PERFORMANCE WORK STATEMENT (PWS)
FOR
GROUNDS MAINTENANCE SERVICES
1. DESCRIPTION OF SERVICES. The Contractor shall provide all personnel, equipment, tools, supervision, and other items and non-personal services necessary to ensure that grounds maintenance is performed at Moody Air Force Base (MAFB) and Grassy Pond Recreational Annex in a manner that will maintain healthy grass, shrubs, trees and associated landscaping resulting in a professional appearance in accordance with this PWS. The Contractor shall accomplish all grounds maintenance tasks identified in this PWS, in order to meet the requirements in the Services Summary (SS). Base maps of areas maintained are provided in Appendix A, Moody AFB Grounds Maintenance Map. Specific grounds area measurements are established in Appendix B-1, Base Grounds Maintenance Estimated Quantities and Appendix B- 2, Hospital Grounds Maintenance Estimated Quantities. All work performed by the Contractor shall be performed in accordance with all applicable laws, regulations, standards, instructions, and commercial practices. Application of non-selective herbicides to control grass, weeds, and vegetation around obstacles, to include fence lines, shall only be allowed with Contracting Officer Representative (COR) approval. The Contractor shall apply Integrated Pest Management or government approved alternate control measures (mechanical removal, heat) or other management strategies to maintain healthy turf. Application of herbicides shall comply with all provisions in this PWS and paragraph 1.7. Adjuvants and dyes may be used to enhance and track chemical applications. All damage caused by the Contractor or its employees to government property (to include personal property of base tenants and employees), trees and shrubs due to Contractor performance of mowing and trimming operations shall be repaired or replaced by the Contractor within 15 days after identification/notification at no additional cost to the Government.
1.1. Maintain Grounds. The Contractor shall provide services described below for all grounds maintenance as identified in appendices A, B-1 and B-2. MAFB and Grassy Pond Annex grounds include turf/grass, bushes, shrubs, ornamental trees, special landscape beds, trees, ditches, slopes, berms, hills and irrigation systems which will be maintained according to the assigned category and zone as indicated in appendices A, B-1 and B-2.
1.1.1. Mowing General Instructions. Prior to beginning of each mowing, trimming and edging operation, all tree limbs, branches, pine cones, litter, and other trash or debris shall be picked up and disposed of properly. Mowing, edging and trimming operations shall be performed concurrently. The Contractor shall dispose of collected natural woody debris or leaves and other organic waste material at the base’s natural debris collection area or other area/container as designated by the Contracting Officer's Representative (COR). Man-made trash and/or debris shall be disposed of in the nearest government provided refuse dumpster. Ditches in all areas, except unimproved grounds, shall be mowed and maintained free of shrubs, trees, trash and debris in order to keep water free flowing. Mowing shall be free of scalping, rutting, and uneven or rough cutting. No loose vegetation or debris created from operations shall be left on paved areas. Loose vegetation, debris and/or Foreign Objects (FO) created from operations shall be immediately removed from paved areas of the semi-improved-airfield area.
1.1.1.1. Improved Grounds Turf; Base/Grassy Pond/Hospital. Mowing Height: two (2) inches - four (4) inches. Grass shall be mowed every 14 days in the months of March and November and every 7 days April through October.
1.1.1.2. (OPTION) Distinguished Visitor (DV)/Special Cuts. This option will only be executed as funding becomes available.
Contractor shall mow, trim and edge specific areas identified by COR in support of DV visits or other special wing functions. Contractor shall be provided up to 7 days notice for acreage increased up to 20 acres and up to 14 days for increased acreage from 21-50 acres. Specific areas and acreage to be mowed will be discussed with contractor prior to work being accomplished.
Performing DV/special cuts shall not prevent contractor from performing or completing other work as described in the PWS.
1.1.1.3. (OPTION) Non-Turf Areas. This option will only be executed as funding becomes available.
Maintain Low Crushed Rock Beds/Areas in improved grounds to present a neat and professional appearance. Maintenance activities include weeding, dead vegetation and litter removal. The contractor may use mechanical, chemical or physical methods of weed control that do not degrade the quality of the mulch surfaces. Dyes may also be used, sparingly, to enhance and track applications. Application of herbicides shall comply with all provisions in PWS paragraph 1.7.
1.1.1.4. Semi-Improved, Non Airfield. Mowing Height: four (4) inches, one (1) time per month (including drainage ditches).
