STATEMENT OF WORK

Replace Flooring, Building 1454 - XLWU 17-2053A

Tyndall Air Force Base, Florida 31 January 2017

1.0. SCOPE: Contractor shall furnish all labor, materials, transportation, tools, equipment, and supervision needed to replace the existing flooring of Building 1454 on Tyndall Air Force Base, Florida.

2.0. DESCRIPTION OF WORK:

2.1. Demo existing carpet, tile, and base and dispose IAW Sec 7.0. See attached drawing.

2.2. Repair/patch floor as needed to provide smooth / level surface for the new carpet and tile.

2.3. Install new ceramic tile with cove base and new carpet with carpet-cove base.

2.3.1. New tile and cove base to be installed in the restroom is 18”x18” Heathland, Glazed Floor/Wall, Raffia (HL02) with 3/16” TEC Power Grout, Light Buff, #945 (OR EQUAL).

2.3.2. New tile and cove base to be installed in other designated ares is 24”x24” Dal-Tile, San Michele, Dorato (OR EQUAL) (Cross Cut). With a San Michele, Dorato 3” x 12” (OR EQUAL) bullnose tile base with 3/16” TEC Power Grout, Light Buff, #945 (OR EQUAL).

2.3.3. New carpet to be installed is Shaw Lush Life/Moritz 5B091, Panache (OR EQUAL) with carpet base of matching color and approved by 325 CES.

2.4. New floor elevation shall match existing.

2.5. Remove metal/aluminum thresholds to demo tile and reinstall over new tile.

2.6. Replace transition strips at Men’s and Woman’s restrooms and each adjacent room.

2.7. Tile joints shall be at the centerline of the closed doors.

2.8. Repair walls at cove base to match adjacent finish as needed.

2.9. Old and new door stop and reinstall new one at same location.

3.0. HAZARDOUS MATERIAL INVENTORY AND TRACKING: The contractor shall submit TAFB

Form 81 (Contractor’s Hazardous Material Questionnaire – Attachment 3) and TAFB Form 82 (Short- Term Contractor Hazardous Material Inventory – Attachment 4), prior to starting work. The Contractor should note that Tyndall AFB is required to report chemicals used such as (but not limited to) compressed gases, adhesives, aerosol cans, sealants, paints, lubricants, oils, cleaners, degreasers, pesticides. Copies of manufacturer-specific Material Safety Data Sheets (MSDS) must be attached to TAFB Form 82. After submission, 325 CES/CEI will notify the Project Manager and CONS of the reportable chemicals and of any special instructions. The Contractor is required to submit TAFB Form 83 (Contractor Environmental Reporting Entry Form – Attachment 6) at the completion of the project. The CO must be notified of any changes from the original submittal (i.e. new chemical is added, size of container or unit of issue changes or if the manufacturer changes). These MSDSs shall also be readily accessible at the location of each hazardous material. MSDS shall be provided to CE Environmental Engineering and to the Quality Assurance Personnel, and approved by the government, prior to commencement of work.

4.0. WORK CLEARANCE REQUEST: Contractor shall obtain a blank AF Form 103, BASE CIVIL

ENGINEERING WORK CLEARANCE REQUEST from 325 CES Customer Service, 283-4949, prior to the any performance of any work. Contractors shall request a Work Clearance at least 10 working days in advance. It is the Contractor’s responsibility to have a completed and approved AF Form 103 prior to beginning any construction activities.

5.0. HOURS: The normal duty hours of work on Tyndall AFB are from 6:30 AM to 5:00 PM, Monday thru

Friday excluding Federal Holidays. All work shall be performed during normal duty hours. All other hours shall be requested through the Contracting Officer for approval. This request shall be submitted no less than three (3) workdays prior to the time requested.

6.0. MATERIALS: All material shall be high quality from a well-known Manufacturer and used in accordance with the manufacturer’s recommendations.

7.0. DEBRIS AND MATERIALS: The contractor shall dispose of all debris and waste materials generated by any work under this contract on a daily basis off-base.

8.0. COORDINATION: The contractor shall notify Civil Engineering Quality Assurance (QA) Personnel at 850-283-1380 one week prior to start of work. The QAP shall inspect all work.

9.0. DRAWINGS: Drawings are provided for reference and are “For Official Use Only”. All dimensions, measurements and quantities etc. are approximate and shall be verified by the Contractor.

10.0. SAFETY: The contractor shall comply with all Federal, State and Local Regulations.

The Ammendment of this solicitation is due to the slight change in square footage for installing new tile.

The original quantity was 2,714 square feet, it is now 2,766 square feet.
