MEMORANDUM FOR ALL PROSPECTIVE BIDDERS

SUBJECT: Request for Quote (RFQ) FA4809-09-Q-9010, Service and Maintain Elevators, Site Visit Minutes

1. A site visit for subject project was held on 23 Sep 08 at 10:00 a.m. at the 4th Contracting Squadron, Seymour Johnson AFB, NC. The following persons were in attendance:

2. The conference began by SSgt Latta introducing the government employees and their respective roles on the project. She proceeded with the briefing and provided detailed information on the following key areas for this procurement:

a. This requirement will result in a firm fixed-price contract to Service and Maintain Elevators in accordance with the Statement of Work.

b. RFQ is issued as a 100% Small Business Set-Aside.

c. The buyer directed attention to and advised offeror to complete Representations and Certifications at FAR 52.212-3 Alt I, Offeror Representations and Certifications-Commercial Items only if offeror has not completed Online Representations and Certifications Applications (ORCA).

d. She also reminded offeror to visit the Federal Business Opportunities website periodically (prior to time set for receipt of offers) to view any amendments issued to subject RFQ.

e. She advised offeror to pay close attention to the Statement of Work.

3. After going over the RFQ, all parties departed the conference room and proceeded to the actual site. SSgt Latta requested that any questions that could not be answered during the site visit or any questions that come up prior to the RFQ closing time and date of 1600 hrs, 25 Sep 08 be submitted to the

4th Contracting Squadron in writing. Questions should be faxed to (919) 722-5404, Attn:

4 CONS/LGCB/SSgt Latta or emailed to: nitra.latta@seymourjohnson.af.mil.

4. Below is the list of questions asked during the site visit with the corresponding answers.

a. Question: Who is responsible for performing annual safety inspections on the elevators?

Response: Seymour Johnson AFB has a separate contract in place for annual certification inspections.

b. Question: Are wiring diagrams available for the elevators?

Response: The diagrams are not presently in the elevator rooms. The Government will provide diagrams to the awardee.

5. Buildings visited during the site visit were 3808, 3809, 4814, 4745, and 4517. With there being no further questions, the site visit adjourned at 10:45 a.m.
