STATEMENT OF WORK
Replace Flooring
Building 1230 Tyndall Air Force Base, Florida
Period of Performance: Demolition and installment of all work shall be completed no longer than 6 Months prior to award of contract.
1.0. SCOPE: Contractor shall furnish all labor, materials, transportation, tools, equipment, and supervision needed to remove carpet/tile and install new carpet/tile at Building 1230; Force Development Center Tyndall Air Force Base, Florida.
2.0. DESCRIPTION OF WORK:
2.1. Demo existing flooring in the entire building.
2.1. 1. Remove carpet throughout the facility.
2.1.2. Remove existing cove base at all walls getting new flooring.
2.1.3. Remove all existing transition strips for entire project.
2.2. Repair/patch floor as needed to provide smooth I level surface for the carpet/tile.
2.3. Installation of new flooring.
2.3.1. Install approximately 400SY of new tile and 1170SY of new carpet and 400SY of tile. The contractor is responsible for their own measurements.

2.3.2. Install new transition strip at all locations where tile and carpet meet.
2.4. Move furniture, cabinets, bookshelves, etc. as needed. Replace the furniture once the carpet is installed.
3.0. HAZARDOUS MATERIAL INVENTORY AND TRACKING: The contractor shall submit TAFB Form 81 (Contractor's Hazardous Material Questionnaire ‐ Attachment 3) and TAFB Form 82 (Short‐Term Contractor Hazardous Material Inventory‐ Attachment 4), prior to starting work. The Contractor should note that Tyndall AFB is required to report chemicals used such as (but not limited to) compressed gases, adhesives, aerosol cans, sealants, paints, lubricants, oils, cleaners, degreasers, pesticides. Copies of manufacturer‐specific Material Safety Data Sheets (MSDS) must be attached to T AFB Form 82. After submission, 325 CES/CEI will notify the Project Manager and CONS of the reportable chemicals and of any special instructions. The Contractor is required to submit T AFB Form 83 (Contractor Environmental Reporting Entry Form – Attachment 6) at the completion of the project. The CO must be notified of any changes from the original submittal (i.e. new chemical is added, size of container or unit of issue changes or if the manufacturer changes). These MSDSs shall also be readily accessible at the location of each hazardous material. MSDS shall be provided to CE Environmental Engineering and to the Quality Assurance Personnel, and approved by the government, prior to commencement of work.
4.0. Contractor Generated Hazardous Wastes/Excess Hazardous Materials: The contractor shall comply with all provisions of 40 CFR 260 through 281 regarding the generation, storage, and disposal of hazardous waste. The contractor shall submit a hazardous waste management plan to the COR for approval by 325 CES/CEIE. The contractor shall not commence any work until this approval is obtained.
The contractor shall stop all work in the event 325 CES/CEIE identifies noncompliance with federal and state regulations and shall correct any discrepancies immediately within 2 hours of notification by 325
CES/CEIE.
4.1. All hazardous waste shall be labeled and an inventory management system will be initiated to insure timely removal and proper disposal. No on‐base disposal will be allowed.
4.2. All drums will be labeled with a hazardous waste label. The label shall include the proper DOT shipping name, UN or NA, EPA waste number, generator information, and accumulation start date. The label shall be placed on the side of the drum.
4.3. All drums used to store hazardous waste shall be non‐leaking and safe to handle. Contractor shall be responsible to insure drums are not over‐packed, rusted, dented, leaking or damaged in any way. Drums shall be provided by the contractor. All drums shall be "new" DOT approved containers.
4.4. The 90‐day storage location for the hazardous waste drums shall be approved by the 325 CES/CEIE prior to the generation of hazardous waste. The contractor shall document inspection of drums for leaks on a daily basis or if not working in the area daily, then a weekly inspection will suffice. A copy of the inspection checklist shall be forwarded to 325 CES/CEIE every Friday.
4.5. Hazardous waste transportation and disposal shall be coordinated through 325 CES/CEIE. The contractor shall be responsible for transportation and disposal of all hazardous waste at an EPA approved treatment, storage, disposal facility (TSDF). The transportation and disposal facilities shall be approved by 325 CES/CEIE prior to their use. Manifests shall be signed only by 325 CES/CEIE. Drums shall be disposed of within 90 days of placing the first drop in the container.
4.6. The contractor shall reimburse the Government for any remediation undertaken to clean up releases by the contractor and for any civil or criminal fines or penalties for any environmental infraction caused by the contractor.
5.0. WORK CLEARANCE REQUEST: Contractor shall obtain a blank AF Form 103, BASE CIVIL ENGINEERING WORK CLEARANCE REQUEST from 325 CES Customer Service, 283‐4949, prior to the any performance of any work. Contractors shall request a Work Clearance at least 10 working days in advance. It is the Contractor's responsibility to have a completed and approved AF Form 103 prior to beginning any construction activities.
6.0. HOURS: The normal duty hours of work at Force Development Center are from 8:00 AM to 4:00 PM, Monday to Friday excluding Federal Holidays. All work shall be performed after duty hours and weekends. Each areas worked shall be ready to support program flow and functions on the next duty day. All other hours shall be requested through the Contracting Officer for approval. This request shall be submitted no less than three (3) workdays prior to the time requested.
7.0. MATERIALS: All material shall be high quality from a well‐known Manufacturer and used per the manufacturer's recommendations. Carpet/tile color TBD.
8.0. FACILITY OCCUPANCY: The area of the building getting the new flooring will require phasing with the customer due to the continuation of the mission. The contractor shall work with CE and the user for the phasing schedule.
9.0. DEBRIS AND MATERIALS: The contractor shall dispose of all debris and waste materials generated by any work under this contract on a daily basis off‐base at no additional cost to the government.
10.0. COORDINATION: The contractor shall notify Civil Engineering Quality Assurance (QA) Personnel at 850‐283‐1380 one week prior to start of work. The QAP shall inspect all work.
11.0. SAFETY: The contractor shall comply with all Federal Occupational Safety and Health Act (OSHA) requirements and standards as published in the 29 Code of Federal Regulations (CFR). Additionally, the contractor will comply with Air Force Occupational Safety and Health (AFOSH) standards, along with installation safety and health requirements. The Contracting Officer, QAP, Base Civil Engineer or designated representative, ground safety and security police may issue an order to the contractor or employees to stop all or part of the contractor's work until all violations are corrected. Bio‐ environmental engineering personnel, who Notice violation of OSHA, AFOSH, or local standards, will notify the QAP who has the authority to stop all or part of the contractor's work. No part of the time lost during a stop work order shall be allowed to be subject of claim by the contractor for time and money compensation. Contractor shall not be excused unsatisfactory performance that may result from a stop work order. Signs must be displayed at all entrances to the Air Field Management Facility indicating work in progress.
