1. DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, supplies, equipment, and labor necessary to provide the service of 0900 Contemporary Service Musician & Coordinator. This position supports the religious ministries of the Protestant Faith Community at Moody AFB GA.

1.1. SCOPE OF SERVICES.

1.1.1. The 0900 Contemporary Service Musician & Coordinator (hereafter called “The Contractor”) will be responsible for the 0900 Worship Service Program.

1.1.2. The Contractor will work with the chaplains in interpreting and preparing policies and procedures applicable to the total Protestant Music program.

1.1.3. 0900 Contemporary Service Musician/Coordinator will be the POC for all music programs.

1.2. OBJECTIVE OF SERVICE: This service is being provided to Moody AFB Chapel.

1.3. SPECIFIC RESPONSIBILITIES.

1.3.1. Provide instrumental accompaniment while leading congregational singing in a contemporary worship setting.

1.3.2. Coordinate all praise team/praise band activities and support requests with the chaplain or chaplains assigned to the Contemporary Service.

1.3.3. Direct weekly scheduled praise team/praise band rehearsals and warm-up sessions prior to the Contemporary Service or other designated program.

1.3.4. Coordinate, direct, and play music for special worship services and choir performances (cantatas, etc.) such as Christmas, Easter and Memorial Services, with advance notice and scheduling arrangements.

1.3.5. Prepare an annual budget and inventory of required music/choir supplies and submit it to the chaplain responsible for the Contemporary Service.

1.3.6. Periodically enhance skills by attending education seminars, etc.

1.3.7. Ensure copyright laws are followed.

1.3.8. Submit a monthly statement of services performed and request for payment to the chaplain(s) in charge of the Worship Service

1.4. SCHEDULE. 3 days wk/4h day

1.5. STAFF RELATIONS.

1.5.1. The contractor will coordinate all services performed, as defined in this performance work statement, with the sponsoring chaplain, or in his/her absence, the Senior Faith or Wing Chaplain.

1.5.2. Musician/Coordinator must work well with the chapel staff and promote a spirit of cooperation and unity.

1.5.3. The sponsoring chaplain will certify all services provided by this contract.

1.5.4. The Contractor will ensure the Wing Chaplain or his/her delegated representative IAW 23 WG/HC Operating Instructions approves all requests for chaplain funds and/or facility usage.

1.6. CALL/TASK ORDER

1.6.1. Maintains the following scheduled of hours: Call consists of monthly services to include, weekly practices and Sunday services.

1.6.2. Additional services may be necessary and shall be issued on a separate call/task order.

1.6.3. Each call/task order shall be issued on a monthly basis.

1.7. SUBSTITUTE PERSONNEL. When the contractor is unable to perform contracted duties, he/she will provide advance notice of a suitable substitute who meets the qualifications of this Performance Work Statement.

2. SERVICES SUMMARY. The contractor’s service requirements are summarized into performance objectives that relate directly to mission essential items. This determines acceptable and unacceptable performance of a task. The SS table indicates the minimum acceptable levels of service required for each requirement. These thresholds are critical to mision success and promote continuous improvement in contractor performance. The government expects the contractor to perform as a minimum, the identified acceptable levels throughtout the life of the contract,, and the contractor;s performance shall be a significant consideration when exercising contract option year(s) and evaluating recommendation for awards of future government contracts. 3. GOVERNMENT-FURNISHED PROPERTY AND SERVICES. (Computer/Printer)

3.1. Telephone Services. The use of Government supplied telephone(s) is provided solely for official government business and shall not be used to transact personal business by the contractor or his/her employees. The contractor shall contact the QAP for repair service. The government will not be responsible for charges for long distance telephone calls made or accepted by contractor personnel. Telephone services will be subject to the standard monitoring requirements of the government-telephone network.

3.2. Base Distribution. The contractor may use the Base Information Transfer Center (BITC) to send mail on base.

3.3. Refuse Collection. The base refuse and recycling contractor is responsible for the collection and removal of all trash and refuse. However, when the contractor finds it necessary, at various times, to dispose of refuse and trash generated as a result of this contract requirement, the contractor may use the base dumpsters.

3.4.

Recycling Program. The contractor shall be required to participate in the base-recycling program.

3.5.

Emergency Medical Service. Moody Air Force Base hospital will provide emergency (cases where loss of life, limb, sight or hearing is possible) medical services, if required, on a reimbursable basis.