1.1.1.5. Semi-Improved, Airfield. Runway/Taxiway/Aprons, Edge Lights, Signs, Airfield.
Mowing Height: seven (7) inches, one (1) time per month (including drainage ditches). The perimeter of the airfield, runways, aprons, and taxiways shall be mowed to a height of two (2) inches ten (10) feet from edge of the paved surfaces as well as a (10) foot diameter around signs and obstructions, one (1) time per month. Airfield mowing is considered an essential service and requires the Contractor to provide plans to continue service should a national emergency or contingency situation be declared; reference paragraph 4.4
1.1.1.5.1. Airfield Coordination. The Contractor shall coordinate all activities on the airfield by contacting Airfield Management. Flight-line driver’s training must be completed prior to operating vehicles, tractors, or mowers on the flight line. All employees working on airfield grounds shall be required to have an AF Form 483, Competency Card (flight-line driver’s license) with them at all times. Vehicles/equipment must have a valid POV pass issued by Airfield Management and displayed on the vehicle prior to operating on the flight line. No equipment shall be left unattended while working in these areas. The Contractor shall be responsible for ensuring that all foreign objects and debris produced by Contractor grounds maintenance is immediately removed from taxiways, runways, and aircraft parking aprons.
1.1.1.5.2. Bird(s). Birds following or attracted to the mowing operations, shall immediately be reported to the Chief of Airfield Management Representative, i.e., Base Operations, via the “Ramp Net.” When required by the airfield manager, due to imminent bird hazards, the Contractor shall move to a different location on the airfield or cease operations and remove mowing equipment from the airfield environment.
1.1.1.5.3. Airfield Tower Communications. When operating near runways, overruns, and taxiways, the Contractor shall be radio-equipped to maintain constant two-way radio contact with the control tower and Airfield Management. Only those employees whose license is marked "Controlled Movement Areas" shall operate vehicles, tractors or mowers within 150 feet of the runways or 'inside' the controlled movement areas to include pedestrian trimmer operators. The Contractor shall maintain contact with the control tower when operating on or within 150 feet of the runway. The Contractor shall respond immediately to the directions of the Air Traffic Control Tower Personnel. The government will provide radios to the contractor for use while performing airfield mowing operations.
1.1.1.6. (OPTION) Maintain Un-Improved Grounds. This option will only be executed as funding becomes available.
Grass, weeds and vegetation shall be maintained to prevent woody encroachment in un-improved grounds as identified in Appendix A. Where drainage ditches exist, mow to the top edge of the ditch.
1.1.2. Trimming/Edging General Instructions. During each trimming operation, the Contractor shall trim grass, weeds, and vegetation concurrently with mowing to match surrounding area grass height, as cited in paragraphs 1.1.1.1., 1.1.1.2., 1.1.1.4., and 1.1.1.5. All areas not identified as special beds shall be trimmed to match the surrounding grass height. Trimming includes, but is not limited to, around all fixed obstacles such as buildings, curbs, trees, shrubs, fences, poles, posts, fire hydrants, parking lot bumper blocks, and boulders, etc. Finished trimming height shall match or be less than surrounding area grass height. Grass at asphalt paved edges shall be maintained to prevent vegetation encroachment onto paved surface. Aircraft E-5 barriers shall be trimmed during each trimming cycle.
1.1.3. Edging General Instructions. Frequency: one (1) time per month. Sidewalks, driveways, curbs, patios, pavilions, and other concrete edges or obstructions shall be edged no more than 1/2-inch from the hard surface edge, maintaining an even contour and uniform appearance with the edge surface. Grass, vegetation, and dirt from edging operations shall not be left on paved surfaces.
1.2. (OPTION) Fertilization. This option will only be executed as funding becomes available.
The Contractor shall have a pH adjustment and fertilizer application program for all improved grounds turf. The type and amount of fertilizer or amendments applied shall be based on the needs of the individual types (warm season grasses, cool season grasses) of turf to maintain
Contract Title: Grounds Maintenance Services optimal root growth. Chemicals procured for fertilization shall be approved by the base HAZMART as described in paragraph 1.8. (inclusive) prior to delivery to MAFB.
1.3. Leaf Removal - Hospital. Leaves, sticks, branches, tree debris, and other natural materials shall be removed from improved hospital grounds. The Contractor shall dispose of collected natural wood and organic waste material at the base’s natural debris collection area or other area/container as designated by the COR.
1.3.1. (OPTION) Leaf Removal - Main Base Grounds. This option will only be executed as funding becomes available
Leaves, sticks, branches, tree debris and other natural materials shall be removed from improved grounds. Sparse amounts of leaves may be mulched in place if the residual mulch does not excessively cover turf. The Contractor shall dispose of collected material at the base’s natural debris collection area or other area/container as designated by the COR.
