Government performance.

4.10.2. Contractor further agrees under such circumstances to permit the Government to use any essential Contractor-furnished property. The Government will equitably compensate the Contractor for use of such property.

4.10.3. Contractor shall not be entitled to payment for any performance period or parts thereof during which the Government assumes performance.

4.11. Records Management.

4.11.1. All records, files, and documents, regardless of media (e.g., paper, electronic, etc.), as described in this PWS, which are the responsibility of the Contractor are the property of the Government and shall remain so upon termination or completion of the contract.

4.11.2. The Contractor shall keep these item(s) current and maintain and dispose of them in accordance with the requirements established in AFI 33-322, Records Management Program, AFMAN 33-363, paragraph 6.4, AFMAN 33-364, paragraph 2.10 and the Air Force Records Disposition Schedule.

4.12. Contractor Manpower Reporting.

4.12.1. The Contractor shall report all Contractor labor hours (including subcontracting labor hours) required for performance of services provided under this contract for the Refuse Services

Contract Title: Medical Linen Services at Moody AFB via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address: https://www.ecmra.mil/. Reporting inputs will be for the labor executed during the period of performance each government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2020. Contractors may direct questions to the help desk at https://www.ecmra.mil/.

4.12.2. Reporting Period. Contractors are required to input data by 31 October of each year.

Example: FY15 ends on 30 September 2015. Manpower Reporting inputs for FY15 is due no later than 31 October 2015.

4.12.3. Uses and Safeguarding of Information. Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.

4.12.4. User Manuals. Data for Air Force Service requirements must be input at the Air Force Enterprise Contractor Manpower Reporting Application (ECMRA) link. However, user manuals for government personnel and contractors are available at the Department of Defense ECMRA link at http://www.ecmra.mil.

APPENDIX A - DEFINITIONS

GENERAL DEFINITIONS.

Contingency - An emergency involving military forces caused by natural disasters, terrorists, subversives, or by required military operations. Contingencies require plans, rapid response, and special procedures to ensure the safety and readiness of personnel, installations, and equipment.

Contingency Operations - Operations involving the use of US military forces to achieve US objectives, usually in response to an emerging or unexpected crisis. Contingency operations may evolve into sustained military operations.

Contracting Officer (CO) - The only person who can bind, limited to the extent of the authority delegated by the head of the agency, through a warrant or certificate of appointment. The CO enters into, administers, or terminates contracts and makes related determinations and findings.

Contracting Officer's Representative (COR) - Individuals designated to perform quality assessment functions, and oversee performance in accordance with the Quality Assurance Surveillance Plan (QASP). They serve as on-site technical managers assessing contractor performance against contract performance standards.

Defective Service - A service output that does not meet the standard of performance specified in the contract for that service.

Functional Commander/Functional Director (FC/FD) - The person responsible for a functional area at the Squadron level. For example, the Civil Engineer Squadron Commander is the Functional Commander for the Civil Engineering performance-based work statements.

Lot - The total number of potential service outputs in a surveillance period.

Market Research - A continuous process for gathering data on service characteristics, suppliers' capabilities, and current business practices plus the analysis of that data to make acquisition decisions. Multi-Functional Team (MFT). A team of stakeholders responsible for a service acquisition throughout its lifecycle, to include representatives of the functional and procurement communities, as well as the contractor after contract award.

Performance Management - The use of performance assessment information to effect positive change in organizational culture, systems, and processes, by helping to set agreed upon performance goals, allocating and prioritizing resources, informing managers to either confirm or change current policy or program directions(s) to meet those goals, sharing results of performance in pursuing those goals.

Performance Objective - The outcome associated with successful contract performance in a specific area. This is a critical success factor in achieving the organization's mission, vision and strategy, which, if not achieved, would likely result in a significant decrease in customer satisfaction or risk mission failure.

Performance Threshold - The minimum standard for a performance objective required by the Government.

Quality Assurance - A process that measures success towards achieving defined performance goals or objectives defined within the performance thresholds in the services summary or the process of assessing progress towards achieving the objectives/goals developed in a Quality Assurance Surveillance Plan (QASP).

Quality Assurance Program - A quality assurance program that is developed, planned, and managed to carryout cost-effectively, all efforts to affect the quality of services. The Performance Management Assessment Program includes performance measurement and performance management by the multi- functional team (MFT).

Quality Control - Those actions taken by a Contractor to control the production of outputs to ensure that they conform to the contract standards/requirements.

Random Sampling - A sampling method where each service output in a lot has an equal chance of being selected for quality assurance surveillance.

Sample - A sample consists of one or more service outputs drawn from a lot for quality assurance surveillance.

Services Summary (SS) - This table lists the performance objectives, the performance thresholds (standard(s) and the paragraph in the PWS where it can be located. These are the mission essential requirements for that particular service.

TECHNICAL DEFINITIONS.

Clean - Finished articles are dry and free of visible soil, lint, and objectionable odors.

Spots and stains will be removed without damaging fabric.

Contaminated Linens - The linens originating in isolation rooms, or other areas of the Medical Treatment Facility (MTF) which are soiled and/or contaminated with infectious micro-organisms. Damaged Items. Items rendered unserviceable due to contractor processing.

Laundering - The process by which soiled fabric items are cleaned and treated. The term includes cleaning by mechanical action or by hand in water solutions of detergents and other additives. For the purpose of this contract, the term includes repellent treating, bleaching, blurring, chemical rinsing (sour), starching, ironing and pressing operations as appropriate to the type of fabric and nature of soil. (Reference A, Guidelines for Environmental Infection Control in Healthcare Facilities, Chapter 3).

Lost/Missing Articles - Any articles not delivered on the regularly scheduled delivery date or the next scheduled delivery.

Major Repairable Items - All articles that require repairs of 4" or greater, and/or zipper replacements as defined in Schedule B.

Maximum Shrinkage Factor - Factors which are computed by comparing total pounds for the month applied to the total number of pounds delivered. The maximum shrinkage factor permissible in the process of performing the laundry for this PWS is 10%.

Minor Repairable Items - All articles that are serviceable. Those articles with minor (3 inches or less) rips, tears, open seams, zipper repairs and holes, loose or missing buttons and ties.

Pressed - Finished articles shall have no puckering around the stitching lines, and no wrinkles running the length of the front, back or sleeves.

Reimbursable Repairs - All major and minor repairable items that are separately priced as line items in bid schedule.

Unserviceable Articles - Those articles that are worn beyond economical repair.

Water Repellent Treatment - Treatment by hand or machine of fabric items with either

1.1.3. Each item is identified as well as estimated number of articles to be processed for the month.

1.1.4. The items in 1.1.10. will not be combined when delivered. Like items will be packaged together and kept separate from unlike items.

1.1.5. Surgical linen will be separately packaged from white linen. This also includes all surgical scrubs and other surgical linens.

1.1.6. All linen will be wrapped with a transparent protective material. The material will be sufficiently durable to withstand normal handling, storage, and transportation.

Wrapping shall be done in such a manner as to not perceptibly increase the bulk or size of the package. Wrapping shall be easily removable and easily compressed to eliminate bulky disposal.

1.1.7. Packages wrapped in accordance with paragraph 1.1.2. above need not be marked with the name of the contents and the item count, with the exception of those packages containing less than the specified quantity or packaged in a quantity determined by the Contractor, which shall be marked with the item count.

1.1.8. To facilitate counting, items will be folded and packaged with round edges (fold) in one direction.

1.1.9. All finished items (laboratory coats and scrubs) will be on hangers and covered with transparent protective material.
