STATEMENT OF WORK
(SOW)
95 AMU / Support Section Epoxy Floors

1.0. DESCRIPTION OF SERVICES
The contractor shall provide all management, labor, equipment and tools necessary to apply an epoxy based resin to the floor. All work performed by the contractor shall be performed in accordance with all applicable laws, regulations, instructions and commercial practices.
2.0. SERVICES SUMMARY
2.1. SCOPE
Contractor shall provide the following services to re-floor the unit’s support section:

This shall be a turnkey project, however support personnel will remove equipment and storage modules to clear the area for work. This will open the work area allowing contractors to operate uninterrupted for the duration of the project. The contractor will apply the resin accordingly and notify the customer point of contact should any delays are encountered. It is understood this project may take up to two weeks to allow proper application and curing. The contractor shall verify all existing conditions and quantities at a pre-bid meeting, and from there determine if any stripping, or other preparation of old flooring is required to allow better adhesion and factor that in the timeline to completion.
2.1.1. EQUIPMENT, MATERIALS, AND LABOR
The contractor will provide all equipment, materials and labor necessary to prepare floor surface and install the epoxy floor system in strict accordance with the manufacturer’s recommended procedures. Government personnel will relocate all mobile storage racks and equipment to make room for the contractors to conduct work.
2.1.2. MATERIALS AND HARDWARE SPECIFICATIONS.
The contractor shall install approximately 3,614.5 square feet of Dura-Flex Shop Floor 3 part epoxy flooring system or equivalent as follows. The contractor shall use manufacturer’s recommended procedures to prepare the floor for the epoxy floor system. The contractor shall apply medium stone, or gray colored epoxy grout coating and black Armor urethane top-coat with shark grip additive that meets the slip 5 percent coefficient.
2.1.3. SITE SURVEY
A site survey shall be conducted to evaluate the above requirements.
2.3. PLACE/PERIOD OF PERFORMANCE
95th Aircraft Maintenance Unit 773 Florida Ave. Bldg 182 (Hangar 1) Tyndall AFB FL, 32403
- Contractor work can be conducted Monday through Friday 0700 to 1700 hours.
- Contract should be planned to start in August 2018, specific date to be determined while the unit is Temporary Duty, this will ensure convenience for both contract and customer to have area free with reduced work interference for both parties.
3.0. GOVERNMENT FURNISHED PROPERTY AND SERVICES.
The Government will furnish the following materials: None.
3.1. GOVERNMENT FURNISHED FACILITIES.
The Government will make all reasonably required utilities available to the Contractor from existing outlets and supplies, as specified in the contract.
4.0. GENERAL INFORMATION.
4.1. SECURITY.
This project is unclassified.
4.1.1. PERSONNEL.
All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the governmental installation shall abide by all security instructions and directives. Employees are responsible for safeguarding all government property provided for contractor use. At the close of each work period, governmental facilities, equipment and materials shall be secured, lights, heat, safety, security, general well-being or operational mission of the installation and its population. The contractor shall submit any request for compensation on delays to the Contracting Office for approval. The contractor shall notify the Contracting office or authorized representative as soon as the contractor identifies/determines any potential delays in accordance with the terms and conditions of this requirement.
4.1.2. SUITABILITY INVESTIGATIONS.
Personnel working in a government facility for less than 6 months or working anywhere on Tyndall AFB property are required to have a Local Files Check. The Unit Security Manager will coordinate the Background Affidavit (with Pass & ID) and DBIDS Application– allow 3-5 business days process time. A favorable local files check is required to be granted base access.
Personnel working in a government facility 6 months or longer or at any time requiring access to the Local Area Network will require a National Agency Check with Written Inquiry (NACI).
An open NACI Investigation, at a minimum, is required before access to government furnished workstations that have access to Air Force e-mail systems can be granted. A Network Waiver may be granted by the Unit Commander after completion of an “Open” NACI reflects in JPAS. These investigations shall be submitted by the government at no additional cost to the contractor.