1.4. General Instructions, Special Landscaped/Flower/Rock Beds. Maintain beds located in Appendix B-2 to present a manicured appearance. Beds can include shrub beds, rock beds, hedge beds, flower beds, or a mixture of all types of vegetation arrangements to include individual trees. Maintenance activities include pruning shrubs, hedges and trees, trimming flowers, adding soil amendment/fertilizing, pine straw maintenance, watering, weed eradication, trash/debris removal, and replacement of plants and/or shrubs. Prune shrubs, hedges, and trees, in their entirety, to maintain their natural growth characteristics, existing shape (when possible), and health of the plant. Minimum clearances for trees are: 14 feet over streets, 12 feet over driveways, 8 feet over walk areas, 4 feet over buildings, and 1 foot from buildings. Prune shrubs to maintain a minimum of twelve (12) inches from buildings and other obstructions. Shrubs which are in front of windows shall be pruned to the lower edge and/or sides of the window opening. Maintain the existing shape and form (when possible) of the shrubs by removing new growth. Prune entire shrubs and hedges to maintain balance and their natural mature form. The Contractor may use mechanical, chemical, or physical methods of weed removal that do not degrade the quality of the mulched surfaces. Mulched areas may be refreshed using pine straw collected during leaf removal. Dyes may be used to enhance and track herbicide applications.
Application of herbicides shall comply with all provisions in this PWS.
1.4.1. (OPTION) Special Landscaped/Flower/Rock Beds - Main Base, Grassy Pond.
Frequency: Upon execution of this OPTION, performance in the main base’s identified special landscaped beds will occur on a monthly basis.
1.4.2. Special Landscaped/Flower/Rock Beds - Hospital. Frequency: Hospital campus special landscaped beds shall be maintained on a monthly basis.
1.5. (OPTION) Prune Shrubs - Main Base/Grassy Pond Annex Grounds. This option will only be executed as funding becomes available.

Prune shrubs in main base improved grounds not included in PWS paragraph 1.4 and 1.4.1 to maintain their natural growth characteristics, existing shape, and health of the plant and to promote safety and security. Maintain the existing shape and form of the shrubs by removing new growth. Prune hedges to maintain their natural mature height. Prune shrubs to maintain a minimum of twelve (12) inches from buildings and other obstructions. Shrubs which are in front of windows shall be pruned to the lower edge and/or sides of the window opening. Foreign weeds, shrubs or trees shall be removed to the ground during pruning operations.
1.6. (OPTION) Control Vegetation in Sidewalks and Parking Areas. This option will only be executed as funding becomes available.
Grass, weeds and vegetation growing in parking lots and sidewalks shall be maintained vegetation free using mechanical, chemical, or physical methods that do not degrade the quality of these surfaces. Adjuvants and dyes may also be used to enhance and track chemical applications. Application of herbicides shall comply with all provisions in PWS 1.7 (inclusive).
All airfield pavements to include aprons, aircraft parking areas, taxiways, runways and over-runs are excluded from this requirement.
1.7. General Instructions – Tree Pruning. The Contractor shall prune/trim trees all trees (ornamental and urban forest) in accordance with the standards stated in paragraph 1.7.
(inclusive). The Contractor shall be responsible for identifying trees requiring pruning/trimming and preparing a monthly tree work schedule. The contractor shall provide a copy of the tree schedule prior to start of work and coordinate start/completion of pruning/trimming operations with the COR. Pruning shall be performed to lift, remove, and/or cut back branches which conflict with normal vehicle or pedestrian traffic. In addition, the Contractor shall prune or trim trees that pose public safety hazards as identified by the COR. Pruning shall include removal of dead limbs (potential safety hazard) and encompass the entire tree resulting in the tree maintaining an overall balanced appearance upon completion. Topping and de-horning are not permitted. Minimum clearances for trees are: fourteen (14) feet over streets; twelve (12) feet over driveways; eight (8) feet over walk areas; four (4) feet over buildings; and one (1) foot from buildings. Trees that pose threats to structures or buildings, as determined by the government, shall be removed. Trimming or pruning of trees which touch or hang over energized utility poles or power lines is the responsibility of the Contractor. Minimum clearance from primary lines (over 600 volts) shall be eight (8) feet. Minimum clearance from secondary lines (under 600 volts, i.e., electric service drops, telephone, and cable TV) shall be four (4) feet. The Contractor shall be responsible for removing all debris generated from trimming or pruning operations.
Debris shall be disposed of at the base’s natural debris collection area or other area/container as designated by the COR. The Contractor shall brace, cable, guy (to keep tree vertical), using straps or hose lengths to prevent damage, and deep water all new or damaged trees after planting or identification. The Contractor shall adjust wires, cables, and straps as required, to prevent girdling. The Contractor shall remove all guy wires, cables, straps, and stakes when the trees remain vertical (normally, after one growing season).