4. GENERAL INFORMATION.

4.1 SECURITY REQUIREMENTS. (NOTE: USE ONLY APPLICABLE PORTIONS OF THE PARAGRAPHS BELOW. ALLOW SUFFICIENT TIME BETWEEN CONTRACT AWARD AND CONTRACT START DATE TO COVER THE SECURITY CLEARANCE PROCESSING TIMES.)

4.1.1. The contract administrator shall complete a "Request for Identification Credential (AFMC Form 496)" for each employee of the contractor requiring access to Moody AFB. The requests shall be submitted to Pass and Registration (South Gate Visitors’ Center).

4.1.2 The contract administrator shall complete an “Unescorted Entry Authorization Certificate” (AF Form 2586) for each employee requiring entry into controlled areas. The request shall be submitted to Pass and Registration (Visitors’ Center).

4.2 QUALITY CONTROL.

4.2.1 The contractor shall only tender for acceptance those items that conform to the requirements of the contract.

4.2.1.1. Quality Assurance. The POC or alternate(s) will inform the CO when work does not conform to the terms and conditions of the contract.

4.2.1.2. Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the CO.

4.2.1.3. The services to be performed by the contractor during the period of this contract shall at all times and places be subject to review by the CO and/or other authorized Government representative(s).

4.3. PHYSICAL SECURITY. The contractor shall be responsible for safeguarding all government property provided for contractor use. At the close of each work period, government facilities, property, and materials shall be secured. .

4.3.1. Key Control. The contractor shall establish and implement methods of ensuring that all keys/key cards issued to the contractor by the government are not lost or misplaced and are not used by unauthorized persons. (NOTE: ALL REFERENCES TO KEYS ALSO INCLUDE KEY CARDS. NO KEYS ISSUED TO THE CONTRACTOR BY THE GOVERNMENT SHALL BE DUPLICATED. THE CONTRACTOR SHALL DEVELOP PROCEDURES COVERING KEY CONTROL THAT SHALL BE INCLUDED IN THE QUALITY CONTROL PLAN. SUCH PROCEDURES SHALL INCLUDE TURN-IN OF ANY ISSUED KEYS BY PERSONNEL WHO NO LONGER REQUIRE ACCESS TO LOCKED AREAS).

4.3.1.1. The contractor shall immediately report the occurrences of a lost or duplicate key to the CO.

4.3.1.2. In the event keys, other than master keys, are lost or duplicated, the contractor shall, upon written direction of the CO, rekey or replace the affected lock or locks; however, the government, at its option, may replace the affected lock or locks or perform rekeying. When the replacement of locks or rekeying is performed by the government, the total cost of rekeying or the replacement of the lock or locks shall be deducted from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the government and the total cost deducted from the monthly payment due the contractor.

4.3.1.3. The contractor shall prohibit the use of keys issued by the government by any persons other than the contractor’s employees. The contractor shall prohibit the opening of locked areas by contractor employees to permit entrance of persons other than contractor’s employees engaged in the performance of assigned work in those areas, or personnel authorized entrance by the CO.

4.3.2. Lock Combinations. (NOTE: OPTIONAL DEPENDING ON LOCAL REQUIREMENTS). The contractor shall establish and implement methods of ensuring that all lock combinations are not revealed to unauthorized persons. The contractor shall ensure that lock combinations are changed when personnel having access to the combinations no longer have a need to know such combinations.

4.4 HOURS OF OPERATION AND PLACE OF PERFORMANCE.

4.4.1. Standard Hours of Operation. The contractor shall perform the services required under this contract during the hours of 0730- 1630, Monday through Friday, EXCEPT for the holidays cited below, when applicable.

4.4.1.1. The period of performance shall be from 1 January to 31 December, and the place of performance shall be the Base Chapel, Building #110/440, 5120 Austin Ellipse, Moody AFB GA.

4.4.1.2. Recognized Holidays. See Wage Determination for recognized Holidays.

4.4.2. Emergency Services. On occasion, services may be required to support an activation or exercise of contingency plans outside the standard duty hours described above. The contractor’s responsibilities for emergency support are described in Section 1.

4.5 CONSERVATION OF UTILITIES. The contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions which prevent the waste of utilities which include the following:

4.5.1. Lights shall be used only in areas where and when work is actually being performed.

4.5.2. Mechanical equipment controls for heating, ventilation, and air conditioning systems shall not be adjusted by the contractor or by contractor employees unless authorized.