Personnel requiring Unescorted Entry to Restricted Areas, require a favorably completed NACI clearance – IAW DoD 5200.2-R, Personnel Security Program.
4.1.3. PASS AND IDENTIFICATION ITEMS.
The contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-government owned vehicles:
a. DBIDS, Defense Biometrics Identification System - Contractor Identification Card.
Process a DBIDS Application to obtain a DBIDS contractor ID card. A DBIDS contractor ID card is required for performance on the installation in a contractor capacity, regardless of alternate base access forms of identification (ex: retired military ID).
b. AF Form 1199, USAF Restricted Area Badge, or a locally developed badge (if applicable).
c. AF Form 75, Visitor/Vehicle Pass (AFI 31-204): The contractor shall adhere to local security policy when applying for short/long term visitors’ passes. Any contractor requesting a pass for more than 3 days will complete a Local Files Check with 325 SFS/Pass & ID. Vehicle passes (personal vehicles) are no longer required.
4.1.4. RETRIEVING IDENTIFICATION MEDIA.
The contractor shall retrieve all identification media from employees who depart for any reason before the contract expires; e.g., terminated for cause, retirement and submit them to the COR or to the office that issued the media.
4.1.5. LISTING OF EMPLOYEES.
The contractor shall maintain a current listing of employees performing services on TAFB. The list shall include employee's name, social security number, date of birth, driver’s license number and state of issue, and level of security clearance or base access (to include if escort privileges are authorized) on company letterhead. Foremen and supervisors must be identified on the list of employees. The contractor shall submit the list to the Contracting Officer, the COR, and the unit’s Security Manager, prior to the contract start date, and submit an updated list when an employee's status or information changes, within 5 working days of the change. The name of any contractor employee that is terminated during the period of the contract must be reported to the Contracting Officer in writing within one workday of the employee's termination.
4.1.7. PHOTOGRAPHY.
No photos are permitted to be taken on the flight line. If photographs are needed in the performance of their duties, contractor shall notify the COR and photos will be taken and provided for them as outlined in Tyndall IDP 31-101.
4.1.8. CONTRACTOR IDENTIFICATION AND CREDENTIALS.
All on-site contractor personnel shall wear clothing appropriate to the tasks assigned and display the name of the company. Employees shall wear reflective vests at all times while operating on the flightline. The contractor shall ensure all employees have the legal right to perform work within the United States and can pass appropriate security checks.
4.2. ANTI-TERRORISM/FORCE PROTECTION.
Contractor personnel, including subcontractors, must comply with and participate in the installation AT/FP Program, specifically including the conduct of Random Antiterrorism Measures (RAM).
4.3. TRAFFIC LAWS.
Contractor employees shall comply with all base traffic regulations, and are subject to random vehicle speed control checks. Contractor personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action. The use of seat belts is mandatory for all vehicle occupants and the use of cell phones while driving is strictly prohibited, except hands-free cell phones, while driving on TAFB.
4.4. RANDOM PERSONNEL AND VEHICLE SEARCHES.
Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges, debarment from the base, or other administrative action.
4.5. WEAPONS, FIREARMS, AND AMMUNITION.
Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor owned vehicle or privately owned vehicle while on Tyndall
AFB.
4.6. FOR OFFICIAL USE ONLY (FOUO) MATERIAL.
If the contractor determines that unauthorized personnel have access to For Official Use Only (FOUO) material, such as the Base map, the contractor Site Manager will immediately notify the COR and cooperate fully with any investigation. The contractor shall comply with DoD 5400-7- R, Chapter 4, DoD Freedom of Information Act (FOIA) Program. This regulation establishes policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.
4.7. WARRANTY.
Industry standard warranty of work and material.
4.8. PRIVACY ACT.
Work on this project may require that personnel have access to Privacy Information. Personnel shall adhere to the Privacy Act, Title 5 of the U.S. Code, Section 552a and applicable agency rules and regulations.