1.7.1. Tree Pruning Standards. The Contractor shall perform all tree work in accordance with contract specifications and the following tree care standards, latest addition, unless otherwise directed by the CO/COR.
1.7.1.1. ANSI A300 - Standard Practices for Tree Care Operations (including referenced Combined Federal Regulations (Utility Pruning and Emergency Service Restoration)).
1.7.1.2. ANSI A300 (Part 1) – Tree Pruning
1.7.1.3. ANSI A300 (Part 3) – Tree Support Systems (Cabling, Bracing, and Guying)
1.7.1.4. ANSI Z60.1 – American Standards for Nursery Stock
1.7.1.5. ANSI Z133.1 – Safety Requirements for Tree Care Operations
1.7.2. (OPTION) Prune Trees – Main Base/Grassy Pond. This option will only be executed as funding becomes available.
Tree pruning shall be accomplished as per paragraph 1.7 (inclusive). Upon execution of this OPTION the contactor shall prune trees on the main base and Grassy Pond Annex as identified by the CO, COR or designated representative.
1.7.3. Prune Trees - Hospital. Tree pruning shall be accomplished as per paragraph 1.7 (inclusive). Upon execution of this contract the Hospital campus’ trees (other than identified in PWS para 1.4) shall be pruned when identified by the CO, COR or designated Hospital representative. Trees shall be pruned at a minimum of once per year.
1.7.4. (OPTION) Emergency Spot Pruning/Trimming. This option will only be executed as funding becomes available.
The contractor shall trim/prune limbs from identified tree(s) which pose safety risks to people or potential damage to facilities and equipment, as identified by the COR. The Contractor shall provide service within 24 hours of notification from the COR. Tree(s) will be identified as sapling up to 4" diameter, small over four (4’’) up to twelve (12”) diameter and medium over twelve (12”) up to 24” diameter, and large over 24” diameter. The diameter of tree(s) will be determined using a logger’s tape and measuring around the circumference of the trunk at approximately 4' above ground level. If a loggers tape is not available use a regular measuring tape and divide measurement by pi ((π) or 3.14). Contractor shall be responsible for removing all debris generated from trimming or pruning operations. Debris shall be disposed of at the base’s natural debris collection area or other area/container as designated by the COR.
1.7.5. (OPTION) Tree and Stump Removal. This option will only be executed as funding becomes available.

The contractor shall remove identified tree(s) and associated stump(s) to include all debris generated during removal. When removing trees, grinding of stumps is considered to be part of the same operation and shall be priced to include both tree removal and stump grinding.
Following stump grinding, the area will be backfilled with topsoil, lightly compacted and raked smooth to match the surrounding grade. Tree(s) sizes will be identified as described in paragraph 1.7.4.
1.7.6. (OPTION) Emergency Tree and Stump Removal. This option will only be executed as funding becomes available.
The contractor shall remove identified tree(s) and associated stump(s) to include all debris caused during removal as a result of storm or posing an immediate risk to people, facilities, or equipment. Contractor shall be able to respond within four (4) hours to mitigate immediate danger. When removing trees, grinding of stumps is considered to be part of the same operation and shall be priced to include both tree removal and stump grinding. Following stump grinding, the area will be backfilled with topsoil, lightly compacted and raked smooth to match the surrounding grade. Tree(s) will be identified as described in paragraph 1.7.4.
1.7.7. (OPTION) Irrigation Systems. This option will only be executed as funding becomes available
The contractor shall maintain, adjust and repair existing lawn irrigation systems during the growing season. Irrigation systems are located at Heritage Park, 23D Wing Headquarters, 93D Wing Headquarters, and Base Operations.
1.7.7.1. Operation and Maintenance. The contractor shall operate and maintain irrigation systems in order to maintain a uniform and healthy lawn or landscaped area. Operation includes initial (growing season start) setting of timer clocks, and periodic checks and adjustments to preclude water run-off or increases/decreases in watering times to maintain the lawn and to minimize water consumption to the maximum extent possible. Maintenance includes adjustment of sprinkler heads and periodic checks to preclude water spray on streets, parking lots, or sidewalks. Irrigation system initialization shall commence at a pre-established time each year as specified by the COR.
1.7.7.2. Initial Annual and Normal Seasonal Repairs. The contractor shall survey and test irrigation systems at the start of the watering/growing season to determine the extent of repairs required to bring the system(s) to an operational status. The contractor shall provide a list of operational and non-operational systems within 30 days to the COR. The contractor’s repair responsibility is only for water lines, sprinkler heads, timers, switches and all other system components downstream of the vacuum breaker or other appropriate valve. Unscheduled repairs, for any reason, are included in the scope of normal seasonal repairs.
