23RD FORCE SUPPORT SQUADRON
STATEMENT OF WORK (SOW)
HUMAN PERFORMANCE FITNESS
Dated: 20 November 2018
23rd Contracting Squadron
Moody AFB GA
1. DESCRIPTION OF SERVICES.
1.1. SCOPE: The Contractor shall provide all qualified personnel required to instruct CrossFit type classes to support the 23d Force Support Squadron (23d FSS) as defined in this Statement of
Work (SOW). For the purpose of this SOW, classes are defined as one (1) hour sessions; three
(3) sessions per day for a Monday-Friday (excluding Federal Holidays and ACC days) will be offered during this contract period.
1.2. LOCATION: Sessions are to be performed at Moody AFB, Freedom 1 Fitness Center, 4851
Burrell St, Bldg 404, Moody AFB, GA 31699.
2. PERSONNEL.
2.1. Contractor (s) is reminded that off-duty active duty military personnel hired may be the subject to permanent change of station, change of duty hours, or deployment. Their absence does not constitute a valid excuse for nonperformance under this agreement.
3. CONSERVATION OF UTILITIES.
3.1. Contractor (s) shall instruct their employees in the utilities conservation practices and shall be responsible for operating under conditions, which preclude the waste of government utilities and resources.
4. GOVERNMENT FURNISHED UTILITIES AND SERVICES.
4.1. The government shall provide, without cost to the contractor, the utilities necessary to accomplish the task (s) specified in this Statement of Work, fire protection, and 911 services.
4.2. The government shall provide a designated area at the fitness center suitable for group exercise instruction. In addition, the government shall provide materials and equipment necessary to conduct sessions (free-weights, dumbbells, mats), when government funds are available. Any needs which are not met and for which the government funds are not available are the responsibility of the contractor and their employees.
5. GERNERAL REQUIREMENTS.
5.1. SPECIFIC TASKS
5.1.1. The Contractor (s) shall accomplish the following:
5.1.2 Contractor must maintain CrossFit affiliation. Copy of the license agreement must be made available to the Fitness Center Director (FCD) or Designee and/or Contracting Officer upon request.
5.1.3. All contractors/instructors shall present a neat appearance and be easily recognized. Proper athletic attire will be worn at all times. Attire will be in good taste and subject to the approval of the Quality Assurance (QA)/ Contract Officer Representative (COR).
5.1.4. Contractor (s) shall provide input on a master program; on a monthly basis depicting group exercise instructor’s name and type of session. The master program must be submitted to the
Fitness Center Director (FCD) or Designee at least 10 days prior to the effected month.
5.1.5. Contractor (s) must maintain a file on each instructor, which includes a copy of his or her
Certification, and Cardiopulmonary Resuscitation/ Basic Life Support (CPR/BLS) and first aid card. These files must be made available to the FCD or his Designee and/or Contracting Officer upon request.
5.1.6. Contractor (s) must provide instruction for special events, such as events similar to the
CrossFit Games, any holiday themed aerobic marathons and specific aerobic seminars during any given month. The FCD or Designee shall coordinate with contractor at least one month in advance to express any special needs for the coming month.
5.1.7 Contractor (s) shall ensure each instructor incorporates standardized safe warm-up, stretching, and exercises techniques. A minimum of two (2) target heart rate checks will be performed during session as well as a safe cool down period to conclude the cardio segment of each session. Instructors will provide specialized instruction for students of each session.
Instructors will provide specialized instruction for students to demonstrate beginner and advanced techniques/options/workouts.
5.1.8. Contractor (s) will conduct an annual or semi-annual survey of session participants to collect feedback on quality of instructors and instruction given during the preceding period.
Survey materials shall be provided to the contractor. Results of survey shall be summarized and submitted to the Fitness Director or Designee upon demand.
5.1.9. Contractor (s) shall conduct monthly communication with all instructors to keep them informed of facility/base policy changes or any other information needed to ensure a quality program. This will be at no cost to the government.
5.1.10. Contractor (s) shall provide and ensure instructors maintain an instructor sign-in log.
Instructors must sign-in and out prior to leaving the facility.
5.1.11. Contractor (s) shall provide and ensure instructors maintain a daily attendance log, which includes his/her name, date and session time. As well as a breakdown of active duty, 3 dependents, Weight and Body Fat Management Program, Fitness Improvement Program; self-paced and monitored personnel attending sessions. A roster will be provided by the Fitness
Center. Rosters will be maintained on the sign-in area in the group exercise room. This log must be available to the FCD or Designee upon request.
5.1.12. Contractor (s) will consolidate daily attendance rosters and customer surveys into a one-month report, and shall provide this information to the FCD at the end of each month.
5.1.13. Contractor (s) shall ensure instructors arrive at least 10 minutes prior to their scheduled session time to prepare to instruct.
5.1.14. Contractor (s) shall ensure instructors are aware that they must notify the on-duty fitness center staff of any injuries or accidents associated with the group exercise customer. Should ambulatory services be needed, the fitness center staff shall notify the proper entity for assistance. If the fitness center staff is not readily available, the group exercise instructor has the authority to call 911.
5.1.15. The government will provide emergency medical treatment and emergency patient transportation service for contractor personnel who are injured or become critically ill while at
Moody AFB. Treated personnel will be responsible for reimbursing the government for the cost of medical treatment and patient transportation at the current inpatient or outpatient treatment rate.
5.1.16. Contractor (s) will ensure instructors use a sound system and microphone and be familiar with their operation. Sound system and microphone will be provided by the government.
5.1.17. Contractor (s) will ensure an instructor will be provided for all scheduled sessions or a qualified substitute; if the primary instructor cannot teach. Unless for a bonafide emergency, the
FCD or Designee must be notified one day prior of a substitute instructor.
5.2 QUALIFICATIONS.
5.2.1. Contractor (s) will recruit and schedule qualified instructors who possess, at a minimum
1 year experience instructing groups 2-8 students and have one of the following national certifications-CrossFit Coach Level 1 and/or 2 course certifications. In addition, contractor will have liability insurance and will ensure all instructors possess a current BLS or CPR card. All certifications must be valid prior to instructor beginning sessions and must remain in effect throughout the period of performance.
5.2.1. Contractor (s) shall provide a one-hour Introduction to CrossFit type class. The intent/purpose of this class should be to introduce, the individual who is completely unfamiliar with exercising, to the CrossFit type culture, healthy culinary/nutrition tips, terminology, form, basic Olympic lifts, current state of CrossFit type sports in the sports, and avenues to graduate to the regular classes.
5.3. HOURS OF OPERATIONS.
5.3.1. Contractor (s) shall ensure that certified group exercise instructors are available to conduct scheduled sessions as specified. Monday through Friday; “subject to scheduling of the
Government”. NOTE: Session schedule is subject to change as needed based on patron/program requirements.
5.3.2. Contractor (s) must inform FCD or designated representative of anticipated absences at least forty-eight (48) hours prior to first schedule session. No other absences are permitted.
5.3.3. The following Holidays and ACC Family Days will be observed by the fitness center, and
NO sessions will be scheduled:

Memorial Day
Fourth of July
Labor Day
New Year’s Day
6. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE
NATIONAL COMMAND AUTHORITY.
6.1. During times of crisis or national emergency, the contractor (s), shall not continue to provide services as scheduled.