4.5.3. Water faucets or valves shall be turned off after the required use has been accomplished.

4.5.4. Cell phone Usage.

4.5.4.1. Cell phone usage is prohibited while driving on base, unless a completely

“hand-free” device is used. Personnel should pull over out of traffic and park to use a cell phone.

4.6. RECORDS. All records, files, documents, regardless of media (e.g., paper, electronic, etc.), as described in the SOW, that are the responsibility of the contractor are the property of the Government and shall remain so upon termination or completion of the contract. The contractor shall keep these items current and maintain and dispose of them in accordance with the requirements established in Air Force Instruction (AFI) 33-322, Records Management Program, paragraph 10, Air Force Manual (AFMAN) 33-363, paragraph 6.4, Air Force Manual (AFMAN) 33-364, paragraph 2.13, and the Air Force Records Disposition Schedule which may be accessed from the AF-RIMS Link located on the Air Force Portal. Records shall be turned over to the Government upon completion of the contract unless otherwise stated. All records are subject to the Freedom of Information and Privacy Act. The Moody AFB Records Management Office (23 CS/SCXKR), 257-3500/3445, will provide assistance and training to the contractor to ensure compliance with Federal record keeping requirements.

4.7. ENVIRONMENTAL CONTROLS.

4.7.1. Compliance with Laws and Regulations. The contractor shall be knowledgeable of and comply with all applicable Interstate, Federal, State, and Local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost as a result of the change, the contractor shall inform the CO pursuant to notice requirements and negotiate a modification to the contract.

4.7.2. Notification of Environmental Spills. If the contractor spills or releases any substance contained in 40 CFR 302 into the environment, the contractor or its agent shall immediately report the incident to Moody AFB Fire Dept at 257-. The liability for the spill or release of such substances rests solely with the contractor and its agent. (NOTE: IF THE CONTRACTOR WILL ROUTINELY HANDLE MATERIALS WHICH COULD HAVE A HARMFUL EFFECT ON THE ENVIRONMENT, ADD APPROPRIATE SUBPARAGRAPHS ON HOW TO DISPOSE OF AND HANDLE SPILLAGES OF THESE MATERIALS.)

4.7.3. Material Storage and Use. The contractor shall follow manufacturer’s guidelines and professional recommendations for control of humidity, temperature, cleanliness, and materials handling. This includes hazardous materials.

4.8. SAFETY REQUIREMENTS. In performing work under this contract, the contractor shall:

4.8.1. Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work.

4.8.2. Take such additional immediate precautions as the CO may reasonably require for safety and mishap prevention purposes.

4.8.3. Develop and provide at the start of the orientation period or the start of the first operational performance period (if there is no orientation period) a safety plan for the protection of government facilities and property and to provide a safe work environment for contractor personnel. (NOTE: FOR NEGOTIATED CONTRACTS, RESERVE THIS PARAGRAPH AND REQUIRE BIDDERS TO INCLUDE THE SAFETY PLAN IN THEIR TECHNICAL PROPOSALS IF APPLICABLE.)

4.8.4. Provide protection to government property to prevent damage during the period of time the property is under the control or in possession of the contractor.

4.8.5. Record and report promptly (within one (1) hour) to the CO or authorized government representative (GR), all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.

4.8.6. In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the CO.

4.8.7. If the government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed.

4.9. TRAINING. N/A

4.10. SPECIAL QUALIFICATIONS.

4.10.1. Must have one year experience in directing a choir in church setting

4.10.2. Must have knowledge of seasonal religious music and able to select appropriate music in line with the church calendar

4.10.3. Must be familiar with Christian music (traditional and contemporary) and able to sight-read music.

4.10.4. Must be able to teach songs to the choir

4.10.5. Must be knowledgeable /experienced in directing the choir and understanding piano and organ music.

4.10.6. Must be able to work effectively with people in a religiously pluralistic environment.

4.11. PUBLICATIONS.

4.11.1. Compliance with all publications, regulations and operating instructions provided by the

Government is required when:

4.11.2. They pertain to the procedures for materials expediting herein and where the contractor is authorized by the performance work statement to accomplish the work specified in the publication, regulation or operating instructions.

4.11.3. The publications prescribe USAF policies, use of materials, procedures and processes applicable to the work requirements.

4.11.4. The contractor is required to acquire and work on the latest version of the publication.