1.7.7.3. Winterize Irrigation Systems. The Contractor shall winterize all irrigation systems to prevent freezing/damage during the non-growing/dormant season.

1.8. Herbicide Application Procedures. Should the Contractor elect to use herbicides versus mechanical measures to control vegetation at fence lines or other obstacles, the following procedures must be used and coordinated with the Contracting Officer (CO), COR, Integrated Pest Management Coordinator (IPMC), and base HazMart personnel. The Contractor (and sub-contractor, if used) shall maintain a contractor’s license issued by the Georgia Dept. of Agriculture. Contractor personnel handling and applying herbicides shall possess current State of Georgia certifications, business licenses, and training in the appropriate herbicide/pesticide categories (24 - Ornamental & Turf, 26 - Aquatic Plant & Animal, and 27 - Right of Way) at all times during performance of this contract. All licenses and certifications must be provided to IPMC and the COR seven (7) days prior to the application of any pesticide/herbicides. No uncertified technicians shall be authorized to mix or apply herbicides. Reference the following sites for more information:

1.8.1. Weed Control Plan. The Contractor shall submit a weed control plan to the CO and IPMC for approval prior to any herbicide application. The weed control plan must be updated, and approved by the IPMC, prior to any new herbicide being used, to include justification as to why alternative, non-chemical control, methods are not being used. The weed control plan shall detail:
1.8.1.1. The type of herbicide(s) to be applied; with the name & EPA number & application rate (amount per acre).
1.8.1.2. Timing of application(s).
1.8.1.3. The type of application equipment.
1.8.2. Herbicide Approval. The contractor shall only use herbicides listed in ARMED FORCES PEST MANAGEMENT BOARD (AFPMB) STANDARD PESTICIDES, and only after approval from the IPMC. The latest version of this publication is available at the following web site:
http://www.afpmb.org/sites/default/files/pubs/standardlists/dod_pesticides_list.pdf The Contractor shall coordinate approval of all herbicides with the base IPMC and the Command Entomologist as well as processing and approval through the base HAZMART, using ESSOH- MIS, prior to the chemical being brought or used on Moody AFB. Herbicides shall only be applied in strict accordance with label directions to include required personal protective equipment, application rates, and use restrictions. Copies of all herbicide labels, Material Safety Data Sheets (MSDS) and application logs shall be maintained by the contractor in the Contractor work center and made immediately available to the COR or other government representatives for inspection. The Contractor shall submit a locally generated Pest Management Application Log, see appendix D, to collect data (target pest, temperature, wind speed, direction, the type and amount of chemical) used per location and pesticide applicator(s). Wind speed information can be obtained from the Base Weather Office at 257-3457 during standard base duty hours.

Applicators must complete the log and forward a copy to the Integrated Pest Management Coordinator NLT the fifth (5th) work day of the month following application. The contractor shall submit a final inventory, not later than ten (10) working days following termination/completion of this contract, of all hazardous materials used since the last report to the IPMC. The final inventory shall list all hazardous materials used, total amount used, and the description of the disposition of any remaining hazardous materials. This information will then be entered into the Integrated Pest Management Information System (IPMIS) by the IPMC in accordance with (IAW) with AFI 32-1053, 4.7.7.3.
1.8.3. Storage and mixing of herbicides by the Contractor is allowed on base; however mixing must occur only in the approved mixing area located at Building 965, Civil Engineer (CES) Entomology Shop. Mixing will be coordinated with and observed by the IPMC. Contractors shall have a spill kit on each application vehicle or piece of equipment capable of containing 110% of the greatest container volume. All wastes shall be the responsibility of the Contractor and shall be disposed of offsite in accordance with all federal and state regulations pertaining to hazardous chemicals, wastes, and herbicides/pesticides. The Government will not be responsible for off-base disposals under any circumstances.
1.9. Contractor-Provided Equipment and Supplies.
1.9.1. Contractor-Furnished Equipment/Vehicles. The Contractor shall provide and maintain all equipment and vehicles necessary to perform the requirements of this contract. The Contractor may perform minor hand or pneumatic cleaning, minor repair, and other standard minor equipment maintenance requirements on site. Major repairs of equipment and/or maintenance and repair of Contractor vehicles or their privately-owned vehicles shall not be accomplished on site. Contractor vehicles shall have the company name prominently displayed on both sides of the vehicle and be maintained to present a neat and professional appearance.
1.9.2. Equipment Condition. All equipment used in the performance of this contract shall be in good operable condition and carry an Underwriters Laboratory (U/L) listing, if applicable.
Equipment found to be unsafe and unable to function as designed shall not be used in performance of this contract. The CO/COR or designated government representative may inspect the Contractor’s equipment and vehicles at any time and direct the repair/removal of any unsafe or unusable equipment or vehicle from the installation.
1.9.3. Equipment Availability. The Contractor shall coordinate availability of a sufficient quantity of equipment and vehicles to effectively fulfill the scope of this contract. Equipment failure or maintenance requirements shall not alleviate the Contractor from meeting the performance standards contained in this contract.

1.10. Work Delays. The Contractor shall notify the COR of any condition, including adverse weather or special requests from Government personnel, that may interrupt or delay performance under this PWS. Once the condition is resolved the Contractor shall resume interrupted work as soon as practical. When this period exceeds twenty-four (24) hours, the COR must approve the delay.

2. SERVICES SUMMARY (SS). The contract service requirements are summarized into performance objectives which are the mission essential requirements that relate directly to the results the Government requires for this particular service. The performance thresholds are the standards required by the Government for each performance objective. The SS and the Contractor’s Quality Control Plan provide information on contract requirements, the expected level of Contractor performance, and the expected method of Government surveillance and confirmation of services provided. These thresholds (standards) are critical to successful accomplishment and assessment of PWS requirements. Procedures, as set forth in the FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies.

2.1. Contractor Quality Control (CQC). The Contractor shall develop and maintain a written CQC program (CQCP) to ensure grounds maintenance services are performed in accordance with this PWS and commonly accepted commercial standards or practices, as applicable to grounds maintenance services. The Contractor’s CQCP shall include a written Quality Control Plan (QCP) establishing, developing and implementing procedures to identify, prevent, and ensure non-performance and/or consistent/continual repetition of defective services does not occur. The QCP shall establish daily, weekly and monthly duties for the Quality Control (QC) manager. The QCP shall be submitted to the CO for review, feedback, and validation no later than the pre-performance conference. The contractor shall use the approved QCP in daily performance of self-inspection and shall keep written records of completed inspections. The plan shall specifically address the Contractor’s strategy to provide quality workmanship, continual process improvement, and correction of government and contractor identified deficiencies.

2.2. Government Quality Assurance (QA). The Government will inspect and evaluate the Contractor’s performance to ensure services are received in accordance with requirements set forth in this contract. The COR will evaluate by physically checking an attribute of the completed task, checking a management information report, investigating customer complaints, conferring with facility managers, or otherwise inspecting the task or its results to determine whether or not performance meets the standards contained in this PWS. The COR will use the Contractor’s work schedule, or modified version thereof, and the CQCP and QCP to schedule and perform surveillance. The COR will use a locally developed surveillance worksheet to document performance of government QA surveillance actions. The contractor shall initial (or digitally sign) the worksheet acknowledging surveillance performance and any discrepancies discovered. The contractor shall return the worksheet to the COR within three (3) duty days after providing comment and corrective action which shall include contractor effort to preclude future discrepancies. Results of the surveillance then become part of the official Air Force record of Contractor performance.
2.2.1. When a performance threshold has not been met or Contractor performance has not been accomplished, the COR will initiate and provide the CO a DD Form 2772, Contract Discrepancy Report (CDR) for issuance to the Contractor. The Contractor shall respond to the CDR IAW CO provided instructions, and return it to the CO within ten (10) calendar days of receipt.
2.3. Surveillance Methods.
2.3.1. Periodic Evaluation and Surveillance. The COR employs a “spot check” style of evaluation based on the Contractor’s submitted schedule. This type of surveillance is based upon selecting samples for evaluation on other than 100% inspection or on a statistically random basis.
Periodic inspections/surveillances will be conducted on a scheduled basis (daily, weekly, monthly, quarterly, semi-annual or annually) and frequency may be adjusted, based on COR evaluation of documented quality trends.
2.3.1.1. Satisfactory and unsatisfactory evaluation (discrepancy/defect) results shall be recorded by the COR. The Contractor shall re-perform the discrepant or defective service within 8 duty hours following notification by the COR. The contractor shall notify the COR when repeat work has been completed.
2.3.1.2. Failing to meet the performance threshold as outlined in the SS for any of these performance objectives in any one-month period shall result in initiation of a CDR by the COR, and a warning or letter of concern from the CO.
2.3.1.3. Failing to meet the performance threshold as outlined in the SS for any of these performance objectives, in any combination, for any two (2) or more consecutive or non-consecutive months during a contract performance period shall result in initiation of a CAR, and constitute an immediate Multi-Functional Team/Progress Meeting. All remedies shall be in accordance with FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance.

2.4. Quarterly MFT/Progress Meetings. The CO, Functional Commander (FC), COR, and other Government personnel, as appropriate, and the Contractor shall meet (following contract award) to discuss the Contractor’s performance. The following issues shall be discussed:
opportunities to improve the contract, any modifications required to the contract, unsatisfactory inspections and observed trends against each performance objective, positive performance, and steps taken by the Contractor to prevent unsatisfactory occurrences in the future. The Contractor shall provide a summation of unsatisfactory inspections and customer complaints and provide insight into any identified trends.
2.4.1. Minutes of these meetings will be reduced to writing, signed by the CO and any other signatures as deemed appropriate, distributed to the functional area and the Contractor. Should the Contractor not concur with the minutes, the Contractor shall provide a written request for resolution to the CO identifying areas of non-concurrence.
2.5. Government Receipt and Acceptance of Goods.
2.5.1. Wide Area Work Flow. The COR will certify receipt of contractor services via the Wide Area Work Flow (WAWF) website. Certification of services will be accomplished using the contractor’s combination invoice/receiving report following completion of each contract payment period.
2.5.2. All work requirements shall be completed and accepted by the COR prior to any payments being made for services performed during the month.
3. GOVERNMENT-FURNISHED EQUIPMENT, FACILITIES, FORMS, PROPERTY,
PUBLICATIONS, AND SERVICES.
3.1. Government-Furnished Facilities. The Government will provide office and storage space in Building 966 (approximately 850 sq ft), open paved storage space (approximately 10,000 sq ft), and a portion of Building 958 (approximately 2000 sq ft). The last two bays on the east side of building 958 will be utilized by the government (approximately 1,000 sq ft). Additionally, the Contractor may use the Government equipment-wash-rack located in the heavy equipment and grounds maintenance yard to clean the exterior of vehicles and equipment only. No disposal of Hazardous Material or Hazardous Waste (HM/HW) is allowed in the wash rack. Engine and equipment steam or pressure cleaning is not allowed in the wash rack or on base. The Contractor shall ensure the equipment-wash-rack is clean and free of debris after each Contractor use.
Discrepancies with the wash rack equipment shall be immediately reported to the COR and Heavy Equipment Shop supervisor.
3.1.1. Alterations. No alterations to office space or other facilities shall be made without specific written permission from the 23d CES Operations Flight Deputy Commander and the CO, as coordinated and approved via AF Form 332, Civil Engineer Work Order submitted by the contractor to the COR. In case of alterations necessary for compliance with OSHA, such permission shall not be unreasonably withheld. The Contractor shall return the office space to the

Government in the same condition as received, excluding fair wear and tear (as determined by the Government) and approved modifications.
3.1.1. Housekeeping. All Government-provided facilities are the contractor’s sole responsibility for housekeeping. The Contractor shall ensure daily, weekly, and periodic cleaning of assigned buildings providing a clean, neat, orderly and well maintained appearance to assigned facilities.
Restrooms shall be thoroughly cleaned at least every other duty day. Office and breakroom shall be thoroughly cleaned at least weekly. Outside and covered storage shall be clean and trash/debris free at the end of each duty day. Cleaning of windows, stripping/ waxing/polishing of office/break area floors shall be accomplished semi-annually.
3.1.2. Government-Furnished Equipment (GFE). The government will provide the following radios and chargers to the contractor for use during airfield operations. Repair or replacement of this equipment due to misuse, abuse, or loss, as determined by the government, shall be the responsibility of the contractor.

3.2. Government Furnished – Contractor Maintained Equipment.
3.2.1. 10 HP-Air Compressor Assembly (Schulz compressor, American tank, Baldor motor). The Government will provide an air compressor at Building 958 for accomplishment of work in accordance with this contract until equipment is no longer cost effective to repair, as determined by the Government. The Contractor shall be responsible for daily user maintenance in accordance with the manufacturer’s specifications. The Contractor shall be responsible for replacement of the air compressor when the Government determines repair is no longer cost effective or failure was a result of the Contractor’s negligence.
3.3. Forms/Publications. The Government will make all publications, forms, references, and report formats listed available via public internet. Publications can be accessed on-line at http://www.e-publishing.af.mil/. Supplements or amendments to listed publications from any organizational level may be issued during the life of the contract. The Contractor shall immediately implement those publications changes, which result in a decrease, or no change in the contract price. Prior to implementing any such revision, supplement, or amendment which will result in an increase in contract price, the Contractor shall submit to the CO a price proposal and obtain prior approval. Price proposals shall be submitted within fifteen (15) calendar days from the date the Contractor receives notice of the revision, supplement, or amendment giving rise to the increase in cost of performance. Changes in the contract price due to supplements and amendments shall be considered under the FAR 52.212-4, Contract Terms and Conditions - Commercial Items. Contract Title: Grounds Maintenance Services calendar days from the date of receipt of any change may endanger Contractor performance. Any required publications not available online publicly will be provided upon contractor request.
3.4. Government-Furnished Utilities. The Government will furnish electricity, water, and sewage service, as necessary, for accomplishment of work in accordance with this contract.
3.4.1. Utility Conservation. The Contractor shall adhere to all base level utility conservation practices or requirements. The Contractor shall be responsible for operating under conditions that prevent waste of utilities.
3.5. Telephone Service. The Government shall provide one (1) telephone, located in building 966, with base and local commercial telephone service. Long distance service, if desired, shall be at the Contractor’s expense and shall be coordinated by the contractor through the 23d Communications Squadron.
3.6. Security, Fire and Medical Services. The Government will provide police and fire protection. In the event of a medical emergency, base ambulance service for transporting an injured employee to a local hospital is available on a cost reimbursable basis.
3.7. Refuse Collection and Disposal. The Contractor shall use existing bulk refuse containers to dispose of trash generated or collected during accomplishment of services detailed in this PWS.
The Contractor shall adhere to all base level recycling programs, to include disposal of recyclables at the solid waste collection center or other designated on-base areas. The Contractor shall dispose of collected yard/tree waste material at the base’s natural debris collection area or other area/container as designated by the COR.
3.8. Mail Service. The Contractor shall be responsible for coordination with, and cost of, US Postal Service mail delivery or post office box.
3.9. Computer/Internet Service. The Government will not provide computer equipment, service or internet support. The Contractor shall be responsible for the cost of, and coordination with, the 23d Communications Squadron and the Contractor's choice of communications companies for computer or internet service at Contractor expense.
4. GENERAL INFORMATION.
4.1. Hours of Operation.
4.1.1. Standard Base Hours. Base hours of operation are 0700-1700, (7:00 a.m.-5:00 p.m.), Monday through Friday, excluding federal holidays. The Contractor may find it necessary to deviate from the standard base hours of operation to ensure timely completion of work under this PWS, which may include weekend work hours, at no additional cost to the Government.

4.1.2. If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the Contractor shall be responsible for rescheduling services for the first work day following the holiday observance. The State of Georgia Department of Labor Wage Determination lists Good Friday as a paid holiday in addition to the Federal holidays listed above.
4.1.3. The Contractor may substitute the “Good Friday” holiday time-off for the day after Thanksgiving, which is usually considered a “family day” for the general base population. If the contractor decides to take advantage of this option, this arrangement must be finalized between the CO, Contractor, and COR two (2) weeks prior to the holiday.
4.2. Base Closures. Work scheduled, but not accomplished because of base closure due to weather, exercises, or actual alert, shall be accomplished as soon as possible after reopening the base.
4.3. Contractor/Employee Base Pass and Identification, Special Clearances and Vehicle Passes. The Contractor shall comply with all requirements and procedures in accordance with AFFAR 5353.242-9000, Contractor Access to Air Force Installations. All documentation shall be submitted at the pre-performance conference.
4.4. Continuation of Essential DoD Contractor Services During Crises.
4.4.1. Semi-Improved Airfield mowing operations accomplished under this contract shall be performed during any wartime operation(s). Wartime operations are those actions, including contingency planning, which will be required to support current or future United States Air Force wartime requirements; in addition to functions that will be continued during wartime-related contingencies. Notwithstanding any other provision of this contract, the Contractor agrees that, in the event of the occurrence of any wartime-related contingency, the Government shall have the unilateral right to extend performance of this contract as long as the wartime contingency(ies) exist.
4.4.1.1. The Contractor’s contract manager will be notified by the Government by any means of communication within twenty-four (24) hours of notification of any contingency or crisis situation. Contract payment will be in accordance with the existing contract rate.
4.4.1.2. The Contractor shall provide the CO at the pre-performance conference the names and title of all employees who shall be responsible to work during such crisis or wartime operations.
4.4.1.3. The Contractor is responsible for providing the requested information as required in clauses 252.237.7023 and 252.237.7024 of this contract.
4.5. HAZMART. The Contractor shall provide the CO an inventory of all chemicals the Contractor plans to bring onto Moody AFB or any property under the control of Moody AFB.
This inventory will be performed and appropriate approvals coordinated by the Contractor as required per AFI 32-7086, through HAZMART and BEE prior to commencement of work.
HAZMART personnel will use this inventory to enter the chemicals into the Environmental Management Information System (EMIS) and provide bar code labels to the Contractor.
